1.Erchentang Ameliorates SiO2-induced Lung Injury by Regulating Oxidative Stress and Metabolic Disorders via Nrf2/HO-1 Signaling Pathway

ObjectiveTo observe the protective effect of Erchentang (ECT) on SiO2-induced lung injury in rats and to explore its underlying mechanism. MethodsA rat model of lung injury was established by a single intratracheal instillation of 50 mg·mL-1 SiO2 suspension. Thirty male Sprague-Dawley (SD) rats were randomly assigned to five groups: control, model, low and high-dose (4.5 g·kg-1·d-1 and 9 g·kg-1·d-1, respectively) ECT, and dexamethasone (0.2 mg·kg-1·d-1). All the groups were treated for 4 consecutive weeks. Histopathological alterations in the lung tissue were examined by hematoxylin and eosin (HE) staining. The levels of malondialdehyde (MDA), superoxide dismutase (SOD), and glutathione peroxidase (GSH-Px) in the lung tissue were measured through biochemical assays. The expression of key molecules in the nuclear factor erythroid 2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) pathway was determined by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR), Western blot, and immunofluorescence assay. The primary active components of ECT were identified by ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS), and their binding affinity to Nrf2/HO-1 was assessed by molecular docking. Untargeted metabolomics of the lung tissue was performed based on UPLC-quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF-MS), and correlation analysis was performed to identify differential metabolites and parameters closely associated with the Nrf2/HO-1 pathway. ResultsCompared with the control group, the model group exhibited a reduction in body weight gain, an increase in lung index, increased MDA content, weakened SOD and GSH-Px activities in the lung tissue, down-regulated mRNA and protein levels of Nrf2 and protein levels of HO-1 and GPX4, and an up-regulated protein level of Keap1 (P<0.05, P<0.01). Treatment with ECT attenuated the SiO2-induced decline in body weight (P<0.05), alleviated inflammatory cell infiltration and silicotic nodule formation in alveoli, and reduced the MDA content and enhanced the SOD and GSH-Px activities in the lung tissue (P<0.05, P<0.01). UPLC-MS/MS and molecular docking revealed that core components of ECT, such as hesperidin and glycyrrhizic acid, displayed strong binding affinity to Nrf2/HO-1. Molecular biological experiments demonstrated that ECT promoted nuclear translocation of Nrf2, up-regulated the mRNA and protein levels of HO-1 and GPX4, and down-regulated Keap1 expression (P<0.05, P<0.01). Metabolomic analysis indicated that ECT reversed the SiO2-induced aberrant expression of metabolites, including linoleic acid and glutamine (P<0.05, P<0.01). Correlation analysis showed that Nrf2 and HO-1 were positively correlated with SOD and GSH-Px (P<0.05, P<0.01), but negatively correlated with glutamine and serine (P<0.05, P<0.01). ConclusionECT may activate the Nrf2/HO-1 pathway through its core active components, thereby regulating oxidative stress and metabolic disorders to ameliorate SiO2-induced lung injury in rats. This study provides experimental evidence for ECT in the prevention and treatment of occupational lung injury.
2.Mitochondrial Dysfunction and Diabetic Retinopathy: From Pathogenesis to Therapeutic Targets

