1.Chinese expert consensus on pre-hospital and emergency diagnosis and treatment of adult traumatic brain injury

Traumatic brain injury(TBI)is a serious condition characterized by high rates of mortality and disability.Deaths caused by severe TBI usually occur within the first few hours after the injury.Timely and effective management of TBI during pre-hospital and emergency treatment phases is crucial for improving patients'prognosis.To address this issue,the Emergency Physicians Branch of the Chinese Medical Doctor Association,the Emergency Medicine Professional Committee of the People's Liberation Army,the Beijing Emergency Medicine Society,and the Chinese Emergency Medicine Specialist Association have jointly selected national experts in emergency medicine and neurosurgery to formulate the"Chinese Expert Consensus on the Pre-hospital and Emergency Diagnosis and Treatment of Adult Traumatic Brain Injury".This consensus consists of two parts focusing on pre-hospital and emergency diagnosis and treatment of TBI,with 24 recommendations put forward to guide relevant clinical practices in pre-hospital and emergency management.

This article presents a comprehensive review of new methods for target identification of complex components in tradi-tional Chinese medicine(TCM)and research progress in their applications.It systematically summarizes classical approaches and cutting-edge technical systems for target identification,cov-ering multiple strategies such as proteomic analysis,fluorescence resonance energy transfer(FRET)technology,network pharma-cology prediction models,high-throughput biochip screening,tar-get capture strategies based on molecular affinity fishing,gene site-directed mutagenesis verification,and co-crystallization structure analysis of target proteins.The review emphasizes the critical role of target identification in elucidating the action mechanisms of TCM and facilitating new drug development,lay-ing a foundation for promoting the modernization of TCM.

5.Research progress of histone deacetylase and its inhibitors in osteogenesis and odontogenic differentiation of odontogenic stem cells

Histone deacetylases(HDACs)can deacetylate histones,leading to tighter DNA binding,and thereby playing a role in inhibiting gene transcription.On the contrary,histone deacetylase inhibitors(HDACis)can promote chromatin relaxation,enabling various transcription factors to bind specifically to DNA and activate transcription genes.Dental stem cells(DSCs)are human adult stem cells.These cells have the characteristics of less damage and low immune rejection during sampling,and are especially important seed cells in the process of osteogenesis,odontogenesis and other differentiation.A large number of experimental studies have shown that HDACs and HDACis together play important roles in cell division and differentiation,signal transduction,regulation of cellular inflammation and other life processes.This review summarizes the research progress of HDACs and HDACis in regulating osteogenic and odontogenic differentiation of DSCs,aiming to provide insights into the study of the interaction between HDACs and HDACis,and potentially guide clinical application of DSCs in the treatment of tooth and bone injury.
6.Qualitative research on disease experiences of women with endometriosis:a meta-synthesis

Objective To understand the disease experiences of women with endometriosis(EMs),so as to provide a basis for improving the diagnosis,treatment,nursing and support of this population.Methods The databases of PubMed,Web of Science,Scopus,Embase,Cochrane Library,VIP,Wanfang Data,CNKI,CBM were retrieved on qualitative research about the disease experiences of endometriosis patients from inception to Jun 2023.The quality of the literature was evaluated by Joanna Briggs Institute(JBI)Critical Appraisal Tool for qualitative studies.Results A total of 17 studies were included,51 clear research findings were extracted,which were summarized into 10 new categories and 3 integrated results:(1)Cyclical episodes of the disease not only bring physical and psychological distress,but also lead to decreasing the patient's sense of female identity,destroying social and intimating relationships;(2)The doctor and patient interaction is influenced by imbalance of cognitive,the process of diagnosis and treatment is full of challenges,and patients have a demand for professional information and social support;(3)Growing up in pain,patients actively self-adjust and positively cope with the disease.Conclusion EMs affects patients'quality of life physiologically and psychologically,with prevalent issues of delayed diagnosis and repeated treatments.The professional information supported by health professionals needs to be improved.Healthcare providers should pay more attention to patients'physical and emotional experiences in their clinical work,improve their informal support,participate in long-term management,and improve patients'ability to manage their diseases.
7.Concordance study of anthropometric indicators in diagnosing abdominal obesity in MHD patients

