1.The mediating effect of discharge guidance and discharge readiness after coronary artery bypass grafting

Objective To explore the mediating effect of negative emotion and rumination on the quality of discharge guidance and discharge readiness of patients after coronary artery bypass grafting.Methods A total of 288 patients who underwent coronary artery bypass surgery in the Second Affiliated Hospital of Nanchang University from February 2022 to February 2024 were selected as study objects.General data questionnaire,self-rating anxiety scale(SAS),self-rating depression scale(SDS),Chinese version of event related rumination inventory(CERRI),quality of discharge teaching scale(QDTS),readiness for hospital discharge scale(RHDS)were used to investigate,and Amos 24.0 structural equation model was used to analyze the chain mediation effect.Results A total of 288 questionnaires were sent out,and 280 valid questionnaires were recovered,with an effective recovery rate of 97.22%.The patients'SAS scores were(57.83±5.32)points,SDS scores were(63.31±4.97)points,CERRI total scores were(31.03±4.76)points,QDTS total scores were(116.78±9.22)points,and RHDS total scores were(79.30±7.21)points.The RHDS scores of patients who lived alone after discharge,paid at their own expense and had grade Ⅲ heart function were significantly lower than those of patients with nursing care,medical insurance payment and grade Ⅰ heart function after discharge(P<0.05).Anxiety was positively correlated with depression,intrusive rumination,but negatively correlated with purposive rumination,quality of discharge guidance and discharge readiness.Depression was positively correlated with intrusive rumination,but negatively correlated with purposive rumination,quality of discharge guidance and discharge readiness.Invasive rumination was negatively correlated with purposive rumination,quality of discharge guidance and discharge readiness.Purposive rumination was positively correlated with the quality of discharge guidance and discharge readiness.The quality of discharge guidance was positively correlated with discharge readiness(P<0.05).The chain-mediated model was well fitted,and the results showed that the quality of patients'discharge guidance could affect their discharge readiness,while negative emotions and rumination pathways also affected their discharge readiness(P<0.05).Conclusion The quality of discharge guidance can directly or indirectly affect the discharge readiness of patients after coronary artery bypass grafting through negative emotions and rumination,and there is a chain mediation effect.
2.Chidamide triggers pyroptosis in T-cell lymphoblastic lymphoma/leukemia via the FOXO1/GSDME axis.

BACKGROUND:
T-cell lymphoblastic lymphoma/acute lymphoblastic leukemia (T-LBL/ALL) is an aggressive form of hematological malignancy associated with poor prognosis in adult patients. Histone deacetylases (HDACs) are aberrantly expressed in T-LBL/ALL and are considered potential therapeutic targets. Here, we investigated the antitumor effect of a novel HDAC inhibitor, chidamide, on T-LBL/ALL.
METHODS:
HDAC1, HDAC2 and HDAC3 levels in T-LBL/ALL cell lines and patient samples were compared with those in normal controls. Flow cytometry, transmission electron microscopy, and lactate dehydrogenase release assays were conducted in Jurkat and MOLT-4 cells to assess apoptosis and pyroptosis. A specific forkhead box O1 (FOXO1) inhibitor was used to rescue pyroptosis and upregulated gasdermin E (GSDME) expression caused by chidamide treatment. The role of the FOXO1 transcription factor was evaluated by dual-luciferase reporter and chromatin immunoprecipitation assays. The efficacy of chidamide in vivo was evaluated in a xenograft mouse.
RESULTS:
The expression of HDAC1, HDAC2 and HDAC3 was significantly upregulated in T-LBL/ALL. Cell viability was obviously inhibited after chidamide treatment. Pyroptosis, characterized by cell swelling, pore formation on the plasma membrane and lactate dehydrogenase leakage, was identified as a new mechanism of chidamide treatment. Chidamide triggered pyroptosis through caspase 3 activation and GSDME transcriptional upregulation. Chromatin immunoprecipitation assays confirmed that chidamide led to the increased transcription of GSDME through a more relaxed chromatin structure at the promoter and the upregulation of FOXO1 expression. Moreover, we identified the therapeutic effect of chidamide in vivo .
CONCLUSIONS
This study suggested that chidamide exerts an antitumor effect on T-LBL/ALL and promotes a more inflammatory form of cell death via the FOXO1/GSDME axis, which provides a novel choice of targeted therapy for patients with T-LBL/ALL.

4.Correlation of serum metabolites and clinical features in patients with peripheral T-cell lymphoma

