Thousands of variant gene sequences of SARS-CoV-2 have been emerged since the COVID-19 epidemic broke out in Dec.2019.Alpha(B.1.1.7),Beta(B.1.351),Gamma(P.1),Delta(B.1.617.2)and Omicron(B.1.1.529)were the most representative variants.With the continuous emergence of new variants,the predomi-nant strain in the global pandemic as of Jan.2025 is the Omicron BA.2.86-derived mutant,JN.1.The effectiveness of drugs against Omicron variants remains a key research focus in the treatment of SARS-CoV-2 infections.The infectiousness and pathogenicity of the variants altered remarkably due to mutations in the genome on S protein of the mutant strains,and these emerging variants are more likely to evade immunity and were more infectious than the previous prevalent variants.During the process of combating with the constantly emerging novel variants,drugs showed various effects on treatment of diseases caused by different variants.New drugs and treatment coun-termeasures are constantly updated with the prevalence of various variants.The current status of research on pres-ent drugs for treatment of SARS-CoV-2 and the therapeutic effects on emerging variants are reviewed in the article so as to provide reference for prevention and treatment of the upcoming evolved variants.
2.Potential of mitochondrial transplantation in treatment of sarcopenia

BACKGROUND:Sarcopenia is a comprehensive condition of aging induced decline in skeletal muscle mass and strength and represents a major health challenge for the elderly.Accumulating evidence suggests that mitochondrial dysfunction plays a key role in the pathogenesis of sarcopenia.OBJECTIVE:To summarize the mechanisms by which dysregulation of mitochondrial quality control leads to sarcopenia and to explore whether mitochondrial transplantation may be a potential target for the treatment of sarcopenia.METHODS:We searched PubMed and CNKI databases for relevant articles published from 2009 to 2023 using the keywords "sarcopenia,mitochondrial dysfunction,mitochondrial quality control,mitochondrial transplantation,limitations."RESULTS AND CONCLUSION:Given the key role of mitochondrial dysfunction in the pathogenesis of sarcopenia,mitochondrial transplantation may serve as a possible strategy for the treatment of sarcopenia by improving mitochondrial bioenergetics and modulating mitochondria related signaling pathways.Although some preclinical and clinical studies have confirmed the potential of mitochondrial transplantation for the treatment of various diseases,there are still some urgent questions regarding the specific details of mitochondrial transfer.
3.The Clinical Mechanism of Improvement of Cognitive Impairment After Ischemic Stroke through Tongdu Xingshen Acupuncture by Regulating Gut Microbes

Objective To explore the mechanism of Tongdu Xingshen acupuncture,the clinical efficacy,systemic inflammatory response,blood-brain barrier and intestinal flora in patients with cognitive impairment after ischemic stroke(IS)were studied.Methods Thirty patients(3 cases shedding)with cognitive impairment after IS were included as the disease group,including patients before treatment as the disease group,patients after Tongdu Xingshen acupuncture treatment as the electroacupuncture group,and 30 healthy controls(3 cases shedding)were included as the healthy group.In the electroacupuncture group,on the basis of the basic treatment,Tongdu Xingshen acupuncture was applied,which was 30 min each time,once a day for 14 days.The MMSE,MoCA and MBI scores of the three groups were observed.The fecal and serum samples from all study subjects were collected,and 16S rDNA sequencing technology and ELISA were used to detect the changes of proinflammatory factors IL-6,IL-1β,TNF-α and S100β in serum in intestinal flora and feces.Results Compared with the healthy group,the MMSE,MoCA,and MBI score of patients in the disease group decreased significantly(P<0.05),serum proinflammatory factors and S100β protein content increased significantly(P<0.05),and the Shannon index(P<0.01)and Simpson index(P<0.001)increased significantly.Compared with the disease group,the MMSE,MoCA,and MBI score of the EA group increased significantly(P<0.05),the serum levels of proinflammatory factors and S100β decreased significantly(P<0.05),Shannon index and Simpson index decreased(P>0.05).The dominant bacterial flora in the healthy group mainly included Bacteroides,Bifidobacterium,Bacteroides,Faecalibacterium,Bifidobacteriaceae,Ruminococcaceae,and Bacteroides and other beneficial bacteria(P<0.05).The dominant flora in the disease group included Proteobacteria,Enterobacteriaceae,Escherichia,Klebsiella and other opportunistic bacteria(P<0.05),while the dominant flora in the EA group was consistent with the healthy group,the relative abundance of beneficial bacteria increased significantly(P<0.05),and the relative abundance of opportunistic bacteria decreased significantly(P<0.05).Spearman correlation analysis found that beneficial bacteria were positively correlated with clinical efficacy related indicators,but with serum proinflammatory factors and the content of S100β was negatively correlated.Conclusion Tongdu Xingshen acupuncture can regulate the diversity of intestinal flora to increase the abundance of Bacteroides,Bifidobacterium,Faecalibacterium,and other beneficial bacteria,regulate the intestinal microecological balance,Thereby regulating systemic inflammation and blood-brain barrier function,which plays a role in improving cognitive function.
4.Therapeutic effects of dental pulp stem cells in a mouse model of autoimmune hepatitis and related immunoregulatory mechanisms

