1.Effect of Optimized New Shengmai Powder (优化新生脉散方) on Exercise Tolerance in Patients with Chronic Heart Failure of Qi Deficiency,Blood Stasis and Fluid Retention Syndrome:A Randomized,Double-Blind,Placebo-Controlled Trial

ObjectiveTo evaluate the effects and safety of the optimized new Shengmai Powder (优化新生脉散方) on exercise tolerance in patients with chronic heart failure (CHF) of qi deficiency, blood stasis, and fluid retention syndrome. MethodsA randomized, double-blind, placebo-controlled trial was conducted. A total of 78 CHF patients with qi deficiency, blood stasis, and fluid retention syndrome were recruited and randomly assigned to a treatment group (39 cases) and a control group (39 cases). On the basis of conventional western medical therapy, patients in the treatment group additionally received the optimized new Shengmai Powder granules, while the control group was given an oral placebo of optimized new Shengmai Powder granules. Patients in both groups took 30.6 g each time, twice a day, mixed with water for administration, with a total treatment course of 4 weeks. The primary outcomes were 6-minute walk distance (6MWD) and peak oxygen uptake (Peak VO2) measured by cardiopulmonary exercise testing. Secondary outcomes included New York Heart Association (NYHA) functional classification, B-type natriuretic peptide (BNP) levels, cardiac function indexes including left ventricular ejection fraction (LVEF), left ventri-cular end-systolic diameter (LVESD) and left ventricular end-diastolic diameter (LVEDD), Minnesota Living with Heart Failure Questionnaire (MLHFQ) scores, and scores of four diagnostic information of traditional Chinese medicine (TCM). All indicators were assessed once before and after treatment respectively. Safety indicators were evaluated, and adverse events during the trial were recorded. ResultsAll patients in both groups were included in the full ana-lysis set (FAS) and safety set (SS). Compared with baseline, the 6MWD and Peak VO₂ of cardiopulmonary exercise test in the treatment group significantly increased after treatment, while the MLHFQ scores, serum BNP levels and scores of TCM four diagnostic information significantly decreased, and the NYHA cardiac function grade significantly improved (P<0.01). After treatment, the 6MWD and Peak VO₂ of cardiopulmonary exercise test, as well as their changes from baseline in the treatment group were higher than those in the control group; the MLHFQ scores, serum BNP levels and scores of TCM four diagnostic information in the treatment group were lower than those in the control group; and the improvement of NYHA cardiac function grade in the treatment group was superior to that in the control group (P<0.01). There was no statistically significant differences in all indicators after treatment in the control group (P>0.05). The incidence of adverse events was 5.1% (2/39) in the treatment group and 2.6% (1/39) in the control group, with no statistically significant difference between groups (P>0.05). ConclusionOn the basis of conventional western medicine treatment, the addition of the optimized new Shengmai Powder can further improve exercise tolerance, cardiac function and quality of life in patients with CHF of qi deficiency, blood stasis and fluid retention syndrome, and show good safety.

