1.Clinical analysis of 32 cases of temporal lobe epilepsy treated by microsurgery under intraoperative cortical EEG monitoring

Objective:To investigate the effect of microsurgery by modified pterional approach in the treatment of temporal lobe epilepsy under intraoperative cortical encephalon electricity graph (EEG) monitoring.Methods:The clinical data of 32 patients with temporal lobe epilepsy who were admitted to the Department of Neurosurgery of Shangqiu First People's Hospital from January 2012 to June 2021 were retrospectively analyzed, all patients underwent microsurgical resection of epileptogenic foci by modified pterional approach under cortical EEG monitoring.Results:The postoperative follow-up was from half a year to 6 years. According to the Tan's classification, 25 cases (78.1%(25/32)) of seizures disappeared completely, 3 cases (9.4%(3/32)) of seizures decreased by more than 75%, and 4 cases (12.5%(4/32)) of seizures decreased by more than 50%. Isotropic hemianopia occurred in 1 case (3.1%) after operation, and there was no operative death.Conclusion:Microsurgical resection of epileptogenic foci through modified pterional approach under intraoperative cortical EEG monitoring was a safe and effective method for the treatment of temporal lobe epilepsy.

Population aging increases the demand for human aging research and its clinical applications.Traditionally, the chronological age(CA), that is, the age based on the calendar, is used to describe the state of aging.However, the aging process and speed among individuals are not consistent and often show clear individual differences in biological aging.Therefore, CA cannot truly reflect people's conditions of body structure and function, has drawbacks leading to unreliable and wrong assessment, and is unable to accurately describe the human body's state of aging.In recent years, it has been proposed that the biological age(BA)should be used to more comprehensively and accurately describe the stage of human aging.Combining mathematical algorithms with a variety of biomarkers, predictive models can be constructed to quantify BA.These approaches have been increasingly appreciated for their improved accuracy and received further investigation.This article reviews the value of BA, currently commonly used calculation methods and their progress and prospects in healthy aging.

4.Advances in the application of Mendelian randomization in investigating the etiologies of common diseases in the elderly

Mendelian randomization is a genetic epidemiological method for confounding adjustment of cross-sectional data by introducing genotypes as instrumental variables of exposure factors and possesses unique advantages for the adjustment of unmeasured and unknown confounding factors.In recent years, Mendelian randomization has been widely used and achieved rapid progress in the etiology research of common diseases in the elderly.This article reviews this method and its application in these diseases, including cardiovascular disease, type 2 diabetes mellitus, osteoporosis, Alzheimer's disease and malignant tumor.
5.Bottlenecks and modification strategies of 1,3-propanediol biosynthesis from glycerol.

Crude glycerol is the main by-product of biodiesel production. A few microorganisms can transfer crude glycerol to 1,3-propanediol (1,3-PD) that is an important chemical material. There exist many limitations such as substrate inhibition, product inhibition when wild strains are used in 1,3-PD biosynthesis. In this review, based on the microbial transformation of 1,3-propanediol from glycerol and its limitations, some strategies using genetic engineering such as knockout or gene overexpression were summarized. The latest research progresses in biosynthesis of 1,3-propanediol from glycerol by genetically engineered strains are discussed.
6.IDH1 mutation and MGMT expression in astrocytoma and the relationship with prognosis after radiotherapy.

