1.Analysis of Mechanism of Xingpi Capsules in Treatment of Functional Dyspepsia Based on Transcriptomics

ObjectiveTo investigate the ameliorative effect of Xingpi capsules on functional dyspepsia(FD) and the potential mechanism. MethodsSixty SPF-grade male SD neonatal rats(7 days old) were randomly divided into the normal group(n=12) and the modeling group(n=48), and the FD model was prepared by iodoacetamide gavage in the modeling group. After the model was successfully prepared, the rats in the modeling group were randomly divided into the model group, the low-dose and high-dose groups of Xingpi capsules(0.135, 0.54 g·kg-1) and the domperidone group(3 mg·kg-1), with 12 rats in each group. Rats in the normal and model groups were gavaged with distilled water, and rats in the rest of the groups were gavaged with the corresponding medicinal solution, once a day for 7 d. The general survival condition of the rats was observed, and the water intake and food intake of the rats were measured, the gastric emptying rate and the small intestinal propulsion rate were measured at the end of the treatment, the pathological damage of the rat duodenum was examined by hematoxylin-eosin(HE) staining, and the expressions of colonic tight junction protein(Occludin) and zonula occludens protein-1(ZO-1) were detected by immunofluorescence. The differentially expressed genes in the duodenal tissues of the model group and the normal group, and the high-dose group of Xingpi capsules and the model group were detected by transcriptome sequencing after the final administration, and Gene Ontology(GO) and Kyoto Encyclopedia of Genes and Genomes(KEGG) enrichment analyses were carried out. The transcriptomic results were validated by Western blot, immunofluorescence, and real-time fluorescence quantitative polymerase chain reaction(Real-time PCR), and the active ingredients of Xingpi capsules were screened for molecular docking with the key targets. ResultsCompared with the normal group, the general survival condition of rats in the model group was poorer, and the water intake, food intake, gastric emptying rate and small intestinal propulsion rate were all significantly reduced(P<0.05), inflammatory infiltration was seen in duodenal pathology, and the fluorescence intensities of Occludin and ZO-1 in the colon were significantly reduced(P<0.01). Compared with the model group, the general survival condition of rats in the high-dose group of Xingpi capsules improved significantly, and the water intake, food intake, gastric emptying rate and small intestinal propulsion rate were all significantly increased(P<0.05), the duodenal pathology showed a decrease in inflammatory infiltration, and the fluorescence intensities of colonic Occludin and ZO-1 were significantly increased(P<0.01). Transcriptomic results showed that Xingpi capsules might exert therapeutic effects by regulating the phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt) through the key genes such as Slc5a1, Abhd6. The validation results showed that compared with the normal group, the phosphorylation levels of PI3K and Akt proteins, the protein expression level of interleukin(IL)-1β, and the fluorescence intensities of IL-6 and IL-1β were significantly increased in the model group(P<0.05, P<0.01), and the mRNA levels of Slc5a1, Abhd6, Mgam, Atp1a1, Slc7a8, Cdr2, Chrm3, Slc5a9 and other key genes were significantly increased(P<0.01). Compared with the model group, the phosphorylation levels of PI3K and Akt, the protein expression level of IL-1β and the fluorescence intensities of IL-6 and IL-1β in the high-dose group of Xingpi capsules were significantly reduced(P<0.05, P<0.01), and the mRNA levels of Slc5a1, Abhd6, Mgam, Atp1a1, Slc7a8, Cdr2, Chrm3 and Slc5a9 were significantly reduced(P<0.05). Weighted gene co-expression network analysis and molecular docking results showed that E-nerolidol and Z-nerolidol in Xingpi capsules were well bound to ABDH6 protein, and linarionoside A, valerosidatum and senkirkine were well bound to Slc5a1 protein. ConclusionXingpi capsules can effectively improve the general survival and gastrointestinal motility of FD rats, its specific mechanism may be related to the inhibition of PI3K/Akt signaling pathway to alleviate the low-grade inflammation of duodenum, and E-nerolidol, Z-nerolidol, linarionoside A, valerosidatum and senkirkine may be its key active ingredients.

3.Sex and age distribution of global disease burden of calcific aortic valve disease.

OBJECTIVES:
To analyze sex and age distribution of global disease burden of calcific aortic valve disease (CAVD) from 1990 to 2021.
METHODS:
CAVD data during 1990-2021 were obtained from the IHME website for Global Burden of Disease (GBD). The prevalence, mortality, years lived with disability (YLDs), and disability-adjusted life years (DALYs) were analyzed by gender and age groups. Joinpoint regression was used to calculate annual percentage change (APC) and average annual percentage change (AAPC).
RESULTS:
In 2021, there were 13.32 million CAVD patients and 142 000 deaths caused by CAVD globally. Age-standardized prevalence was higher in males (193.2/105) than that in females (128.9/105). Patients in 65-<85 age group accounted for 64.0% of total cases, while those ≥85 years old accounted for 16.1%. From 1990 to 2021, prevalence increased in both sexes with an AAPC of 0.72% for males and 0.57% for females, respectively. Prevalence grew fastest from 2000 to 2010, slowed thereafter, and declined from 2015 to 2021. In <65 years old, the mortality of males was 2.4 times higher than that of females, while in ≥85 years old, mortality of females (117.3/105) exceeded that of males (99.1/105). YLD rates increased with age, and were higher in males for all age groups. DALY rates decreased overall but increased in ≥85 years old, with a greater increase in females.
CONCLUSIONS
There are significant gender and age disparities in global disease burden of CAVD, with the elderly, especially super-elderly females deserving particular attention. It is recommended to develop personalized intervention strategies for these populations.

