1.Diagnosis and Treatment of a Case of Spironolactone-Associated Asymptomatic Hyperuricemia After Renal Transplantation

Objective To explore the identification method,pathogenesis,clinical characteristics and individualized pharmacotherapy of asymptomatic hyperuricemia after renal transplantation.Methods The pharmacist was on duty at the organ transplant outpatient clinic.During this time,they analyzed and sorted out the medications,identified and differentiated a case of asymptomatic hyperuricemia related to spironolactone in a patient who had undergone a renal transplant,and provided comprehensive care throughout the entire process.Results The asymptomatic hyperuricemia in this patient might be associated with spironolactone,and the adverse reactions of the patient were alleviated by pharmacists through optimizing clinical treatment.Up to now,no hyperuricemia occurred.Conclusions Pharmacists are required to collaborate closely with clinicians to establish medication profiles for patients under long-term follow-up and to closely monitor and evaluate drug-related adverse reactions.Additionally,they should assess the renal function and immune status of transplant recipients promptly and formulate individualized treatment plans in order to enhance the long-term survival of both the transplanted kidneys and the recipients.
2.Analysis of the Application Effect of Quantitative Fecal Immunochemical Test in Colorectal Cancer Opportunistic Screening in Sichuan

[Purpose]To analyze and evaluate the application effect of quantitative fecal immuno-chemical test(FIT)in the opportunistic screening of colorectal cancer(CRC)in health examination population.[Methods]10 025 healthy individuals who completed quantitative FIT at the Health Management Cancer of Sichuan Cancer Hospital from January 1,2021 to December 31,2023 were included.Participants with positive FIT results were recommended for diagnostic colonoscopy.Quantitative FIT positivity rate,colonoscopy screening compliance rate,and colorectal lesion de-tection rate in the queue population were analyzed,the detection rate of colorectal lesions and colonoscopy resource load between quantitative FIT positive and negative colonoscopy screening groups at all levels were compared and the diagnostic efficacy of quantitative FIT for non progres-sive lesions and progressive lesions were explored.[Results]The positive rate of quantitative FIT was 6.24%(95%CI:5.77%~6.71%).The compliance rate of colonoscopy screening was 20.93%(95%CI:17.74%~24.12%).Among 284 colonoscopy patients,7 cases were detected with CRC,21 cases with advanced adenomas,52 cases with non advanced adenomas,40 cases with non adenomatous polyps,51 cases with other benign lesions,and 113 cases without abnormalities.The detection rates of non advanced adenomas,advanced adenomas,colorectal cancer,non pro-gressive lesions and progressive lesions in the quantitative FIT positive group were significantly higher than those in the quantitative FIT negative group.The detection rates of those without obvi-ous abnormalities were significantly lower than those in the quantitative FIT negative group,and the differences were statistically significant(all P<0.05).The resource burden of colonoscopy in the quantitative FIT positive and negative groups was 5.46(95%CI:1.56~9.34)and 38.25(95%CI:30.55~45.95),respectively.The level of FIT measurement gradually increases with the severity of the lesion,and the difference between groups is statistically significant(H=48.308,P<0.001).Quan-titative FIT has high sensitivity and negative predictive value for non progressive lesions and pro-gressive lesions,with values of 71.15%,85.71%and 90.20%,97.39%,respectively.The AUC values for quantitative FIT screening of non progressive lesions and progressive lesions were 0.653(95%CI:0.572~0.734,P=0.001)and 0.720(95%CI:0.631~0.808,P<0.001),respectively.[Conclusion]Quantitative FIT,as a suitable technique for CRC opportunistic screening in health check-up population,has good diagnostic efficacy for non progressive lesions and progressive le-sions.Performing colonoscopy on FIT positive individuals can improve lesion detection rates while reducing colonoscopy resource consumption,aligning with China's healthcare resource landscape.

5.Effect of information-motivation-behavior skill model intervention on quality of life in patients with coronary heart disease

