1.Ethical dilemmas and solutions of informed consent in psychological counseling

Informed consent in psychological counseling is the first step for clients to initiate the counseling process, and the degree and effectiveness of informed consent are important factors that determine the subsequent effectiveness and development of psychological counseling. By elaborating on the connotation and importance of informed consent in psychological counseling, the ethical dilemmas of the issue of informed consent in psychological counseling were classified and summarized. There were ethical dilemmas, such as the lack of consensus, procedural guarantee mechanisms, special clients procedures and informed consent principles, as well as non-standard informed consent procedures in online psychological counseling. The paper also proposed to clarify the explanatory obligations of counselors, strengthen procedural ethical constraints, formulate special norms for informed consent for special subjects, and enhance the ethical education and capacity building of psychological counselors.
2.Lipoxygenase inhibits SFTSV replication by down-regulating autophagy

Objective To explore the effects of lipoxygenase(LOX)on severe fever with thrombocytopenia syndrome virus(SFTSV)in human umbilical vein endothelial cells(HUVECs).Methods The CCK-8 assay was used to assess the cytotoxicity of LOX in HUVECs.Real-time quantitative PCR(qPCR)was adopted to detect the replication of viral RNA in cells.The infection rate of SFTSV to HUVECs was observed via indirect immunofluorescence assay(IFA).The expressions of intracellular viral NP protein and autophagy related proteins microtubule-associated protein 1 light chain 3(LC3)and adenosine 5'-monophosphate-activated protein kinase(AMPK)/mammalian target of rapamycin(mTOR)were detected via Western blotting.Lipid metabolomics was used to analyze the differences in expressions of intracellular metabolites in the SFTSV-infected Huh 7 with or without LOX treatment.Results The results of CCK-8 showed no obvious cytotoxicity to HUVECs with LOX≤8 mg/mL.With the increase of LOX concentrations,the viral RNA level,viral infection rate,and expressions of SFTSV nucleoprotein(NP)protein were gradually decreased,respectively.The autophagy reaction was activated in SFTSV-infected HUVECs,evidenced by the increased expression level of LC3 Ⅱ protein.Compared with the untreated group,LOX treatment resulted in decreased levels of LC3 Ⅱ and AMPK phosphorylation,and enhanced phosphorylation of mTOR in SFTSV-infected HUVEC.Conclusion LOX negatively regulates autophagy by inhibiting AMPK/mTOR signaling pathway,thereby inhibiting SFTSV replication.
3.Clinical Study of Ultrasound-Guided Acupuncture from Tiaokou(ST38)to Chengshan(BL57)in the Treatment of Acute Scapulohumeral Periarthritis

Objective This study aim to evaluate the therapeutic efficacy of ultrasound-guided acupuncture,specifically targeting the Tiaokou(ST38)acupoint through Chengshan(BL57)to elicit the Deqi sensation,in the context of acute scapulohumeral periarthritis.Methods Utilizing a randomized controlled trial design,a total of 60 subjects diagnosed with acute scapulohumeral periarthritis underwent random allocation into either the observation or control group,with 30 subjects in each.Under ultrasound guidance,acupuncture was administered unilaterally at the specific acupoint ST38 through BL57.The observation group received needling traversing the calf interosseous membrane,reaching the deep fascia of the tibialis posterior muscle,employing lifting and inserting twisting techniques to induce Deqi.In contrast,the control group received acupuncture targeting the sub-superficial fascia without needle manipulation or the requirement for Deqi.Both groups underwent a singular acupuncture session,with a standardized 20-minute needle retention period.Results Post-acupuncture intervention,the observation group exhibited a statistically significant total effective rate of 76.66%,greater than that in the control group at 30%(P<0.01).Comparative analysis revealed substantial enhancements in the observation group,including improvements in VAS score,Constant shoulder joint function score,the highest △T of red external thermal imaging around the shoulder,and patients'subjective acupuncture sensation scores(P<0.05).Additionally,within the observation group,post-treatment reductions were observed in VAS score and △ T of red external thermal imaging,coupled with an elevation in Constant shoulder joint function score(P<0.01).Importantly,neither group manifested discernible adverse reactions.Conclusion Ultrasound-guided acupuncture,facilitating Deqi at Tiaokou(ST38),emerges as a clinically significant and secure intervention for acute scapulohumeral periarthritis.These findings substantiate its potential as a viable therapeutic modality,promoting broader clinical adoption.
4.Meta-analysis of the immunogenicity of inactivated quadrivalent influenza virus vaccine in people over 60 years old