Background Public places are important areas for daily human activities. Frequent contact with public items promotes their role as vehicles for microbial spread, creating a substantial risk for the transmission of pathogenic microorganisms. Objective To understand the hygiene status and influencing factors of public items in typical public places in Shanghai from 2010 to 2024, and to provide a scientific basis for optimizing the hygiene management of public items. Methods Based on the monitoring data of public items in public places in Shanghai from 2010 to 2024, the hygiene status was evaluated in three stages: 2010–2019, 2020–2022, and 2023–2024. Background As common public facilities essential to daily life, hair and beauty salons frequently contain various airborne toxic and hazardous pollutants potentially leading to adverse health effects for salon practitioners. Objective To characterize the indoor air pollution profiles of common contaminants in hair and beauty salons in Shanghai and to evaluate the associated health risks for practitioners, in order to provide a scientific basis for strengthening the public health management in Shanghai and protecting the health of practitioners. Methods The air quality monitoring data of hair and beauty salons in Shanghai from 2016 to 2024 were obtained from the “Health Hazard Factors Monitoring Program for Public Places” of the National Institute of Environmental Health, Chinese Center for Disease Control and Prevention. Monitoring indicators included particulate matter ≤10 μm in aerodynamic diameter (PM10), particulate matter ≤2.5 μm in aerodynamic diameter (PM2.5), formaldehyde, ammonia, benzene, toluene, and xylene. Indicator compliance rates were calculated across various years in accordance with GB 9666-1996 Hygienic standard for barber shop and beauty shop and GB 37488-2019 Hygiene indicators and limit for public places; specifically, PM2.5 was assessed against the limits stipulated in GB/T 18883-2022 Standards for indoor air quality. A questionnaire survey was conducted among salon practitioners to collect weekly working days and daily working hours. The non-carcinogenic risks associated with inhalation exposure to formaldehyde, ammonia, benzene, toluene, and xylene as well as the carcinogenic risks posed by formaldehyde and benzene were evaluated following WS/T 777-2021 Technical guide for environmental health risk assessment of chemical exposure and the U.S. Environmental Protection Agency inhalation risk model. Results The overall compliance rates of PM10, formaldehyde, ammonia, benzene, and toluene in the air of hair and beauty salons in Shanghai from 2016 to 2024 were 92.13%, 96.59%, 96.15%, 94.93%, and 94.97%, respectively; the overall compliance rate of xylene was a little lower (85.92%), and the overall compliance rate of PM2.5 was 57.18%. The P50 concentrations of PM10, PM2.5, formaldehyde, ammonia, benzene, toluene, and xylene did not exceed the corresponding limits. The P50 of non-carcinogenic risk indicator (hazard quotient, HQ) for formaldehyde, ammonia, benzene, toluene, and xylene were <1. The probabilities of non-carcinogenic risk HQ >1 for formaldehyde and xylene were 41.4% and 10.9%, respectively, which were higher than that of other pollutants. The P50 of carcinogenic risk (CR) for formaldehyde and benzene were between 1.0×10−6 and 1.0×10−4, while the probabilities of CR >1.0×10−4 were 16.9% and 14.0%, respectively. Conclusion The overall compliance rate of common pollutant concentrations in the air of hair and beauty salons in Shanghai is high, and the hygienic condition meets the requirements of national standards. The non-carcinogenic health risks posed by formaldehyde and xylene to employees (with formaldehyde being more prominent), as well as the carcinogenic risks associated with formaldehyde and benzene, deserve heightened attention in future health supervision.
4.Research Progress in the Mechanism of Targeted Intervention of Chinese Materia Medica in PI3K/Akt Pathway for the Treatment of Fibrosis

Organ fibrosis is a chronic tissue injury caused by multiple causes,and the continuous dynamic progression will lead to abnormal organ structure and function.Recent studies have shown that Chinese materia medica can inhibit organ fibrosis by regulating the PI3K/Akt pathway,which holds potential significance for developing anti-fibrosis strategies.This article systematically reviewed the role of the PI3K/Akt signaling pathway in the entire process of fibrosis,and summarized the potential mechanisms of Chinese materia medica monomers,drug pairs and compounds that can target and intervene in PI3K/Akt signaling transduction in preventing and treating organ fibrosis,in order to provide reference for the intervention of Chinese materia medica in the PI3K/Akt pathway for the treatment of organ fibrosis.
5.Analysis on Clinical Trial Registration of TCM in the Treatment of Coronary Artery Microvascular Diseases

Objective To review clinical trial registration status of TCM treatment for coronary microvascular disease;To analyze the effectiveness and safety of TCM in treating coronary microvascular disease.Methods The clinical trials of TCM in the treatment of coronary artery microvascular disease included in the Chinese Clinical Trials Registry and the US Clinical Trials Registry from the establishment of the database to January 31,2024 were retrieved,and the general characteristics(time,region,funding source),design type,intervention regimen and outcome indicators of the included clinical trials were extracted and analyzed using Excel 2019 software.Results A total of 17 clinical trials were included,including 16 pre-registrations.The registered units were distributed in 5 provinces across the country,involving 17 registration institutions.The two regions with the most distribution were Shanghai(6 studies,35.29%)and Beijing(5 studies,29.41%).The types of studies were mainly interventional studies,and most of the study designs were randomized parallel controlled studies(16 studies,94.12%).A total of 8 interventions were reported,including Chinese patent medicine,TCM decoction,TCM intravenous preparation,and acupuncture and moxibustion.A total of 143 outcome indicators were designed,including 10 first-level indicators,including coronary artery microcirculation,clinical efficacy,symptoms and signs,TCM syndromes,quality of life,exercise tolerance,cardiac function,physical and chemical testing,safety,and pharmacoeconomic evaluation.Conclusion The registration of clinical trials by TCM in the treatment of coronary microvascular diseases has been gradually receiving attention from researchers,but the overall number is still small.At present,the study needs to be optimized in terms of study design and index selection.
6.A Case of One-stop Procedure for Cryoballoon Ablation and Left Atrial Appendage Occlusion of Atrial Fibrillation Under Electroacupuncture Anesthesia