Objective To explore the changes in serum energy metabolites in patients with peripheral T-cell lymphoma,and investigate serum biomarkers for monitoring peripheral T-cell lymphoma from the perspective of energy metabolism.Methods Multiple/selected reaction monitoring(MRM/SRM)was used to detect the energy-related metabolites in the sera of 16 patients with newly diagnosed peripheral T-cell lymphoma admitted in the Hematology Medical Center of the Second Affiliated Hospital of Army Medical University from November 2020 to December 2021,as well as 10 recruited healthy volunteers.The corresponding clinical data including medical history,laboratory results and image data were collected and retrospectively analyzed.Results Significant differences were seen in the contents and expression profiles of serum energy metabolism-related products between the patients and the healthy volunteers.The patients had significantly reduced serum contents of cyclic AMP,succinate,citrate and cis-aconitate(P<0.05),and elevated D-glucose 6-phosphate content(P<0.05).The serum contents of citrate and succinate were negatively correlated with the risk stratification(low-,moderate-and high-risk)and clinical stage of the disease(P<0.05).Meanwhile,there was a negative correlation between the contents of L-malic acid and citrate and the mid-term efficacy evaluation results,such as complete/partial response(CR/PR)or stable disease(SD)(P<0.05).For patients with extranodal NK/T cell lymphoma(n=10),there were also significant reductions in the contents of cyclic AMP,succinate,citrate,isocitrate and cis-aconitate in the sera of patients compared with healthy volunteers(P<0.05),and the contents of citrate and succinate were negatively correlated with the clinical stage(P<0.05)and were rather correlated with mid-term efficacy evaluation results(CR/PR or SD)(P<0.05).For patients with angioimmunoblastic T-cell lymphoma(n=6),the serum contents of cyclic AMP,citrate and succinate were significantly lower,while the content of D-glucose 6-phosphate was higher when compared with the healthy volunteers(P<0.05),and the content of succinate was negatively correlated with both clinical stage and risk grade of the patients(P<0.05).Conclusion There are 5 serum differential metabolites identified between patients with peripheral T-cell lymphoma and healthy controls,and succinate and citrate are expected to be serum biomarkers of peripheral T-cell lymphoma.
5.National norms of nursing soft skills for new nurses

Objective To develop national norms of nursing soft skills, and provide a basis for the evaluation of the nursing soft skills for new nurses.Methods With the stratified cluster sampling, 2816 new nurses were choosed from across China, and nursing soft skills evaluation scale was used to investigate.The general descriptive statistics, reliability analysis and confirmatory factor analysis of sample data were done.Results Developed the total national norms of nursing soft skills for new nurses ( the total soft skills was 150.31 ± 15.47,interpersonal skills was 28.28 ± 3.75, organizational management was 33.74 ± 4.10, self-management was 37.50 ±4.58 ,career sentiment was 28.10 ±3.52 ,cognitive kills was 22.70 ±3.11 ) and established a T-rough and percentiles-rough score conversion tables.The level of nursing soft skills of new nurses was divided into five levels: T-score < 30 was poor for the soft skill, T-score at the 30 to 39 for the somewhat less, T-score at the 40 to 59 for the general level, T-score at 60 to 69 for good,T-score ≥ 70 for the best.Conclusion These norms can be used as evaluating standards of nursing soft skills for new nurses.
6.Qualitative research on soft skills of pediatric nurses

Objective To deeply study the soft skills that pediatric nurses should have. Methods 8 pediatric registered nurses were interviewed, the whole interviewing process was fully noted, then the data were analyzed acconting to Colaizzi methods. Results Soft skills that pediatric nurses should have included: affinity, empathy, caring about others, sympathy, communication skills, spirit of contribution, a bridge for com-munication between patients and doctors, resolving conflicts skills, cooperation, seE-regulation, stress manage-ment, emotion management and working achievements. Conclusions Soft skills are very important to pedi-atric nurses, so we need progress it gradually.
7.Discussion on the strategy for developing nursing human resources

Objective The discussion on the strategy for developing nursing human resources was carried out through truthfully understanding the medical managers' experiences of developing nursing human resources. We aimed to supply reference for establishment of policies by relevant government departments. Methods We collected data of 13 medical managers by in-depth interview and took notes (or recording) on the spot. The data were analyzed by Colaizzi's analysis program. Results Two aspects were summarized through reading, analyzing, reflecting, classifying and extracting. One was policy guidance for developing nursing human resource (4 items) and the other was policies and measures for developing nursing human resource (7 items). Conclusions The development of nursing human resource was influenced by many factors. It needs the common efforts by government, society, hospital and nurses.
8.Phenotypic and functional characteristics of T cells in tuberculosis pleurisy

Objective:To elucidate the phenotypic and functional characteristics of CD3+ CD4+ and CD3+ CD8+ T in pleural fluid of patients with tuberculous pleurisy.Methods:PBMCs and PFCs were isolated. The frequency of CD3+,CD3+ CD4+,CD3+ CD8+ T cells and the ratio of CD4/CD8 in PFCs were analyzed by FACS. The concentration of IFN-?,IL-2 and TNF-? in the pleural fluid were analyzed by ELISA.Production of IFN-?,IL-2 and TNF-? by CD3+ CD4+ and CD3+ CD8+ T cells was detected by FACS following stimulation with BCG.Results:Higher frequencies of CD3+ CD4+ T cells and CD4+ T/CD8+ T ratio were demonstrated in PFCs when compared with PBMCs from normal donors and TB patients.ELISA analysis demonstrated that significantly high levels of IFN-? and TNF-? were detected in pleural fluid.Further analysis by FACS indicated that IL-2,TNF-? and IFN-? were predominantly secreted by CD3+CD4+T cells but not by CD3+ CD8+ T cells.Conclusion:The frequencies of CD3+ and CD3+ CD4+ T cells are increased in PFCs. Large amounts of Th1 cytokines are detected,which are produced by CD3+ CD4+ T cells,and might play important roles against TB infection.
