1.Clinical Applications of Circulating Tumor DNA in Response Evaluation and Relapse Monitoring of Primary Mediastinal Large B-Cell Lymphoma.

OBJECTIVE:
To explore the clinical significance of circulating tumor DNA (ctDNA) in response evaluation and relapse monitoring for patients with primary mediastinal large B-cell lymphoma (PMBCL).
METHODS:
The clinical characteristics, efficacy and survival of 38 PMBCL patients in our hospital from January 2010 to April 2020 were retrospectively analyzed. The ctDNA monitoring was conducted by targeted next-generation sequencing (NGS).
RESULTS:
Among the 38 patients, 26 cases were female, and 32 cases were diagnosed with Ann Arbor stage I-II. The 5-year overall survival (OS) rate and progression-free survival (PFS) rate were 74.7% and 61.7%, respectively. Males and those with high aaIPI scores (3 points) had a relatively poor prognosis. The NGS results of 23 patients showed that STAT6 (65.2%), SOCS1 (56.5%), and TNFAIP3 (56.5%) were the most common mutated genes. Patients with stable disease (SD)/progressive disease (PD) exhibited enrichment in cell cycle, FoxO, and TNF signaling pathways. A total of 29 patients underwent end-of-treatment PET/CT (EOT PET/CT), and 16 of them received ctDNA monitoring with 12 negative. Among 6 patients with EOT PET/CT positive (Deauville 4), 4 underwent ctDNA monitoring, and 3 of them were negative, being still in continuous remission without any subsequent anti-tumor therapy.
CONCLUSION
CtDNA may be combined with PET/CT to assess efficacy, monitor relapse, and guide treatment of PMBCL.

Receptor Protein Tyrosine Phosphatases(RPTPs)are a class of membrane proteins that play a crucial role in cellular signal transduction,primarily responsible for regulating the level of protein tyrosine phosphorylation within cells.They participate in various biological processes such as cell proliferation,differentiation,migration,and apoptosis.As a key group of signal transduction regulators,RPTPs are involved in multiple signaling pathways in the nervous system,such as regulating neuronal development,dendritic and axonal growth,and synapse formation.These functions are crucial for the normal development and maintenance of the nervous system.Studies have shown that RPTPs are involved in the occurrence and development of various neuropsychiatric disorders,such as bipolar disorder,autism,schizophrenia,and restless leg syndrome,and are considered potential therapeutic targets.Several studies are exploring how to regulate their expression and function.Here we review the studies of RPTPs in the nervous system,including their roles in axon guidance,synapse formation,and neuropsychiatric disorders,which helps to understand the functions of RPTPs and provides a basis for finding new therapeutic targets for neuropsychiatric diseases.
3.Feasibility of modified LIFT guided by magnetic resonance imaging in the treatment of deep anorectal abscess

Objective To evaluate the efficacy and safety of magnetic resonance-guided modified ligation of the intersphincteric fistula tract in the treatment of deep anorectal abscess.Methods A retrospective study was conducted on 148 patients with deep anorectal abscess treated at our hospital from September 2019 to June 2023,who met the inclusion criteria.Of these,51 patients in the observation group underwent modified LIFT,while 97 patients in the control group received incision and drainage.Using propensity score matching,47 patients from each group with balanced baseline characteristics were selected,resulting in a total of 94 patients being included in the analysis.The study compared operation time,hospital stay,pain index,postoperative complications,anal function,and healing rate between the two groups.Results Primary lesions originating in the sphincter space were identified through preoperative magnetic resonance imaging in all 94 patients.The mean operation time for the observation group was(43.75±11.77)minutes,significantly longer than that of the control group at(28.90±8.67)minutes(P<0.05).At three months post-surgery,the healing rate in the observation group was 80.9%,which was signifi-cantly higher than the 57.4%observed in the control group(P<0.05).Among patients with fistulas in the observation group,the healing rate was 73.1%,although this difference was not statistically significant in the univariate analysis of clinical characteristics(P>0.05).No significant differences were found between the two groups regarding the length of hospital stay,pain index,postoperative complications,or anal function(all P>0.05).Conclusions Modified LIFT guided by MRI can be conditionally applied in the treatment of deep anorectal abscesses.This approach can enhance the initial surgical healing rate without significantly increasing the incidence of postoperative complications,thereby avoiding potential damage to anal function.

8.Analysis of Therapeutic Efficacy and Adverse Prognostic Factors of Secondary Central Nervous System Lymphoma

Objective:To explore the therapeutic efficacy and prognostic factors of induction therapy for secondary central nervous system lymphoma(SCNSL).Methods:Clinical data of patients diagnosed with SCNSL from 2010 to 2021 at Guangdong Provincial People's Hospital were retrospectively collected.A retrospective cohort study was performed on all and grouped patients to analyze the efficacy and survival.Multivariate logistic regression analysis was used to explore the adverse prognostic factors.Results:Thirty-seven diffuse large B-cell lymphoma patients with secondary central involvement were included in the research.Their 2-year overall survival(OS)rate was 46.01%and median survival time was 18.1 months.The 2-year OS rates of HD-MTX group and TMZ group were 34.3%and 61%,median survival time were 8.7 and 38.3 months,and median progression-free survival time were 8.1 and 47 months,respectively.Multivariate logistic regression analysis showed that age,sex,IPI,Ann Arbor stage were correlated with patient survival time.The median survival time of patients with CD79B,KMT2D,CXCR4.ERBB2,TBL1XR1,BTG2,MYC,MYD88,and PIM1 mutations was 8.2 months,which was lower than the overall level.Conclusion:HD-MTX combined with TMZ as the first-line strategy may improve patient prognosis,and early application of gene sequencing is beneficial for evaluating prognosis.

China prospective multicenter birth cohort (Prospective Omics Health Atlas birth cohort, POHA birth cohort) study was officially launched in 2022. This study, in collaboration with 12 participating units, aims to establish a high-quality, multidimensional cohort comprising 20 000 naturally conceived families and assisted reproductive families. The study involves long-term follow-up of parents and offspring, with corresponding biological samples collected at key time points. Through multi-omics testing and analysis, the study aims to conduct multi-omics big data research across the entire maternal and infant life cycle. The goal is to identify new biomarkers for maternal and infant diseases and provide scientific evidence for risk prediction related to maternal diseases and neonatal health.
10.Application research of R language-based autoregressive integrated moving average model for predicting short-term consumption of medical consumables

Objective To explore the effect of a R language-based autoregressive integrated moving average(ARIMA)model for predicting the consumption of medical consumables.Methods The monthly consumption data of a certain type of pre-filled flush syringe from July 2018 to June 2023 was selected as the sample data,which underwent smoothness test and difference operation with R language.An ARIMA model was established and the optimal model was determined according to the Akaike and Bayesian information criteria.The corresponding data of the third quarter of 2023 was used as the validation set to predict the consumption,and the prediction result was compared with the actual values to evaluate the prediction effect of the ARIMA model.Results The ARIMA model with the best fitting was ARIMA(0,1,1)(1,0,0)12,all the predicted data were within 95%confidence interval,and its mean absolute percentage error MAPE was 9.92%.P-value proved to be higher than 0.05 when the residual series were tested using the Ljung-Box statistics,which meant the prediction result was satisfactory.Conclusion The R language-based ARIMA model behaves well in predicting the consumption of medical consumables,and provides references for demand planning,budgeting,purchasing and management of medical consumables.[Chinese Medical Equipment Journal,2024,45(10):84-87]
