OBJECTIVE To compare the efficacy of ceftazidime and avibactam (CZA) monotherapy and combination therapy in the treatment of carbapenem-resistant Gram-negative bacteria (CRGNB) infections, and analyze the influencing factors. METHODS The data of patients with CRGNB infection who received CZA treatment from January 2020 to March 2025 were collected retrospectively. The patients were divided into the CZA monotherapy group (52 cases) and the CZA combination therapy group (85 cases) according to treatment regimen. The therapeutic effects of the two groups were compared, and the drug susceptibility results of isolated strains were recorded. The multivariate Logistic regression model was used to analyze the factors influencing clinical efficacy of CRGNB patients. RESULTS The bacterial clearance rate of patients was significantly higher in the CZA combination therapy group than in the CZA monotherapy group (P=0.012). However, when comparing the 30-day mortality rate and the clinical response rate between the two groups, no statistically significant differences were observed (P>0.05). Among the isolates, carbapenem-resistant Klebsiella pneumoniae had the highest sensitivity to tigecycline (87.3%) and carbapenem-resistant Pseudomonas aeruginosa showed 90.9% sensitivity to amikacin. Five isolates were resistant to CZA. The multivariate Logistic regression showed, lung infection, receiving continuous renal replacement therapy (CRRT), and inadequate treatment courses were significantly correlated with clinical treatment failure (P<0.05). CONCLUSIONS For CRGNB infection, the clinical efficacy of CZA combination therapy is similar to that of monotherapy, but the combination therapy has a higher bacterial clearance rate. Objective To observe the effect of omeprazole enteric-coated capsules on clinical symptoms and serum inflammatory factor levels in children with chronic gastritis and peptic ulcer.Methods Children with chronic gastritis and peptic ulcer were divided into treatment group and control group by random number table method.The control group was given triple therapy of ranitidine hydrochloride tablets,amoxicillin and clarithromycin,while the treatment group was treated with omeprazole enteric-coated capsules combined with amoxicillin and clarithromycin.Clinical efficacy,symptom relief time,and changes in serum motilin(MOT),gastrin(GAS)and inflammatory factors[interlrukin-6(IL-6)and interlrukin-8(IL-8)]were compared between the two groups.Results There were 48 cases in treatment group and 48 cases in control group.After treatment,the total effective rates in treatment group and control group were 93.74%(45 cases/48 cases)and 85.42%(41 cases/48 cases),with significant difference(P<0.05).After treatment,the disappearance time of ulcer induced pain in treatment group and control group were(1.51±0.26)and(2.08±0.42)d;the disappearance time of acid regurgitation were(2.29±0.40)and(2.93±0.33)d;the disappearance time of burning sensation were(2.37±0.21)and(2.85±0.54)d;the length of hospital stay were(6.21±1.07)and(6.94±1.25)d;serum MOT levels were(298.48±35.15)and(273.58±31.25)pg·mL-1;serum GAS levels were(167.28±19.46)and(128.32±18.61)ng·L-1;IL-6 levels were(58.67±5.39)and(76.14±6.63)mg·mL-1;IL-8 levels were(50.08±5.16)and(58.68±5.49)mg·mL-1.The above indexes were significantly different between control group and treatment group(all P<0.05).The total incidence of adverse drug reactions in treatment group and control group were 8.33%and 12.50%,with no statistical significance(P>0.05).Conclusion Omeprazole enteric-coated capsules in the treatment of children with chronic gastritis and peptic ulcer can effectively alleviate various clinical symptoms and improve clinical efficacy.At the same time,it can lower serum levels of inflammatory factors and improve inflammation,with good effect.
