1.Association between magnesium sulfate exposure within 24 hours before delivery and short-term outcomes in preterm infants under 34 weeks: a single-center retrospective cohort study

Objective:To investigate the association between magnesium sulfate exposure within 24 h before birth and short-term outcomes in preterm infants under 34 weeks' gestation.Methods:This retrospective cohort study analyzed data from preterm infants under 34 weeks admitted to the neonatal intensive care unit of the Affiliated Hospital of Qingdao University between June 1, 2020, and December 31, 2024. Infants were categorized into the exposure and the control groups based on magnesium sulfate administration within 24 hours before delivery. Maternal characteristics, birth parameters, and neonatal outcomes were compared using independent t-tests or Chi square tests (Fisher's exact test), with logistic regression assessing magnesium sulfate's effect on non-cerebral palsy outcomes. Results:The cohort comprised 384 preterm infants (24-33 +6 weeks), with 290 (75.5%) in the exposure group and 94 (24.5%) in the control group. (1) Baseline characteristics showed no significant differences in maternal hypertensive disorders, antenatal corticosteroids administration, premature rupture of membranes, delivery mode, or male infant proportion (all P>0.05). The exposure group had lower birth weight [(1 583±451) vs. (1 744±473) g; t=2.97] and gestational age [(31.3±2.1) vs.(31.8±2.4) weeks; t=2.20; both P<0.05)]. (2) The exposure group demonstrated reduced incidence of in-hospital mortality [0.3% (1/290) vs. 6.4% (6/94); Fisher's exact test], grade Ⅲ-Ⅳ intracranial hemorrhage [1.7% (5/290) vs. 9.6% (9/94); χ2=12.86], and white matter injury [2.1% (6/290) vs. 9.1% (8/94); χ2=9.08] (all P<0.01). (3) Univariate logistic regression identified antenatal magnesium sulfate as protective against grade Ⅲ-Ⅳ intracranial hemorrhage ( OR=0.20, 95% CI: 0.04-0.96), white matter injury ( OR=0.11, 95% CI: 0.01-0.91), and in-hospital mortality ( OR=0.93, 95% CI: 0.88-0.99). (4) Multivariate analysis confirmed the independent protective effect of antenatal exposure of magnesium sulfate against intracranial hemorrhage in preterm infants under 34 weeks ( OR=0.19, 95% CI: 0.04-0.95), particularly pronounced in infants <32 weeks ( OR=0.11, 95% CI: 0.01-0.96). (5) No significant differences emerged in secondary outcomes including 5-minute Apgar scores, respiratory distress syndrome, surfactant administration, mechanical ventilation, bronchopulmonary dysplasia, hemodynamically significant patent ductus arteriosus, necrotizing enterocolitis, early-onset sepsis, retinopathy of prematurity, metabolic bone disease, or hospitalization duration (all P>0.05). Conclusions:Magnesium sulfate exposure within 24 hours before delivery reduces grade Ⅲ-Ⅳ intracranial hemorrhage risk in preterm infants under 34 weeks, with enhanced protection in those <32 weeks, without increasing adverse effects in other organ systems.

Through rigorous procedures, including collecting and reviewing achievements, writing achievement reports, organizing expert reviews, and seeking public opinions, remarkable research achievements in TCM during 2012—2022 were compiled.
4.miR-4645-5p regulates malignant biological behavior of esophageal cancer cells by targeting MUC16

Objective To investigate the effect of microRNA(miR)-4645-5p on the proliferation,invasion and epithelial-mesenchymal transition of esophageal cancer cells by targeting mucin 16(MUC16)and its mo-lecular mechanism.Methods The expression of miR-4645-5p in esophageal cancer tissues was analyzed online by TCGA database.The expression level of miR-4645-5p in esophageal cancer cell lines was analyzed by fluo-rescent real-time fluorescence quantitative polymerase chain reaction(qPCR).KYSE-30 cells were transfected with miR-4645-5p mimic and negative control mimic by lipofection technology,and were divided into miR-4645-5p group and control mimic group.The proliferation ability,migration ability and invasion ability of transfected KYSE-30 cells were analyzed by CCK-8 method,scratch test and Transwell test respectively.The target gene of miR-4645-5p was predicted by the bioinformatics website,and the binding of miR-4645-5p to the target gene was detected by the dual-luciferase reporter gene assay.The expression level of MUC16 mR-NA was detected by qPCR,and the protein expression levels of MUC16,transcription factor-1(ZEB-1),zonal atresia protein(ZO-1),tight junction protein-1(Claudin-1)and α-smooth muscle actin(α-SMA)were detected by Western blotting.Results The expression level of miR-4645-5p in esophageal cancer tissues was signifi-cantly lower than that in adjacent tissues(P<0.01).Compared with HET-1 A,the expression of miR-4645-5p was lower in esophageal cancer cell lines(P<0.05).After overexpression of miR-4645-5p,the proliferation a-bility of KYSE-30 cells was significantly reduced(P<0.05),the migration ability was significantly reduced(P<0.01)and the invasion ability was significantly reduced(P<0.01).miR-4645-5p targeted and negatively regulated the expression of MUC16 mRNA(P<0.01).After overexpression of miR-4645-5p,the protein ex-pression levels of MUC16,ZEB-1 and α-SMA were all down-regulated,and the protein expression levels of ZO-1 and Claudin-1 were up-regulated.Conclusion miR-4645-5p regulates the malignant biological behavior of esophageal cancer KYSE-30 cells by targeting MUC16.
5.Effects and Mechanisms of Electroacupuncture at Zhongwan (CV12) on Gastric Nociceptive Response Induced by Gastric Acid Stimulation in Rats

