1.Standardized scheme for deploying and withdrawing surgical module of new-type tent hospital system

Objective To explore the scheme for the deployment and withdrawal of the surgical module of the new-type tent hospital system.Methods A set of standardized scheme for deploying and withdrawing the surgical module of the new-type tent hosital system was proposed and implemented in terms of labor division,operation precedure,operation technique and precaution.The operating time,number of operational errors and number of equipment damages were recorded for each of the five deployment and withdrawal operations before and after the program was executed,and the team members'immediate heart rate,percentage of maximum heart rate(MHR)and rating of perceived exercise(RPE)at the end of the operation were recorded after the program was implemented.SPSS 26.0 software was used for statistical analysis.Results The standardized scheme had the deployment time shortened from(85.15±11.430)min to(58.23±8.513)min,withdrawal time decreased from(65.36±9.369)min to(48.92±7.129)min,with the differences being statistically significant(P<0.05);the numbers of operatio-nal errors and equipment damages were both reduced when compared with those before the implementation of the schemce;the immediate heart rate of the team members at the end of the operation ranged from 43 to 157 beats/min,with an average value of 151.1 beats/min,the individual MHR percentages were from 75%to 87%,with an average value of 81.1%,and the RPE scores were from 14 to 17,with an average value of 15.3,which all could be categorized as moderate-operation intensity.Condusion The standardized deployment and withdrawal scheme for the surgical module meets the needs of actual combat and training assessment,and thus is worthy promoting in medical institutions equipped with the surgical module of the new-type tent hosital system.[Chinese Medical Equipment Journal,2025,46(2):74-79]

Objective:To observe the role of impulsivity in cognitive impairment in manic episodes of bipolar Ⅰ disorder(BPDTI)patients.Methods:180 cases of patients with BPDTI manic episodes admitted to our hospital from February 2024 to December 2024 were selected as the manic group,and another 180 cases of volunteers who were psychologically tested as normal during the same period were selected as the healthy group.The impulse Barratt-11 scale(BIS-11)and the MATRICS Consensus Cognitive Test(MCCB)were used to investigate and evaluate impulsivity and cognitive function,and correlation analysis and risk factor analysis were conducted.Results:In the BIS-11 scale,there was no statistically significant difference between the two groups in the unplanned factor(P>0.05),but there were significant differences compared between the two groups in cognitive,motor factors,and total score(P<0.05).The MCCB scores of the manic group in information processing speed,working memory,word learning,visual learning,social cognition,reasoning and problem solving,and attention vigilance were significantly lower than those of the healthy group(P<0.05).The relationship between BIS-11 total score and cognitive impulsivity factor and MCCB total score and word learning showed there were negative correlation(P<0.05);The exercise factor in the BIS-11 scale were negatively correlated with the MCCB total score and working memory factor(P<0.05);The unplanned factor in the BIS-11 scale showed there were negative correlation with the MCCB total score,word learning(P<0.05),and information processing speed.BIS-11,the number of years of education was an influencing factor for BPDTI mania(P<0.05).Conclusion:BPDTI mania patients had high impulsivity and cognitive impairment,high impulsivity and number of years of education were influencing factors for mania,high impulsivity affected cognitive function through corresponding brain regions,aggravating the condition.

4.Preliminary exploration of the role of miR-429 in human synovial mesenchymal stem cell-derived exosomes in repairing osteoarthritis cartilage damage