5.Clinical trial of dydrogesterone combined with granulocyte colony-stimulating factor in the treatment of patients with thin endometrial infertility

Objective To explore the effects of dydrogesterone combined with granulocyte colony-stimulating factor(G-CSF)on blood flow parameters and sex hormone levels in patients with thin endometrial(TE)infertility.Methods TE infertility patients were randomly divided into control group and treatment group.All patients were treated with freezing-thawing embryo transfer.The control group was treated with desdrogesterone after ovulation monitoring by vaginal ultrasound,10 mg each time,bid,and stopped until the 14th day after ovulation.In treatment group,G-CSF was given on the basis of control group,and the endometrial thickness and hemodynamic parameters were monitored by vaginal ultrasound 3 days later,and then embryo transfer was performed.Blood flow parameters,ultrasound parameters,endometrial morphology,sex hormone levels[follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2),progesterone(P)]and pregnancy status were compared between the two groups.Results There were 49 cases in treatment group and 49 cases in control group.After treatment,endometrial thickness in treatment group and control group were(9.59±1.35)and(7.89±1.14)mm;follicle diameter were(19.35±1.58)and(17.75±1.42)mm;uterine volume were(92.68±7.85)and(85.69±6.74)cm3;PI were 1.29±0.27 and 1.74±0.32;RI were 0.32±0.09 and 0.50±0.11;peak systolic/end-diastolic velocity(S/D)were 2.03±0.28 and 2.21±0.25;P were(45.98±5.75)and(39.15±5.78)nmol·L-1;E2 were(462.58±58.42)and(339.45±45.97)pg·mL-1;FSH were(12.18±2.58)and(9.42±1.42)U·L-1;LH were(29.42±4.18)and(22.25±3.40)U·L-1;there were statistically significant differences between the two groups(all P<0.05).The clinical pregnancy rate(51.02%)and implantation rate(57.14%)in treatment group were higher than those in control group(30.61%and 36.73%),and the differences were statistically significant(all P<0.05).Conclusion Dydrogesterone combined with G-CSF can improve blood flow parameters,endometrial thickness and sex hormone levels in TE infertility patients,and help to increase the embryo implantation rate.
6.Effects of exosomes secreted by induced pluripotent stem cells on keratinocyte proliferation and migration

Objective To explore the role of exosomes secreted by induced pluripotent stem cells(iPSC)in promoting the proliferation,invasion and migration of keratinocytes,thereby facilitating wound healing.Methods Extract iPSC-Exos and identify them through transmission electron microscopy,nanoparticle tracking analysis technology,and Western blotting.Purified iPSC-Exos labeled with PKH26 were added to keratinocytes(HaCaT)for the determination of keratinocyte uptake of exosomes.The optimal working concentration of exosomes was assessed using cell counting kit-8(CCK-8),and cells were divided into control group(cell scratch),and experimental group(cell scratch followed by addition of exosomes at the optimal working concentration).Proliferation,migration,and invasion abilities of cells in each group were evaluated using CCK-8,5-ethynyl-2'-deoxyuridine(EdU),scratch assay,and Transwell assay.Results iPSC-Exos exhibit a membranous vesicular structure with a round or elliptical shape,and their diameter is(120.00±25.00)nm.The expression of characteristic surface markers CD9,CD63,and CD81 on iPSC-Exos is positive in the experimental group,while being negative in the control group.HaCaT cells are capable of internalizing iPSC-Exos.After 24 hours of intervention,the scratch healing rates in the control and experimental groups are(25.70±1.07)%and(71.60±12.76)%,respectively.The Transwell invasion cell numbers are(86.33±10.79)and(166.33±24.13)in the control and experimental groups,and the EdU-positive proportions are(45.30±3.17)%and(78.10±6.29)%,respectively.The above indicators in the experimental group show statistically significant differences compared to the control group(all P<0.05).Conclusion The exosomes secreted by pluripotent stem cells can promote the proliferation,migration,and invasion of keratinocytes,thereby indirectly promoting wound healing.
