OBJECTIVE To evaluate the efficacy, safety and economics of the centrally procured generic versus original esomeprazole in the treatment of acute non-variceal upper gastrointestinal bleeding (ANVUGIB). METHODS A retrospective collection of real-world clinical data was conducted for ANVUGIB patients who received treatment at Shenzhen People’s Hospital and University of Hong Kong-Shenzhen Hospital from January 2018 to March 2024. Patients were divided into imported original drug group (original drug group, 221 cases) and centrally procured generic drug group (generic drug group, 75 cases) according to the types of drug used. Propensity score matching (PSM) was performed at a ratio of 3∶1 to compare the clinical efficacy, safety and economics between the two groups. RESULTS Totally 241 patients were included after PSM, with 170 in the original drug group and 71 in the generic drug group. There were no significant differences between the two groups in terms of rebleeding rate, rate of second endoscopic intervention, blood transfusion rate, length of hospital stay, mortality due to gastrointestinal bleeding, 30-day readmission due to rebleeding, and overall survival rate (P>0.05). The incidence of adverse events among all patients in both groups also showed no statistically significant difference (P>0.05); furthermore, the adverse events reported by the respective hospitals to the National Center for ADR Monitoring were comparable between the two groups. After PSM, the median total drug cost and high-dose esomeprazole cost in the generic drug group were significantly lower than those in the original drug group, while the median nursing fee and bed fee were significantly higher than those in the original drug group (P<0.05). There was no statistically significant difference between the two groups in terms of median total hospitalization expenses, total treatment costs, laboratory fees, examination fees, material costs, or consultation fees (P>0.05). CONCLUSIONS The clinical efficacy and safety of centrally procured generic esomeprazole in the treatment of ANVUGIB are comparable to those of the original drug, and it is more economical.

OBJECTIVE:
To evaluate the effect of manual acupuncture on endometrial blood flow parameters by three-dimensional (3D) power Doppler ultrasound in women undergoing in vitro fertilization embryo transfer (IVF-ET).
METHODS:
Seventy patients undergoing IVF-ET were equally randomized into traditional or sham acupuncture treatment group for totally 4 days (from the day of oocyte aspiration to the day of embryo transfer) of treatment by random envelope method at the Reproductive Medicine Center and Outpatient Department of Integrated Traditional Chinese and Western Medicine, Tongji Hospital, Tongji Medicine College, Huazhong University of Science and Technology from January 2013 to December 2015. Patients in the traditional acupuncture group accepted traditional acupuncture methods with manual acupuncture, and Zhongji (CV3), Qihai (CV 6), Sanyinjiao (SP6), Taichong (LR 3), Tianshu (ST 25), Guilai (ST 29) and Zusanli (ST 36) were chosen. Patients at the sham acupuncture group accepted shallow acupuncture methods at 4 non-meridian points at each shoulder and upper arm. Outcome measures included endometrial ultrasonic indices such as vascularization index (VI), flow index (FI) and vascularization flow index (VFI), endometrial thickness and volume, subendometrial VI (sVI), subendometrial FI (sFI), subendometrial VFI (sVFI), implantation rate, clinical pregnancy rate, abortion rate, live birth rate and number of live births.
RESULTS:
Finally, 34 patients in the traditional acupuncture group and 35 in the sham acupuncture group completed this trial. VI, FI and VFI of the traditional acupuncture group were significantly higher than those in the sham acupuncture group (P<0.05). No significant differences were found in endometrial thickness, endometrial volume, sVI, sFI, sVFI, implantation rate, clinical pregnancy rate, abortion rate, live birth rate and number of live births (P>0.05).
CONCLUSIONS
Manual acupuncture performed after oocyte aspiration and before transplantation improved the endometrial blood flow parameters VI, RI and VFI in women who underwent IVF-ET, instead of sVI, sFI and sVFI. Therefore, acupuncture might be beneficial in women undergoing IVF-ET by increasing endometrial blood flow and endometrial receptivity. (Registration No. ChiCTR2100053354).

OBJECTIVE To investigate a method for dynamic monitoring of drug consumption (DMDC) based on statistical process control (SPC), aiming to improve the macro-supervisory capacity in the process of drug utilization. METHODS The lists of key monitoring drug varieties in our hospital were established based on drug cost and relevant national documents. Monthly consumption data of key monitoring drug varieties in the entire hospital, outpatient pharmacy and inpatient pharmacy were taken as monitoring objects, and the DMDC model was established using SPC’s X control chart, moving range control chart, and exponentially weighted moving-average control chart, monitoring from three dimensions: single-month consumption, range variation, and consumption trend. Rosuvastatin, metoprolol and meropenem were taken as examples to demonstrate the monitoring capabilities of the DMDC model. RESULTS Lists of key monitoring drug varieties were established for entire hospital, outpatient pharmacy and inpatient pharmacy, containing 203, 167 and 200 varieties, respectively. After excluding drug varieties that could not be modeled and for which modeling failed, 179, 116 and 172 DMDC models were successfully established for these three drug consumption areas, respectively. During the first four months of 2024, these three groups of model separately warned 54, 32 and 62 drug varieties. The DMDC model successfully monitored the monthly consumption of drugs, such as rosuvastatin throughout the hospital, metoprolol in outpatient pharmacy, and meropenem in inpatient pharmacy. Compared with the previously used floating rate ranking method in our hospital, the application of the DMDC model significantly improved the scope and depth of drug monitoring, with the monitored drug varieties greatly expanded from about 50 to 179, and the monitoring dimensions increased from a single dimension to three. CONCLUSIONS The DMDC model based on SPC is effective and feasible, suitable for monitoring drug varieties with stable monthly consumption.

Journal of Environmental and Occupational Medicine 2024;41(11):1240-1245
Background Workers in printing jobs are exposed to a variety of organic solvents at low levels for a long period of time. Previous studies have focused on the main components in Material Safety Data Sheet or high-risk occupational hazardous agents, but have not been able to comprehensively and accurately identify the volatile organic compounds (VOCs) in printing jobs. Objective To qualitatively analyze the VOCs in ink, detergent, varnish, fountain solution, and other raw and auxiliary materials by headspace gas chromatography-mass spectrometry, and to accurately identify the occupational hazardous agents in printing jobs. Methods Raw and auxiliary materials used in printing jobs in 6 printing enterprises in Shanghai were sampled by headspace sampling, analyzed by gas chromatography-mass spectrometry, identified by National Institute of Standards & Technology ( NIST) Standard Library and retention time, and quantified by peak area percentage. Results A total of 181 VOCs were reported in the headspace of 25 organic solvent samples, with a total detection frequency of 337 items and 13.2±12.3 VOCs per sample. The most frequently detected VOCs category was naphthene, with a total of 69 items detected (22.5%), dominated by cyclohexane. The characteristics of VOCs components in the headspace of different types of samples were different. The largest number of VOCs was detected in the headspace of detergent samples, with an average of 31.5 VOCs detected. The characteristic components in detergent were aromatic hydrocarbons and ester compounds; alcohols were the characteristic components in varnish; and ester compounds were the characteristic components in ink. The results of cluster analysis showed that ethanol, toluene, ethyl acetate, cyclohexane, and acetone were occupational hazardous agents with high levels of exposure in printing jobs. Conclusion Many kinds of organic solvents used in printing process are difficult to identify by traditional methods. Headspace gas chromatography-mass spectrometry can help to accurately identify VOCs in the workplace, so as to more accurately assess the occupational hazards, and to provide a technical basis for guiding occupational health monitoring and implementing corresponding occupational health risk management and control.