Atrial fibrillation is the most common type of chronic cardiac arrhythmia. Catheter ablation and left atrial appendage occlusion are effective treatment methods for atrial fibrillation, but these procedures require anesthesia support. However, anesthetic drugs often cause side effects such as nausea, vomiting, involuntary movements, and respiratory depression. This paper presents a case of a successful one-stop procedure for cryoballoon ablation and left atrial appendage occlusion of atrial fibrillation performed entirely under acupuncture anesthesia. Thirty minutes before the procedure, acupuncture needles were inserted perpendicularly at bilateral Neiguan (PC 6), Lieque (LU 7), Ximen (PC 4) and (LU 6). After obtaining the deqi (得气) sensation, an electroacupuncture device was connected, and electroacupuncture anesthesia was used for pain control throughout the procedure. The patient exhibited good tolerance and cooperation, with electroacupuncture anesthesia completely replacing intravenous anesthetics, ensuring the smooth completion of the surgery. Postoperative follow-up showed favorable outcomes.
7.Prognostic analysis of laparoscopic-assisted Soave procedure for common hirschsprung's disease in infants younger than 6 months

Objective To evaluate the prognosis of infants with common Hirschsprung's disease who undergo laparoscopic-assisted Soave procedure below 6 months of age,and to explore its feasibility.Methods A retrospective analysis was conducted on the clinical data of 82 cases of common Hirschsprung's disease admitted to our department from January 2018 to August 2022.The patients were divided into two groups based on their age at the time of surgery:Young surgical age group(<6 months,50 cases)and Old surgical age group(≥ 6 months,32 cases).All patients underwent the treatment of laparoscopic-assisted Soave procedure.The surgical time,length of hospital stay,postoperative complications,and anal function were compared between the two groups.Results There were no significant differences between young surgical age group and old surgical age group in terms of surgical time[(145.04±11.21)min vs.(149.25±12.18)min,P=0.20],length of hospital stay[(13.56+0.93)d vs.(13.91±0.99)d,P=0.55],postoperative complications[22.00%(11/50)vs.28.13%(9/32),P=0.53],and Rintala score for anal function[(18.58±1.44)vs.(17.22±1.64),P=0.06].The main postoperative complications,including enterocolitis[10.00%(5/50)vs.12.50%(4/32),P=0.72],fecal/anal incontinence[6.00%(3/50)vs.6.25%(2/32),P=0.96],and constipation[4.00%(2/50)and 6.25%(2/32),P=0.65],showed no significant differences between the two groups(all P>0.05).However,there was a statistically significant difference in terms of bleeding volume[(7.86±2.02)ml vs.(9.13±2.17)ml,P<0.05].No deaths occurred in this study.Conclusion Laparoscopic-assisted Soave procedure is safe and effective for infants below 6 months of age with common congenital Hirschsprung's disease.It can achieve satisfactory anal function and reduce the incidence of adverse events during the waiting period for surgery.
8.Deep learning-based automatic reconstruction of interstitial needles in brachytherapy for cervical cancer