7.Effects of ropivacaine in cognitive dysfunction and synapses after tibial fracture in aged rats

Objective To explore the effects of ropivacaine on cognitive dysfunction and synapses in aged rats after tibial fracture.Methods SD male rats were divided into sham operation group,model group and low,medium,high dose experimental groups.Sham operation group was incised and sutured under local anesthesia,and other four groups underwent open tibial fracture operation.Sham operation group and model group were given sevoflurane anesthesia,low,medium and high dose experimental groups were given 0.5,1.0,2.0 mg·kg-1 ropivacaine on the basis of sevoflurane anesthesia.Open field test and Morris water maze test were performed 7 days after operation;Longa score was evaluated;neurotransmitter levels were detected by kit;and Syn1 in hippocampus of rats was detected by immunohistochemistry.Western blot analysis was used to detected the expression of N-methyl-D-aspartate receptor 2B(NMDAR2B),calmodulin-dependent protein kinase Ⅱ(CaMK Ⅱ)and Syn1.Results The Longa scores of sham operation group,model group and low,medium,high dose experimental groups were 0,(3.50±0.71),(2.80±0.63),(2.20±0.63)and(0.90±0.32)points;the integrated optical density of Syn1 were 0.56±0.09,0.25±0.03,0.34±0.03,0.42±0.03 and 0.50±0.05;the expression of Syn1 protein were 1.08±0.12,0.42±0.05,0.55±0.07,0.72±0.06 and 0.86±0.05;the expression of NMDAR2B protein were 1.28±0.13,0.51±0.07,0.69±0.06,0.84±0.07 and 1.02±0.11;CaMK Ⅱ protein were 0.94±0.08,0.36±0.04,0.50±0.06,0.71±0.06 and 0.86±0.06.There were statistically significant differences between sham operation group and model group(P<0.05);there were significant differences in the above indexes between model group and low,medium,high dose experimental groups(all P<0.05).Conclusion Ropivacaine can improve cognitive dysfunction and synapses in aged rats after tibial fracture.

This experiment aims to study the taste-masking effects of different kinds of corrigent used individually and in combination on ibuprofen oral solution, in order to optimize the taste-masking formulation. Firstly, a wide range of corrigent and the mass fractions were extensively screened using electronic tongue technology. Subsequently, a combination of sensory evaluation, analytic hierarchy process (AHP)-fuzzy mathematics evaluation, and Box-Behnken experimental design were employed to comprehensively assess the taste-masking effects of different combinations of corrigent on ibuprofen oral solution, optimize the taste-masking formulation, and validate the results. The study received ethical approval from the Review Committee of the Beijing University of Chinese Medicine (ethical code: 2024BZYLL0102). The results showed that corrigent fractions and types were screened separately through single-factor experiments. 9.Modified Fuzheng Yiliu Decoction Combined with XELOX Regimen for the Treatment of Postoperative Patients with Advanced Gastric Cancer of Qiand Yin Deficiency Type:A Randomized Controlled Study

Objective To investigate the clinical efficacy of modified Fuzheng Yiliu Decoction(composed of Astragali Radix,Codonopsis Radix,Ligustri Lucidi Fructus,Hedyotis Diffusae Herba,Moutan Cortex,Visci Herba,etc.)combined with XELOX regimen(Oxaliplatin plus Capecitabine)for the treatment of postoperative patients with advanced gastric cancer of qi and yin deficiency type.Methods A total of 80 postoperative patients with advanced gastric cancer of qi and yin deficiency type were randomly divided into the Chinese medicine group and the control group,with 40 cases in each group.Both groups received chemotherapy with XELOX regimen,while the Chinese medicine group was given modified Fuzheng Yiliu Decoction.Three weeks constituted a course of treatment,the medication of Chinese medicine decoction lasted for two weeks or more in each course of treatment,and a total of 8 courses of treatment were performed.The incidence of adverse reactions during chemotherapy was monitored and changes in serum tumor markers of serum carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199)and alpha-fetoprotein(AFP)were observed in the two groups before and after treatment.Moreover,the patients'quality of life was assessed by the scores of Karnofsky's Performance Status(KPS)and World Health Organization Quality of Life Measurement Scale(WHOQOL-100).Long-term follow-up was carried out for the evaluation of the prognostic indicators such as overall survival and one-year and 2-year overall survival rates.Results(1)Patients in the two groups were all followed up,and the median follow-up time was 27 months(95%CI:23.59-27.86).