1.Investigation on the status and barriers of non-accompanied care services in medical institutions in Zhejiang Province

Objective To investigate the current status and barriers to implementing non-accompanied care services in medical institutions in Zhejiang Province,and to provide a basis for the improvement of standardized management of such services.Methods A self-designed questionnaire was conducted from September to October 2024 among all secondary and tertiary medical institutions in Zhejiang Province to assess the implementation status and barriers to non-accompanied care services.Results A total of 397 questionnaires were distributed,with 389 valid responses,yielding a valid response rate of 97.98%.Non-accompanied care services were implemented in 118 institutions(30.33%).Among these,90 institutions(76.27%)had established management systems for non-accompanied wards;71 institutions(60.17%)had a medical nursing assistant-to-bed ratio lower than 1∶5;41 institu-tions(34.75%)provided tiered training for medical nursing assistants;93 institutions(78.81%)required patients to bear the full cost of the service.Compared with secondary medical institutions,tertiary medical institutions have more complete management system for non-accompanied care services.The main obstacles hindering the development of non-accompanied care services include an imperfect management system for non-accompanied wards,a shortage of medical nursing assistants,a lack of standardized training for such assistants,inconsistent charging standards,and low acceptance among patients and their families.Conclusion The promotion of non-accompanied care services in medical institutions in Zhejiang Province has achieved initial success.However,challenges persist,including incomplete management systems,uneven development across hospital tiers,and imperfect charging mechanisms.It is recommended that relevant authorities strengthen policy support,enhance standardized training for healthcare nursing assistants,refine cost-sharing mechanisms,and improve the quality and sustainability of non-accompanied care services through multi-party collaboration.
2.Practice of " no-accompany" services through multi-department collaboration in public hospital

The " no-accompany" service is an important measure to meet the patients′ needs for comprehensive, continuous, and high-quality diversified care and to enhance their medical experience. Starting in March 2024, the Second Affiliated Hospital of Zhejiang University School of Medicine launched a pilot " no-accompany" service in the breast surgery and dermatology wards. The hospital established a working team led by the nursing department and involving multiple administrative departments such as the medical affairs department, medical insurance office, and logistics management department. And the hospital has successfully advanced the implementation of the " no-accompany" ward program by establishing a multi-department collaborative management mechanism, standardizing the management of medical caregivers, developing a tiered fee scheme for the " no-accompany" service, and improving relevant service measures. The acceptance of the " no-accompany" service by patients increased from (3.93±0.83) in the second quarter of 2024 to (4.69±0.59) in the fourth quarter, and overall satisfaction rose from (4.18±0.73) to (4.50±0.54) (all P<0.001). The job competency of medical caregivers also improved significantly from (64.80±3.49) before starting work in March 2024 to (94.00±2.40) in the fourth quarter ( P<0.001). These findings provide a reference for the implementation of " no-accompany" services in public hospitals.

4.A multicenter survey on the nurses'knowledge and attitude of nursing for panvascular disease in Zhejiang Province

Objective To investigate the knowledge and attitude of clinical nurses on Panvascular Disease(PVD)care in Zhejiang Province,and to analyze the influencing factors,so as to provide references for constructing a systematic PVD training program.Methods The convenient sampling method was used to investigate 454 nurses from 36 general hospitals in 11 prefectures and cities in Zhejiang Province in June 2024 with a self-designed questionnaire on general information and a questionnaire on PVD nursing knowledge and attitude questionnaire,and multiple stepwise linear regression was used to analyze the impact factors.Results 452 valid questionnaires were collected and the valid recovery rate was 99.56%.Nurses scored(8.57±2.02)in knowledge dimension,(60.82±6.45)in attitude dimension.The results of multiple stepwise linear regression analysis showed that hospital grade was the factor influencing the score of nurses'knowledge towards PVD nursing in Zhejiang Province(P<0.05).Hospital grade,whether they had received systematic knowledge training related to PVD were the factors influencing the score of nurses'attitude towards PVD nursing(P<0.05).Conclusion Nurses in Zhejiang Province are positive about PVD nursing,but their knowledge level needs to be improved.Nursing managers should pay full attention to the training intention of clinical nurses,establish a PVD knowledge training system,and strengthen the theoretical and practical training of clinical nurses on PVD nursing,so as to promote the development of PVD nursing work.

