In this study, the pharmacokinetic characteristics and tissue distribution of cannabidiol(CBD)/γ-polyglutamic acid-g-cholesterol(γ-PGA-g-CHOL) nanomicelles [CBD/(γ-PGA-g-CHOL)NMs] were investigated by pharmacokinetic experiments, and the effect of CBD/(γ-PGA-g-CHOL)NMs on the lipopolysaccharide(LPS)-induced inflammatory damage of cells was evaluated by cell experiments. CBD/(γ-PGA-g-CHOL)NMs were prepared by dialysis. The CBD concentrations in the plasma samples of male SD rats treated with CBD and CBD/(γ-PGA-g-CHOL)NMs were investigated, and the pharmacokinetic parameters were calculated and compared. UPLC-MS/MS was employed to determine the concentration of CBD in tissue samples. The heart, liver, spleen, lung, kidney, and muscle samples were collected at different time points to explore the tissue distribution of CBD and CBD/(γ-PGA-g-CHOL)NMs. The Caco-2 cell model of LPS-induced inflammation was established, and the cell viability, transepithelial electrical resistance(TEER), and secretion levels of inflammatory cytokines were determined to compare the anti-inflammatory activity between the two groups. The results showed that CBD/(γ-PGA-g-CHOL)NMs had the average particle size of(163.1±2.3)nm, drug loading of 8.78%±0.28%, and encapsulation rate of 84.46%±0.35%. Compared with CBD, CBD/(γ-PGA-g-CHOL)NMs showed increased peak concentration(C_(max)) and prolonged peak time(t_(max)) and mean residence time(MRT_(0-t)). Within 24 h, the tissue distribution concentration of CBD/(γ-PGA-g-CHOL)NMs was higher than that of CBD. In addition, both CBD and CBD/(γ-PGA-g-CHOL)NMs significantly enhanced Caco-2 cell viability and TEER, lowered the secretion levels of inflammatory cytokines, and alleviated inflammation. Moreover, CBD/(γ-PGA-g-CHOL)NMs demonstrated stronger anti-inflammatory effect. It can be inferred that γ-PGA-g-CHOL blank nanomicelles are good carriers of CBD, being capable of prolonging the circulation time of CBD in the blood, improving the bioavailability and tissue distribution concentration of CBD, and protecting against LPS-induced inflammatory injury. The findings can provide an experimental basis for the development and clinical application of oral CBD preparations.

OBJECTIVES: To investigate the chain mediating role of family care and emotional management in the relationship between social support and anxiety among rural primary school students. METHODS: A questionnaire survey was conducted among students in grades 4 to 6 from four counties in Hunan Province. Data were collected using the Social Support Rating Scale, Family Care Index Scale, Emotional Intelligence Scale, and Generalized Anxiety Disorder -7. Logistic regression analysis was used to explore the influencing factors of anxiety symptoms. Mediation analysis was conducted to assess the chain mediating effects of family care and emotional management between social support and anxiety. RESULTS: A total of 4 141 questionnaires were distributed, with 3 874 valid responses (effective response rate: 93.55%). The prevalence rate of anxiety symptoms among these students was 9.32% (95%CI: 8.40%-10.23%). Significant differences were observed in the prevalence rates of anxiety symptoms among groups with different levels of social support, family functioning, and emotional management ability (P<0.05). The total indirect effect of social support on anxiety symptoms via family care and emotional management was significant (β=-0.137, 95%CI: -0.167 to -0.109), and the direct effect of social support on anxiety symptoms remained significant (P<0.05). Family care and emotional management served as significant chain mediators in the relationship between social support and anxiety symptoms (β=-0.025,95%CI:-0.032 to -0.018), accounting for 14.5% of the total effect. CONCLUSIONS Social support can directly affect anxiety symptoms among rural primary school students and can also indirectly influence anxiety symptoms through the chain mediating effects of family care and emotional management. These findings provide scientific evidence for the prevention of anxiety in primary school students from multiple perspectives.