ObjectiveTo observe the effects of electroacupuncture at Zhongwan (CV12) on gastric nociceptive response induced by gastric acid stimulation and explore the underlying mechanisms associated with nuclei of the medullary viscerosensory and visceral motor neurons. MethodsTwenty SD rats were given intragastric administration of 0.5 mol/L diluted hydrochloric acid (0.5 ml/100 g) to induce gastric nociceptive response induction. Eight rats were randomly selected to record the gastric slow wave (GSW) area under the curve, and extracellular discharge frequency of neurons in the nucleus of the solitary tract (NTS) and dorsal motor nucleus of the vagus nerve (DMV) before intragastric administration and at 1, 5, 10, 15, 20, 25, 30, 35, 40, 45, 50, 55, and 60 minutes after intragastric administration. The remaining 12 rats received electroacupuncture intervention at Zhongwan within 5 to 25 minutes after intragastric administration of diluted hydrochloric acid, with a duration of one minute. The GSW area under the curve and extracellular discharge frequency of NTS and DMV neurons were compared between the 1-minute intervals before and after electroacupuncture intervention. ResultsCompared to the baseline before intragastric administration, the area under the curve of GSW significantly increased at 1, 5, 10, 15, 20, and 25 minutes after intragastric administration, and the extracellular discharge frequency of excitatory neurons in the NTS (accounting for 90%, 57/63) significantly increased at 1, 5, 10, 15, 20, 25, 30, 35, and 40 minutes, both reaching peak values at 1 minute after intragastric administration (P<0.01 or P<0.05). The extracellular discharge frequency of inhibitory neurons in the DMV (accounting for 91%, 20/22) showed a non-significant increase at 1 minute after intragastric administration (P>0.05), but significantly decreased at other timepoints (P<0.05). Compared to the baseline before electroacupuncture intervention, the GSW area under the curve and the extracellular discharge frequency of excitatory neurons in the NTS significantly decreased (P<0.05), while the extracellular discharge frequency of inhibitory neurons in the DMV showed no significant difference (P>0.05). ConclusionElectroacupuncture at Zhongwan can improve gastric nociceptive response induced by gastric acid stimulation, possibly by reducing the transmission of visceral sensation and decreasing the excitability of NTS neurons in the medulla.

7.Efficiency of "short time sampling" in noninvasive preimplantation genetic testing: a prospective study

Objective:To explore the efficiency of "short time sampling" for noninvasive preimplantation genetic testing (ni-PGT).Methods:A prospective study was conducted. The blastocysts of patients who were treated by preimplantation genetic testing for aneuploidy (PGT-A) in the Reproductive Center of Xiangtan Central Hospital from March 2021 to June 2022 were biopsied, meanwhile the spent culture medium (SCM) were collected. According to the different sampling methods for SCM, they were divided into two groups, direct sampling method and "short time sampling". The coincidence rate between ni-PGT and PGT-A was analyzed and compared statistically.Results:Totally 108 blastocysts from 36 couples were biopsied and SCMs were collected, including 59 cases of direct collection and 49 cases of "short time sampling". The area under curve (AUC) of direct collection method for diagnosing embryo ploidy was lower than that of "short time sampling" (0.628 vs. 0.785). Conclusion:The efficiency of "short time sampling" in diagnosing embryo ploidy is better than that of direct collection method.

9.Related risk factors for the prognosis of hospital-acquired carbapenem-resistant Klebsiella pneumoniae bloodstream infections in elderly patients with critical illness

Objective:To investigate the related risk factors for the prognosis of hospital-acquired carbapenem-resistant Klebsiella pneumoniae (CRKP) bloodstream infections in elderly patients with critical illness.Methods:Clinical data of elderly patients with nosocomial CRKP bloodstream infection in intensive care unit (ICU) from Jan. 2010 to Dec. 2016 were retrospectively analyzed. Patients were divided into the death and survival groups according to the prognosis. Clinical characteristics were compared between the two groups. Influencing factors for the prognosis of nosocomial CRKP bloodstream infections in elderly ICU patients were screened by multivariate Logistic regression analysis.Results:A total of 119 elderly ICU patients with nosocomial CRKP bloodstream infection were enrolled. The overall ICU mortality rate was 62.2% (74/119 patients), among which the ICU mortality was lower in patients treated with tigecycline than without tigecycline treatment (50.0% or 25/50 vs. 71.0% or 49/69, χ2=4.770, P=0.029). And the ICU mortality was lower in patients with combination therapy than with mono-therapy (54.9% or 39/71 vs. 72.9% or 35/48, χ2=3.940, P=0.047). Multivariate Logistic regression analysis revealed that the administration of vasoactive drugs ( OR=25.545, 95% CI: 9.743-52.242, P=0.001), and the resistance to tigecycline ( OR=8.990, 95% CI: 0.957-24.488, P=0.049) were independent risk factors for ICU mortality. While the early initiated appropriate antibiotics treatment, which was defined as using at least one susceptible antibiotic within 48 hours ( OR=0.081, 95% CI: 0.014-0.463, P=0.005), and appropriate antibiotics and adequate duration ( OR=0.785, 95% CI: 0.631-0.977, P=0.030), were protective factors for the good outcome. Conclusions:Nosocomial CRKP bloodstream infection in elderly ICU patients leads a high ICU mortality rate. The early initiated appropriate antibiotics treatment and optimum antibiotics duration could reduce the risk for death.