Objective To explore the role of miR-429 in synovial mesenchymal stem cell-derived exosomes(SMSC-Exos)in repairing cartilage damage in temporomandibular joint osteoarthritis(TMJ OA)by extracting SMSC-Exos from human synovial tissue and screening differentially expressed microRNA(miRNA)through transcriptome sequencing.Methods Human synovial tissues were obtained from 6 patients who underwent surgery at the First Medical Center of the Chinese PLA General Hospital from June to December 2023,including 3 patients with osteoarthritis(OA group)and 3 control patients(control group),all of whom were male.SMSC-Exos were extracted from the synovial tissues for miRNA sequencing and differential expression analysis.Further,Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analyses were performed on the target genes of differentially expressed miRNA to identify key functional miRNA and construct miRNA-target gene regulatory networks and protein-protein interaction(PPI)networks of target genes.An in vitro model of rabbit condylar cartilage cell inflammatory microenvironment induced by interleukin-1β(IL-1β)was established,with the control group cultured in DMEM/F12 basic medium and the inflammation-induced group cultured in DMEM/F12 basic medium containing 10 ng/ml IL-1β.RT-qPCR was used to detect the effects of overexpressed target miRNA on the mRNA expression levels of cartilage phenotype factors such as type Ⅱ collagen α1 chain(Col2a1),aggrecan(Acan),as well as inflammatory factors including a disintegrin and metalloproteinase with thrombospondin motifs 5(Adamts5)and cyclooxygenase-2(Cox-2).Results(1)SMSC-Exos were successfully isolated,cultured,and identified.(2)miRNA sequencing of SMSC-Exos from OA and control groups revealed 16 differentially expressed miRNAs(|log2FC|>2,P<0.05).Compared with control group,7 miRNAs were up-regulated and 9 were down-regulated in OA group.GO and KEGG analysis indicated that the target genes of miR-429 were mainly involved in development process,anatomical structure development,system development,cell development and differentiation,and were enriched in inflammation-related pathways such as mitogen-activated protein kinase(MAPK)and phosphatidylinositol 3-kinase-protein kinase B(PI3K-Akt).(3)Functional validation of miR-429 in the rabbit condylar cartilage cell inflammatory model showed that overexpression of miR-429 increased the mRNA expression levels of Col2a1 and Acan(P<0.05)and decreased the mRNA expression levels of Adamts5 and Cox-2(P<0.05)in the inflammation-induced group.Conclusions miRNA sequencing of SMSC-Exos isolated and identified from human synovial tissues reveals a specific miRNA expression profile in OA patients,with miR-429 significantly down-regulated.Functional validation demonstrates that overexpression of miR-429 has reparative and anti-inflammatory effects on condylar cartilage cells in an inflammatory microenvironment.

Objective To establish a stable,reliable,and clinically relevant porcine model of endotoxin-induced acute respiratory distress syndrome(ARDS).Methods Ten 8-month-old male Bama minipigs were deeply sedated,followed by invasive mechanical ventilation and electrocardiographic monitoring.Lipopolysaccharide(LPS)was intravenously pumped at 600 μg/(kg·h)for 3 hours,then maintained at 15 μg/(kg·h)thereafter.Dynamic monitoring was performed at five time points after LPS injection(LPS 0,1,3,5,and 8 h),including arterial blood gas analysis and chest computed tomography(CT)scans.Pathological examination of lung tissues obtained via bronchoscopic biopsy(HE staining and transmission electron microscopy)was conducted.These indicators were comprehensively used to evaluate the success of the animal model.Results At 5 hours after LPS administration,8 minipigs developed symptoms such as skin cyanosis,elevated body temperature,and respiratory distress.The oxygenation index decreased to<300 mmHg.Chest CT scans showed diffuse pulmonary infiltrates.Histopathology revealed alveolar edema and hyaline membrane formation.Transmission electron microscopy demonstrated disruption of pulmonary blood-air barrier,depletion of lamellar bodies in type Ⅱ pneumocytes,inflammatory cell infiltration,and exudation of plasma proteins and fibrin.Compared with LPS 0 h,at LPS 8 h,the oxygenation index and arterial blood pH were significantly decreased(P<0.001),while blood lactic acid and serum potassium were significantly increased(P<0.05);serum calcium and base excess were significantly decreased(P<0.05),and the lung injury score based on HE-stained lung sections was significantly increased(P<0.01).Conclusion The porcine ARDS model established by continuous LPS injection can dynamically simulate the pathophysiological characteristics and typical pathological manifestations of clinical septic ARDS,making it an effective tool to study the pathogenesis,prevention,and treatment strategies of septic ARDS.
6.Regulatory effect of oral probiotics adjuvant therapy on imbalance of helper T cell 1/helper T cell 2,intestinal flora and pulmonary function in children with allergic asthma