(2)After treatment,the levels of serum CEA and AFP in the Chinese medicine group were significantly lower than those before treatment(P<0.05 or P<0.01),while serum CA199 tended to decrease compared with those before treatment,but the difference was not statistically significant(P>0.05);in the control group,the levels of serum CEA,CA199,and AFP were not significantly decreased after treatment(P>0.05).The intergroup comparison showed that the decrease of serum CEA,CA199 and AFP levels in the Chinese medicine group was significantly superior to that in the control group(P<0.05 or P<0.01).(3)The adverse reactions during chemotherapy in the two groups mainly involved bone marrow suppression,gastrointestinal reactions and liver function abnormalities,etc.The incidences of all adverse reactions in the Chinese medicine group tended to be lower than those in the control group,but the differences were not statistically significant(P>0.05).(4)After treatment,the KPS scores of patients in both groups were improved compared with those before treatment(P<0.01),and the improvement in the Chinese medicine group was significantly superior to that in the control group(P<0.01).(5)After treatment,the scores of the four dimensions of WHOQOL-100 such as health status,mobility,life feelings,and other activities of daily life in the Chinese medicine group were significantly improved compared with the pre-treatment(P<0.05),whereas there was no significant improvement in the control group(P>0.05).The intergroup comparison showed that the improvement of the scores of each dimension of the WHOQOL-100 in the Chinese medicine group was significantly superior to that in the control group(P<0.05 or P<0.01).(6)The median survival in the Chinese medicine group was 29.0 months(95%CI:25.95-31.70)and that in the control group was 22.0 months(95%CI:19.67-25.58),indicating that the median survival was significantly prolonged in Chinese medicine group(P<0.01).The one-year and 2-year postoperative survival rates were 97.5%and 77.5%in the Chinese medicine group and 92.5%and 47.5%in the control group,respectively.The intergroup comparison showed that the one-year and 2-year postoperative survival rates in the Chinese medicine group were higher than those in the control group(P<0.01).Conclusion Modified Fuzheng Yiliu Decoction can effectively alleviate the adverse reactions during adjuvant chemotherapy for postoperative patients with advanced gastric cancer of qi and yin deficiency type,improve the quality of life of patients,and prolong the survival time of patients.
10.Efficacies of proximal femoral nail anti-rotation internal fixation in different body positions on elderly unstable femoral intertrochanteric fractures

Objective To investigate the efficacies of proximal femoral nail anti-rotation(PFNA)internal fixation in traction bed supine position and non-traction bed lateral position in the treatment of elderly unstable femoral intertrochanteric fractures.Methods The clinical data of patients with unstable femoral intertrochanteric fractures treated with PFNA internal fixation in our hospital were retrospec-tively analyzed,41 patients received treatment in traction bed supine position were included in the supine position group,and 55 patients treated received treatment in non-traction bed lateral position were included in the lateral position group.The perioperative related indicators,surgical reduction,hip Harris score,and incidence of complications in the two groups were analyzed.Results The operation time and incision length of patients in the lateral position group were shorter than those in the supine position group,and the intraoperative blood loss and fluoroscopy times were less than those in the supine position group,with statistically significant differences(P<0.05).There was no significant difference in the anesthesia mode,blood transfusion or hospital stay of patients between the two groups(P>0.05).There was no significant difference in the incidence of postoperative complications of patients between the two groups(P>0.05).There was no significant difference in neck-shaft angle,tip-apex distance or hip Harris score of patients between the two groups(P>0.05).Conclusion PFNA internal fixation in traction bed supine position and non-traction bed lateral position have the same effect in the treatment of elderly unstable femoral intertrochanteric fractures,while the non-traction bed lateral position for treatment has more advantages in shortening operation time,decreasing intraoperative blood loss,and reducing radiation exposure.
