1.Chinese expert consensus on the diagnosis and treatment of chronic pain after lung surgery with integrated Traditional Chinese and Western medicine (2026 edition)

Chronic post-surgical pain (CPSP) is a common long-term complication following lung surgery. Its high incidence significantly impacts patients’ quality of life and functional recovery, and imposes a substantial socioeconomic burden. This consensus aims to systematically establish a standardized integrated Chinese and Western medicine diagnostic and treatment framework for chronic post-lung surgery pain (CPLSP). Based on the latest domestic and international evidence-based medical research and multidisciplinary clinical experience, the working group comprehensively elaborates on core issues regarding CPLSP, including its definition, epidemiology, pathogenesis, clinical assessment, Western medical treatment, traditional Chinese medicine (TCM) treatment, and integrated strategies. The consensus emphasizes a patient-centered approach, adhering to the principles of multimodality, individualization, and stepwise management, highlighting the synergistic advantages of integrating Chinese and Western medicine throughout the entire perioperative management cycle encompassing "perioperative anti-inflammation, acute analgesia, and chronic rehabilitation." Through systematic literature retrieval and evidence integration, a total of 9 core recommendations were established to provide scientifically sound and clinically practical guidance.
2.Sclera Vessel Segmentation Based on Fusion Filtering and Reflection Suppression

ObjectiveIn traditional Chinese medicine (TCM), the foundational doctrine that the eyes reflect the essence of the internal viscera establishes ocular observation as a cornerstone of diagnostic practice. Specifically, the morphological characteristics and coloration variations of the scleral microvasculature serve as critical clinical indicators for assessing the dynamic balance of Qi and Blood, as well as the pathological status of internal organs. Historically, however, TCM eye diagnosis has relied predominantly on the subjective clinical experience and visual acuity of individual practitioners, leading to inherent challenges in standardization and reproducibility. While automated computer-aided diagnostic systems offer a promising solution, existing vessel segmentation algorithms encounter significant domain-specific bottlenecks when applied to scleral imagery. These challenges primarily stem from the highly reflective and moist nature of the ocular surface, which generates severe reflective interference. Furthermore, the inherent low contrast of fine capillary networks against complex background textures, compounded by non-uniform illumination, frequently results in high false-positive rates, misdetections, and severe vessel fragmentation. To address these critical limitations and advance the objective quantification of TCM diagnostics, this paper proposes a novel, highly robust sclera vessel segmentation framework that innovatively integrates Frangi-Sato dual-filter adaptive enhancement with pixel-level reflection detection. MethodsThe proposed methodology systematically addresses the segmentation pipeline through three synergistic stages. First, to overcome the structural limitations of single-filter approaches, a multi-scale weighted fusion strategy is meticulously designed to harness the complementary extraction capabilities of both Frangi and Sato filters. This adaptive enhancement optimally balances the preservation of main vessel trunk continuity with the heightened sensitivity required for delineating delicate, low-contrast peripheral capillaries. Second, to tackle the persistent issue of reflective highlights, a sophisticated multi-feature synergistic reflection detection module is introduced. By jointly analyzing local information entropy, gradient field variations, and intensity statistical distributions, this module achieves precise, pixel-level identification and elimination of reflective artifacts without compromising the underlying vascular structures. Finally, a dual-level adaptive thresholding strategy, featuring an innovative “core protection” mechanism, is implemented. This critical step effectively suppresses complex background noise while rigorously preserving the structural and topological integrity of the intricate vessel network, preventing the structural breaks often seen in conventional binarization methods. ResultsThe efficacy of the proposed framework was rigorously evaluated using both self-constructed clinical datasets specifically acquired for TCM research and standardized public datasets. Extensive experimental results demonstrate that the proposed method consistently outperforms state-of-the-art traditional approaches and contemporary deep learning models. Specifically, the proposed method achieves a Dice similarity coefficient of approximately 0.71 on the private clinical dataset, and secures the best performance across the majority of quantitative metrics on both datasets. Notably, the framework exhibits exceptional robustness and generalization capabilities in highly challenging scenarios characterized by intense reflective interference, low signal-to-noise ratios, and cross-domain image variations. ConclusionThis study successfully realizes the high-integrity, automated segmentation of scleral vessel networks under complex clinical imaging conditions. By overcoming the fundamental algorithmic challenges of reflection interference and micro-vessel loss, the proposed methodology provides potential support for the digitization, objective standardization, and intelligent advancement of modern TCM eye diagnosis systems.

