1.Confirmatory analysis of HBsAg reactive samples from voluntary blood donors

Objective: To systematically analyze the confirmatory positivity of different combinations of HBsAg screening results in blood testing, providing data to support the optimization of blood donor eligibility management. Methods: A retrospective analysis was conducted on blood screening data from 174 266 voluntary blood donor samples at the Chongqing Blood Center between October 2021 and September 2022. Samples with inconsistent results between the two HBsAg enzymelinked immunosorbent assays (ELISA) and individual donor nucleic acid testing (NAT) were confirmed using an electrochemiluminescence immunoassay (ECLIA) and a neutralization test. The detection efficacy of four different HBsAg ELISA reagents was compared using the HBsAg-confirmed positive samples. Results: A total of 767(0.44%) HBV-reactive (HB-sAg and/or HBV DNA reactive) samples were detected. Among them, 344 samples with discordant serological and NAT results were collected, of which 64(18.6%) were confirmed positive by neutralization test. Additionally, 5 samples that were neutralization-negative but double-reactive for HBsAg and HBV DNA were confirmed as positive according to FDA guidance, resulting in a total of 69(20.1%) confirmed HBsAg-positive samples. There were significant differences in the neutralization test confirmation rates among different screening result categories (P<0.05): The group with dual HBsAg reagent reactivity (double reactive) & NAT-negative had the highest confirmation rate (96.9%, 31/32); the group reactive to only reagent 2 (single reactive) had a rate of 25.7% (29/113); while the confirmation rates for samples reactive to only reagent 1 and samples with isolated HBV DNA positivity were extremely low [0(0/34) and 2.4%(4/165), respectively]. The four commercial reagents showed significant differences in their ability to detect confirmed positive samples that were initially single reactive (P<0.05). Conclusion: Given the performance variations among HBsAg screening reagents, thorough performance verification is essential before implementation. When NAT is negative, dual HBsAg reactivity in screening can serve as a basis for confirming infection and directly deferring blood donors. However, confirming infection in donors with single HBsAg reactivity is more challenging, necessitating supplementary tests to rule out infection risk.
2.Effects of COM-B model-based motivation interview combined with transitional care in stroke patients

Objective:To explore the effects of capacity, opportunity, motivation-behavior (COM-B) model-based motivation interview combined with transitional care on self-management ability, health behavior, and social support in stroke patients.Methods:From June 2021 to June 2024, 104 stroke patients admitted to Beijing Anzhen Hospital, Capital Medical University, were selected as study subjects using convenience sampling. Patients were randomly assigned to either the control group or the intervention group, with 52 patients in each group. Control group received routine care, while intervention group received COM-B model based motivation interview combined with transitional care in addition to routine care. Scores of the Exercise of Self-Care Agency Scale (ESCA) , Health Behavior Scale for Stroke Patients (HBS-SP) , 11-item Duke Social Support Index (DUKE-UNK11) , Readiness for Hospital Discharge Scale (RHDs) , and Stroke Specific Quality of Life Scale (SS-QOL) were compared between two groups of patients before and after intervention.Results:After intervention, the ESCA, HBS-SP, DUKE-UNK11, RHDs, and SS-QOL scores of intervention group were higher than those of control group, and the differences were statistically significant ( P<0.05) . Conclusions:COM-B model-based motivation interview combined with transitional care can improve the self-management ability of stroke patients, promote the formation of healthy behaviors, and enhance patients' perception of social support, which is worthy of clinical promotion and application.
3.Study on ultrasound nomogram model based on BI-RADS in predicting malignant tumor of breast