Diabetic retinopathy (DR) is one of the most prevalent and vision-threatening microvascular complications of diabetes mellitus, yet its pathogenesis extends far beyond vascular injury alone. As the retina is among the most energy-demanding tissues in the body, its neurons, glial cells, pigment epithelial cells, pericytes, and endothelial cells are highly dependent on mitochondrial oxidative phosphorylation to maintain visual signal transduction, ionic homeostasis, and neurovascular integrity. This review summarizes current evidence indicating that mitochondrial dysfunction is not merely a downstream consequence of chronic hyperglycemia, but a central pathogenic hub that initiates, amplifies, and perpetuates retinal neurovascular degeneration in DR. Persistent hyperglycemia activates multiple abnormal metabolic pathways, including the polyol pathway, hexosamine pathway, protein kinase C signaling, advanced glycation end-product formation, and angiotensin II-related responses. Although these pathways differ mechanistically, they converge on excessive reactive oxygen species (ROS) generation, antioxidant depletion, and mitochondrial injury. Under diabetic stress, electron transport chain overload promotes mitochondrial ROS leakage, damages mitochondrial DNA, disrupts membrane potential, and impairs the transcription of key respiratory chain components. In parallel, mitochondrial quality-control systems become progressively compromised. The balance between fusion and fission shifts toward pathological fragmentation through reduced MFN1/2 and OPA1 activity and enhanced DRP1-mediated fission. Mitochondrial biogenesis is suppressed through inhibition of the AMPK/SIRT1/PGC-1α/NRF1/TFAM axis, while mitophagy changes from an early compensatory response to a later state of autophagic flux blockade and accumulation of dysfunctional mitochondria. Importantly, damaged mitochondria serve as signal amplifiers linking metabolic stress to inflammation and programmed cell death. Mitochondrial ROS, oxidized mitochondrial DNA, calcium overload, cardiolipin exposure, and membrane permeabilization activate interrelated death pathways, including intrinsic apoptosis, ferroptosis, and pyroptosis. Cytochrome C and apoptosis-inducing factor promote caspase-dependent and caspase-independent apoptosis; iron dyshomeostasis, glutathione depletion, GPX4 dysfunction, and lipid peroxidation drive ferroptosis; and mitochondrial danger signals activate the NLRP3 inflammasome and gasdermin-dependent pyroptosis. These pathways jointly damage the retinal neurovascular unit and contribute to pericyte loss, endothelial barrier breakdown, Müller cell dysfunction, retinal ganglion cell apoptosis, retinal pigment epithelial injury, and photoreceptor degeneration. This review also emphasizes the role of epigenetic regulation in stabilizing mitochondrial pathology. DNA methylation, histone modifications, and non-coding RNAs interact to silence mitochondrial protective genes, alter antioxidant responses, and maintain the “metabolic memory” of DR even after glycemic normalization. Therefore, mitochondrial dysfunction should be understood as a dynamic, multidimensional network rather than a single pathological event. Current clinical approaches, such as laser photocoagulation, intravitreal anti-VEGF therapy, and vitrectomy, mainly target advanced vascular lesions and are limited by invasiveness, incomplete responsiveness, recurrence, and potential adverse effects. Therapeutically, strategies targeting mitochondrial ROS, restoring mitochondrial dynamics, enhancing biogenesis, regulating mitophagy, inhibiting inflammasome activation, correcting epigenetic abnormalities, and improving targeted delivery systems show promising potential. However, major translational barriers remain, including retinal cell heterogeneity, stage-specific mitochondrial responses, insufficient organelle-specific drug delivery, and long-term safety concerns. A deeper understanding of mitochondrial regulatory networks may support earlier, more precise, and multi-target interventions for preventing or slowing DR progression.
3.Mechanism of action of Qiangxin decoction in regulating the IκBα/NF-κB p65 signaling pathway-mediated M1 macrophage polarization for the treatment of chronic heart failure

OBJECTIVE To explore the mechanism of Qiangxin decoction in the treatment of chronic heart failure (CHF) by regulating the inhibitor of nuclear factor-κB alpha/nuclear factor-κB p65 subunit (IκBα/NF-κB p65) signaling pathway-mediated M1 macrophage polarization. METHODS In animal experiments, mice were used as research subjects, and the CHF model was established via coronary artery ligation. Model mice were treated with Qiangxin decoction (21.69 g/kg) for 28 days. After intervention, pathological morphological changes of myocardial tissues and cardiomyocyte apoptosis were observed. Serum levels of inflammatory factors including tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6), myocardial injury marker troponin I3 (TNNI3), and macrophage-related markers F4/80 and cluster of differentiation 80 (CD80) were detected. In cell experiments, monocyte-macrophage leukemia RAW264.7 cells were induced to undergo M1 polarization by 10 ng/mL lipopolysaccharide combined with 10 ng/mL interferon-γ, followed by 48 h intervention with 10% drug-containing serum of Qiangxin decoction. After intervention, the nuclear translocation of NF-κB p65 and NF-κB p50 was observed. The positive rate of CD80 in cells was measured. The mRNA expression of inflammatory factors including IL-1β, IL-6, IL-8, IL-18, TNF-α, inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX2), C-C motif chemokine receptor 7 isoform 2 (CCR7-2), as well as the expression levels of proteins related to the IκBα/NF-κB p65 signaling pathway were detected. RESULTS Animal experimental results showed that Qiangxin decoction alleviated myocardial fibrosis in mice, and significantly reduced the cardiomyocyte apoptosis rate, serum levels of TNF-α and IL-6, as well as the protein expression of TNNI3, F4/80 and CD80 in myocardial tissues (P<0.05). Cell experimental results indicated that Qiangxin decoction significantly suppressed the nuclear translocation of NF-κB p65 and NF-κB p50 (P<0.05). It significantly decreased the CD80 positive rate, mRNA levels of IL-1β, IL-6, IL-8, IL-18, TNF-α, iNOS, COX2 and CCR7-2, total and nuclear expression of NF-κB p65, total, nuclear and cytoplasmic expression of phosphorylated NF-κB p65, total expression of phosphorylated IκBα (P<0.05). Meanwhile, it significantly elevated the total expression of IκBα and cytoplasmic expression of NF-κB p65 (P<0.05). CONCLUSIONS Qiangxin decoction can inhibit the activity of the IκBα/NF-κB p65 signaling pathway, reduce the release of pro-inflammatory factors, thereby restraining M1 macrophage polarization, mitigating myocardial fibrosis, and ultimately exerting therapeutic effects against CHF.