Objective:To explore the clinical efficacy of acupuncture therapy in treating post-stroke pulmonary(PSP)and its impact on diaphragm function based on the theory of liver ascending and lung descending.Methods:This study was a prospective study,70 PSP patients who were admitted to our hospital from April 2023 to August 2024 were selected,they were divided into control group and study group according to the random number table method,with 35 patients in each group.Both groups received routine treatment,the control group received respiratory training,while the study group received respiratory training combined with acupuncture therapy.Clinical efficacy,recovery time of signs,related scale scores[clinical pulmonary infection score(CPIS),National Institute of HealthStroke Scale(NIHSS)],blood gas and lung function indicators[(partial pressure of oxygen(PaO2),partial pressure of carbondioxide(PaCO2),forced expiratory volume in one second(FEV1),peak expiratory flow(PEF)]and diaphragm function between two groups were compared.Results:The total clinical effective rate of the study group was higher than that of the control group,and the time for fever resolution,relief of pulmonary infection,disappearance of pulmonary wet rales and disappearance of cough were shorter than those of the control group(P＜0.05).The CPIS scores,NIHSS scores and PaCO2 of the study group at 14 days after intervention were lower than those of the control group,FEV1,PEF,PaO2,thickening rate and activity of diaphragm were higher than those of the control group(P＜0.05).Conclusion:Acupuncture therapy in treating PSP patients based on the theory of liver ascending and lung descending,can shorten recovery time of signs,improve clinical treatment effectiveness,reduce the degree of infection and brain nerve damage,and improve patients' lung function and diaphragm function.

Objective:To investigate the association of serum levels of tumor necrosis factor receptor-related factor 6(TRAF6)and histone deacetylase 3(HDAC3)with severity of coronary artery disease and prognosis in patients with acute myocardial infarction(AMI).Methods:A total of 122 AMI patients admitted Macheng Traditional Chinese Medicine Hospital between July 2019 and December 2020 were selected,divided into low score group(n=73,＜20 points)and high score group(n=49,≥20 points)according to Gensini score.Association of serum levels of TRAF6 and HDAC3 with Gensini score was analyzed by Pearson method.According to survival condition during hospitalization within 28d,they were divided into survival group(n=85)and death group(n=37).Cox regression model was used to analyze the influencing factors for death within 28d in AMI patients;receiver operating charac-teristic(ROC)curve was used to analyze the predictive efficacy of TRAF6 and HDAC3 for death within 28d in AMI patients.Results:Patients in high score group had significant higher serum levels of TRAF6[(6.01±2.39)μg/ml vs.(5.06±1.74)μg/ml,P=0.012]and HDAC3[(4.14±1.94)ng/ml vs.(2.87±1.37)ng/ml,P＜0.001]compared with low score group.Pearson correlation analysis indicated that serum levels of TRAF6 and HDAC3 were positively correlated with Gensini score(r=0.879,0.837,P＜0.001 all).Patients in death group had significant higher serum levels of TRAF6[(6.89±1.67)μg/ml vs.(4.81±1.24)μg/ml,P＜0.001]and HDAC3[(5.37±1.77)ng/ml vs.(2.52±0.76)ng/ml,P＜0.001]compared with those in survival group.Cox regression analysis indicated that Gensini score[HR=1.857,95％CI 1.259～2.737,P=0.001],TRAF6[HR=1.659,95％CI 1.083～2.543,P=0.022],HDAC3[HR=1.779,95％CI 1.192～2.653,P=0.004]were independent risk factors for death within 28d in AMI patients.ROC curve analysis indicated that AUC of TRAF6,HDAC3 and their combina-tion predicting death within 28d in AMI patients was 0.862,0.859 and 0.971 respectively,and AUC of combination was significantly higher than those of TRAF6 and HDAC3 alone(Z=2.535,2.032,P=0.011,0.042).Conclu-sion:Serum levels of TRAF6 and HDAC3 are closely related to severity of coronary artery disease and prognosis in AMI patients,and dual combination possesses good predictive value for prognosis in AMI patients.

