Medical therapy and surgical intervention are the two primary approaches for treating myocardial bridge. However, there remains controversy regarding the use of coronary artery bypass grafting(CABG)and myocardial bridge unroofing. Here,we report a case of myocardial infarction following CABG in a patient with a myocardial bridge. The patient was admitted to Lanzhou First Peopie's Hospital with persistent chest pain, chest tightness, and shortness of breath lasting 2 hours.Physical examination revealed no significant abnormalities.Electrocardiography(ECG)indicated extensive anterior wall myocardial infarction.Laboratory findings showed myoglobin levels of 140.1 ng/ml and troponin Ⅰ levels of 2.59 ng/ml,with no other significant abnormalities. The initial diagnosis was acute extensive anterior wall myocardial infarction.Emergency coronary angiography revealed a myocardial bridge in the mid-segment of the left anterior descending artery(LAD).Emergency CABG using the left internal mammary artery to the LAD was performed, leading to symptomatic improvement, and the patient was discharged in stable condition. However, the patient experienced a recurrent myocardial infarction seven years post-surgery and received secondary preventive medical therapy. The patient is currently under ongoing follow-up care.CABG is an effective treatment for myocardial bridge. However, based on the case reported in this study,we recommend careful evaluation of whether a patient may benefit from CABG.

Objective:To analyze the characteristics and trends of chronic obstructive pulmonary disease (COPD) burden attributable to non-optimal temperature in China and globally from 1990 to 2021.Methods:This retrospective cross-sectional study utilized data from the Global Burden of Disease (GBD) 2021 database to extract indicators including death numbers, disability-adjusted life years (DALYs), age-standardized mortality rate (ASMR), and age-standardized DALY rate for COPD due to non-optimal temperature in China and globally. The Joinpoint regression model was applied to analyze temporal trends and identify inflection points. Disease burden distribution characteristics were analyzed by gender and age groups. The Bayesian Age-Period-Cohort (BAPC) model was used to predict the disease burden from 2022 to 2035.Results:Compared with 1990, in 2021 the global ASMR and DALY rate for COPD decreased by 44.59% and 50.02%, respectively, while China saw larger reductions of 69.61% and 72.57%. However, the absolute number of deaths and DALYs increased globally by 31.55% and 12.23%, respectively; in China, the number of deaths remained stable while DALYs decreased by 21.59%. The Joinpoint regression model showed statistically significant differences in trend inflection points between China and the world. The disease burden was higher in males than in females and was significantly higher in people aged≥65 years. The projected burden of COPD mortality and DALYs due to non-optimal temperature is expected to continue rising in both China and the world from 2022 to 2035. By 2035, China′s ASMR and DALY rate are predicted to increase to 21.32/100 000 and 284.68/100 000, respectively, and the global ASMR and DALY rate are predicted to rise to 7.88/100 000 and 122.16/100 000.Conclusions:From 1990 to 2021, the burden of COPD due to non-optimal temperature showed a declining trend in both China and globally, but the absolute burden continued to rise, particularly among elderly males and advanced age populations. The burden in both China and globally is projected to continue increasing through 2035, with a higher growth rate in China than globally.

Atrial fibrillation (AF) is the most common arrhythmia in clinical practice. It has a high prevalence and poor prognosis. The application of antiarrhythmic drugs and even surgery cannot completely treat the disease, and there are many sequelae. AF can be classified into the category of "palpitation" in Chinese medicine according to its symptoms. Acupuncture has a significant effect on AF. The authors find that an important mechanism of acupuncture in AF treatment is to regulate the cardiac vagus nerve. Therefore, this article intends to review the distribution and function of vagus nerve in the heart, the application and the regulatroy effect for the treatment of AF.

Progress in Modern Biomedicine 2025;25(14):2378-2390
Purine nucleotides, fundamental constituents of nucleic acids, represent essential organic molecules within cells. These molecules serve critical roles not only in nucleic acid biosynthesis, the transmission of genetic information and signal transduction cascades, but also function as precursors for the synthesis of energy substances.Consequently, purine metabolism is subject to stringent regulatory mechanisms that preserve cellular homeostasis and function integrity.Emerging evidence demonstrates that dysregulated purine metabolism exhibits significant associations with tumorigenesis and existing anti-tumor drugs have developed varying degrees of drugs resistance during therapy.Elucidating the mechanistic links between purine metabolic aberrations and tumorigenesis holds dual significance:it enhances our understanding of cancer pathophysiology while potentially informing novel therapeutic paradigms. This review systematically examines the pathophysiological contributions of key purine metabolic enzymes and intermediate metabolites to malignant transformation, while critically evaluating emerging anti-tumor strategies that target purine metabolism pathways.
