1.Application of Acostream thrombus suction device in the treatment of acute medium-high risk pulmonary embolism

Objective To investigate the safety and effectiveness of Acostream thrombus suction device in the treatment of acute medium-high risk pulmonary embolism(APE).Methods Ten patients with APE medium-high risk treated with Acostream thrombus suction device in Wenzhou Central Hospital from June 2022 to January 2024 were selected as subjects.Clinical observation indicators such as vital signs and auxiliary examination before surgery and 72h after surgery were collected to record the relief of symptoms and complications.The safety and efficacy of the Acostream thrombus suction device for thrombus removal in the pulmonary arteries were evaluated.Respiratory status and complications were assessed at the 1,2 and 3 months.Results All patients completed the operation through the Acostream thrombus suction device.One patient was transferred to the Intensive Care Unit after 3d of automatic discharge because of the septic shock aggravation,and the remaining 9 patients showed significant improvement in strenuous breathing and other symptoms.At 72h after surgery,arterial blood oxygen partial pressure,finger pulse oxygen,heart rate,troponin I,N-terminal prohormone of brain natriuretic peptide,D dimer,and right ventricular ratio to left ventricular diameter of patients were improved compared with preoperative value,and the difference was statistically significant(P<0.05).During the follow-up period,one patient relapsed due to the withdrawal of anticoagulants.Conclusion Acostream thrombus suction device is safe,effective,and has high clinical application value in the treatment of medium-high risk APE.
2.Status quo of health information screening ability of rural elderly and its influencing factors

Objective:To understand the current situation of the ability to screen health information of the elderly in rural areas and analyze its influencing factors, so as to provide a basis for taking targeted measures.Methods:A cross-sectional survey was adopted, and rural elderly people in Yonghe Town, Dahu Town and Dayao Town of Liuyang City, Hunan Province were selected as the survey subjects by the convenience sampling method. The investigation was conducted using the general information Questionnaire, the Health Information Discrimination Ability Scale, the filial Piety Expectation Scale and the two-way Social Support Scale.Results:A total of 228 questionnaires were distributed, and 215 valid questionnaires were retrieved, with an effective recovery rate of 94.30%. Among the 215 rural elderly people surveyed, 86 were male and 129 were female. The ability to discriminate health information was (69.19 ± 13.41) points, the expectation of filial piety was (37.67 ± 6.12) points, and the two-way social support was (46.82 ± 8.11) points. The ability of rural elderly people to discriminate health information was positively correlated with filial piety expectations and two-way social support ( r = 0.340, 0.456; both P<0.01). The results of hierarchical regression analysis showed that educational level, filial piety expectations and two-way social support were the influencing factors of the current status of health information discrimination ability of rural elderly people ( t values were 2.60 to 5.35, all P<0.05). Conclusions:The ability of discriminating health information of the elderly in rural areas is affected by the educational level, filial piety expectation and two-way social support. The health information of rural elderly people with low education level should be screened and targeted, so as to enrich their cultural life and combine education with fun. Actively build a filial piety culture to promote children′s attention to their parents. In addition to providing social support for the elderly, we should promote their interpersonal and social communication and exert their self-value, so as to improve the health information screening ability of the elderly in rural areas.

5.Relationship between MTHFR C677T gene polymorphism and serum Hcy with restenosis after intervention in lower limb arteriosclerosis obliterans

Objective To explore the relationship between methylene tetrahydrofolate reductase(MTHFR)C677T gene polymorphism and serum homocysteine(Hcy)levels with postoperative restenosis in patients with lower limb arteriosclerosis obliterans(ASO)after intervention.Methods A total of 92 patients with lower limb ASO treated in Wenzhou Central Hospital from January 2021 to July 2022 were divided into non-restenosis group and restenosis group according to whether patients with postoperative restenosis.General and surgical data from all patients were collected,and preoperative neutrophils,lymphocytes,platelets,fibrinogen,D-dimer,C-reactive protein,low-density lipoprotein,homocysteine,MTHFR C677T gene polymorphism,etc were tested.x2 test and Logistic regression analysis were used to compare the relationship between lower limb ASO postoperative restenosis after intervention and MTHFR C677T gene polymorphism and serum Hey.A risk scoring system was built to predict postoperative restenosis after intervention and evaluate its effectiveness.Results There were significant differences(P<0.05)between the two groups in terms of lesion location,stent placement,stent length,serum Hcy,and MTHFR C677T gene polymorphism.Multivariate Logistic regression analysis showed that MTHFR C677T TT type,serum Hey 17.15μmol/L,lesion location in the femoral artery,and no stent placement were independent risk factors for postoperative restenosis in lower limb ASO patients(P<0.05).The area under the curve(AUC)of receiver operating characteristic curve of the risk scoring system constructed based on independent risk factors was 0.818(95%CI:0.731-0.905),which was significantly higher than those other independent risk factors and had high efficacy.Conclusion The MTHFR C677T gene polymorphism and serum Hcy are closely related to postoperative restenosis in lower limb ASO patients.A risk scoring system based on MTHFR C677T gene polymorphism,serum Hcy,lesion location,and stent placement can help predict the occurrence of restenosis in lower limb ASO patients.
6.Chronic Intermittent Hypobaric Hypoxia Ameliorates Renal Vascular Hypertension Through Up-regulating NOS in Nucleus Tractus Solitarii.

