Objective To explore the correlation between fasting blood glucose(FBG)level and fractional flow reserve(FFR)in patients with borderline coronary artery disease, and to clarify its potential influence on FFR measurement. Methods From August 2020 to August 2023,the data of 135 patients with coronary atherosclerotic heart disease who received coronary angiography and FFR evaluation in the Fourth Affiliated Hospital of Harbin Medical University were retrospectively collected.According to the exclusion and inclusion criteria,85 cases of borderline diseased vessels of single coronary artery with stenosis degree of 50%-80%were screened out,and they were divided into FBG≥6.1 mmol/L group(47 cases)and FBG<6.1 mmol/L group(38 cases). The baseline data, angiographic and functional indexes of the two groups were compared, and the correlation between FBG and FFR was analyzed. Results Compared with the FBG<6.1 mmol/L group, the FBG≥6.1 mmol/L group had a higher proportion of FFR negative results(72.3%vs.23.7%,P<0.001),and the FFR measurement values were generally increased[0.84(0.80,0.90)vs.0.75(0.68,0.80),P<0.001],with statistically significant differences.Pearson correlation analysis was performed on all lesions, and FFR>0.80(negative result)was positively correlated with FBG≥6.1 mmol/L(r=0.484,P<0.001).Conclusions Among the patients with borderline coronary artery disease(50%-80%stenosis)included in this study,FBG≥6.1 mmol/L is significantly correlated with FFR>0.80.For patients with borderline coronary lesions with elevated FBG,the influence of blood glucose factors should be carefully considered in clinical interpretation of FFR results.

Objective To investigate the clinical characteristics of preoperative intestinal symptoms in patients with bowel endometriosis and to compare the effects of shaving versus segmental bowel resection on postoperative intestinal function. Methods A total of 105 patients diagnosed with bowel endometriosis and treated by the same surgical team at the Obstetrics and Gynecology Hospital,Fudan University between Aug 1,2013 and Dec 30,2017 were prospectively enrolled in this study.Clinical data were collected via outpatient visits and telephone follow-ups at four time points:preoperative(T0)and postoperative(T1:Nov 2018;T2:Nov 2020;T3:Apr 2024).The primary outcome was bowel symptoms and gastrointestinal function scores;secondary outcome was pain scores.A generalized estimating equation(GEE)model was used to analyze the interaction effect of surgical approach and follow-up time on outcomes. Results Ultimately,a total of 89 patients were included(15.24%loss to follow-up),among whom 79 patients(88.76%)underwent shaving excision.Preoperatively,46 patients(51.68%)presented with bowel symptoms, primarily anus bulge(21 cases,46.65%)and diarrhea(15 cases,32.61%)during menstruation.Postoperatively, there was a significant increase in constipation rates(T1:71.43%;T2:50.00%;T3:72.00%).Both surgical groups exhibited significant improvements in dysmenorrhea, gastrointestinal discomfort scores as well as gastrointestinal quality of life index(P<0.000 5).However,the segmental resection group had significantly higher scores for low anterior resection syndrome, constipation compared with the shaving excision group(P=0.02 and P=0.05).Conclusion Approximately half of the patients with bowel endometriosis exhibit typical bowel symptoms preoperatively, such as anus bulge and diarrhea. Both shaving excision and segmental resection effectively alleviate pain;however, shaving excision demonstrates an advantage regarding preservation of bowel function, whereas segmental resection may elevate risks associated with postoperative constipation or altered defecation patterns due to structural changes. The selection of surgical approach should carefully balance lesion removal and functional preservation, moreover,be sure that potential risks are thoroughly informed to patients prior to surgery.

Aim To investigate the regulatory effects and underlying mechanisms of 4-methoxybenzyl alcohol(4-MBA),an active ingredient of Gastrodia elata,on hepatic cholesterol metabolism. Methods Acute hy-perlipidemia mouse models were established via egg yolk emulsion induction, and hyperlipidemia rat models were constructed using a high-fat diet.Serum and he-patic total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),and high-den-sity lipoprotein cholesterol(HDL-C)levels were quan-tified via enzymatic assays.Hepatic histopathological changes were evaluated through hematoxylin-eosin(HE)and Oil Red O staining.Interactions between 4-MBA and key cholesterol metabolism targets were sim-ulated using molecular docking.mRNA and protein ex-pression levels of LDL receptor(LDLR),proprotein convertase subtilisin/kexin type 9(PCSK9),liver X receptor α(LXRα),peroxisome proliferator-activated receptor γ(PPARγ),ATP-binding cassette transporter G1(ABCG1),and cholesterol 7α-hydroxylase(CYP7A1)were assessed using quantitative polymer-ase chain reaction(qPCR)and immunohistochemis-try.Results In acute hyperlipidemic mice,4-MBA administration significantly reduced serum TG and LDL-C levels while elevating HDL-C(P<0.05).Hy-perlipidemic rats exhibited decreased serum TG and LDL-C,increased HDL-C(P<0.01),reduced hepatic LDL-C(P<0.01),and elevated hepatic HDL-C(P<0.01).Although TC levels showed a downward trend, the difference lacked statistical significance.He-patic lipid accumulation and steatosis were alleviated.Upregulated mRNA and protein expression of LDLR,PPARγ,LXRα,and ABCG1(P<0.01),alongside downregulated PCSK9(P<0.05),were observed. Conclusion 4-MBA modulates cholesterol metabolism primarily via the LDLR/PCSK9 pathway to enhance cholesterol uptake and the PPARγ-LXRα-CYP7A1/ABCA1 axis to promote cholesterol utilization and ef-flux.

Objective:To construct a clinical early warning model for bladder spasm occurred after transurethral resection of the prostate(TURP)in patients with benign prostatic hyperplasia(BPH)based on LASSO regression. Methods:The clinical data of 139 patients with BPH who underwent TURP treatment in Yellow River Sanmenxia Hospital from January 2022 to June 2024 were prospectively selected, and they were divided into spasm group(39 cases)and non-spasm group(100 cases)according to whether bladder spasm occurred after surgery. The characteristics of bladder spasm occurred after TURP in patients with BPH were statistically analyzed, and the general data between the two groups were compared.LASSO regression was used to screen the characteristic variables related to bladder spasm occurred after TURP,multivariate logistic regression was used to analyze the influencing factors of bladder spasm after TURP.The prediction model of bladder spasm after TURP was constructed, and the receiver operating characteristic(ROC)curve was drawn to evaluate the efficacy of the prediction model. Results:The incidence of bladder spasm after TURP was 28.06%.There were significant differences in age, history of diabetes, operation time, temperature of irrigating fluid, traction tension of urinary catheter, prostate volume and postoperative hospital stay between non-spasm group and spasm group(P<0.05).LASSO regression showed that operation time, age, traction tension of urinary catheter, temperature of irrigating fluid, history of diabetes were important characteristics of bladder spasm occurred after TURP.Multivariate logistic regression analysis showed that old age, history of diabetes and long operation time were independent risk factors of bladder spasm occurred after TURP(P<0.05).ROC curve showed that the area under the curve(AUC)of the risk prediction model for bladder spasm occurred after TURP was 0.885(95%CI:0.752-0.921).Conclusions:Old age, history of diabetes and long operation time are independent risk factors of bladder spasm occurred after TURP,the clinical warning model based on LASSO regression has a certain value in predicting the risk for bladder spasm occurred after TURP,which helps to identify high-risk patients early.
