Objective To study the risk factors and strain resistance of Escherichia coli with extended-spectrum β-lactamases(ESBL)-producing bloodstream infection,so as to provide clinical basis for rational use of antibiotics and effective prevention and control of bloodstream infection. Methods The clinical data of patients with bloodstream infections caused by Escherichia coli in a tertiary hospital in Chongqing from January 2018 to December 2022 were retrospectively collected. The clinical characteristics and drug resistance of bloodstream infections caused by Escherichia coli were statistically analysed.According to the ESBL confirmation test of Escherichia coli strains, the patients were divided into the ESBL-producing group and the non-ESBL-producing group. The chi-square test was used to compare the differences in influencing factors between the two groups, and then the independent influencing factors of ESBL production were analyzed through multivariate Logistic regression. Results A total of 408 patients were included. The detection rate of ESBL-producing strains was 60.3%(246/408),and the detection rates in the nephrology department and the intensive care unit were relatively high(both>76.0%).Diabetes[OR=1.98,95%CI(1.24,3.17)]and urinary tract intubation[OR=1.60,95%CI(1.02,2.51)]were independent influencing factors for bloodstream infection with ESBL-producing Escherichia coli. The resistance rate of ESBL-producing Escherichia coli to levofloxacin and ceftriaxone was>90.0%.Moreover,the resistance rates of the second-generation cephalosporins(except ceftazidime),compound sulfamethoxazole, ciprofloxacin and amtronam were significantly higher than those in the non-ESBL-producing group(P<0.05).Both groups of strains showed high sensitivity to amikacin and carbapenem drugs. Conclusion The severe current situation of bloodstream infections caused by ESBL-producing Escherichia coli in this region showing a high prevalence of drug resistance characteristics.Diabetes and urinary tract intubation,as independent risk factors, suggest that key monitoring should be implemented for such high-risk populations in clinical practice.Given that ESBL-producing strains remain sensitive to carbapenems and amikacin,it can be recommended as the first empirical medication. It is of great public health significance to achieve the effect of curbing the spread of such multi-drug resistant bacteria by establishing an early warning system based on risk factor assessment and standardized management of invasive operations.

Microglial pyroptosis and neuroinflammation have been implicated in the pathogenesis of sepsis-associated encephalopathy (SAE). OGT-mediated O-GlcNAcylation is involved in neurodevelopment and injury. However, its regulatory function in microglial pyroptosis and involvement in SAE remains unclear. In this study, we demonstrated that OGT deficiency augmented microglial pyroptosis and exacerbated secondary neuronal injury. Furthermore, OGT inhibition impaired cognitive function in healthy mice and accelerated the progression in SAE mice. Mechanistically, OGT-mediated O-GlcNAcylation of ATF2 at Ser44 inhibited its phosphorylation and nuclear translocation, thereby amplifying NLRP3 inflammasome activation and promoting inflammatory cytokine production in microglia in response to LPS/Nigericin stimulation. In conclusion, this study uncovers the critical role of OGT-mediated O-GlcNAcylation in modulating microglial activity through the regulation of ATF2 and thus protects against SAE progression.

Neuropathic pain, a debilitating condition caused by dysfunction of the somatosensory nervous system, remains difficult to treat due to limited understanding of its molecular mechanisms. Bioinformatics analysis identified cerebellin 2 (CBLN2) as highly enriched in human and murine proprioceptive and nociceptive neurons. We found that CBLN2 expression is persistently upregulated in dorsal root ganglia (DRG) following spinal nerve ligation (SNL) in mice. In addition, transcription factor SOX11 binds to 12 cis-regulatory elements within the Cbln2 promoter to enhance its transcription. SNL also induced SOX11 upregulation, with SOX11 and CBLN2 co-localized in nociceptive neurons. The siRNA-mediated knockdown of Sox11 or Cbln2 attenuated SNL-induced mechanical allodynia and thermal hyperalgesia. High-throughput sequencing of DRG following intrathecal injection of CBLN2 revealed widespread gene expression changes, including upregulation of numerous NF-κB downstream targets. Consistently, CBLN2 activated NF-κB signaling, and inhibition with pyrrolidine dithiocarbamate reduced CBLN2-induced pain hypersensitivity, proinflammatory cytokines and chemokines production, and neuronal hyperexcitability. Together, these findings identified the SOX11/CBLN2/NF-κB axis as a critical mediator of neuropathic pain and a promising target for therapeutic intervention.

Pyroptosis is an identified programmed cell death that has been highly linked to endoplasmic reticulum (ER) dynamics. However, the crucial proteins for modulating dynamic ER membrane curvature change that trigger pyroptosis are currently not well understood. In this study, a biotin-labeled chemical probe of potent pyroptosis inducer α-mangostin (α-MG) was synthesized. Through protein microarray analysis, reticulon-4 (RTN4/Nogo), a crucial regulator of ER membrane curvature, was identified as a target of α-MG. We observed that chemically induced proteasome degradation of RTN4 by α-MG through recruiting E3 ligase UBR5 significantly enhances the pyroptosis phenotype in cancer cells. Interestingly, the downregulation of RTN4 expression significantly facilitated a dynamic remodeling of ER membrane curvature through a transition from tubules to sheets, consequently leading to rapid fusion of the ER with the cell plasma membrane. In particular, the ER-to-plasma membrane fusion process is supported by the observed translocation of several crucial ER markers to the "bubble" structures of pyroptotic cells. Furthermore, α-MG-induced RTN4 knockdown leads to pyruvate kinase M2 (PKM2)-dependent conventional caspase-3/gasdermin E (GSDME) cleavages for pyroptosis progression. In vivo, we observed that chemical or genetic RTN4 knockdown significantly inhibited cancer cells growth, which further exhibited an antitumor immune response with anti-programmed death-1 (anti-PD-1). In translational research, RTN4 high expression was closely correlated with the tumor metastasis and death of patients. Taken together, RTN4 plays a fundamental role in inducing pyroptosis through the modulation of ER membrane curvature remodeling, thus representing a prospective druggable target for anticancer immunotherapy.