Objective To investigate the therapeutic effect of dental pulp stem cells(DPSCs)on autoimmune hepatitis in in vivo and in vitro experiments and the related mechanism.Methods An in vitro co-culture system was used to evaluate the immunoregulatory effect of DPSCs,and 32 mice were randomly divided into healthy control group,model group,positive drug group,and DPSCs treatment group,with 8 mice in each group.The serum levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),and inflammatory factors were measured,and HE staining was used to assess liver pathological injury.An analysis of variance was used for comparison of normally distributed continuous data between multiple groups,and the least significant difference t-test was used for further comparison between two groups.Results The in vitro experiment showed that the positive rates of CD105,CD73,and CD90 in DPSCs were 99.97%,100%,and 99.53%,respectively,while the positive rates of CD34,HLA-DR,and CD45 were 0.56%,0.17%,and 0,respectively.DPSCs significantly inhibited the proliferation of Th1 and Th17 subsets,with inhibition rates of 31.32%and 45.76%,respectively;DPSCs promoted the proliferation of Treg(CD4+CD25+FoxP3+),with a promoting rate of 52.29%.DPSCs had an inhibition rate of 93.70%on the proliferation of lymphocytes.In the mouse model of autoimmune hepatitis,compared with the model group,the DPSCs treatment group had significant reductions in the serum levels of ALT and AST,with reduction rates of 66.8%and 60.0%,respectively(t=3.321 and 2.907,P=0.007 5 and 0.017 5)and significant reductions in the inflammatory factors tumor necrosis factor-α and interleukin-1β,with reduction rates of 57.5%and 71.3%,respectively(t=2.484 and 2.796,P=0.039 8 and 0.020 6),and histopathological examination showed no significant improvement in periportal bridging necrosis(t=1.969,P=0.098).Conclusion DPSCs effectively alleviate immune-mediated liver injury through immunoregulation,which provides an experimental basis for clinical translation.
5.Current Status Analysis and Comparison on Innovative Patent Technology Transfer and Transformation in the Cardiovascular Field Between China and the United States From 2014 to 2023