Objective: To investigate the clinical features, treatment regime, and outcome of pediatric acute myeloid leukemia (AML) with DEK-NUP214 fusion gene. Methods: The clinical data, genetic and molecular results, treatment process and survival status of 7 cases of DEK-NUP214 fusion gene positive AML children admitted to the Pediatric Blood Diseases Center of Institute of Hematology & Blood Diseases Hospital, Chinese Academy of Medical Sciences from May 2015 to February 2022 were analyzed retrospectively. Results: DEK-NUP214 fusion gene positive AML accounted for 1.02% (7/683) of pediatric AML diagnosed in the same period, with 4 males and 3 females. The age of disease onset was 8.2 (7.5, 9.5) years. The blast percentage in bone marrow was 0.275 (0.225, 0.480), and 6 cases were M5 by FAB classification. Pathological hematopoiesis was observed in all cases except for one whose bone marrow morphology was unknown. Three cases carried FLT3-ITD mutations, 4 cases carried NRAS mutations, and 2 cases carried KRAS mutations. After diagnosis, 4 cases received IAE induction regimen (idarubicin, cytarabine and etoposide), 1 case received MAE induction regimen (mitoxantrone, cytarabine and etoposide), 1 case received DAH induction regimen (daunorubicin, cytarabine and homoharringtonine) and 1 case received DAE induction regimen (daunorubicin, cytarabine and etoposide). Complete remission was achieved in 3 cases after one course of induction. Four cases who did not achieved complete remission received CAG (aclarubicin, cytarabine and granulocyte colony-stimulating factor), IAH (idarubicin, cytarabine and homoharringtonine), CAG combined with cladribine, and HAG (homoharringtonine, cytarabine and granulocyte colony-stimulating factor) combined with cladribine reinduction therapy, respectively, all 4 cases reached complete remission. Six patients received hematopoietic stem cell transplantation (HSCT) after 1-2 sessions of intensive consolidation treatment, except that one case was lost to follow-up after complete remission. The time from diagnosis to HSCT was 143 (121, 174) days. Before HSCT, one case was positive for flow cytometry minimal residual disease and 3 cases were positive for DEK-NUP214 fusion gene. Three cases accepted haploid donors, 2 cases accepted unrelated cord blood donors, and 1 case accepted matched sibling donor. The follow-up time was 20.4 (12.9, 53.1) months, the overall survival and event free survival rates were all 100%. Conclusions: Pediatric AML with DEK-NUP214 fusion gene is a unique and rare subtype, often diagnosed in relatively older children. The disease is characterized with a low blast percentage in bone marrow, significant pathological hematopoiesis and a high mutation rate in FLT3-ITD and RAS genes. Low remission rate by chemotherapy only and very high recurrence rate indicate its high malignancy and poor prognosis. Early HSCT after the first complete remission can improve its prognosis.