Objective:To develop a nomogram model that combined radiomics and the Breast Imaging Reporting and Data System(BI-RADS),which was used to predict breast cancer in category 4 or 5 lesions of BI-RADS ultrasound(US).Methods:A total of 315 female patients with breast tumors who admitted to The First People's Hospital of Chuzhou and Xinjiang Uygur Autonomous Region Hospital of Traditional Chinese Medicine from November 2021 to September 2024 were selected.In them,211 patients from January 2022 to August 2023 were included in the training group from Xinjiang Uygur Autonomous Region Hospital of Traditional Chinese Medicine,and 104 patients from November 2021 to September 2024 were included in the validation group from The First People's Hospital of Chuzhou.The nomogram model was developed according to the results of multiple regression analysis of the training group.Then,the discrimination,calibration and clinical practicality of nomogram were assessed in predicting breast cancer in the validation group.Results:Radiomics score and BI-RADS category were independent influencing factors in predicting malignant tumors of breast(OR=4.66,4.87,P<0.05).In the ROC curve analysis,the area under curve(AUC)values of the ROC curves of the nomogram models of training group and the validation group were respectively 0.928 and 0.883 in the ability of distinguishing malignant and benign lesions,which all were better than AUC value(0.791,0.864)of the radiomics score,and that(0.825,0.857)of the BI-RADS category,and the differences were statistically significant(Ztraining group=4.026,3.716,and Zverification group=3142,2.847,P<0.05).Conclusion:The nomogram model based on radiomic score and BI-RADS category has potential application value in the prediction for malignant tumor of breast that is BI-RADS US category 4 or 5.
4.Relationship between high DNA stainability of sperm and routine semen parameters

Objective To investigate the relationship between high DNA stainability(HDS)and routine semen parameters.Methods Semen samples were collected from 396 men of childbearing age who were admitted to our department.Correlations of HDS with routine semen parameters and age were analyzed.Multiple linear regression analysis was performed to identify the routine semen parameters that had the greatest influence on HDS.The correlation of HDS and DNA fragmentation index(DFI)with routine semen parameters and age in 244 patients with teratozoospermia was analyzed.The 244 patients were divided into extremely severe,severe,moderate,and mild teratozoospermia groups,and differences in HDS,DFI,and routine semen parameters were compared among the four groups.Results HDS was negatively correlated with total sperm count,sperm concentration,sperm progressive motility,and normal sperm morphology rate(NSMR)(P<0.01).After adjusting for potential confounders,including total sperm count,sperm concentration,sperm progressive motility,NSMR,and DFI,NSMR had the most significant negative effect on HDS(P<0.05).In the 244 patients with teratozoospermia,HDS was negatively correlated with NSMR(P<0.01)and positively correlated with the percentage of sperm head and tail abnormalities(P<0.05),while DFI was positively correlated with the percentage of sperm tail abnormalities(P<0.01).There was a significant difference in the percentage of sperm head abnormalities among the four teratozoospermia groups.The more severe the malfor-mation,the higher the percentage of sperm head abnormalities was(P<0.001).HDS in the extremely severe teratozoospermia group was significantly higher than that in the mild and moderate teratozoospermia groups(P<0.05).No significant differences in DFI were found among the four groups(P>0.05).Conclusion HDS was closely correlated with routine semen parameters and was a crucial biomarker for assessing sperm quality,particularly the extent of sperm head abnormalities.
5.Research status of quality assessment of hospice care for children with cancer abroad

The effectiveness and professionalism of hospice care services for children with cancer are of great significance for improving their quality of life and reducing the mental and economic burden on their families. Currently, a relatively complete quality evaluation system for hospice care for children with cancer has been established abroad, while research in this field in China is still in its infancy. By elaborating on the concept and connotation of quality assessment of hospice care for children with cancer in detail, systematically reviewing the research status of quality assessment of hospice care for children with cancer abroad, and addressing the problems and challenges faced by quality assessment of hospice care for children with cancer in China, corresponding inspirations and improvement strategies were proposed, aiming to provide a reference for improving the quality of hospice care for children with cancer in China.
6.Effect and mechanism of Jingangteng capsules in the treatment of non-alcoholic fatty liver disease based on gut microbiota and metabolomics

