Objective To investigate the impact of adequate lumen preparation on the 12-month primary patency rate of drug-eluting stents(DES)in patients with lower extremity arteriosclerosis obliterans(ASO)and to identify risk and protective factors for in-stent restenosis. Methods A retrospective cohort study was conducted on 141 patients who underwent DES implantation for ASO at the Department of Vascular Surgery of the First Affiliated Hospital of Chongqing Medical University between April 2022 and April 2024.According to post-percutaneous transluminal angioplasty(PTA)dissection grade[severe dissection is defined as type C or higher based on National Heart,Lung, and Blood Institute(NHLBI)criteria]and residual stenosis rate(severe stenosis is defined as≥30%),the patients were categorized into an adequate preparation group(no severe dissection or severe residual stenosis;n=59)and an inadequate preparation group(n=82).DES primary patency at 12 months was assessed by color Doppler ultrasonography or angiography, defined as a peak systolic velocity ratio≤2.4 on duplex ultrasound or angiographic stenosis<50%.Kaplan-Meier analysis was used to compare intergroup differences in DES patency rate and freedom from clinically driven target lesion revascularization(CD-TLR).Multivariate logistic regression analysis was employed to identify risk and protective factors for DES restenosis and to evaluate the risk contributions of post-PTA dissection and residual stenosis to DES restenosis. Results Post-PTA,vascular dissection occurred in 94.3%(133/141)of treated limbs, among which 42.1%(56/133)were severe dissections, and severe residual stenosis was present in 44.0%(62/141)of limbs.Kaplan-Meier analysis revealed that the 12-month primary patency rate was significantly higher in the adequate preparation group than the inadequate group(HR=0.322,95%CI:0.132~0.789,P=0.013),while no significant difference was found in the rate of freedom from CD-TLR(HR=0.608,95%CI:0.187~1.937,P=0.407).Multivariate logistic regression analysis identified adequate lumen preparation(OR=0.228,95%CI:0.069~0.747,P=0.015)and Rutherford category 2~3(OR=0.205,95%CI:0.058~0.725,P=0.014)as independent protective factors against DES restenosis. The patency rate in the subgroup with coexisting severe dissection and residual stenosis was significantly lower than that in the uncomplicated group(55.6%vs 89.8%,P<0.001),with a 7.067-fold increased risk of re-stenosis(95%CI:2.424~20.602,P<0.001).Conclusion Adequate lumen preparation significantly improves the 12-month primary patency rate of DES.It,along with Rutherford category 2~3,serves as an independent protective factor against in-stent restenosis.Concurrent severe dissection and significant residual stenosis should be actively avoided during the procedure.

Cardiac fibrosis is characterized by an elevated amount of extracellular matrix (ECM) within the heart. However, the persistence of cardiac fibrosis ultimately diminishes contractility and precipitates cardiac dysfunction. Circular RNAs (circRNAs) are emerging as important regulators of cardiac fibrosis. Here, we elucidate the functional role of a specific circular RNA CELF1 in cardiac fibrosis and delineate a novel feedback loop mechanism. Functionally, circ-CELF1 was involved in enhancing fibrosis-related markers' expression and promoting the proliferation of cardiac fibroblasts (CFs), thereby exacerbating cardiac fibrosis. Mechanistically, circ-CELF1 reduced the ubiquitination-degradation rate of BRPF3, leading to an elevation of BRPF3 protein levels. Additionally, BRPF3 acted as a modular scaffold for the recruitment of histone acetyltransferase KAT7 to facilitate the induction of H3K14 acetylation within the promoters of the Celf1 gene. Thus, the transcription of Celf1 was dramatically activated, thereby inhibiting the subsequent response of their downstream target gene Smad7 expression to promote cardiac fibrosis. Moreover, Celf1 further promoted Celf1 pre-mRNA transcription and back-splicing, thereby establishing a feedback loop for circ-CELF1 production. Consequently, a novel feedback loop involving CELF1/circ-CELF1/BRPF3/KAT7 was established, suggesting that circ-CELF1 may serve as a potential novel therapeutic target for cardiac fibrosis.