Objective To evaluate the immunogenicity of inactivated quadrivalent influenza vaccine (QIV) in people over 60 years old. Methods Databases including PubMed, EMBASE, Cochrane Library, China Biology Medicine disc, Wangfang Database and China National for Biotechnology Information were searched for random control studies comparing and analyzing the immunogenicity and safety between QIV and inactivated trivalent influenza vaccine (TIV) in people over 60 years old. Meta-analysis of the included literature data was performed using Stata11.0 software. Results A total of 9 articles were included in this study. There was no difference in seroconversion rates (SCR) and seroprotection rates (SPR) for the same vaccine strain after inoculation between QIV and TIV, while for the B vaccine strain (B/Victoria or B/Yamagata) not included in TIV, the SCR and SPR of QIV were significantly higher: SCR RR of 2.04 (95%CI:1.48~2.83) and SPR RR of 1.21 (95%CI:1.11~1.31) for B/Victoria; SCR RR of 2.35 (95%CI:2.04~2.69) and SPR RR of 1.12 (95%CI:1.02~1.23) for B/Yamagata. Conclusion For people over 60 years old, QIV has good immunogenicity, and produces better immunogenicity against influenza B vaccine strains not included in TIV.

6.Predictive value of vascular endothelial growth factor expression and microvascular density for the depth of infiltration in early gastric cancer

Objective:To investigate the predictive value of vascular endothelial growth factor (VEGF) expression and microvascular density (MVD) for the depth of infiltration in early gastric cancer.Methods:The pathological tissues of 24 patients with early gastric cancer (early gastric cancer group), 23 patients with advanced gastric cancer (advanced gastric cancer group) and 10 patients with gastritis (gastritis group) who admitted to Fenyang Hospital Affiliated of Shanxi Medical University from January 2020 to January 2022 were retrospectively collected. Immunohistochemistry was used to detect VEGF expression and MVD in the lesion tissues of each group, and the correlation of VEGF expression and MVD in gastric cancer tissues with the clinicopathological characteristics of patients was analyzed. Postoperative pathological diagnosis was treated as the gold standard. The efficacy of VEGF and MVD in predicting the depth of infiltration in gastric cancer and early gastric cancer was assessed by using the receiver operating characteristic (ROC) curve.Results:The VEGF positive expression rate was 10.00% (1/10), 29.17% (7/24) and 78.26% (18/23) in gastritis group, early gastric cancer group and advanced gastric cancer group, respectively, and the MVD was (21±5) strips/field, (23±9) strips/field and (43±15) strips/field, respectively. The positive expression rate of VEGF and MVD were related with the tumor diameter [>2 cm vs. ≤2 cm:69.70% (23/33) vs. 14.29% (2/14), (39±15) strips/field vs. (20±8) strips/field] and infiltration depth of gastric cancer [intramucosal carcinoma vs. submucosal carcinoma vs. advanced gastric cancer: 26.31% (5/19) vs. 40.00% (2/5) vs. 78.26% (18/23), (20±7) strips/field vs. (36±3) strips/field vs. (43±15) strips/field] (all P > 0.01), while not related with gender, age, tumor location, differentiation degree (all P > 0.05). The ROC curve analysis showed that the area under the curve (AUC) of VEGF and MVD in predicting the depth of infiltration in gastric cancer was 0.716 (95% CI 0.581-0.828) and 0.711 (95% CI 0.573-0.823), respectively; the optimal cut-off value of VEGF and MVD was positive and 24.8 strips/field, with the sensitivity of 53.19%, 61.70%, and the specificity of 90.00% both. The AUC of VEGF and MVD in predicting the depth of infiltration in early gastric cancer was 0.596 (95% CI 0.414-0.760) and 0.506 (95% CI 0.330-0.681) , respectively; the optimal cut-off value of VEGF and MVD was positive and 32.5 strips/field, with the sensitivity of 29.17% , 70.83%, and the specificity of 90.00%, 0, respectively. Conclusions:VEGF expression and MVD are elevated with the increase of depth of gastric cancer infiltration, while the value of the combination of both in predicting the depth of infiltration in early gastric cancer is not high.
7.Characterization of the antigens in inactivated porcine circovirus type 2 vaccines and virus-like particle vaccines by high-performance size-exclusion chromatography coupled with multi-angle laser light scattering.