Objectives:To explore the progress in the application of cardiovascular related innovative patent technologies between China and the United States from 2014 to 2023,and to further analyze the differences in driving forces and policy orientations in the transformation of scientific and technological achievements between the two countries,providing reference data for improving the efficiency of the transformation of cardiovascular scientific and technological achievements in China.Methods:Based on the Himmpat patent database,we searched invention patent application data in the cardiovascular fields by using the Fuwai Subject Headings and adopting the method of the international patent classification(IPC)-title-claims-abstract,from January 1,2014 to December 31,2023(excluding utility model and design patents).Fourteen indicators regarding patent value evaluation were selected as independent variables and the transfer and transformation rate of invention patent family as the dependent variable.Then we construct univariate and multivariate prediction models by utilizing logistic regression to compare and analyze the differences in various predictors for the transformation rate of patent family in the cardiovascular field between the two countries,and calculate the respective population attributable fraction(PAF)for each predictor.Results:Over the past 10 years,there have been 45 413 invention patent family in the cardiovascular field in China,which exceeded the 35 582 invention patent family in the United States.The transformation rate of cardiovascular invention patent family in the United States was 49.77%,while in China it was only 9.18%.In the United States,the transformation rate of invention patent family's applicants involving medical institutions is significantly higher than that of universities,research institutions,and enterprises,with transformation rates of 63.75%,56.74%,50.00%,and 49.76%,respectively.In contrast,in China,the transformation rate of invention patent family in the cardiovascular field is 6.87%in medical institutions and 7.90%in universities,slightly higher in research institutions(10.99%),while the highest rate is in enterprises(15.83%).Moreover,the transformation rates of biomedical invention patent family is higher than that of medical devices in China(9.45%vs.7.81%),while the opposite is true in the United States(42.39%vs.59.03%).The results of PAF based on full multivariate logistic regression,the top three predictors for the transfer and transformation of invention patent family in China are applicants including enterprises(PAF=71.17%),overseas distribution country number more than one(PAF=30.76%),and invention authorized(PAF=29.95%).In the United States,citation frequency exceeding 10 times(PAF=46.92%),overseas distribution country number more than one(PAF=32.56%),and applicants including enterprises(PAF=20.54%)are the top three predictors for the transfer and transformation of invention patent family.Conclusions:Since the implementation of the 13th Five-Year Plan,the number of patent family applications for cardiovascular related inventions in China has surpassed that of the United States.However,the transfer and transformation efficiency of cardiovascular invention patents in China is significantly lower than that of the United States.In depth analysis of the reasons,this study finds it mainly lies in China's insufficient research and development potential for high-value patents,weak awareness of overseas market layout,insufficient motivation for universities and medical institutions to transfer and transform invention patents,and the underutilization of the transformation potential of medical device invention patents as well.
6.Effect of side-to-end anastomosis on postoperative bowel function in rectal cancer surgery: a prospective single-center randomized controlled trial

Objective:To compare bowel function 12 months after surgery between side-to-end anastomosis (SEA) and end-to-end anastomosis (EEA) groups of patients who had undergone rectal cancer resection.Methods:This single-center, prospective, open-label, phase III randomized controlled trial was approved by the Ethics Committee of Peking University People's Hospital (2018PHB040-01) and registered at ClinicalTrials. org (NCT03669237). Inclusion criteria were as follows: (1) histologically confirmed rectal adenocarcinoma; (2) tumor located 0 to 12 cm from the anal verge; (3) age≥18 years; and (4) planned R0 resection with primary reconstruction. Exclusion criteria included: (1) emergency surgery; (2) cognitive impairment; (3) non-primary anastomosis; (4) history of left-sided colonic or anorectal surgery; and (5) preexisting chronic defecation dysfunction. Eligible rectal cancer patients scheduled for elective sphincter-preserving surgery at Peking University People's Hospital were prospectively enrolled between October 2018 and March 2021 and randomly assigned to either the EEA group or the SEA group via computer-generated numbers prior to entering the operating room. All patients underwent standard radical tumor resection. Bowel function was evaluated by the low anterior resection syndrome (LARS) questionnaire. It consists of five single-choice questions and yields a total score ranging from 0 to 42. Defecation function is categorized into three levels: no LARS (0-20 points), minor LARS (21-29 points), and major LARS (30-42 points). The primary endpoint was the LARS score 12 months after surgery. Secondary endpoints included LARS scores from 1 to 11 months and during long-term follow-up(>12 months). The final follow-up was completed in July 2022. All randomized patients were included in the intention-to-treat set (ITTS). The full analysis set (FAS) was defined as ITTS patients with valid outcome data. All primary statistical analyses were performed in the FAS, and results were further compared in the per-protocol set (PPS) based on the actual treatment received.Results:A total of 323 patients underwent eligibility assessment, of whom 71 did not meet the inclusion criteria and 52 declined to participate. Ultimately, 200 patients were randomized. Median age was 64 years and 85 were women. The SEA and EEA groups comprised 102 and 98 patients, respectively. A total of 181 patients (90.5%) were included in the FAS, and 170 (85.0%) were included in the PPS. Among these, the 12-month LARS score was evaluated in 178 patients (98.3%) in the FAS and in 167 (98.2%) in the PPS. Median LARS score at 1–12 months were significantly lower in the SEA group in both the FAS dataset [12 months:8 (interquartile range [IQR], 0–22) vs. 14 (IQR, 8–29); Z=2.687, P=0.007] and the PPS dataset [12 months: 8 (IQR, 0–22) vs. 14 (IQR, 6–29); Z=2.543, P=0.011]. During long-term follow-up, the median LARS score was also significantly lower in the SEA group in the FAS dataset [2 (IQR, 0–4) vs. 11 (IQR, 2–23); Z=2.968, P=0.003] and the PPS dataset [2 (IQR, 0–14) vs. 11 (2, 27); Z=2.687, P=0.007]. Conclusion:Compared with the EEA group, bowel function was superior in the SEA group 1 year after surgery and during long-term follow-up.
7.Evaluation of Chinese and English Reports Quality of Randomized Controlled Trial of Acupuncture in the Treatment of Smoking Cessation Based on CONSORT Statment and STRICTA Checklist