4.Distribution and antimicrobial resistance analysis of bacteria isolated from the celiac cavity in Sichuan from 2019 to 2023

Objective To investigate the distribution and antimicrobial resistance of bacteria isolated from celiac specimens in Sichuan region, and to provide a reference for the selection of clinical antibiotics. Methods According to the technical protocol of the Sichuan Provincial Antimicrobial Resistance Technical Program, the data of bacteria isolated from celiac specimens and drug sensitivity results reported by members of Sichuan bacterial resistance monitoring network from 2019-2023 were statistically analyzed by WHONET 5.6 software, and only the first strain of the same bacteria from the same patient was included. Results A total of 22 204 non-repetitive bacterial isolates were obtained from celiac specimens in Sichuan region from 2019 to 2023, including 6 023 Gram-positive bacteria (27.1%) and 16 181 Gram-negative bacteria (72.9%). The top 10 bacteria in celiac specimens were Escherichia coli 45.8% (10 179/22 204), Klebsiella pneumoniae 8.1% (1 788/22 204), Enterococcus faecium 6.1% (1 361/22 204), Pseudomonas aeruginosa 5.8% (1 289/22 204), Enterococcus faecalis 3.0% (676/22 204), Streptococcus anginosus 2.8% (624/22 204), Staphylococcus aureus 2.2% (484/22 204), Staphylococcus epidermidis 2.1% (473/22 204), Enterobacter cloacae 2.0% (439/22 204), and Streptococcus constellatus 1.8% (399/22 204). The results of in vitro antimicrobial susceptibility testing showed that from 2019 to 2023, the resistance rates of Escherichia coli(E.coli) to ciprofloxacin and levofloxacin remained stable (average resistance rates were 42.5% and 38.8% respectively), with a decreasing trend in the rate of their resistance to third-generation cephalosporins, and carbapenem-resistant Escherichia coli was 0.7%-0.8%. Klebsiella pneumoniae had five-year average resistance rates of 24.2%, 28.3%, and 17.8% to ceftriaxone, cefotaxime, and ceftazidime, respectively, with 8.2%-9.9% carbapenem-resistant Klebsiella pneumoniae. The 5-year average resistance rate of Pseudomonas aeruginosa showed average resistance rates over five years not exceeding 5.0% to carbapenems, quinolones, ceftazidime, and piperacillin/tazobactam. Enterococcus faecium had average resistance rates over 80.0% to ciprofloxacin, levofloxacin, and ampicillin, while Enterococcus faecalis showed average resistance rates below 24.0% to these three antimicrobial drugs. The average detection rate of methicillin-resistant Staphylococcus aureus was 33.9%, while the average detection rates of vancomycin-resistant Enterococcus faecium and Enterococcus faecalis were 0.8% and 0.1%, respectively. Conclusions Among the bacteria isolated from celiac specimens in Sichuan region, the ratio of Gram-negative to Gram-positive bacteria is about 7∶3, mainly including E.coli, K. pneumoniae, E. faecium, P. aeruginosa, and E.faecalis. In addition, carbapenems still maintain a high antibacterial activity in vitro against Enterobacterales and P. aeruginosa.
5.Novel coronavirus pneumonia (COVID-19) after pulmonary surgery: A case report

There was a male novel coronavirus (2019-nCoV, SARS-CoV-2) pneumonia (COVID-19) patient after pulmonary surgery at age of 61 years. The patient had no clear history of contact COVID-19 patient before surgery. He developed transient fever on the 4th day after surgery. The body temperature returned to normal on the 5th day after antibiotic adjustment. The patient developed fever and fatigue again on the 6th day after surgery. A chest CT scan revealed postoperative pneumonia. The patient was treated by ganciclovir and moxifloxacin hydrochloride. The patient's temperature gradually decreased on the 7th to 9th days after the operation. CT scan on the 10th day after surgery showed viral pneumonia, so we immediately raised the level of protection. The novel coronavirus nucleic acid test was positive. The patient was immediately transferred to the designated hospital for treatment. The patient was treated by arbidol, moxifloxacin, human immunoglobulin (PH4), ambroxol and other nutritional symptomatic and supportive treatment. The patient's condition is currently stable. Ten people in close contact with the patient developed symptoms, and their CT scans showed viral pneumonia. Six of them were positive in nucleic acid tests, and the others were still under quarantine observation. This shows that it is easy to confuse the imaging manifestations of pneumonia with novel coronavirus pneumonia after lung surgery. We should perform nucleic acid detection as soon as possible in the early diagnosis of CT and reformulate the treatment protocol.
6.Development, Application Details, and Prospects of Uni-VATS on Lung Cancer Radical Operation under the Concept of"Precise Medical Treatment"

Uniportal video-assisted thoracic surgery (Uni-VATS) has become one of the most exciting new develop-ments in minimally invasive thoracic surgery in recent 5 years. With the development of better thoracoscopic cameras and the availability of endoscopic linear mechanical staplers, experts in the technique are able to use uniportal VATS to encompass the most complex procedures such as bronchial sleeve/vascular reconstructions or carinal resections. A large number of multi-center reports have conifrmed that Uni-VATS lobectomy is safe and feasible;dissected node group number and total number of mediastinal lymph nodes were not less than that of traditional muli VATS;Uni-VATS gained positive results in pain, trauma and recovery time. Although the large sample multi center clinical data, such as the ifve year survival rate has not yet been obtained, gradually carring out of uni-VATS will be an important direction for the future development of minimally invasive thoracic sur-gery. hTis article will focus on the principle of Uni-VATS and the speciifc operation details, combined with the basic operation concept, with a view to put forward the orderly and standard utilization of the Uni-VATS.
