Ischemic stroke(IS)research has faced bottlenecks due to the limitations of conventional technologies in resolving cellular heterogeneity and spatiotemporal dynamics. The development of single-cell and spatial omics technologies provides revolutionary tools to break through these constraints.Single-cell omics technologies,by performing high-throughput sequencing on thousands of cells, reveal the high heterogeneity and dynamic state transitions of neurons, glial cells, immune cells, and others post-IS.For instance, microglia contain pro-inflammatory and anti-inflammatory functional subsets, while astrocytes exhibit distinct activation state spectra.Pseudotime analysis further reconstructs the fate trajectories of cells during the damage and repair processes.Spatial omics technologies, conversely, reconstruct spatial maps of gene expression through in situ capture, elucidating molecular gradients between the ischemic core, penumbra, and healthy brain regions, and enabling the analysis of critical cell-cell interaction net-works.Integrating the deep phenotyping capability of single-cell sequencing with the in situ localization information from spatial omics constitutes the current core strategy. This multimodal analysis allows for precise anchoring of cell subtypes to their spatial microenvironments, revealing their distribution patterns and functions, and constructing a more accurate atlas of cell-cell communication. This significantly ad-vances the refined dissection of IS mechanisms. This strategy has already accelerated the discovery of po-tential biomarkers and spatiotemporally specific therapeutic targets.Although challenges remain in sample preparation, data integration, and technical noise, future interdisciplinary collaboration,multi-omics in-tegration,and in-depth mining with artificial intelligence promise to comprehensively transform our under-standing of IS.Ultimately,it holds the potential to promote advances in its early diagnosis, precise sub-typing,and the development of individualized treatment strategies.

Objective To investigate the association of endometrial thickness(EMT)with obstetric and neonatal outcomes of monoparous pregnancy in fresh cleavage embryos transfer. Methods A total of 1 845 patients of monoparous pregnancy after fresh cleavage embryos transfer cycles from Jan 2016 to Mar 2022 at Shanghai First Maternity and Infant Hospital,Tongji Universtiy were analyzed retrospectively.Patients were categorized into three groups by EMT on transferation day:≤8 mm(group A),8-14 mm(group B)and≥14 mm(group C).The primary outcomes were preterm birth(PTB),birth weight and birth weight z-score,small-for-gestation age,large-for-gestation age, very low birth weight, low birth weight and macrosomia. The second outcomes were pregnancy and perinatal complications. The relationship between EMT and adverse neonatal outcomes was estimated by Logistic regression analysis. Results The rate of ectopic pregnancy was increased significantly in group A.No significant differences were found among the three groups in gestation age, birth weight, birth weight z-score,PTB,small for gestation age, large for gestation age,low birth weight, very low birth weight and macrosomia.Compared with group B,the odds of adverse neonatal outcomes did not show significant differences before and after adjustment in both group A and group C by Logistic regression analysis. Conclusion Thinner EMT in fresh cleavage embryos transfer is associated with higher rate of ectopic pregnancy, while it is not independently associated with adverse perinatal outcomes.

Traditional Chinese Medicine (TCM), as a treasure of the Chinese nation, plays a significant role in maintaining public health. In 2019, the Central Committee of the Communist Party of China and the State Council proposed for the first time the establishment of a TCM registration and evaluation evidence system that integrates TCM theory, "personal experience" and clinical trials (referred to as the "Three-in-One" System) to promote the inheritance and innovation of TCM. Subsequently, the National Medical Products Administration issued several guiding principles to advance the improvement and implementation of this system. Owing to the complexity of its implementation, there are still differing understandings within the TCM industry regarding the positioning of the "Three-in-One" Registration and Evaluation Evidence System, as well as the connotation and value orientation of the "personal experience." To address this, Academician WANG Qi, President of the TCM Association, China International Exchange and Promotion Association for Medical and Healthcare and TCM master, led a group of academicians, TCM masters, TCM pharmacology experts and clinical TCM experts to convene a "Seminar on Promoting the Implementation of the ′Three-in-One′ Registration and Evaluation Evidence System for Chinese Medicinals." Through extensive discussions, an expert consensus was formed, clarifying the different roles of the TCM theory, "personal experience" and clinical trials within the system. It was further emphasized that the "personal experience" is the core of this system, and its data should be derived from clinical practice scenarios. In the future, the improvement of this system will require collaborative efforts across multiple fields to promote the high-quality development of the Chinese medicinal industry.

Readiness for Hospital Discharge(RHD)refers to a multidimensional assessment of a patient's ability to transition safely from hospital to home, encompassing physiological stability, psychological preparedness, and social support adequacy. For patients requiring Home Nutrition Support(HNS),discharge readiness is particularly critical due to their heightened need for post-discharge specialized care, which significantly influences long-term recovery and quality of life. This paper reviews the concept, influencing factors, and unmet needs of RHD in patients with HNS and proposes targeted strategies to enhance discharge preparedness.By addressing gaps in current practices,we aim to optimize RHD in this vulnerable population and provide clinicians with evidence-based guidance for developing effective discharge plans.

Phosgene is a highly reactive chemical substance and a prevalent chemical warfare agent, and it is vitally important for rapid and accurate detection of phosgene to counteract terrorist threats and industrial accidents. In this work,a phosgene probe, designated as SX-Pho,which incorporated benzimidazole and hydroxyl groups as recognition motifs, was prepared through Suzuki coupling and Debus-Radziszewski methodologies to incorporate an electron-donating carbazole moiety. This probe exhibited a large Stokes shift(Approximately 130 nm).Upon exposure to triphosgene/triethylamine conditions(in situ phosgene generation),the fluorescence emission of probe at 470 nm underwent significant quenching, with a 20-fold reduction in intensity, while the fluorescence lifetime decreased from 3.30 ns to 3.06 ns.Concentration titration experiments demonstrated that SX-Pho achieved a lower detection limit of 57.8 nmol/L with high specificity and interference resistance.Preton nuclear magnetic resonance spectroscopy(1H NMR),high-resolution mass spectrometry, and density functional theory(DFT)calculations confirmed the cyclization reaction between hydroxyl groups, imines, and phosgene. The extent of overlap between the highest occupied molecular orbital(HOMO)and the lowest unoccupied molecular orbital(LUMO)was notably decreased, leading to the suppression of radiative transitions. The energy gap underwent a reduction of 0.43 eV,while the non-radiative transition was augmented, resulting in fluorescence quenching and achieving rapid detection of phosgene.Based on this,probe-loaded test strips were prepared. The color change under 365 nm illumination allowed visual discrimination of phosgene at concentrations below 20 μL/L.Furthermore,using a smartphone's built-in RGB application to measure the intensity of the blue(B)channel after the test strips were exposed to phosgene enabled both qualitative and quantitative detection. The detection range was 1.82-50 μL/L,with a limit of detection(LOD)of 1.814 μL/L.