Objective To evaluate the reporting quality of randomized controlled trials on acupuncture cessation by using CONSORT statement and STRICTA checklist.Methods According to inclusion and exclusion criteria,9 Chinese and English databases such as CNKI and PubMed were searched by computer,and the retrieved literatures were evaluated according to the CONSORT statement 2010 and STRICTA checklist 2010.Results After exclusion,34 Chinese and English literatures were included,including 27 in Chinese and 7 in English.In a risk review using ROB2,the Cochrane Collaboration's bias risk assessment tool,29 reviews were judged to be high risk overall,2 to be at some risk,and 3 to be at low risk.In the CONSORT statement evaluation,the reporting rate of 23 items was less than 76.4%,including the reporting rate of 17 items was less than 47%.In the STRICTA checklist evaluation,the reporting rate of 9 items was less than 76.4%,including the reporting rate of 4 items was less than 29.4%.Conclusion There are still shortcomings in the methodological design and item reporting standards of acupuncture smoking cessation RCT.It is suggested that researchers should strictly follow the CONSORT declaration and STRICTA list in designing RCT schemes and reporting RCT results in the future.

9.Ecological factors impacting genetic characteristics and metabolite accumulations of Gastrodia elata.

OBJECTIVE:
The investigation of the correlation between ecological factors and the genetic characteristics or metabolites of plants offers valuable insights into the regional causes of genetic and metabolic diversity. Here, Gastrodia elata, a medicinal plant, is employed as a model to explore the environmental factors that influence its genetic characteristics and metabolic accumulations.
METHODS:
A total of 23 G. elata populations from six cultispecies and 11 cultivated regions were selected based on the predictions of the global geographic information system. The genetic characteristics of these populations were evaluated using highly polymorphic simple sequence repeat markers. Additionally, the metabolic accumulations and antioxidant capacity of mature tubers were measured employing colorimetry and high performance liquid chromatography (HPLC). Ecological data of each region were obtained from the WorldClim-global climate database and harmonized world soil database. To assess the influence of ecological factors on the genetic characteristics and metabolic profiles of G. elata, Pearson's correlation analysis was conducted.
RESULTS:
Genetic variation among G. elata populations exceeded that within populations. Genetic diverisity, distance and structure manifested regional and species-specific patterns. Metabolic profiling and antioxidant capacity exhibited regional variations. Notably, the Lueyang region demonstrated that a content range of total polysaccharide, total protein, and phenolic glycosides was 9.34%-189.67% higher than the average. Similarly, in the Hubei region, total phenolic content, p-hydroxybenzyl alcohol content, and antioxidant indicators were observed to be higher than the average levels, by 106.57%, 136.47% and 12.50%-91.14%, respectively. Furthermore, ecological factors had a significant comprehensive impact on G. elata genetic characteristics (r > 0.256 and P < 0.05). Multivariate metabolite accumulations in G. elata were influenced by dominant ecological factors. Temperature notably impacted the accumulation of total protein (|r| > 0.528 and P < 0.05). Moisture, encompassing precipitation and soil content, significantly affected the production of phenolic glycosides (|r| > 0.503 and P < 0.05).
CONCLUSION
The genetic characteristics of G. elata manifested regional and species-specific patterns, with the metabolic accumulations and antioxidant capacity of mature tubers exhibited regional variations. Specifically, multivariate ecological factors comprehensively influenced genetic characteristics. Temperature and moisture played pivotal roles in regulating the accumulations of proteins and phenolic glycosides, respectively. These findings underscore the significant impact of ecological factors on the shaping of G. elata, highlighting their crucial role in enhancing the quality of Chinese medicinal materials.
10.Correlation between transabdominal bowel ultrasonography parameters and disease activity of ulcerative colitis