OBJECTIVETo study the correlation between IDH1 mutation, MGMT expression, clinicopathologic features and post-radiotherapy prognosis in patients with astrocytoma.
METHODSDetection of IDH1 mutation and MGMT expression was carried out in 48 cases of astrocytoma (WHO grade II to III) by EnVision method with immunohistochemical staining. Follow-up data, including treatment response and overall survival time, were analyzed.
RESULTSThe rates of IDH1 mutation and MGMT expression in astrocytomas were 62.7% (30/48) and 47.9% (23/48), respectively. There was a negative correlation between IDH1 mutation and MGMT expression (r = -0.641, P < 0.01). The age of patients with IDH1 mutation was younger at disease onset. The IDH1 mutation rate in patients with WHO grade II astrocytoma was higher than that in patients with WHO grade III tumor (P < 0.05). The age at onset was an independent factor affecting the expression of mutant IDH1. After radiotherapy, patients with IDH1 mutation+/MGMT- tumor carried a longer overall survival time than patients with IDH1 mutation-/MGMT+ tumor (P < 0.05).
CONCLUSIONSThere is a correlation between IDH1 mutation and MGMT expression in WHO grade II to III astrocytoma. Age at onset is an independent factor affecting the expression of mutant IDH1. Tumors with IDH1+/MGMT- pattern show better response to radiotherapy than tumors with IDH1-/MGMT+ pattern. Detection of IDH1 mutation and MGMT protein expression can provide some guidance in choice of treatment modalities in patients with astrocytoma.

7.CT value characteristics and significance of Graves disease and Hashimoto's thyroiditis

Objective To investigate the change and clinical significance of thyroid CT value in patients with Graves disease and Hashimoto' s thyroiditis (HT).Methods One hundred and twelve Graves patients (GD group),54 HT patients (HT group) and 36 healthy people (NC group) were selected as our subjects.Computed tomography (CT) value of thyroid were measured.Results The CT value of the left thyroid gland was (70.53 ± 15.44) Hu in GD group,while (70.50 ± 16.01) Hu at the right side.In HT group,the CT value of the left thyroid gland was (53.77 ± 4.88) Hu,and (52.38 ± 6.67) Hu at the right side.The CT value of the left and right sides was (99.66 ±9.80) Hu and (100.77 ± 10.66) Hu in the NC group respectively.The CT value of GD and HT group were lower than that of the NC group (P < 0.01),and the CT value of the HT group lower than that of the GD group more significant (P < 0.01).Conclusion (1) The CT value of the GD group and the HT group are significantly lower than the NC group and the CT value of the HT group reduces more apparently than that of the GD group.(2) CT values can be used as an early,non-invasive diagnosis of GD and HT secondary indexes.
8.Lentivirus-mediated RNAi against xenoantigen in mouse vascular endothelial cells

Objective To investigate the feasibility of inhibiting Galα (1,3)-Gal expression in mouse vascular endothelial cells by lentivirus-mediated RNAi.Methods The shRNA specified to α1,3-GT mRNA was designed and synthesized in vitro and cloned into the lentivirus vector.EOMA cells were infected by recombinant lentivirus.Real-time RT-PCR was used to detect mRNA transcriptional levels of αl,3-GT as well as immunofluorescence and flow cytometry were applied to detect Galα(1,3)-Gal antigen level after gene transfection.Co-culture of infected EOMA and serum of human was done and the survival rate was measured by MTT.Results The αl,3-GT shRNA sequences were cloned into the recombinant lentivirus vector correctly and the lentivirus was produced successfully.The transfection efficiency to EOMA was 75 %.Real-time PCR revealed that the mRNA transcription of α1,3-GT was obviously inhibited by α1,3-GT shRNA recombinant lentivirus with the rate of 88 % (P<0.05),while there were no obvious differences among control group,no shRNA lentivirus group and negative-shRNA lentivirus group (P> 0.05).Immunofluorescence and flow cytometry demonstrated the same results that Galα(1,3)-Gal antigen expression in EOMA transfected by α1,3-GT shRNA lentivirus was less than that of control group,no shRNA lentivirus group and negative-shRNA lentivirus group (P<0.05),but there were no obvious differences among the later three groups (P>0.05).After co-culture with serum of human,MTT showed the survival rate of EOMA infected by α1,3-GT shRNA lentivirus was obviously increased (P< 0.05).Conclusion Recombinant α2,3-GT shRNA 1entivirus is constructed successfully,which can inhibit the expression of α1,3-GT and Galα1,3-Gal in EOMA by RNAi and control hyperacute rejection in vitro.