Objective: To clarify the phenotypes of the newborns with SLC26A4 single-allele mutation in deafness genetic screening and second variant; to analyze the SLC26A4 genotype and hearing phenotype. Methods: 850 newborns born in Beijing from April 2015 to December 2019 were included and there were 468 males and 382 females. They received genetic deafness screening for 9 or 15 variants, with the result of SLC26A4 single-allele mutation. Firstly, three step deafness gene sequencing was adopted in this work, i.e., the first step was "SLC26A4 gene whole exons and splice sites" sequencing; the second step was "SLC26A4 gene promoter, FOXI1 gene and KCNJ10 gene whole exons" sequencing; and the third step was detection for "SLC26A4 gene copy number variation". Secondly, we collected the results of newborn hearing screening for all patients with the second mutation found in the three step test, and conducted audiological examinations, such as acoustic immittance, auditory brainstem response and auditory steady state response. Thirdly, for novel/VUS mutations, we searched the international deafness gene database or software, such as DVD, ClinVar and Mutation Taster, to predict the pathogenicity of mutations according to the ACMG guideline. Lastly, we analyzed the relationship between genotype and phenotype of newborns with SLC26A4 single allele mutation. Results: Among 850 cases, the median age of diagnosis was 4 months. In the first step, 850 cases were sequenced. A total of 32 cases (3.76%, 32/850) of a second variants were detected, including 18 cases (2.12%, 18/850) with identified pathogenic variants; 832 cases were sequenced and 8 cases of KCNJ10 gene missense variants were detected among the second step. No missense mutations in the FOXI1 gene and abnormal SLC26A4 gene promoter were detected; the third step sequencing results were all negative. Genotypes and hearing phenotypes included 18 cases combined with the second clear pathogenic variant, 16 cases (16/18) referred newborn hearing screening and 2 cases (2/18) passed in both ears; degree of hearing loss consisted of 18 profound ears (18/36), 13 severe ears (13/36) and 5 moderate ears (5/36); audiogram patterns comprised 17 high frequency drop ears (17/36), 14 flat ears (14/36), 3 undistinguished ears (3/36), and 2 U shaped ears (2/36); 11 cases underwent imaging examination, all of which were bilateral enlarged vestibular aqueduct. As for 22 cases of other genotypes, all passed neonatal hearing screening and the hearing diagnosis was normal, including 9 cases with VUS or possibly novel benign variants, 8 cases with KCNJ10 double gene heterozygous variants, and 5 cases with double heterozygous variants. Conclusions: The probability of individuals with SLC26A4 single-allele variant who merge with a second pathogenic variant is 2.12%, all of which are SNV, which can provide scientific basis for the genetic diagnosis and genetic counseling of SLC26A4 variants. Those who have merged with second pathogenic variant are all diagnosed with sensorineural hearing loss. Patients with KCNJ10 gene mutations do not manifest hearing loss during the infancy, suggesting the need for further follow-up.

ObjectiveTo investigate the mechanism of Sishencong electroacupuncture on preventing the apoptosis of hippocampal neurons in rats with acute sleep deprivation and its effect on anxiety behavior. MethodsAdult Sprague-Dawley （SD） rats were randomly divided into Sham group （control group）， SD group （sleep deprivation group）， and EA+SD group （eletroacupuncture group）. Modified multiple platform method was used to induce sleep deprivation for 4 days. EA+SD group received Sishencong electroacupuncture in each day during sleep deprivation with 20 minutes needling for 4 days. The open field test was used to detect the exploration of rats to evaluate the degree of anxiety， and the co-labeling of NeuN⁺ and Cleaved Caspase-3 in different regions of the hippocampus was detected by immunofluorescence staining； the astrocyte marker glial fibrillary acidic protein （GFAP） in the hippocampus was labeled by immunofluorescence. GFAP and the morphological changes of astrocytes were analyzed by Sholl analysis， and the co-labeling with C3+ and S100A+ were detected to analyze the A1 and A2 types of astrocytes in the hippocampus. ResultsOpen field test showed that the time spent in the center of rats was increased in EA+SD group， but the movement speed was decreased compared with SD group. Immunofluorescence staining confirmed that the expression of Cleaved caspase-3 on neurons in different regions of the hippocampus （CA1， CA2， CA3， DG1， DG2） in the EA+SD group was lower than that of the SD group （P<0.05）. The number of intersections of astrocytes and concentric circles in the EA+SD group at 12 μm，15 μm， 18 μm and 21 μm were higher than those of the SD group. Immunofluorescence showed that electroacupuncture could regulate the proportion of C3⁺/GFAP⁺ and S100A⁺/GFAP⁺ in the hippocampus of rats during sleep deprivation. ConclusionElectroacupuncture could improve the exploration during sleep deprivation by reducing the apoptosis of neurons in the hippocampus， repairing the morphological damage of astrocytes and balancing the proportion of A1 and A2 astrocytes in the hippocampus.
