To establish a method for detecting Toxoplasma gondii based on recombinant polymerase amplification(RPA)technology and apply it to clinical sample validation of pet cats.Using the 529 repeat sequence of the Toxoplasma gondii gene as the target gene sequence, primers and probes were designed, and the Rep-529 recombinant plasmid was constructed as the standard.A fluorescent RPA reaction system was established.Dilute the plasmid standard 10 times to different concentrations as the detection template for sensitivity testing;Specific testing was conducted using genomic DNA from several parasitic spe-cies,including Toxoplasma gondii,Cryptosporidium,Neosporidium,Trichinella spiralis,Giardia flagellata,Babesia bo-vis and Theileria annulata as templates;Simultaneously, fluorescence RPA and RT-PCR were used to detect 52 positive and 40 negative cats clinical samples, and the coincidence rate of the detection results of the two methods were compared and ana-lyzed.The RPA reaction system was successfully established using PTRep recombinant plasmid as the standard,ToxD-F/ToxD-R as the primer, and RepD-P as the fluorescent probe. The reaction temperature was constant at 39 ℃,the reaction time was 30 minutes, and the detection sensitivity was 1 copy/μL.There is no significant cross reaction with parasites such as Cryptosporidium,Neosporidium,Trichinella spiralis,Giardia,Babesia bovis and Theileria annulata, and the specificity is good.A total of 92 clinical fecal samples from cats were tested, and the positive coincidence rate of fluorescence RPA detection method was higher than that of conventional RT-PCR method(98.08%vs.82.69%),and the difference of the positive rate was not statistically significant(X2=1.392,P>0.05).The fluorescence RPA detection method for Toxoplasma gondii suc-cessfully established in this study has the characteristics of being fast, sensitive, specific, accurate, and reliable. It can be used as a rapid clinical detection kit for Toxoplasma gondii in cats and other animals, providing new technical support for the subsequent epidemiological monitoring and precise clinical diagnosis of toxoplasmosis in cats, other animals, and humans in the future.

Objective To investigate the stability and reproducibility of the treatment fractions during the intensity-modulated radiotherapy(IMRT)for breast cancer and the effect of respiratory motion on the dose irradiation of breast cancer radiotherapy by comparing the results of breast cancer dosimetric verification based on fractionated images by an electronic portal imaging device(EPID).Methods A total of 28 IMRT patients admitted to some hospital from January to June 2023 were grouped according to the pathological results and effects of respiratory motion on the accuracy of radiotherapy during clinical treatment, including 14 cases in a breast group and 14 cases in a non-breast group with 8 ones of head and neck tumors,5 ones of esophageal cancer and 1 case of cervical cancer. All the patients underwent a scan with cone beam computed tomography(CBCT)before the first radiotherapy, and image registration was carried out with a positioning CT.An EPID was used to acquire transmission dose images of 10 fractions of radiotherapy, and γ analysis was performed using the RIT 113 QA software to compare the images of the subsequent 9 fractions with those of the first fraction, with the images of the first fraction of radiotherapy as the baseline values.Absolute maximum dose normalization was implemented under the condition of 10%dose assessment threshold, and the γ-pass rates under the 3 criteria of 2%/2 mm,3%/2 mm and 3%/3 mm were counted separately. The fraction dose verification results of the 28 patients were divided into 3 treatment phases of 2-4 times(T1),5-7 times(T2)and 8-10 times(T3)to analyze the stability of dose irradiation during the radiotherapy.SPSS 22.0 software was used for statistical analysis. Results Under the condition of 10%dose assessment threshold, the breast and non-breast groups had the γ-pass rates being(95.80±2.65)%and(94.60±6.59)%under the 2%/2 mm criterion and(98.46±1.31)%and(97.50±3.30)%under the 3%/2 mm criterion respectively, and the differences were statistically significant(all P<0.05).Under the assessment criteria of 2%/2 mm,3%/2 mm and 3%/3 mm,the breast group had the γ-pass rates of fractions of treatment significantly lower than those of the non-breast group(all P<0.05),while the γ-pass rates showed no significant differences at T1,T2 and T3 treatment phases(all P>0.05).Conclusion EPID fraction images contribute to evaluating IMRT accuracy effectively.IMRT has high stability and reproducibility during the treatment cycle, while respiration may result in dose deviation during the fraction radiotherapy for breast cancer, and optical surface tracking technology or active breathing control technology is suggested to be involved in to relieve dose deviation.[Chinese Medical Equipment Journal,2025,46(6):54-58]

