1.Role of GLUT1-dependent glycolysis in attenuation of oxygen-glucose deprivation-reoxygenation injury by dexmedetomidine in HK-2 cells

Aim To evaluate the role of the glucose transporter protein 1(GLUT1)-dependent glycolytic in the attenuation of oxygen-glucose deprivation-reoxygen-ation(OGD/R)injury in HK-2 cells by dexmedetomi-dine(Dex).Methods C57/BL6 mice were random-ly divided into three groups(n=6),namely,sham operation group(Sham group),renal ischemia reper-fusion group(I/R group)and Dex group(I/R+Dex group).Serum creatinine(Cr)and urea nitrogen(BUN)were measured,while the levels of key glyco-lytic enzymes HK2,PFKFB3 and GLUT1 were meas-ured.HK-2 cells were cultured and randomised into seven groups(n=6),which was treated with OGD/R,overexpression or interference with GLUT1,Dex and glycolysis inhibitor 2-DG.CCK-8 and LDH activi-ty were used to detect cellular damage.Glycolysis lev-els were detected by lactate and ECAR.The inflamma-tory level was reflected by qRT-PCR for IL-6 and TNF-α.qRT-PCR and Western blot were performed to de-tect the levels of GLUT1,HK2,and PFKFB3.Results Dex significantly ameliorated kidney injury and HK-2 cell injury(P<0.05).Dex inhibited the OGD/R-induced rise in lactate and extracellular acidification rate(ECAR),as evidenced by suppression of the ex-pression of GLUT1,HK2 and PFKFB3(P<0.05).In vitro experiments showed that GLUT1 knockdown sig-nificantly improved OGD/R-induced cellular damage.Lactate,ECAR,glycolysis-related mRNAs and pro-teins were inhibited by GLUT1 knockdown(P<0.05).Significantly,there were no significant differ-ences in above indexes after Dex treatment based on GLUT1 knockdown.Overexpression of GLUT1 abroga-ted the protective effects of Dex,while reversing the inhibitory effects of Dex on the expression of GLUT1,HK2,and PFKFB3(P<0.05).Conclusions Dexmedetomidine attenuates OGD/R induced injury in HK-2 cells by inhibiting GLUT1-dependent glycolysis.
2.Correlation between serum CA72-4 level and pathological differentiation degree of gastric cancer and its clinical significance

Objective:To explore the correlation between serum carbohydrate antigen 72-4 (CA72-4) level and pathological differentiation degree of gastric cancer, and to provide reference for early diagnosis, prognosis evaluation and efficacy monitoring of gastric cancer.Methods:A retrospective study was conducted on 136 patients with gastric cancer admitted to Zhejiang Provincial People′s Hospital from April 2022 to December 2024. According to postoperative pathological diagnosis, patients were divided into well-differentiated group ( n=9), moderately differentiated group ( n=36) and poorly/undifferentiated group ( n=91). Preoperative CA72-4 levels and clinical characteristics of patients were collected to analyze the relationship between differentiation degree and CA72-4 level, and to compare the changes of CA72-4 before and after surgery in gastric cancer patients with different differentiation degrees. Results:The preoperative CA72-4 level in the poorly/undifferentiated group was significantly higher than that in the well-differentiated group and moderately differentiated group (all P<0.001). Preoperative CA72-4 level in gastric cancer patients was weakly negatively correlated with differentiation degree ( R=-0.341). After surgery, the CA72-4 level in the poorly/undifferentiated group decreased significantly compared with that before surgery ( P=0.043), while the changes in the well-differentiated group and moderately differentiated group were not significant. Conclusions:There are significant differences in CA72-4 levels among gastric cancer groups with different differentiation degrees. Preoperative CA72-4, as a tumor marker, is of great significance for early diagnosis, prognosis evaluation and postoperative monitoring of gastric cancer. Especially in patients with poorly/undifferentiated gastric cancer, the CA72-4 level is relatively high, which can be used for auxiliary diagnosis and formulation of treatment plans.
3.Pharmacokinetics and tissue distribution of fluorescent-labeled Astragalus polysaccharides in mice.