Objective To observe the value of MR 3D-T1WI and T2WI radiomics for diagnosing early and middle stage Parkinson disease(PD).Methods A total of 96 patients with early or middle stage PD(Hoehn-Yahr[H-Y]stage≤2.5)and 96 matched healthy adults were retrospectively collected and divided into training set(n=135,including 67 cases of PD and 68 healthy adults)and validation set(n=57,including 28 cases of PD and 29 healthy adults)at the ratio of 7∶3.The optimal radiomics features of left red nucleus(LRN),right red nucleus(RRN),left substantia nigra(LSN)and right substantia nigra(RSN)were extracted and screened from cranial 3D-T1WI and T2WI in training set.Then radiomics models of single MR sequence and combined MR sequences were constructed,respectively,the radiomics scores(Radscore)were obtained,the diagnostic efficacy of each model for diagnosing early and middle stage PD was validated using validation set,and the correlations of Radscore of each model and clinical scale scores of PD patients were analyzed.Results Based on LRN,RRN,LSN and RSN on 3D-T1WI and T2WI,15,14,11 and 14,and 15,12,14 and 12 optimal radiomics features were obtained,respectively.Then models of single sequence,including LRN3D-T1W,I RRN3D-T1W,I LSN3D-T1W,I RSN3D-T1W,I LRNT2W,I RRNT2W,I LSNT2WI and RSNT2W,I as well as models of combined sequences,including LRN3D-T1WI+T2WI,RRN3D-T1WI+T2WI,LSN3D-T1WI+T2WI and RSN3D-T1WI+T2WI were constructed.The AUC of models in training and validation sets based on 3D-T1WI were 0.75-0.86,of models based on T2WI in training and validation sets were 0.82-0.90,while of combined models were 0.85-0.93.The Radscore of LRN3D-T1WI model in PD patients was negatively correlated with Hamilton depression scale(HAMD)and Hamilton anxiety scale(HAMA)scores(rs=-0.255,-0.242,P=0.011,0.016),while of RSNT2WI model was negatively correlated with HAMD score(rs=-0.254,P=0.010).Conclusion 3D-T1WI and T2WI radiomics could be used to diagnose early and middle stage PD.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
7.Association between serum vitamin A and E levels and asthma risk in children aged 1-14 years in some areas of Hunan Province

To analyze the relationship between serum vitamin A and E levels and asthma risk in children aged 1-14 years. Based on a multi-center case-control study design, general information and vitamin A and E test data were collected from asthmatic children in the respiratory outpatient departments of four medical institutions in Hunan Province and control children undergoing health check-ups in the child healthcare departments during the same period from July 2017 to October 2019.After 1∶1 matching age and gender using propensity scores, a total of 2 454 research subjects were included. 1 730 boys and 724 girls, with an age range of 1-14 years, a median age of 3 years and a mean age of (3.58±2.83) years. The levels of vitamin A and E in the asthma group and the control group were compared. Binary Logistic regression was used to analyze the association between vitamin levels and asthma, and stratified analysis by age and gender was conducted. The results showed that a total of 3 447 research subjects were initially included in this study, and 2 454 research subjects were included after propensity scores matching (PSM). After matching, there was no statistically significant difference in age and gender between the two groups ( χ2=0.00 ,P>0.05), and the standardized mean difference (SMD) values were all less than 10%.The levels of vitamin A and E in the asthma group were significantly lower than those in the control group [(0.32±0.09)mg/L vs.(0.34±0.08)mg/L, t=4.94 ,P<0.001;(8.90±2.47)mg/L vs.(9.19±2.58) mg/L, t=2.79 ,P=0.005]. Compared to the normal vitamin A group, the high-risk subclinical deficiency group ( OR=1.53, 95% CI: 1.28-1.83) and the subclinical/clinical deficiency group ( OR=2.68, 95% CI: 1.81-3.97) were significantly positively associated with asthma risk. Compared to the normal vitamin E group, the vitamin E insufficient group was significantly associated with increased asthma risk ( OR=1.28, 95% CI: 1.03-1.57). Subgroup analysis revealed that subclinical/clinical vitamin A deficiency was significantly associated with increased asthma risk across all age groups and both genders (all P<0.05), whereas vitamin E insufficiency was only associated with higher asthma risk in the 1-3 age group and among girls ( P<0.05).In conclusion, subclinical or clinical deficiency of vitamin A may significantly increase the risk of asthma. Vitamin E deficiency is mainly associated with an increased risk of asthma in younger age groups (1-<3 age group) and girls.