Objective To explore the regulatory effect of oral probiotics as adjuvant therapy on the imbalance of helper T cell 1(Th1)/helper T cell 2(Th2),intestinal flora and lung function in children with allergic asthma and its clinical efficacy.Methods A retrospective analysis was con-ducted in the clinical data of 122 children with allergic asthma.According to different treatment regi-mens,the children were divided into control group(n=60)and observation group(n=62).The control group received conventional treatment combined with budesonide,while the observation group received oral probiotics as adjunctive therapy in addition to the treatment in the control group.The asthma control effects[Childhood Asthma Control Test(C-ACT)score],intestinal flora(Escherichia co-li,Bifidobacterium,Lactobacillus),relevant lung function indicators[peak expiratory flow as a percentage of predicted value(PEF%pred),fractional exhaled nitric oxide(FeNO),forced expiratory volume in one second as a percentage of predicted value(FEV1%pred)],respiratory mechanics parameters[work of breathing(WOB),airway resistance(Raw),peak inspiratory pressure(PIP),dynamic lung compliance(Cdyn)],levels of sputum adhesion molecules[integrin α4β1,vascular cell ad-hesion molecule-1(VCAM-1),intercellular adhesion molecule-1(ICAM-1)],levels of Th1/Th2 cytokine-related indicators[interleukin-4(IL-4),interferon--y(IFN-γ),the ratio of IFN-γ to IL-4(IFN-γ/IL-4),the ratio of Th1 to Th2(Th1/Th2)],symptom disappearance time,clinical effica-cy,and the incidence of adverse reactions were compared between the two groups.Results After treatment,the observation group demonstrated significantly higher C-ACT scores,overall clinical re-sponse rates,IFN-γ/IL-4 and Th1/Th2 ratios,along with elevated levels of Lactobacillus,Bifidobacterium,PEF%pred,FEV1%pred,Cdyn and IFN-γ compared to the control group(P<0.05).Conversely,adverse reaction rates,and levels of Escherichia coli,FeNO,Raw,WOB,PIP,integrin α4β1,VCAM-1,ICAM-1 and IL-4 were significantly lower in the observation group(P<0.05).Symptom resolution time was also shorter in the observation group(P<0.05).The total Naranjo scale score was 3,indicating a"possible"causal relationship between adverse drug re-actions and probiotic use.Conclusion Oral probiotics as adjunctive therapy can effectively regulate Th1/Th2 imbalance induced by allergic asthma in children,improve clinical outcomes,pulmonary function,respiratory mechanics,intestinal microbiota composition,and sputum adhesion molecule levels.
7.Clinical efficacy of hyperbaric oxygen therapy on patients with tracheostomy for consciousness disturbance

Objective To explore the therapeutic effect and safety of hyperbaric oxygen therapy in the treatment of patients underwent tracheostomy for consciousness disturbance.Methods A total of 60 patients underwent tracheostomy for consciousness disturbance in our hospital from January 2019 to December 2021 were selected and randomly divided into the control group(30 cases)and the treatment group(30 cases).Patients in the control group were treated with tracheotomy and general treatment for primary disease,while patients in the treatment group were treated with hyperbaric oxygen on the basis of the control group.After 1 month of treatment,the total effective rate,Glasgow coma scale(GCS)score,persistent vegetative state(PVS)score,white blood cell count,neutrophil count,hemoglobin,C-reactive protein(CRP),and adverse reactions of patients were compared between the two groups.Results The total effective rate of the treatment group(96.67%)was higher than that of the control group(76.67%),and the difference was statistically significant(P<0.05).The GCS and PVS scores of patients in the two groups increased compared with those before treatment(P<0.05),and the GCS and PVS scores of patients in the treatment group were higher than those in the control group(P<0.05).There was no statistically significant difference in white blood cell count of patients in the two groups before and after treatment(P>0.05).The neutrophil count and CRP level of patients in both groups after treatment were lower than those before treatment(P<0.05),and the neutrophil count and CRP levels of patients after treatment in the treatment group were lower than those in the control group(P<0.05).The hemoglobin level of patients in both groups after treatment increased compared with those before treatment(P<0.05),and the hemoglobin level of patients after treatment in the treatment group was higher than that in the control group(P<0.05).The incidence of adverse reactions in the treatment group(6.67%)was lower than that in the control group(26.67%),and the difference was statistically significant(P<0.05).Conclusion Hyperbaric oxygen therapy has a good therapeutic effect and high safety for patients who treated by tracheostomy for consciousness disturbance.

Objective To evaluate the clinical outcome of neurosurgical robot-assisted stereotactic puncture and drainage for brain abscess.Methods A retrospective analysis was conducted on the clinical data of 53 patients with brain abscess admitted to our hospital from January 2018 to December 2024.Among them,29 cases underwent craniotomy for abscess resection(craniotomy group),while 24 cases received neurosurgical robot-assisted stereotactic puncture and drainage(robot-assisted group).The operation time,intraoperative blood loss,decompressive craniectomy rate,proportion of postoperative antibiotic regimen adjustment,postoperative hospital stay,incidence of postoperative complications,mortality rate and Glasgow outcome scale(GOS)scores 6 months after surgery of patients were compared between the two groups.Results Compared with the craniotomy group,the robot-assisted group demonstrated significantly shorter operation time,less intraoperative blood loss,and lower incidence of postoperative complication,the differences were all statistically significant(P<0.05).However,there were no statistically significant differences in terms of decompressive craniectomy rate,postoperative hospital stay,mortality rate,GOS score,or proportion of the postoperative antibiotic regimen adjustment between the two groups(P>0.05).Conclusion As a precise and minimally invasive surgical method,neurosurgical robot-assisted stereotactic puncture and drainage for patients with brain abscess can effectively improve the operational efficiency,shorten the operation time,reduce intraoperative injury,and lower the risk of postoperative complications.It has high clinical application value and potential for widespread adoption.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