Objective:To analyze the application value of information-motivation-behavior(IMB)application model in patients with coronary heart disease(CHD).Methods:This randomized controlled study enrolled 110 CHD patients admit-ted to the Second Affiliated Hospital of Chinese PLA Air Force Military Medical University between March 2021 and March 2023.They were divided into intervention group(n=55,IMB model intervention)and control group(n=55,rou-tine management intervention).Both groups were intervened for 2 months.The aerobic exercise indicators,heart func-tion,coagulant function,self-management behavior,quality of life and medical compliance were compared between the two groups.Results:Compared with patients in control group,those in intervention group had significant higher 6min walking distance(6MWD)[(459.17±32.08)m vs.(429.88±37.25)m],maximal oxygen uptake(VO2max)[(69.89±3.71)L/minvs.(63.91±4.56)L/min],prothrombin time(PT)[(16.79±3.40)s vs.(13.26±3.21)s],dimensional scores of emotional management[(18.25±2.05)points vs.(14.70±1.73)points],daily life management[(48.52±3.09)points vs.(40.55±3.05)points],medical management of disease[(30.36±2.57)points vs.(25.21±2.40)points]of coronary self-management scale(CSMS)and quality of life questionnaire core 30(QLQ-C30)score[(87.89±4.73)points vs.(75.78±4.91)points](P<0.001 all),and significant lower left ventricular end-systolic volume in-dex(LVESVI)[(35.66±0.76)ml/m2 vs.(39.80±0.79)ml/m2],left ventricular end-diastolic volume index(LV-EDVI)[(45.66±1.64)ml/m2 vs.(56.29±2.10)ml/m2],Ddimer(D-D)[(373.47±23.20)g/L vs.(494.03±26.10)g/L],fibrinogen(Fg)[(2.70±0.81)mg/dl vs.(3.71±0.89)mg/dl](P<0.001 all).Medical compliance rate(89.09%vs.63.64%)in intervention group was significantly higher than that of control group(P=0.002).Conclu-sion:IMB model could significantly improve heart function and coagulation function,and significantly improve self-man-agement behavior,quality of life and compliance in CHD patients.
6.Design of sinusoidal excitation phased array ultrasound focusing system

Objective To design a phased array ultrasonic focusing control system based on sinusoidal signal excitation in order to avoid the introduction of high-frequency interference components into the ultrasonic transducer and improve the electronic focusing performance of phased array ultrasound.Methods The system designed mainly used the high-speed field programmable gate array(FPGA)chip combined with the high-speed digital to analog converter(DAC)to realize synchronous output and control of multi-channel sinusoidal signals.There were 8 sinusoidal excitation emission modules based on FPGA and DAC and one sinusoidal ultrasonic excitation control module based on ZYNQ MPSoC involved in the hardware part of the system,in which the emission modules generated multi-channel sinusoidal excitations and timing control of sinusoidal signals within the module and the control module was responsible for controlling the triggering timing between each sinusoidal excitation transmitter module.The system developed had its software designed with MATLAB App Designer to improve the human-computer interaction experience.Performance verification was carried out for the system by testing the output waveform,inter-channel delay error and focused sound field of each channel.Results The system developed achieved generation and timing control of 64-channel sinusoidal ultrasound excitations,with the output channel main frequency being 0.5 MHz,amplitude within 0 and±12.5 V and the inter-channel delay errors not higher than 26.0 ns;a focused sound field with a focal spot diameter of 4.2 mm(-3 dB)at a depth of 50 mm was obtained when the system was applied to driving a 64-array phased-array transducer.Conclusion The system designed is capable of realizing ultrasonic electron focusing under sinusoidal excitation,which helps to improve the focusing resolution of non-invasive and precise deep brain stimulation techniques relying on the accuracy of ultrasonic focusing such as transcranial magneto-acoustic stimulation(TMAS)and transcranial ultrasonic stimulation(TUS).[Chinese Medical Equipment Journal,2025,46(5):14-20]

9.Mechanism of action of D-limonene on steatosis in primary hepatocytes based on AMPK/ACC/CPT1A signaling pathway

Aim To explore the effects of D-limonene on the steatosis of primary mouse hepatocytes and its potential mechanism of action.Methods Oleic acid-induced steatosis in primary mouse hepatocytes was used as a model to observe the effects of D-limonene on cell viability,cellular lipid content,and intracellular expression of proteins such as AMP-activated protein kinase(AMPK),acetyl-coenzyme A carboxylase 1(ACC1),and carnitine palmitoyl transferase 1A(CPT1A).Results It was found that a low dose of D-limonene could effectively enhance the viability of primary mouse hepatocytes.When oleic acid at a con-centration of 300 μmol·L-1 successfully induced steatosis in primary mouse hepatocytes,D-limonene re-duced the lipid content of the cells,and D-limonene up-regulated the cellular AMPK expression level,down-regulated the cellular ACC1 and fatty acid synthetase(FAS)expression levels,which in turn promoted the overexpression of CPT1A.Conclusions D-limonene has the effect of reducing lipid deposition in primary mouse hepatocytes,and the mechanisms may be related to the activation of AMPK,the inhibitions of ACC1 and FAS,and the up-regulation of CPT1A protein expres-sion level.

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