In this study, the reductive amination method was used to label IR783 on Astragalus polysaccharides(APS) for the first time, which was verified by ultraviolet-visible spectroscopy and infrared spectroscopy. Quantitative analysis methods of APS-IR783 in plasma and various tissue were established using a multifunctional microplate reader. The pharmacokinetics and tissue distribution of APS-IR783 in mice were investigated after a single intravenous injection of 30 mg·kg~(-1) APS-IR783, and pharmacokinetic parameters were calculated using DAS 2.0 software. The results showed that the APS used had a mass fraction of 93.69%, a relative molecular weight of 1.55×10~5, and a polydispersity index(PDI, M_w/M_n) of 1.73, close to a homogeneous polysaccharide. The IR783 labeling yield reached 86.50%, and the content of IR783 in APS-IR783 was 0.72%. After a single intravenous injection of 30 mg·kg~(-1), the pharmacokinetic parameters of APS in mouse plasma were as follows: T_(max) was(0.67±0.26) h; C_(max) was(1 599.29±159.30) mg·L~(-1); T_(1/2α) and T_(1/2β) were(2.29±3.06) h and(0.44±0.05) h, respectively; AUC_(0-t) was(23 398.91±2 907.03) mg·h·L~(-1); AUC_(0-∞) was(27 710.55±3 506.55) mg·h·L~(-1); MRT_(0-∞) was(34.38±12.59) h; CL was 0.001 L·h~(-1)·kg~(-1); V_z was(0.042±0.017) L·kg~(-1). The in vivo biodistribution study demonstrated that the in vivo exposure ratios of APS in different tissue were in the following order: spleen > liver > kidney > lung > heart > small intestine > muscle > large intestine > brain > stomach, where the top five tissue accounted for 87.54% of the total area under the curve(AUC). This study successfully labeled APS with a water-soluble near-infrared fluorescent probe of IR783 for the first time and revealed the pharmacokinetics and tissue distribution of APS in mice. The paper provides detailed in vivo behavior of APS after intravenous injection, which lays the foundation for the development and utilization of APS and related natural medicines.

Abstract
In recent years, the prevalence of overweight and obesity among children and adolescents has risen rapidly, posing a serious threat to their physical and mental health. To provide scientific, systematic, and standardized weight management guidance for overweight and obese children and adolescents, the study focuses on the core concept of healthy lifestyle intervention, integrates multidisciplinary expert opinions and research findings,and proposes a comprehensive multidisciplinary intervention framework covering scientific exercise intervention, precise nutrition and diet, optimized sleep management, and standardized psychological support. It calls for the establishment of a multi agent collaborative management mechanism led by the government, implemented by families, fostered by schools, initiated by individuals, optimized by communities, reinforced by healthcare, and coordinated by multiple stakeholders. Emphasizing a child and adolescent centered approach, the consensus advocates for comprehensive, multi level, and personalized guidance strategies to promote the internalization and maintenance of a healthy lifestyle. It serves as a reference and provides recommendations for the effective prevention and control of overweight and obesity, and enhancing the health level of children and adolescents.
5.Practice and reflection of reception and rescue team aboard simulated hospital ships

Based on medical rescue operations aboard simulated hospital ships,this paper explores the medical care practice mode employed during maritime rescue missions,points out the deficiencies in current medical practices during rescue operations,and puts forward countermeasures to improve the practice based on past casualty reception and rescue training exercise.With a comprehensive analysis of casualty reception team practices on simulated hospital ships,this paper aims to provide insights for the continuous optimization of maritime medical care.
6.Predictive value of myocardial enzyme changes for the risk of late microvascular obstruction in patients with acute myocardial infarction after percutaneous coronary intervention