Aim To evaluate the bioequivalence of two oral preparations of rivaroxaban tablets(test preparation T and refe-rence preparation R)in fasting/postprandibular state in healthy Chinese subjects.Methods A randomized,open,single-dose,four-cycle,completely repeated crossover experiment was used in this study.A total of 70 healthy male and female subjects were enrolled,including 38 subjects in the fasting group and 32 sub-jects in the postprandial group.Rivaroxaban tablets(2.5 mg/tablet)were taken orally once per cycle and their reference preparations were tested.The plasma rivaroxaban concentration was determined by LC-MS/MS method.The pharmacokinetic parameters of rivaroxaban tablets were calculated by WinNonlin software,and the parameters were analyzed and processed.Re-sults The PK parameters of rivaroxaban tablets and reference preparations in fasting group were as follows:Cmax was(72.48±17.08)and(66.36±15.64)μg·L-1,respectively.AUC0-t were(383.49±101.06)and(370.43±102.16)h·ng·mL-1,and AUC0-inr were(389.58±102.28)and(375.84±103.01)h·μg·L-,respectively.Main PK parameters of subjects taking rivaroxaban tablets orally after meals:Cmax were(66.48±15.64 and 60.87±13.44)μg·L-1,AUC0-t were(404.44±72.58)and(381.80±79.93)h·μg·L-1,re-spectively.AUC0_inf was(410.88±73.55)and(393.64±69.71)h·μg·L-1,respectively.Under fasting and postmeal conditions,subjects took rivaroxaban test and reference prepara-tion orally,one tablet(2.5 mg/tablet)each time.The geometric mean of the main pharmacokinetic parameters of rivaroxaban in plasma(Cmax,AUC0-t,AUC0-inf)and their corresponding values had a 90%confidence interval ranging from 80.00%to 125.00%.No serious adverse events or unexpected adverse e-vents occurred in both groups.Conclusion Rivaroxaban tablets are bioequivalent and safe in vivo under fasting and postprandial conditions.

Objective:To investigate the distribution of CGG repeat numbers in the FMR1 gene among reproductive-age women in China, providing data reference for carrier screening and genetic counseling of Fragile X syndrome. Methods:This cross-sectional study recruited 16 610 reproductive-age women from 12 medical institutions between July 2022 and October 2023. Peripheral venous blood samples (3 mL) were collected, and genomic DNA was extracted. The number of CGG repeats in the FMR1 gene was determined using the triplet-primed polymerase chain reaction (TP-PCR) combined with capillary electrophoresis technology. Statistical analyses were performed to assess the prevalence and distribution of CGG repeat expansions. Results:Among 16 610 women of childbearing age, 5 684 (34.220%) women had the same number of CGG repeats in the two alleles of FMR1 gene, and 10 926 (65.780%) women had different numbers of repeats in the two alleles. Among the 33 220 FMR1 alleles in 16 610 women of reproductive age, the most common CGG repeat numbers were 29 [48.645% (16 160/33 220)] and 30 [26.276% (8 729/33 220)], while the most frequent CGG genotype was CGG 29/29 [24.726% (4 107/16 610)]. The CGG repeat numbers of FMR1 gene were normal in 16 498 women (99.326%). Among the 112 women (0.674%) with CGG repeat abnormities, 96 (0.578%) women were classified as intermediate carriers, 15 (0.090%) as premutation carriers, and 1 (0.006%) as a full mutation carrier, whose CGG genotype was (36, >200). Conclusion:In the general reproductive-age female population in China, the normal CGG repeat numbers of the FMR1 gene account for 99.326%, while the intermediate carrier rate is 0.578%, and the combined carrier rate of the premutation and full mutation types is 0.096%.