OBJECTIVE To investigate the effect and mechanism of Jingangteng capsules in the treatment of non-alcoholic fatty liver disease (NAFLD). METHODS Thirty-two SD rats were randomly divided into normal group and modeling group. The modeling group was fed a high-fat diet to establish a NAFLD model. The successfully modeled rats were then randomly divided into model group, atorvastatin group[positive control, 2 mg/(kg·d)], and Jingangteng capsules low- and high-dose groups [0.63 and 2.52 mg/(kg·d)], with 6 rats in each group. The pathological changes of the liver were observed by hematoxylin-eosin staining and oil red O staining. Enzyme-linked immunosorbent assay was performed to determine the serum levels of triglycerides (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), alanine transaminase (ALT), aspartate transaminase (AST), tumour necrosis factor-α (TNF-α), interleukin-1β (IL-1β), IL-6, IL-18. 16S rDNA amplicon sequencing and metabolomics techniques were applied to explore the effects of Jingangteng capsules on gut microbiota and metabolisms in NAFLD rats. Based on the E-mail:591146765@qq.com metabolomics results, Western blot analysis was performed to detect proteins related to the nuclear factor kappa-B (NF-κB)/NOD-like receptor family protein 3 (NLRP3) signaling pathway in the livers of NAFLD rats. RESULTS The experimental results showed that Jingangteng capsules could significantly reduce the serum levels of TG, TC, LDL-C, AST, ALT, TNF-α, IL-1β, IL-6, IL-18, while increased the level of HDL-C, and alleviated the hepatic cellular steatosis and inflammatory infiltration in NAFLD rats. They could regulate the gut microbiota disorders in NAFLD rats, significantly increased the relative abundance of Romboutsia and Oscillospira, and significantly decreased the relative abundance of Blautia (P<0.05). They also regulated metabolic disorders primarily by affecting secondary bile acid biosynthesis, fatty acid degradation, O-antigen nucleotide sugar biosynthesis, etc. Results of Western blot assay showed that they significantly reduced the phosphorylation levels of NF-κB p65 and NF-κB inhibitor α, and the protein expression levels of NLRP3, caspase-1 and ASC (P<0.05 or P<0.01). CONCLUSIONS Jingangteng capsules could improve inflammation, lipid accumulation and liver injury in NAFLD rats, regulate the disorders of gut microbiota and metabolisms, and inhibit NF-κB/NLRP3 signaling pathway. Their therapeutic effects against NAFLD are mediated through the inhibition of the NF-κB/NLRP3 signaling pathway.

9.Neuroform Atlas stent-assisted coil embolization for middle cerebral artery bifurcation aneurysms: a multicenter retrospective study

Objective:To evaluate the efficacy and safety of Neuroform Atlas stent-assisted coil embolization in patients with middle cerebral artery bifurcation aneurysms.Methods:A retrospective analysis was performed; the clinical data of 46 patients with middle cerebral artery bifurcation aneurysms accepted Neuroform Atlas stent-assisted coil embolization in First Affiliated Hospital of Zhengzhou University, Beijing Tiantan Hospital Affiliated to Capital Medical University, First Affiliated Hospital of Harbin Medical University, Zhujiang Hospital of Southern Medical University and First Affiliated Hospital of Chongqing Medical University from January 2022 to March 2024 were collected. There were 28 ruptured aneurysms (60.87%) and 18 unruptured aneurysms (39.13%). Follow-up was performed for more than 3 months; Raymond-Roy grading was used to evaluate the aneurysm embolization immediately after embolization and during follow-up; perioperative hemorrhagic or ischemic complications were recorded; modified Rankin Scale (mRS) was used to evaluate the prognosis of the patients at discharge and during follow-up (mRS score≤2: good prognosis, and mRS score>2: poor prognosis).Results:Coil embolization was successful in all 46 patients. DSA immediately after embolization showed that 41 patients (89.13%) had completely occluded aneurysms (Raymond-Roy grading I), 2 patients (4.35%) had residual aneurysm neck (Raymond-Roy grading Ⅱ) and 3 patients (6.52%) had partially occluded aneurysms (Raymond-Roy grading Ⅲ). Perioperative complications occurred in 5 patients, including 2 with postoperative cerebral infarction, 1 with hydrocephalus, 1 with postoperative pneumonia leading to respiratory failure, and 1 with stent thrombosis during embolization. Both at discharge and 3 months after embolization, 43 patients (93.48%) had good prognosis and 3 patients (6.52%) had poor prognosis. No obvious ischemic complications (such as stent restenosis) or hemorrhagic complications (such as re-rupture of the aneurysms) were found in all patients. Thirty patients (65.22%) had imaging follow-up for 6-12 months: 26 (86.67%) had Raymond-Roy grading I, 3 (10.00%) had Raymond-Roy grading II, and 1 (3.33%) had Raymond-Roy grading III.Conclusion:Neuroform Atlas stent-assisted coil embolization has good short-term efficacy and high safety in middle cerebral artery bifurcation aneurysms, but long-term follow-up observation is still needed to verify its efficacy.
10.Correlation analysis of genetic and environmental factors with clinical characteristics of pediatric upper urinary tract calculi