This paper aims to detect the antigens in porcine circovirus type 2 (PCV2) vaccines by high-performance size-exclusion chromatography (HPSEC) coupled with multi-angle laser light scattering (MALLS). With purified inactivated PCV2 and PCV2 virus-like particles (VLP) as references, two inactivated vaccines (a and b) and two VLP vaccines (c and d) for PCV2 from four manufacturers were analyzed by HPSEC-MALLS after demulsification. The antigen peaks in HPSEC-MALLS were identified by PCV2 antigen test strips, Western blotting and transmission electron microscope (TEM). The repeatability and linearity of the method were investigated. The results showed the virus antigens in the two inactivated vaccines were eluted at about 13.3 min in HPSEC. The molecular weight of these antigens was 2.61×106 (±4.34%) Da and 2.40×106 (±2.51%) Da, respectively, as calculated by MALLS. The antigen peaks of the two VLP vaccines also appeared at 13.3 min and the molecular weight was 2.09×106 (±2.94%) Da and 2.88×106 (±11.85%) Da, respectively, which was close to the theoretical molecular weight of PCV2. Moreover, an antigen peak of VLP vaccine c was observed at 11.4 min and the molecular weight was 4.37×106 (±0.42%) Da. The antigen was verified to be the dimer of VLP by TEM. Vaccine d and purified Cap VLP antigens were tested repeatedly, and the RSD of the peak area (n=3) was all < 1.5%, indicating that the method was repeatable. The purified VLP were diluted in serial and tested for linearity. The result suggested good linear relationship between the peak area of VLP or VLP aggregates and the protein concentration of the sample with R2 of 0.999 and 0.997, respectively. Thus, the method met the requirement for quantification and aggregate analysis. This method is accurate and efficient in in vitro quality evaluation and improvement of PCV2 vaccine.

Objective To evaluate the safety of a Chinese thimerosal-free trivalent split influenza virus vaccine after being marketed in a large population. Methods Through the information management system of adverse event following immunization (AEFI), the adverse events in healthy people aged 6 months and above who were vaccinated with split influenza virus vaccine in Hubei Province from October to December 2015 were collected. The data was analyzed by descriptive methodology. Results From October 1, 2015 to June 30, 2016, among the 227 920 people in Hubei Province who were vaccinated with split influenza virus vaccine, the common adverse reactions were mainly fever, redness, irritability, pain and itching. Four cases of AEFI were passively observed and reported in the system, with a reporting rate of 1.76/100 000, among which 3 cases were anaphylactic rash and 1 case was optic neuritis. Conclusion The Chinese thimerosal-free trivalent split influenza virus vaccine used in Hubei Province had a good safety record and is suitable for the general vaccination of people without vaccination contraindications.
10.ANALYSIS OF THE CLINICAL FEATURES OF 4 PATIENTS WITH DETECTABLE SEVERE FEVER WITH THROMBOCYTOPENIA SYNDROME VIRUS IN THEIR THROAT SWABS

The suspected severe fever with thrombocytopenia syndrome ( SFTS) cases of pharyngeal swab detection of severe fever with thrombocytopenia syndrome virus ( SFTSV) , to discuss the possibility of spread of respiratory tract. According to the suspected cases of inclusive criteria from national Ministry of Health issued " National guideline for prevention and control of severe fever with thrombocytopenia syndrome (2010)". May to September in 2013, we collect serum and throat swab of 72 suspected SFTS cases from the sentinel surveillance hospital in SFTS high?incidence area ( Xinyang, He′nan Province) . Real?time RT?PCR and ELISA methods for the detection of SFTSV were used in this study. In 72 suspected SFTS cases, 52 (72?2%) were detected positive, 46 were positive for RT?PCR, 6 were positive for ELISA. In 52 SFTSV positive cases, 4 ( 7?7%) cases were found with positive nuclei acid of SFTSV in throat swab samples. in 20 SFTSV negative cases, all the throat swab samples from the patients were also negative. The 4 SFTSV positive cases were severe, with cough and the highest viral load of up to 108 copies/mL. SFTSV can be detected in the throat swabs from patients, and the positive rate of throat swabs is much lower than that of serum samples. The feasibility of detecting SFTSV is disputable. SFTS patients with positive throat swab samples possessed the high viral load and cough symptoms, which may increase the risk of SFTSV transmission through the respiratory tract.