Chronic intermittent hypobaric hypoxia (CIHH) is known to have an anti-hypertensive effect, which might be related to modulation of the baroreflex in rats with renal vascular hypertension (RVH). In this study, RVH was induced by the 2-kidney-1-clip method (2K1C) in adult male Sprague-Dawley rats. The rats were then treated with hypobaric hypoxia simulating 5000 m altitude for 6 h/day for 28 days. The arterial blood pressure (ABP), heart rate (HR), and renal sympathetic nerve activity (RSNA) were measured before and after microinjection of L-arginine into the nucleus tractus solitarii (NTS) in anesthetized rats. Evoked excitatory postsynaptic currents (eEPSCs) and spontaneous EPSCs (sEPSCs) were recorded in anterogradely-labeled NTS neurons receiving baroreceptor afferents. We measured the protein expression of neuronal nitric oxide synthase (nNOS) and endothelial NOS (eNOS) in the NTS. The results showed that the ABP in RVH rats was significantly lower after CIHH treatment. The inhibition of ABP, HR, and RSNA induced by L-arginine was less in RVH rats than in sham rats, and greater in the CIHH-treated RVH rats than the untreated RVH rats. The eEPSC amplitude in NTS neurons receiving baroreceptor afferents was lower in the RVH rats than in the sham rats and recovered after CIHH. The protein expression of nNOS and eNOS in the NTS was lower in the RVH rats than in the sham rats and this decrease was reversed by CIHH. In short, CIHH treatment decreases ABP in RVH rats via up-regulating NOS expression in the NTS.

7.Effect of methanol-ethyl acetate partitioned fractions from on proliferation and apoptosis of human non-small cell lung cancer H1975 cells.

OBJECTIVE:
To investigate the effects of methanol-ethyl acetate partitioned fractions from (MEDS) on the proliferation and apoptosis of human non-small cell lung cancer H1975 cells.
METHODS:
The systemic solvent extraction method was used to preliminary separation of the effective fractions in the methanol extract of . The cytotoxicity of each extract (5, 10, 20, 40, and 80 μg/mL) was tested using MTT assay. Colony cloning method was used to assess the effect of different concentrations of methanol-ethyl acetate partitioned fractions from MEDS (5, 10, 20, 40, and 80 μg/ mL) on the proliferation of H1975 cells. Flow cytometric analysis with Annexin V-FITC/PI staining was performed to detect the apoptosis of the cells after treatment with different concentrations of MEDS fractions (10, 20, and 40 μg/mL). Western blotting was used to evaluate the effects of MEDS fractions on the expressions of apoptosis-related proteins Akt, Bax, and Bcl-2. The anti-tumor activity of 100 mg/kg MEDS fractions was tested in a nude mouse model bearing H1975 cell xenografts.
RESULTS:
MTT assay and colony forming experiment showed that MEDS fractions significantly inhibited the proliferation of H1975 cells in a dose-and time-dependent manner ( < 0.05). The results of flow cytometry showed that MEDS fractions induced obvious apoptosis of H1975 cells in a concentration-dependent manner ( < 0.05). MEDS fractions also significantly decreased the expressions of Bcl-2 and Akt protein and increased the protein expression of Bax ( < 0.05). In the tumor-bearing nude mouse model, MEDS fractions showed potent anti-tumor effects with a low toxicity to affect the body weight and organs of the mice.
CONCLUSIONS
The methanol-ethyl acetate partitioned fractions from MEDS show potent anti-tumor activity both and , suggesting their value as promising therapeutic agents against lung cancer.

8.Survey on nutritional knowledge, attitude and practice of medical staff in the nerve profession