Objective:To explore the feasibility of autosegmentation and reconstruction of interstitial needles in intracavitary / interstitial brachytherapy (IC-ISBT) for cervical cancer based on deep learning.Methods:The data of 180 treatment plans from 98 patients who received IC-ISBT were retrospectively collected and divided into the training, validation, and testing sets in a 16:1:1 ratio. Masks of needles were created using the dwell positions of radiation sources, and a 3D U-Net model was trained. The performance of the model was evaluated using the Dice similarity coefficient (DSC). Absolute and relative accuracy rates were used to assess the results of this method, and the position bias was used to evaluate the precision of predictions in the transversal plan of CT scans. Wilcoxon rank-sum test was performed to evaluate the reconstruction efficiency by comparing the time required for automated versus manual reconstruction.Results:DSC of the model was 0.93 ± 0.02. The absolute and relative accuracy rates were 0.44 ± 0.09 and 0.95 ± 0.03, respectively. The distance deviation on the CT horizontal plane was (0.58 ± 0.54) mm. The average time of autosegmentation and reconstruction was (6.2 ± 0.4) s, leading to a significant reduction in time consumption compared with manual construction ( P<0.001). Conclusions:Based on deep learning, using the dwell positions of radiation sources for data annotation, combined with post-processing algorithms, accurate automated segmentation and digital reconstruction of needles in IC-ISBT three-dimensional CT images can be achieved, significantly improving reconstruction efficiency.
9.Research progress and clinical application prospects of proton arc therapy

Proton arc therapy (PAT), as an emerging radiation therapy technique, has attracted increasing widespread attention in the field of cancer radiotherapy in recent years. In this review, technological developments, clinical application potential, biological effects, dosimetric optimization, and current challenges of PAT were comprehensively summarized. A systematic analysis of recent studies indicates that PAT offers significant advantages in improving target coverage and sparing normal tissues, particularly in mitigating organ motion-induced uncertainties. However, PAT remains in the clinical validation stage and still faces challenges related to technical optimization and cost control. Further clinical studies are required to confirm its long-term efficacy and safety.
10.Meta-analysis of exercise intervention on cardiopulmonary function in stroke patients

OBJECTIVE:Clinically,stroke patients experience significant cardiopulmonary dysfunction during rehabilitation,which increases the likelihood of recurrent stroke.However,exercise interventions can effectively improve cardiopulmonary function in stroke patients.This paper employs meta-analysis to conduct a comprehensive quantitative evaluation of domestic and international studies on the improvement of cardiopulmonary function in stroke patients through exercise interventions.It examines the effects of different exercise modalities on the cardiopulmonary function of stroke patients and explores the optimal exercise regimen.METHODS:China National Knowledge Infrastructure(CNKI),WanFang Data,VIP,Web of Science,Cochrane Library,PubMed,and EBSCO databases were searched for articles published from inception to May 15,2024,primarily including randomized controlled trials on exercise interventions improving cardiopulmonary function in stroke patients.Statistical analyses were conducted using Review Manager 5.4 and Stata 17 software.RESULTS:(1)A total of 32 articles meeting the criteria were included,involving 1 826 stroke patients.(2)The results showed that exercise interventions significantly improved the oxygen uptake capacity of stroke patients[MD=2.10,95%CI(1.57,2.63),P<0.000 01].The optimal exercise regimen for improving oxygen uptake capacity was high-intensity interval training,conducted 5-6 times per week,with each session lasting 41-60 minutes,for a duration of more than 12 weeks.(3)Exercise interventions also significantly enhanced the aerobic endurance of stroke patients[MD=38.00,95%CI(29.55,46.45),P<0.000 01].The optimal regimen for improving aerobic endurance was high-intensity interval training,conducted 2-3 times per week,with each session lasting 41-60 minutes,for a duration of more than 12 weeks.CONCLUSION:Exercise interventions can significantly improve the cardiopulmonary function of stroke patients,with high-intensity interval training being the most promising exercise modality.Therefore,the optimal exercise regimen for improving cardiopulmonary function in stroke patients is to conduct high-intensity interval training sessions lasting 41-60 minutes,starting with an intervention frequency of twice a week,gradually increasing the frequency based on tolerance,and continuing the intervention for more than 12 weeks.Future research should focus on collecting more high-quality,multi-faceted clinical studies and evidence to validate these conclusions,particularly in exploring the optimal exercise-to-rest ratio of high-intensity interval training for improving cardiopulmonary function in stroke patients.
