1.Relationship between coping style and psychological distress in stroke patients:the pathway of resourcefulness

Background Stroke patients commonly experience varying degrees of psychological distress.Previous studies have shown that the psychological distress of stroke patients is closely related to their coping styles and resourcefulness.However,the path of wisdom in coping style and psychological pain of stroke patients is still unclear.Objective To explore the path of resourcefulness between coping styles and psychological distress in stroke patients,so as to provide insights for interventions targeting psychological distress.Methods A total of 350 patients who met the diagnostic criteria for stroke according to the Diagnostic Criteria of Cerebrovascular Diseases in China(version 2019)and were admitted to the neurology departments of two tertiary grade-A hospitals in Luzhou City from January to May 2023 were selected as the research subjects.Basic information was collected using a self-designed questionnaire,and Resourcefulness Scale(RS),Medical Coping Mode Questionnaire(MCMQ)and Distress Thermometer(DT)were used for assessment.Pearson correlation analysis was conducted to examine the correlation between the scores of each scale.Model 4 of the Process 3.4.1 program was employed to analyze the path of resourcefulness between coping styles and psychological distress,and the Bootstrap method was used to test the mediation effects.Results The DT score showed a negative correlation with the confrontation and RS scores(r=-0.581,-0.716,P<0.01),and a positive correlation with the avoidance and resignation scores(r=0.432,0.758,P<0.01).The confrontation score was positively correlated with RS score(r=0.700,P<0.01),while the avoidance and resignation scores were negatively correlated with RS score(r=-0.460,-0.566,P<0.01).Resourcefulness might serve as a mediating path between confrontation,avoidance,resignation,and psychological distress,with the effect value of-0.359,0.203 and 0.183,accounting for 81.78%,62.65%and 27.07%of the path effect,respectively.Conclusion The coping styles of stroke patients can directly affect their psychological distress,and can also indirectly influence it through resourcefulness.Resourcefulness may play a mediating role between coping styles and psychological distress.
2.Medical narrative competence and its influencing factors among nurses in Luzhou

Objective:To explore the current status and influencing factors of medical narrative competence among nurses in Luzhou, offering theoretical insights for improving nurses′ narrative competence.Methods:This study was a cross-sectional survey. From July 2021 to January 2022, using convenience sampling method, 267 nurses from Zhongshan Area of Affiliated Hospital of Southwest Medical University, Kangjian Area of Affiliated Hospital of Southwest Medical University and the Traditional Chinese Medicine Hospital Affiliated to Southwest Medical University were investigated by the general information questionnaire, Narrative Competence Scale, Nurses′ Perceived Professional Benefits Questionnaire And Perceived Social Support Scale.Results:The total scores of narrative competence of the 267 nurses was (142.25 ± 19.03), nurses′ perceived professional benefits was (137.16 ± 15.89) and perceived social support was (65.86 ± 10.38). The results of multiple linear regression showed that nurses′ perceived professional benefits and perceived social support were the important factors affecting their narrative ability ( t = 5.09 and 5.95, both P<0.05), which could account for 35.7% of the total variation of narrative competence. Conclusions:Nurses have narrative competence at low level in Luzhou. Managers should pay more attention to nurses who have poor level of perceived professional benefits and perceived social support, so as to keep the nurses′ mental health and improve nurses′ narrative competence by taking corresponding measures.
3.Long-pulsed 1064 nm Nd: YAG laser combined with terbinafine against chromoblastomycosis caused by and the effect of laser therapy in a Wistar rat model.