Progress in Biochemistry and Biophysics 2025;52(10):2663-2676
ObjectiveFat infiltration has been shown to be closely related to muscle mass loss and a variety of muscle diseases. This study proposes a method based on phase-angle electrical impedance tomography (ΦEIT) to visualize the electrical characteristic response caused by muscle fat infiltration, aiming to provide a new technical means for early non-invasive detection of muscle mass deterioration. MethodsThis study was divided into two parts. First, a laboratory pork model was constructed to simulate different degrees of fat infiltration by injecting1 ml or 2 ml of emulsified fat solution into different muscle compartments, and the phase angle images were reconstructed using ΦEIT. Second, a human experiment was conducted to recruit healthy subjects (n=8) from two age groups (20-25 years old and 26-30 years old). The fat content percentage ηfat of the left and right legs was measured by bioelectrical impedance analysis (BIA), and the phase angle images of the left and right calves were reconstructed using ΦEIT. The relationship between the global average phase angle ΦM and the spatial average phase angle ΦMi of each muscle compartment and fat infiltration was further analyzed. ResultsIn the laboratory pork model, the grayscale value of the image increased with the increase of ηfat and ΦM showed a downward trend. The results of human experiments showed that at the same fat content percentage, the ΦM of the 26-30-year-old group was about 20%-35% lower than that of the 20-25-year-old group. The fat content percentage was significantly negatively correlated with ΦM. In addition, the M2 (soleus) compartment was most sensitive to fat infiltration, and the spatial average phase angles of the M2 (soleus), M3 (tibialis posterior and flexor digitorum longus), and M4 (tibialis anterior, extensor digitorum longus, and peroneus longus) compartments all showed significant inter-group differences. ConclusionΦEIT imaging can effectively distinguish different degrees of fat infiltration, especially in deep, small or specially located muscles, showing high sensitivity, demonstrating the potential application of this method in local muscle mass monitoring and early non-invasive diagnosis.

OBJECTIVES:
This study aimed to investigate the impact of foam macrophages (FMs) on the intracellular survival of Mycobacterium tuberculosis (MTB) and identify the molecular mechanisms influencing MTB survival.
METHODS:
An in vitro FM model was established using oleic acid induction. Transcriptomic and metabolomic analyses were conducted to identify the key molecular pathways involved in FM-mediated MTB survival.
RESULTS:
Induced FMs effectively restricted MTB survival. Transcriptomic and metabolomic profiling revealed distinct changes in gene and metabolite expression in FMs during MTB infection compared with normal macrophages. Integrated analyses identified significant alterations in the cyclic adenosine monophosphate (cAMP) signaling pathway, indicating that its activation contributes to the FM-mediated restriction of MTB survival.
CONCLUSIONS
FMs inhibit MTB survival. The cAMP signaling pathway is a key contributor. These findings enhance the understanding of the role of FMs in tuberculosis progression, suggest potential targets for host-directed therapies, and offer new directions for developing diagnostic and therapeutic strategies against tuberculosis.

OBJECTIVE To explore the effects of meropenem exposure and degradation levels on clinical efficacy in patients with purulent meningitis (PM). METHODS A total of 131 PM patients treated with meropenem at the Affiliated Suzhou Hospital of Nanjing Medical University from January 2022 to June 2025 were prospectively included. Relevant data were collected and divided into a cured group (91 cases) and a non-cured group (40 cases) based on the efficacy. High-performance liquid chromatography-tandem mass spectrometry was used to determine the concentration of meropenem and its open-loop metabolites. Risk factors that affect efficacy were screened, and their predictive power and correlation were evaluated by univariate analysis, and multivariate Logistic regression analysis, receiver operating characteristic (ROC) curves, and correlation analysis. RESULTS Univariate analysis showed that serum creatinine, creatinine clearance rate, minimum inhibitory concentration of meropenem ≥16 μg/mL, cerebrospinal fluid red blood cell count, cerebrospinal fluid white blood cell count, cerebrospinal fluid glucose content, blood trough concentration, blood open-loop metabolite concentration/trough concentration ratio, and intrathecal injection were all correlated with efficacy (P<0.05). The results of multiple Logistic regression analysis showed that serum creatinine blood open-loop metabolite concentration/trough concentration ratio, intrathecal injection, and cerebrospinal fluid glucose content were influencing factors for suboptimal anti-infective ltt efficacy (P<0.05). ROC curve analysis showed that when the blood open-loop metabolite concentration/trough concentration ratio was greater than 2.854 (AUC=0.647), serum creatinine was less than 59.5 μmol/L (AUC=0.647), and cerebrospinal fluid glucose content was less than 3.37 mmol/L (AUC=0.709), the risk of treatment failure significantly increased (P<0.05). Correlation analysis showed that the blood trough concentration of meropenem was positively correlated with the concentration of its open-loop metabolites (R 2=0.134 5, P<0.000 1). CONCLUSIONS Insufficient exposure level and rapid degradation of meropenem are key mechanisms affecting the anti-infective efficacy of PM. Elevated blood open-loop metabolite concentration/ trough concentration ratio, low serum creatinine level, lack of intrathecal injection, and low cerebrospinal fluid glucose content are independent risk factors for poor efficacy.