7.Construction and practice of a multidisciplinary collaborative nursing management model in the integrated ward for pan-vascular disease patients

Objective To explore the construction and practical effects of a multidisciplinary collaborative nursing management model of"organization-training-emergency response-integration-quality control"for pan-vascular disease patients in integrated wards.Methods On the basis of literature research and expert meetings,a multidisciplinary collaborative nursing management model for patients with pan-vascular disease in intergrated wards has been formed.It includes the implementation of accurate dynamic and intelligent overall allocation of beds in the hospital,the formulation of multidisciplinary training programs,the establishment of an emergency and critical case treatment guarantee mechanism,the construction of a standardized path for the joint management of multidisciplinary nursing teams,and the establishment of an intelligent and homogeneous nursing quality control chain.From July 2022 to December 2023,the program was implemented in the cardio-cerebrovascular ward of a tertiary A hospital in Zhejiang province,and indicators such as the average length of stay,cure and improvement rate,and cardiopulmonary resuscitation success rate were collected,and the satisfaction of patients with pan-vascular disease was investigated.Results During the application of multidisciplinary collaborative nursing management model for patients with pan-vascular disease,5416 patients were admitted,with an average length of stay of 6.11±0.73 days,a cure and improvement rate of 98.06%(5 311/5 416),a cardiopulmonary resuscitation success rate of 94.12%,and no adverse events.The overall satisfaction score of patients improved from 92.74±1.68 points in the 4th quarter of 2022 to 98.21±0.55 points in the 4th quarter of 2023(P<0.001).Conclusion The construction of the multidisciplinary collaborative nursing management model in the integrated pan-vascular ward has explored a new path for the hospital to cultivate cross-professional nursing personnel,promoted effective resource integration,and optimized the health management path,effectively improved the utilization rate of existing resources in the hospital and patients'medical experience.
8.Application of Snyder’s hope theory in patients with pelvic floor exercise after prophylactic stoma retraction

Objective:To explore the effect of pelvic floor muscle functional exercise based on Snyder hope theory in patients after prophylactic stoma retraction.Methods:74 patients with low rectal cancer who underwent prophylactic stoma retraction from July 2019 to June 2021 were randomly divided into intervention group and control group. The patients in the control group received routine nursing and pelvic floor muscle functional exercise. The patients in the intervention group received functional exercise intervention based on Snyder′s hope theory on the basis of the control group. The hope level and self-care ability of the patients in the two groups were evaluated before the intervention and 3 months after stoma restitution. The anal function of the patients in the two groups was evaluated 1 month and 3 months after stoma restitution.Results:Before the intervention, there was no significant difference in the score of hope level and self-care ability between the two groups ( P>0.05). Three months after the operation, the score of hope level in the observation group was 36.20 ± 3.82, which was higher than that in the control group (31.26 ± 5.03) ( t = 4.63, P<0.05). Three months after the operation, the self-care ability score of the observation group was 123.57 ± 10.82, which was higher than that of the control group (108.23 ± 9.48) ( t = 6.31, P<0.05). One month and three months after stoma retraction, the anal function scores of the observation group were 12.03 ± 3.94, 5.91 ± 2.05 respectively, which were lower than those of the control group (13.86 ± 2.19, 7.26 ± 1.74) ( t = 2.40, 2.99, both P<0.05). Conclusion:Pelvic floor muscle functional exercise based on Snyder′s hope theory can improve the hope level of patients after stoma retraction, improve their anal function and improve their self-care ability.
9.The protection of isoflavones on myocardium in myocardial infarction mouse

Aim By establishing mouse acute myocardial infarction model,to observe the protection of isoflavones on ischemic myocardium and research the mechanism.Methods Mouse acute myocardial infarction model was established by ligating the left anterior descending(LAD)coronary artery.Danshen was used as the positive control.The effect of isoflavones on myocardial infarct area,serum myocardium creatase and serum levels of SOD and MDA was observed.By Real Time PCR,it was found that isoflavones could affect the expression of β-adrenergic receptor kinase(β-ARK_1).Results Isflovones could obviously reduce the myocardial infarct area and lower the levels of serum myocardial creatase and MDA.It could downregulate the expression of β-ARK_1 as the doses are increased.Conclusions Isoflavones can protect the myocardium of acute myocardial infarction mouse.The mechanism is related to the reduction of the oxidative damage,and the downregulation of the expression of β-ARK_1.