We report a case of chromoblastomycosis caused by , which was successfully treated by long-pulsed 1064 nm Nd: YAG laser combined with terbinafine. A 60-year-old man was admitted for the presence of a 30 mm×40 mm erythematous plaque on the dorsum of his right hand for about 10 months without any subjective symptoms. Both microscopic examination and tissue biopsy of the lesion showed characteristic sclerotic bodies of chromoblastomycosis. Lesion tissue culture on SDA at 26 ℃ for 2 weeks resulted in a black colony, and slide culture identified the isolate as Fonsecaea species. ITS sequence analysis of the isolate showed a 99% homology with strain KX078407. The susceptibility of the isolate to 9 antifungal agents was determined using the microdilution method according to the guidelines of CLSI M38-A2 protocol, and terbinafine showed the lowest MIC (0.125 μg/ml). We subsequently established a Wistar rat model of chromoblastomycosis using the clinical isolate and treated the rats with long-pulsed 1064 nm Nd: YAG laser (pulse width of 3.0 ms, fluence of 24 J/cm, spot size of 3 mm, frequency of 4 Hz, repeated 3 times at an interval of 30 s) twice a week for a total of 8 sessions. Although the laser treatment alone was not able to eliminate the fungi, histopathological examination showed the aggregation of numerous lymphocytes in the local affected tissue, indicating an immune response that consequently facilitate the regression of the lesion. The patient was successfully treated by long-pulsed 1064 nm Nd: YAG laser once a week combined with terbinafine (0.25 /bid) for 8 weeks, and follow-up for 20 months did not reveal any signs of recurrence.

4.Online health education by family physician based on the wearable devices

Objective To explore the effect of online health education by family physician using the wearable devices, and provide the basis in theory and practice to improve the level of primary health care in community. Methods A total of 132 community residents (78 for male and 36 for female, all of them are WeChat user) with hypertension in one residential area who had regularly participated in the activity of volunteer blood pressure measurement organized by community health service center in wujiaochang street from November 2015 to February were chosen as the participants. The residents who wore the wearable device given by the community hospital uniformly were set as the study group (60 people), while the remaining 72 residents were set as the control group. Both groups received the daily online health education provided by the family physicians through their smartphone. We compared the diversity of health cognition, controlling behavior and controlling results to blood pressure of both groups before and after the studyrelatively, then we comprehensively analyzed the effect of use of wearable devices in family physicians ' online health education. SPSS 18.0 was used for data analysis. The categorical data of two groups were compared with chi-square test and the comparison of continuous data was performed with t test. Results Totally 114 community residents were qualified in the study, and the initial backgrounds of study group and control group had no significant difference. After receiving the online health education information for three months, both groups achieved improvement on the cognition and related self-control behavior of hypertension except for several items, and the study group did better than control group in most items. The physical examination showed that the systolic blood pressure of the study group after the intervention [(131.46 ± 12.89) mmHg] (1 mmHg=0.133 kPa) was lower than before [(137.24 ± 12.27) mmHg] and the difference was significant (t=9.2, P<0.01); the diastolic blood pressure of the study group after the intervention [(78.29±8.91) mmHg] was lower than that of before intervention [(80.75±10.25) mmHg] and the difference was significant (t=2.3, P<0.05). Meanwhile, after intervention the systolic blood pressure of study group [(131.46 ± 12.89) mmHg] was lower than that of control group [(133.27 ± 12.7) mmHg] and the intervention was significant (t=2.1, P<0.05). Conclusion Long-term use of wearable devices help family physicians improve the effect of health education, consequently the community hospitals are responsible to enhance the input and management of the informatization of family physicians' service, and assist them to broaden the form and content of health education.
5.One case of adult Kawasaki disease.

Kawasaki disease is far more frequent in children than in adults. The pathogenesis of Kawasaki disease is unknown, but it involves changes to the coronary artery and other diverse clinical manifestations. There are currently no specific laboratory diagnostic indexes, and especially since the disease is rare in adults, so it is extremely easy to misdiagnose or to overlook entirely. Our retrospective analysis of an diagnosis of and treatment for Kawasaki disease in an adult provides a guide to clinical doctors in terms of understanding Kawasaki disease, early diagnosis of it, and improved prognosis.

6.Autologous granular fat transplantation in facial rejuvenation

Objective To investigate methods and experience of autologous granular fat transplantation by correction of aging face. Methods 178 cases of aging face subjects were treated by liposuction. Then autologous granular fat was harvested by liposuction,centrifuged and purified, and injected into marked areas of aging face. The injection process should be controled by multilevel and multiple tennel, in order to supplement the loss of facial soft tissue volume and improve the aging face. Results The follow-up period ranged from 3 months to 3 years. All the subjects had a satisfactory results with no obvious complications such as fat liquefaction, infection and ulceration. The effect of autologous granular fat transplantation was obvious and permanent. Conclusions Autologous granular fat transplantation is a safe and effective surgical technique for facial soft tissue augmentation that can effectively improve aging face and worthy of clinical application.