Objective To explore the protective effects of genetically modified porcine erythrocyte suspension as a subnormothermic normoxic mechanical perfusate on hypoxic-ischemic brain injury in cynomolgus monkeys caused by traumatic hemorrhage. Methods Cynomolgus monkeys were randomly divided into positive and negative control groups(a total of 3 monkeys, with 3 left cerebral hemispheres as the positive control group and 3 right cerebral hemispheres as the negative control group)and the subnormothermic perfusion group(n=3).The positive control group was directly sampled 1 hour after circulatory arrest, while the negative control group was placed at subnormothermic conditions for 6 hours after circulatory arrest. The subnormothermic perfusion group underwent 6 hours of subnormothermic normoxic mechanical perfusion of the bilateral common carotid arteries of the cynomolgus monkey hypoxic-ischemic brain injury model using genetically modified porcine erythrocyte suspension 1 hour after circulatory arrest. Before perfusion,cross-matching experiments were conducted between the six genetically modified pig and the cynomolgus monkeys. After the start of perfusion, the levels of routine blood indicators in the perfusate were detected at 0,1,2,3,4,5 and 6 hours.Blood oxygen saturation was recorded, and the levels of Na+,K+,Ca2+,glucose and blood pH in the perfusate were measured,as well as the levels of IgG and IgM in the perfusate. After 6 hours of perfusion, the water content of the brain tissue was measured.Nissl staining was performed on the frontal cortex and hippocampal regions, and immunofluorescence staining was used to detect the expression of glial fibrillary acidic protein(GFAP),ionized calcium-binding adapter molecule 1(Iba1)and neuronal nuclear antigen(NEUN).Results The cross-matching results between the six genetically modified pig and the cynomolgus monkeys were negative. The number of red blood cells in the perfusate decreased significantly at 3 hours of perfusion, and the hemoglobin level showed a downward trend at 1,3,5 and 6 hours. The number of white blood cells and platelets decreased at all time points. The blood oxygen saturation in the subnormothermic perfusion group remained stable at 95%-98%,and the levels of blood oxygen saturation,Na+,Ca2+,glucose and pH were stable, while the K+level first increased and then decreased. There was no significant difference in the levels of IgG and IgM before and after perfusion. The water content of brain tissue at the end of perfusion in the subnormothermic perfusion group was significantly higher than that in the positive control group(P<0.001).Nissl staining results showed that compared with the positive control group, the pyramidal neurons in the prefrontal cortex of the subnormothermic perfusion group maintained better morphological integrity, with no significant increase in enlarged and deformed cells. In the hippocampal CA1 region, there was a slight increase in enlarged and deformed cells, and a few cells with undamaged structures showed reduced cell size.In the hippocampal dentate gyrus, fewer granule neurons had compromised structural integrity, with increased cell edema.NEUN immunofluorescence staining showed that compared with the positive control group, the pyramidal neurons in the prefrontal cortex and hippocampal CA1 region of the subnormothermic perfusion group had better morphological states, with clear axons. The granule cells in the hippocampal dentate gyrus were well preserved, but the nuclei were less well protected.GFAP immunofluorescence staining showed that compared with the positive control group, the subnormothermic perfusion group had sparser protrusions that were more tightly associated with neurons.Iba1 immunofluorescence staining showed that compared with the positive control group, the subnormothermic perfusion group had thicker and fewer protrusions. Conclusions Compared with the positive control group, subnormothermic normoxic mechanical perfusion with genetically modified porcine erythrocyte perfusate increases brain tissue edema in cynomolgus monkeys, but better preserves the morphological integrity of neurons and glial cells. The protective effects may be related to the continuous oxygen and energy supply, maintenance of ion homeostasis and perfusate pH,reduced rejection, and low metabolic state of the whole brain.

Objective:To analyze the clinic effects of arthroscopic partial trapeziectomy and suture button suspensionplasty in the treatment of first carpometacarpal joint (CMCJ) Eaton stage II/III arthrosis. Methods:A retrospective study was conducted on a total of 15 cases (16 hands) of patients including 5 males (1 bilateral) and 10 females with CMCJ stage II/III arthrosis who underwent surgical treatment at the first affiliated hospital of Shenzhen university from January 2020 to June 2022, with mean age of 56.7±6.4 years (range, 46-75 years). The duration from pain to treatment was 7.8±3.2 months (range, 4-14 months). X-ray showed narrowing of CMCJ with osteophytes and distal radial subluxation. All the patients were treated with arthroscopic partial trapeziectomy and suture button suspensionplasty. The preoperative and last postoperative follow-up radiographs, visual analogue scale (VAS), thumb's Kapandji scores, disabilies of the arm, shoulder, and hand (DASH) scores, grip and pinch strength and time to return to work were compared. Results:All cases were followed up for 19.6±6.3 months (range, 11-36 months). The postoperative X-ray showed all the CMCJs were reduced with a normal height of first metacarpal. The mean time for patients to return to their daily activities was 18.69±3.70 d and the mean time to return to work was 24.63±4.91 d. The average VAS score decreased from 6.56±1.15 preoperatively to 1.00 (0.75, 1.25). The preoperative Kapandji's score was 8.00±0.82 and the postoperative Kapandji's score was 8.00 (7.25, 9.00). The average DASH values improved from 24.06±3.19 to 4.00 (3.00, 5.00). The were significant differences except for Kapandji score ( Z=-4.905, P<0.001; Z=-0.121, P=0.905; Z=-4.846, P<0.001). The mean grip and pinch strength showed improvement from an average of 16.4 (14.13, 18.68) kg and 1.70±0.35 kg to 26.14±3.27 kg and 3.58±0.91 kg with significant difference ( Z=-4.617, P<0.001; t=-7.669, P<0.001). Conclusion:Arthroscopic partial trapeziectomy and suture button suspensionplasty is a minimally invasive surgery for the treatment of first CMCJ Eaton stage II/III arthrosis. By this technique, the patients' existing instability and pain problems can be solved.