Objective:To investigate the correlation between transabdominal bowel ultrasongraphy (TBUS) parameters and disease activity of ulcerative colitis (UC).Methods:The results of TBUS, endoscopy and laboratory tests of 68 UC patients (108 examinations) who visited the Department of Gastroenterology of the Second Hospital (Eastern Campus) of Hebei Medical University From January 2022 to February 2024 were retrospectively analyzed. According to Mayo endoscopic score (MES), there were 10 cases of remission (MES=0) and 98 cases of active phase (MES>0); patients in active phase further classified into 16 cases of mild (MES=1), 19 cases of moderate (MES=2), and 63 cases of severe (MES=3). The TAUS parameters were compared between patients in remission and active phases. Spearman rank correlation analysis was used to analyze the correlation between the TBUS parameters including bowel wall thickness (BWT), Limberg score, wall layer stratification, fat wrapping and endoscopic activity score, laboratory inflammatory indicators. Receiver operating characteristic curve (ROC) analysis was applied to evaluate the predictive efficacy of the TBUS parameters and laboratory inflammatory indicators in identifying remission under endoscopy. Independent sample t-test and Fisher′s exact probability method were used to compare the two groups. Results:The BWT of patients in remission was significantly thinner than that of patients in active phase ((2.99±0.41) mm vs. (5.66±1.57) mm, t=-5.34, P<0.001). The proportions of patients in remisson with Limberg score ≤2, normal wall layer stratification, and absence of fat wrapping were significantly higher than those of patients in active phase (10/10 vs. 34/98, 10/10 vs. 48/98, 10/10 vs. 41/98, Fisher′s exact test, P<0.001, =0.002, =0.001). BWT, Limberg score, wall layer stratification, and fat wrapping were positively correlated with MES ( r=0.676, 0.677, 0.441, and 0.493, all P<0.001). BWT and Limberg score were moderate positively correlated with C-hypersensitive reactive protein, erythrocyte sedimentation rate and fecal calprotectin (BWT: r=0.561, 0.420 and 0.458, all P<0.001; Limberg score: r=0.576, 0.469 and 0.403, all P<0.001), and were negatively correlated with serum albumin and hemoglobin (BWT: r=-0.604 and -0.453, both P<0.001; Limberg score: r=-0.573 and -0.532, both P<0.001). The results of ROC analysis showed that BWT achieved the best predictive efficacy in identifying endoscopic remission, and the best cut-off value was 3.45 mm (area under the curve was 0.972, 95% confidence interval: 0.944 to 1.000, P<0.001), with a sensitivity of 93.9% and specificity of 100.0%. Conclusions:TBUS parameters can accurately reflect disease activity in UC. It is a powerful tool for UC monitoring. The BWT<3.45 mm is the best cut-off value in prediction of endoscopic remission.
