1.Effects of preoperative coagulation status on the dynamic changes of postoperative coagulation indices in ATAAD patients and the different changes by gender

Objective To investigate the effects of preoperative coagulation status on the dynamic changes of postoperative coagulation indices in patients with acute type A aortic dissection(ATAAD)and the differences between different genders.Methods We recruited 337 patients with ATAAD diagnosed by CT angiography who visited The First Affiliated Hospital of Xi'an Jiaotong University from September 2020 to January 2022 and divided them into fibrinogen(FIB)≥2 g/L group and FIB<2 g/L group.We compared the differences in changes of perioperative coagulation indices of patients in the two groups and differences in the changes between different genders.Results The FIB<2 g/L group had a larger proportion of male patients,more intraoperative blood transfusion(P=0.020),higher re-operation rate(P=0.035),significantly higher preoperative D-dimer(P<0.001)and FIB degradation product levels(P<0.001),but lower platelet counts and plateletcrit(both P<0.001)than the control group.Platelet counts and plateletcrit for five consecutive postoperative days were lower in the FIB<2 g/L group than in the control group(both P<0.001).D-dimer and FIB degradation products were significantly higher in the FIB<2 g/L group than in the control group before operation and in the 5-day postoperative period.The FIB<2 g/L group showed a decreasing trend of D-dimer and FIB degradation products in the postoperative period(both P<0.001);however,there was no significant change in the control group.Preoperative FIB was lower in male patients than in female patients(P<0.001),and the proportion of low FIB was significantly higher than in females(P=0.003).Preoperative D-dimer(P=0.004)and FIB degradation products(P=0.003)were higher in male group.In male patients,postoperative D-dimer and FIB degradation products were higher in patients with preoperative FIB<2 g/L at 5 days postoperatively,but there was no statistically significant difference in the changes of D-dimer or FIB degradation products between the two groups in female patients.Conclusion The preoperative coagulation status of ATAAD patients may exert some effects on the dynamic changes of postoperative coagulation indices,which is more pronounced in men.

3.Bafa in Tianhui medical bamboo slips.

In 2012, a total of eight medical books were compiled from Tianhui medical bamboo slips unearthed in Chengdu, Sichuan, China, which for the first time, recorded bafa (ba technique) and acupuncture technique. Bafa is only found in Tianhui medical bamboo slips. Based on the records in medical bamboo slips, it is believed that the essential theory of bafa refers to an outlook of life for the harmony between form and spirit, meaning "the heart mind is the monarch of form". It is the initial view of physique centered on five-zang organs. In clinical application, bafa is used for prevention of diseases in four seasons, specially for wubi (five-bi disorders) and wufeng (five-wind disorders), covering a extensive parts of the body. Compared with the stone technique, "bafa must work on meridian points, while stone on meridians". Bafa is often used for deficiency syndrome, while stone technique is for excess syndrome. Hence, these two types of technique should be applied cautiously. It is speculated that bafa is an external therapeutic method based on the theory of meridian medicine in the Han Dynasty.

4.Predictive value of serum milk fat globule-EGF factor 8 protein and leucine rich alpha-2 glycoprotein 1 levels for acute kidney injury in patients with severe pancreatitis

Objective To explore the predictive value of serum milk fat globule-EGF factor 8 protein(MFG-E8)and leucine rich alpha-2 glycoprotein 1(LRG1)for acute kidney injury(AKI)in patients with severe pancreatitis.Methods A study was conducted on 100 patients with severe pancreatitis admitted to our hospital from January 2020 to August 2023.They were grouped into AKI group(43 cases)and non AKI group(57 cases)based on whether AKI occurred within 48 hours.Enzyme linked immunosorbent assay(ELISA)was applied to determine the levels of serum MFG-E8 and LRG1 in each group.Spearman correlation was applied to analyze the correlation between serum MFG-E8,LRG1 expression levels and Acute Physiological And Chronic Health Evaluation Ⅱ(APACHE Ⅱ).Logistic regression was applied to analyze the factors affecting AKI in patients with severe pancreatitis.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum MFG-E8 and LRG1 for AKI in patients with severe pancreatitis.Results Compared with the non AKI group,the expression level of LRG1 in the AKI group was obviously increased(P<0.05),while the expression level of MFG-E8 was significantly reduced(P<0.05).There was a negative correlation between serum MFG-E8 level and APACHE Ⅱ score in severe pancreatic patients(r=-0.444,P<0.05),while there was a positive correlation between serum LRG1 level and APACHE Ⅱ score(r=0.655,P<0.05).The results of multivariate Logistic regression analysis showed that serum MFG-E8 was a protective factor for AKI in patients with severe pancreatitis(P<0.05).LRG1,serum creatinine,and uric acid were all risk factors for AKI in patients with severe pancreatitis(P<0.05).The combination of serum MFG-E8 and LRG1 in predicting AKI in patients with severe pancreatitis was superior to MFG-E8 and LRG1 alone in predicting AKI(Zcombination-MFG-E8=2.952,P=0.003;Zcombination-LRG1=2.385,P=0.017).Conclusion The serum MFG-E8 expression level is significantly reduced and LRG1 expression level is significantly increased in severe pancreatitis patients with AKI.The combination of the two can be used for predicting AKI in severe pancreatitis patients.

6.Xinfukang drug-containing serum on bone marrow stem cell transcription and secretion of SDF-1α

Objective To clarify the effects of Xinfukang containing-serum on stromal cell-derived factor-1α (SDF-1α) translation and protein secretion of bone marrow stem cells (BMSCs).Methods BMSCs were isolated and amplified using bone marrow culture method,and were identified by flow cytometry.mRNA and protein secretion of SDF-1α were detected by quantitative PCR (q-PCR) and enzyme linked immunosorbent assay (ELISA),respectively.Results The expression of SDF-1α mRNA were significantly increased after 72 h in drug-containing serum,and SDF-1α mRNA in the experimental group was approximately 200 times as that in the control group (P<0.05).Secretion of SDF-1 α in the experimental group (277.561 1 ± 15.651 8) pg/ml was nearly doubled compared with that in the control group (153.107 1±14.765 1) pg/ml (P<0.05).Conclusions BMSCs from whole bone marrow adherent culture have high purity,and drug-containing serum can promote BMSCs to express SDF-1 α mRNA and secretion of SDF-1 α.

The development of medical technology draws the doctor-patient relationship ever closer.However,the abundance and complexity of medical knowledge and technology make it a difficult task for medical personnel to grasp medical knowledge and skills,setting up a higher request for handling physician-patient relationship.Besides,medical disputes between doctors and patients caused by the abuse of medical technology in clinical practice also contribute to the contradiction between the advancement of medical technology and the latency of medical morality.Therefore,it is one important part in the study on medical morality from the perspective of medical technology ethics to take a rational view on the development of medical technology and construct a harmonious physician-patient relationship.
