OBJECTIVE To screen a single matrix, prepare dissolving microneedles of Pterocarya hupehensis total flavonoids(PHTF-MN), and characterize it while evaluating its analgesic effects.METHODS Using microneedle morphology and puncture rate as evaluation indicators, the matrix was screened from polyvinyl alcohol (PVA), polyvinylpyrrolidone K30 (PVP K30), PVP K90, and sodium hyaluronate with relative molecular weights of 15 000 and 60 000 (HA 1.5w and HA 6w ). PHTF-MN was prepared by the layer-by-layer casting method with the selected matrix, and the appearance and morphology, total flavonoid content, mechanical properties, puncture performance, dissolution properties, and drug release behavior of the obtained PHTF-MN were characterized. Using male BALB/c mice as test subjects and aspirin (ASA) as a positive control, the analgesic effects of the microneedles were evaluated using a formalin-induced pain model.RESULTS Blank microneedles prepared with 30% HA 1.5w exhibited optimal surface flatness and the membrane puncture rate (through 4 layers of parafilm membrane) reaching 78%. The PHTF-MNs (with PHTF contents of 10, 20, 30, 40, and 50 mg/g in the matrix) exhibited uniform arrays and intact tips, with RSDs for total flavonoid contents all below 7% ( n =8). The single-needle puncture forces were 1.46 and 1.07 N (with PHTF contents of 30 and 50 mg/g in the matrix, respectively). The skin penetration rate approached 100%, with visible puncture marks in the stratum corneum and drug residue (with a PHTF content of 50 mg/g in the matrix). After 30 min of fixation, the needles had almost completely dissolved, and the skin retention rates all exceeded 90% (with PHTF contents of 10, 30, and 50 mg/g in the matrix, respectively). The cumulative release within 8 h was (134.64±2.03) μg/cm 2 , and the fit to the first-order kinetic equation ( R 2 =0.950 1) was highest (with a PHTF content of 50 mg/g in the matrix). Low- and high-dose PHTF-MN (containing 85.26 and 145.03 μg of total flavonoids per patch, respectively; 1 patch/day) significantly shortened the cumulative time of phase Ⅱ pain response in mice ( P <0.05), with pain inhibition rates (43.67% and 59.78%) comparable to those of ASA (53.70%).CONCLUSIONS PHTF-MN using HA 1.5w as a single matrix was successfully prepared. This formulation features a simplified preparation process, uniform drug loading, favorable mechanical properties, resistance to fracture, and rapid dissolution. Its drug release behavior conforms to a first-order kinetic model, and its analgesic effect is comparable to that of ASA.

OBJECTIVE To construct and evaluate a perioperative pharmaceutical care model for orthopedic joint replacement based on clinical pharmacy specialty collaboration and medication therapy management (MTM), so as to provide a reference for improving the level of clinical rational drug use and reducing medical costs.METHODS Patients undergoing orthopedic joint replacement were taken as the research subjects. A specialized collaboration group was established, consisting of the head of the clinical pharmacy group, ward-based clinical pharmacists, and multi-specialty clinical pharmacists. The practice path, service contents, and workflow of MTM for the perioperative period of orthopedic joint replacement were clarified, and a perioperative pharmaceutical care model for orthopedic joint replacement was developed. Data of patients in the orthopedic diagnosis-related groups (DRG)-IC29 group of the department of orthopedics in Daping Hospital,Army Medical University were collected from July 2022 to June 2023 (pre-intervention, 581 cases) and from July 2023 to June 2024 (post-intervention, 843 cases). The work-implementation indicators in the post-intervention period were counted, and the efficiency indicators and pharmaceutical management indicators were compared between the pre- and post-intervention periods. Meanwhile,200 patients were randomly selected from the pre-intervention and post-intervention groups, respectively, to evaluate the rate of rational medical orders.RESULTS From July 2023 to June 2024, the collaboration group implemented MTM for 843 patients undergoing orthopedic joint replacement via the established perioperative pharmaceutical care model. A total of 1 163 drug‑related problems (DRPs) were identified, and 79 patients with high medication‑related risks were screened out. The most common types of DRPs included (possible) adverse drug events/reactions (48.93%), unnecessary drug therapy (21.58%), and ineffective drug therapy (16.25%). Among the 1 163 DRPs, 895 (76.96%) were completely resolved. Compared with the pre‑intervention period, the post‑intervention period showed reductions in average hospitalization cost per case, average drug cost per case, average length of hospital stay, proportion of drug cost, utilization rate and use intensity of antibacterial agents, and per‑capita drug cost of six main drug categories. The rate of rational medical orders increased from 78.25% to 98.11%.CONCLUSIONS The perioperative pharmaceutical care model for orthopedic joint replacement based on clinical pharmacy specialty collaboration and MTM can help pharmacists identify and resolve DRPs during the perioperative period of orthopedic joint replacement, improve the level of clinical rational drug use,and reduce treatment costs.

ObjectiveTo investigate the temporal trend of weight-adjusted-waist index (WWI) and its association with the risk of incident cardiovascular diseases (CVD) among middle-aged and older Chinese adults. MethodsBased on the China Health and Retirement Longitudinal Study (CHARLS), this research enrolled participants aged 45 and above with complete physical measurement data from the 2011, 2013, and 2015 waves. Complex sampling weights were applied to calculate the mean WWI, and the Mann-Kendall test was used to analyze its trend. From the 2011 baseline survey, individuals with a history of cardiovascular disease or missing key covariates were further excluded. New-onset cardiovascular outcome events were collected during follow-up until 2020. Participants were categorized into Q1–Q4 groups based on baseline WWI quartiles. Multivariable log-binomial regression models were constructed to assess the association between WWI and CVD, while restricted cubic splines were employed to evaluate potential nonlinear relationships. Sensitivity analyses were conducted to verify the robustness of the findings. ResultsFrom 2011 to 2015, the weighted mean WWI among Chinese middle-aged and older adults remained generally stable (2011: 11.17±1.68; 2013: 11.28±1.35; 2015: 11.24±1.59; P for trend <0.05). During the longitudinal follow-up, 1 784 incident CVD cases were documented. After multivariable adjustment, each unit increase in WWI was associated with a 17% increased risk of CVD (RR: 1.17, 95%CI: 1.10-1.24). Compared with the Q1 group, the risk of CVD in the Q2, Q3, and Q4 groups increased by 23% (RR: 1.23, 95% CI: 1.07-1.40), 27% (RR: 1.27, 95% CI: 1.11-1.44), and 40% (RR: 1.40, 95% CI: 1.22-1.60), respectively. A significant linear trend was observed between the increasing WWI and elevated CVD risk (P for trend <0.001). Subgroup analysis revealed a stronger association in participants aged 45-60 years (P for interaction <0.05). The results of sensitivity analysis were consistent with the main research results. ConclusionsThe overall trend of WWI level in the middle-aged and elderly population in China is stable from 2011 to 2015. The higher level of WWI is associated with an increased risk of CVD, suggesting that WWI may be used as a simple and effective indicator to evaluate the risk of cardiovascular metabolism in middle-aged and elderly people.