Objective:To study the application of conicity index (CI),body mass index (BMI),waist-to-hip ratio (WHR) and body fat ratio (BFR) in abdominal obesity in maintenance hemodialysis (MHD) patients. Methods The patients who received dialysis treatment in the hemodialysis center of the Third Affiliated Hospital of Chongqing Medical University from September to December 2023 were selected by fixed-point cluster sampling,and the age,dialysis age,height,weight,biochemical examination indexes and other information of the study subjects were collected,and the body composition analysis was performed using InBody S10 and the visceral fat area was used as the basis for the determination of abdominal obesity. The applications of body mass index,body fat ratio,waist-to-hip ratio and conicity index in predicting abdominal obesity in MHD patients were compared. Results A total of 151 patients were enrolled,including 65 patients with abdominal obesity MHD and 86 patients with non-abdominal obesity MHD. The results showed that there were significant differences in gender,weight,BMI,visceral fat area,body fat ratio,waist-to-hip ratio,conicity index and the prevalence of diabetes between patients with abdominal obesity and non-abdominal obesity (all P<0.05). The consistency test results showed that the consistency between using BMI,body fat percentage,and waist hip ratio to determine abdominal obesity with visceral fat area method was weak,with the conicity index Kappa coefficient having the largest value (P<0.05).The analysis of receiver operating characteristic (ROC) curve showed that the area under the ROC curve of conicity index was the highest,0.765,indicating the highest diagnostic efficacy (P<0.05) in MHD patients. Conclusion Conicity index,body mass index,waist-to-hip ratio and body fat ratio can be used in the diagnosis of abdominal obesity in MHD patients,among which the conicity index is more effective.

Objective Kelch-like epichlorohydrin-associated protein 1/transcription factor NF-E2-associated factor 2/hemoglobin oxygenase 1(Keap1/Nrf2/HO-1),phosphatidylinositol-3 kinase/protein kinase B/rapamycin protein(PI3K/Akt/mTOR),stem cell growth factor/tyrosinase receptor(SCF/c-kit),liver kinase B1/adenylate-activated protein kinase(LKB1/AMPK)and other signaling pathways have irreplaceable roles in the development of oligospermia(OAS),and the study of targeting their related signals is a hotspot for the prevention and treatment of OAS.Therefore,taking many signaling pathways as breakthroughs.The review systematically summarized the mechanism of action of Chinese medicine extracts,proprietary Chinese medicines,Chinese medicine prescriptions and external Chinese medicine treatments such as acupuncture and moxibustion that intervene in the relevant signaling pathways for the treatment of OAS,with a view to providing theoretical basis and guidance for clinical practice.

10.Independent prognostic value of the congestion and renal index in patients with acute heart failure.

BACKGROUND:
Clinical outcomes are poor if patients with acute heart failure (AHF) are discharged with residual congestion in the presence of renal dysfunction. However, there is no single indication to reflect the combined effects of the two related pathophysiological processes. We, therefore, proposed an indicator, congestion and renal index (CRI), and examined the associations between the CRI and one-year outcomes and the incremental prognostic value of CRI compared with the established scoring systems in a multicenter prospective cohort of AHF.
METHODS:
We enrolled AHF patients and calculated the ratio of thoracic fluid content index divided by estimated glomerular filtration rate before discharge, as CRI. Then we examined the associations between CRI and one-year outcomes.
RESULTS:
A total of 944 patients were included in the analysis (mean age 63.3 ± 13.8 years, 39.3% women). Compared with patients with CRI ≤ 0.59 mL/min per kΩ, those with CRI > 0.59 mL/min per kΩ had higher risks of cardiovascular death or HF hospitalization (HR = 1.56 [1.13-2.15]) and all-cause death or all-cause hospitalization (HR = 1.33 [1.01-1.74]). CRI had an incremental prognostic value compared with the established scoring system.
CONCLUSIONS
In patients with AHF, CRI is independently associated with the risk of death or hospitalization within one year, and improves the risk stratification of the established risk models.