Objective:To analyze the predictive value of dynamic changes of myocardial enzymes for late microvascu-lar obstruction(MVO)in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods:The clinical data of 123 AMI patients who underwent PCI in Peking University Third Hospital Qinhuangdao Hospital between June 2020 and December 2022 were retrospectively analyzed.Patients were divided into late MVO group(n=52)and non-late MVO group(n=71)according to the occurrence of late MVO 4-7 d after PCI.The myocardial enzyme spectrum indexes of the two groups before and 1 d after operation were collect-ed.Myocardial enzyme spectrum indexes were compared between the two groups.Multivariate Logistic regression a-nalysis was used to analyze the related factors affecting late MVO in AMI patients after PCI.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive efficacy of myocardial enzyme changes for late MVO in AMI patients after PCI.Results:At 1 d after operation,compared to patients in non-late MVO group,those in the late MVO group had significant higher lactate dehydrogenase(LDH)[(332.12±34.28)IU/L vs.(308.66±30.9)IU/L],aspartate aminotransferase(AST)[(64.89±6.11)IU/L vs.(58.84±4.96)IU/L],crea-tine kinase(CK)[(240.60±32.51)IU/L vs.(214.69±30.27)IU/L],creatine kinase isoenzyme(CK-MB)[(35.01±4.73)IU/L vs.(30.27±4.60)IU/L]and α-hydroxybutyrate dehydrogenase(α-HBDH)[(55.24±4.20)U/L vs.(50.64±3.87)U/L](P<0.001 all).Multivariate Logistic regression analysis showed that LDH(OR 1.034,95%CI 1.013~1.054),AST(OR 1.173,95%CI 1.052~1.308),CK(OR 1.038,95%CI 1.014~1.062),CK-MB(OR 1.310,95%CI 1.139~1.508)and α-HBDH(OR 1.379,95%CI 1.167~1.630)at 1 d af-ter operation were independent risk factors of late MVO in AMI patients after PCI(P<0.01 all).ROC analysis showed that the area under the curve(AUC)of LDH,AST,CK,CK-MB,α-HBDH alone and combined detec-tion in predicting late MVO in AMI patients after PCI were 0.705(95%CI 0.609~0.801),0.793(95%CI 0.707~0.879),0.712(95%CI 0.621~0.804),0.767(95%CI 0.683~0.851),0.798(95%CI 0.718~0.878)and 0.950(95%CI 0.914~0.986)respectively.The AUC of combined prediction was better than those of single predic-tions(Z=2.807~4.882,P<0.05 or<0.01).Conclusion:Changes in myocardial enzyme spectrum may increase the risk of late MVO in AMI patients after PCI and combined detection have good predictive efficacy for it.

9.Clinical efficacy of microscopic varicocelectomy versus laparoscopic varicocelectomy in the treatment of varicocele with male infertility

Objective:To compare the clinical efficacy between microscopic varicocelectomy and laparoscopic varicocelectomy in the treatment of varicocele(VC)with male infertility.Methods:A total of 307 patients who were diagnosed with VC complicated with male infertility and admitted to the Affiliated Hospital of Qingdao University from October 2018 to October 2022 were recruited for retrospective analysis.The patients were divided into the microscopic group(180 cases)and laparoscopic group(127 cases)according to the surgery method.The pre-and postoperative clinical data of these two groups were analyzed,including the degree of dilatation and reflux time of internal spermatic vein,hemodynamic parameters of testicular capsular artery,proportion of progressive motility spermato-zoa(PR),concentration of spermatozoa,proportion of normal morphology sperm,the pregnancy outcome of spouses and the incidence of complications related with surgery within 2 years postoperatively.Results:All the surgeries for the 307 patients in this study were successful.There was no significant difference in operation time,hospitalization time and management expenses between the microscop-ic group and the laparoscopic group(P>0.05).Compared to the patients in laparoscopic group,the patients in the microscopic group received a better improvement in venous diameter,reflux time of spermatic veins and hemodynamic parameters of testicular capsular ar-tery(P<0.05).Moreover,the semen analysis showed that the PR,spermatozoa concentration and proportion of normal morphology sperm in the microscopic group were also obviously increased than those in the laparoscopic group(P<0.05).During the 2-year fol-low-up period,the conception rate of spouses in the microscopic group was 67.2%,while only 47.2%in the laparoscopic group,in which the difference was statistically significant(P<0.05).Besides,the time-to-pregnancy(TTP)within 2 years postoperatively in the microscopic group was significantly shorter than that in the laparoscopic group(P<0.05).Meanwhile,the incidence of adverse pregnancy outcomes in the microscopic group was also significantly lower than that in the laparoscopic group(P<0.05).It is worth mentioned that the spontaneous conception rate of spouses with successful pregnancy in the microscopic group was also significantly higher than that in the laparoscopic group(P<0.05).Severe complication such as testicular atrophy,bleeding and infection did not appear in both of two groups.However,the incidences of testicular hydrocele and recurrence of VC postoperatively in the laparoscopic group were significantly higher than those in the microscopic group(P<0.05).Conclusion:Both microscopic varicocelectomy and laparoscopic varicocelectomy can be applied to the management of VC combined with male infertility.But microscopic varicocelectomy showed better clinical efficacy in improving the testicular hemodynamic parameters,semen quality,pregnancy outcome and postopera-tive complications,which is worthy of further clinical applications.