Objective To investigate nutritional knowledge,attitude,and practice (K-A-P) of medical staff in the nerve-related departments in Hebei province,with the aim of determining a basis for the effective education of nutrition.Methods A multicenter,observational study was utilized to conduct a questionnaire survey.642 medical staff in nerve-related departments from 15 hospitals in 11 prefecture level cities was surveyed from September 2016 to March 2017 in Hebei Province.Results were statistically analyzed.Results The overall scores of nutrition knowledge,attitude,and practice were 34.88±22.90,69.14± 16.75 and 42.86± 18.22,respectively.There were positive correlations between nutrition knowledge and attitude (r=0.150,P< 0.001),knowledge and practice (r=0.152,P<0.001),and attitude and practice (r =0.432,P<0.001).The nutrition knowledge score of male medical staff was higher than that of female (t =3.546,P<0.01) however,there was no significant difference between males and females in nutrition attitude and practice scores (t =0.685,P=0.494;t=1.649,P=0.091).In addition,the nutrition knowledge scores of physicians and medical technicians were significantly higher than that of nurses (F=46.89,P<0.01) while the nutrition practice scores of physicians were significantly higher than both medical technicians and nurses (F=7.85,P<0.01).It was also found that nutrition attitude scores of senior and secondary medical staff were higher than junior medical staff (F=5.116,P=0.002) and nutrition practice scores of senior and secondary senior medical staff were higher than that of junior medical staff (F=4.074,P=0.007).Furthermore,it was found that nutrition knoledge scores of undergraduate,master and doctoral degree staff were higher than that of junior college staff (F=15.617,P<0.001) and nutrition attitude score of undergraduate and master degree staff were significantly higher than that of junior college staff (F =4.11,P =0.003).It was also found that the nutrition knowledge scores of medical staff with less than 5 years of work experience were significantly higher than those with more than 5 years of work experience (F=3.312,P =0.02).However,the scores of nutrition attitude of medical staff with more than 15 years of work experience were significantly higher than those with less than 15 years of work experience (F=6.356,P<0.001) and the scores of nutrition practice of medical staff with 10-15 years of work experience were higher than that of less than 5 years and 5-10 years of work experience (F=2.959,P=0.032).It was also identified that the top three ways to obtain nutrition knowledge were learning at work (68.85%),learning at university (68.69%) and reading books and journals (57.79%).Conclusion The medical staff of nerve-related departments in Hebei province has positive attitudes towards nutrition treatments.However,their mastery of nutritional knowledge and practice could and should be improved further.Therefore,it is necessary to strengthen the nutrition education for medical staff of the nerve profession.
9.FEN1 expression in oral carcinoma-associated fibroblasts and its relationship with PCNA

Objective:
To research the expression levels of FEN1 and PCNA in carcinoma-associated fibroblasts (CAFs) and analyze their correlation.
Methods:
Fresh specimens of oral squamous cell carcinoma tissues and normal oral mucosal tissues excised during oral and maxillofacial plastic surgery were collected. Primary oral CAFs and normal fibroblasts (NFs) were obtained by tissue culture, identified by immunocytochemistry and divided into the CAF and NF groups. Western blot and quantitative real-time PCR were used to detect the protein and mRNA expression levels of both FEN1 and PCNA in the oral CAFs and NFs. The correlation between FEN1 and PCNA expression in oral CAFs was analyzed.
Results:
Oral CAFs and oral NFs were successfully cultured and identified from 12 samples. Both the protein and mRNA expression levels of FEN1 and PCNA were higher in the oral CAFs than NFs, but there were no significant differences (P > 0.05). In the oral CAFs, the linear correlation coefficient between FEN1 and PCNA was 0.677 (P = 0.016) at the mRNA level, indicating a strong positive correlation; however, at the protein level, no correlation was found (P > 0.05).
Conclusion
In primary cultured oral CAFs and NFs, there were no significant differences in the FEN1 and PCNA protein and mRNA expression levels. However, in the CAFs, the mRNA levels of FEN1 and PCNA had a strong positive correlation. The relationship and the regulatory mechanism of the two genes require further study.
10.Influence of serum resistin level on blood pressure in patients with type 2 diabetes mellitus complicated hypertension

Objective: To study the correlation between serum resistin (SR) level and blood pressure in patients with type 2 diabetes mellitus (T2DM) complicated hypertension. Methods: A total of 124 patients with T2DM complicated hypertension (T2DM + hypertension group), 85 patients with pure T2DM (pure DM group) and 71 normal subjects (normal control group) were enrolled. According to blood pressure level, T2DM + hypertension group was further divided into stage 1 subgroup (n=34), stage 2 subgroup (n=43) and stage 3 subgroup (n=47). Peripheral venous blood was taken to measure levels of SR and other indexes in three groups. Logistic multi-factor regression analysis was used to analyze the risk factors affecting blood pressure level. Kendall’s tau-b correlation analysis was used to analyze the correlation among SR, fasting insulin, quantitative insulin sensitivity check index, homeostasis model-insulin resistance index and blood pressure level. Results: Compared with normal control group, there were significant rise in SR levels [(7.21±2.65) ng/ml vs. (19.87±3.67) ng/ml vs. (31.32±3.89) ng/ml] in pure DM group and T2DM + hypertension group, and that of T2DM + hypertension group was significantly higher than that of pure DM group, P<0.01 all. Logistic multi-factor regression analysis indicated that SR level was an independent risk factor influencing blood pressure level of patients (R2=0.087, P<0.05). Kendall’ s tau-b correlation analysis indicated that SR level was significantly positively correlated with blood pressure level of patients with hypertension (r=0.202, P=0.001); after other influencing factors were controlled, the significant positive correlation still existed between them (r=0.233, P=0.022). Conclusion: Serum resistin level rise is a risk factor of patients with type 2 diabetes mellitus complicated hypertension, and it is significantly positively correlated with their blood pressure level.