High mobility group box protein 1(HMGB1)is one of the most widely expressed protein member in the HMGs family, which is well known for its involvement in the body inflammatory response.Previous researches have found that it plays a significant role in cell migration, immune identification and neuroprotection.Spinal cord injury is a disease that causes severe damage to the nervous system, and neural circuits are disrupted after a spinal cord injury, which leads to many conditions including ischemia and hypoxia, inflammatory responses, demyelinating lesions, and glial scar formation that are detrimental to nerve regeneration and repair, making it one of the most difficult diseases to treat in the modern spinal surgery field.HMGB1 is upregulated after spinal cord injury, thereby regulating neuroinflam-matory responses, and participating in the neuronal apoptosis, promoting neuronal regeneration, and inducing neural stem cell differentiation and migration, which plays an important role in the process of neural function recovery. This paper summarizes the structure and function of HMGB1,as well as its role in spinal cord injury,in order to provide direction for founding therapeutic target for neurological function recovery after spinal cord injury.

Background and Aims:Acute inferior vena cava thrombosis(IVCT)commonly occurs secondary to inferior vena cava filter(VCF)implantation. If not promptly treated,it may lead to serious complications such as bilateral lower limb swelling and pulmonary embolism and can also reduce the likelihood of successful filter retrieval.Percutaneous mechanical thrombectomy(PMT)and catheter-directed thrombolysis(CDT)are currently the main interventional treatments for IVCT,but comparative studies evaluating their efficacy and safety remain limited. This study was to conduct a prospective randomized controlled trial to compare the clinical efficacy and safety of AngioJet mechanical thrombectomy versus conventional CDT in the treatment of acute IVCT and to explore factors influencing filter retrieval rates, thereby providing evidence-based guidance for clinical decision-making.Methods:From January 2022 to December 2024,patients diagnosed with acute IVCT following VCF implantation were prospectively enrolled at the Department of Vascular Surgery,Beijing Jishuitan Hospital,Capital Medical University.Patients were randomly assigned to either the CDT group(n=46)or the PMT group(n=48)according to the interventional procedure used. The two groups were compared in terms of filter retrieval rates, thrombus clearance outcomes, operative time, thrombolytic drug dosage, and incidence of complications.Logistic regression analysis was used to identify factors associated with primary filter retrieval. Results:A total of 94 patients were enrolled, with 46 in the CDT group and 48 in the PMT group.Compared to the CDT group, the PMT group demonstrated a significantly higher primary filter retrieval rate(77.1%vs.43.5%),grade Ⅲ thrombus clearance rate(70.8%vs.37.0%),and better postoperative thrombus scores.Additionally, the PMT group required lower urokinase doses and shorter thrombolysis duration(all P<0.05).The overall filter retrieval rate and 3-month IVC patency were similar between groups, both exceeding 93%.Regarding safety, the CDT group had a higher incidence of catheter-related infections and medical adhesive-related skin injury, while vagal reflex symptoms were more frequent in the PMT group.Logistic regression analysis identified thrombus clearance rate as an independent factor significantly associated with primary filter retrieval in the PMT group(OR=190.773,P<0.05).Conclusion:Compared to CDT,AngioJet mechanical thrombectomy combined with manual aspiration achieves higher thrombus clearance and primary filter retrieval rates in the treatment of acute IVCT while also reducing thrombolysis duration and drug dosage. However, attention should be paid to the increased risk of vagal reflex symptoms. There was no significant difference between the two groups in secondary filter retrieval rates or long-term IVC patency. The choice of intervention should be based on the patient's condition, timing of filter retrieval, and individualized clinical considerations.