Objective:To investigate the influence of genetic and environmental factors on the clinical characteristics of upper urinary tract calculi in pediatric patients.Methods:This study was a retrospective case series. The clinical data of 179 children under the age of 14 with upper urinary tract calculi treated at Beijing Friendship Hospital,Capital Medical University,from August 2014 to February 2023 were analyzed. There were 121 males(67.60%)and 58 females(32.40%),with a median age at onset of 2.10(1.14,5.17)years. Thirty-three cases(18.44%)had a family history of urinary stone disease. Stone characteristics was defined by CT,with a median stone burden(sum of the diameters of all stones)of 1.3(1.00,1.60)cm. Fifty-four(30.17%)children had staghorn calculi. Multiple stones were present in 92 cases(51.40%),and bilateral stones in 52 cases(29.05%),with hydronephrosis was present in 119 children(66.48%). The median follow-up time was 67 months,and 36 children(20.11%)experienced stone recurrence. Dietary habits and related information were collected by electronic questionnaire,including a total of 115 children(64.25%)with an unbalanced diet,101(56.42%)with insufficient water intake,and 32 children(17.88%)with a preference for a high-protein diet. Tap water was used as the source of drinking water by 128 patients(71.51%),and 107(59.78%)took dietary supplements. Whole-exome sequencing revealed that 55 children(30.73%)carried pathogenic mutations in stone-related genes. Binary logistic regression was used for univariate analysis of above risk factors. Variables with P < 0.1 in univariate analysis and without multicollinearity were included in multivariate logistic regression to further screen for independent risk factors. Results:Multivariate analysis confirmed that carrying stone-related pathogenic gene mutations( OR = 3.06,95% CI 1.25?7.45, P = 0.014)and insufficient water intake( OR = 3.28,95% CI 1.14?9.47, P = 0.028)were independent risk factors for higher stone burden. A high-protein diet( OR = 2.40,95% CI 1.03?5.63, P = 0.044),carrying stone-related pathogenic gene mutations( OR = 4.57,95% CI 2.21?9.46, P<0.01),and a family history of stones( OR = 3.18,95% CI 1.28 ~ 7.91, P = 0.013)were independent risk factors for staghorn calculi. Multiple stones were closely associated with a family history of stones( OR = 2.66,95% CI 1.15-6.17, P = 0.022)and carrying stone-related pathogenic gene mutations( OR = 3.22,95% CI 1.60-6.48, P = 0.001). Moreover,carrying stone-related pathogenic gene mutations( OR = 5.19,95% CI 2.52?13.82, P < 0.01)were an independent risk factor for stone recurrence,whereas dietary supplement intake was a protective factor( OR = 0.26,95% CI 0.11?0.62, P = 0.002). Conclusions:Genetic and environmental factors play significant roles in the occurrence and development of pediatric upper urinary tract stones. A high-protein diet as well as a positive family history of stones are independent risk factors for staghorn calculi,and insufficient water intake is a critical environmental factor for stone formation,while appropriate use of dietary supplements may help reduce the risk of stone recurrence. Genetic testing indicates that approximately 30% of children carry stone-related pathogenic gene mutations,and these patients prone to severe stone and an increased risk of recurrence.