4.Effect of autologous bone and allogeneic bone in repair of bone defect after microwave in situ inactivation for bone giant cell tumor around the knee

Objective To investigate the clinical effect of autologous bone combined with allogeneic bone in repairing bone defect after microwave in situ inactivation for bone giant cell tumor around the knee.Methods A retrospective analysis was conducted on the clinical data of 39 patients with bone giant cell tumor around the knee treated in our hospital from January 2015 to December 2022.The tumor tissues of all patients were performed microwave in situ inactivation first,then followed by curettage;the subchondral bone defect adjacent to the articular surface was reconstructed with autologous bone graft,and the remaining tumor cavity was filled with a composite of autologous and allogeneic bone;then a locking plate was applied for protective fixation.The early outcomes including operative time,intraoperative blood loss,hospital stay,wound healing,and surgery-related complications were recorded.The regular follow-up was conducted after surgery,then the tumor recurrence or metastasis was observed and the status of internal fixation and bone graft fusion along with its fusion time were recorded.The knee functions before operation and at the end of follow-up were evaluated with the Musculoskeletal Tumor Society(MSTS)scoring system.The radiographic joint degeneration was evaluated by the Aboulafia classification system at the end of follow-up.Results The patients showed a mean operative time of(134.7±14.2)minutes,an averaged intraoperative blood loss of(153.9±23.0)mL,and a mean hospital stay of(17.4±3.2)days.The patients obtained Ⅰ phase wound healing without any surgery-related complications such as neurovascular injury.All patients underwent the postoperative follow-up,without local recurrence or metastasis cases during the follow-up.At the end of follow-up,the MSTS score of patients was higher than that before operation(P<0.05),with an excellent and good rate of knee function recovery of 89.7%.At the end of follow-up,X-ray films showed good filling of the lesion and bone graft fusion,with a mean fusion time of(9.4±2.1)months.No collapse or fracture of the articular surface occurred.According to the Aboulafia classification,there were 20 cases of grade 0,11 cases of grade 1,and 8 cases of grade 2.Conclusion The autologous bone and allogeneic bone in repair of bone defect after microwave in situ inactivation for bone giant cell tumor around the knee has perfect knee function recovery,and can effectively prevent the collapse and degeneration of articular surfaces,which helps in the reconstruction of tumor cavity.
5.Correlation between Serum MEGF6,SAMSN1 Expression Levels and Acute Lung Injury in Patients with Sepsis

Objective To investigate the relationship between the expression levels of serum multiple epithelial growth factor like domains 6(MEGF6),sterile alpha motif(SAM)domain,scr homology 3(SH3)domain and nuclear localization signals 1(SAMSN1)and acute lung injury(ALI)in patients with sepsis.Methods A total of 182 patients with sepsis(sepsis group)who were admitted to Huazhong University of Science and Technology Union Jiangnan Hospital from February 2021 to February 2024 were selected and divided into ALI group(n=60)and non-ALI group(n=122)according to whether they were complicated with ALI or not.80 healthy people who underwent physical examination during the same period were selected as the control group.Serum MEGF6 and SAMSN1 levels were detected by enzyme-linked immunosorbent assay(ELISA).Pearson correlation analysis was used to analyze the correlation between serum MEGF6,SAMSN1 and clinical indicators.Multivariate Logistic re-gression analysis was used to analyze the risk factors of sepsis complicated with ALI.The receiver operating characteristic(ROC)curve was used to analyze the value of serum MEGF6 and SAMSN1 in the evaluation of sepsis complicated with ALI.Results Serum MEGF6(37.52±7.87 ng/L)and SAMSN1(24.26±7.45 ng/L)in the sepsis group were lower than those in the control group(78.17±15.26 ng/L,62.31±14.12 ng/L),and the differences were statistically significant(t=28.397,28.477,all P<0.001).Serum MEGF6(29.22±7.64 ng/L)and SAMSN1(13.28±7.12 ng/L)in the ALI group were lower than those in the non-ALI group(41.60±7.90 ng/L,29.66±7.67 ng/L),and the differences were statistically significant(t=10.045,13.961,all P<0.05).Se-rum MEGF6 and SAMSN1 in patients with sepsis complicated with ALI were negatively correlated with APACHE II score and SOFA score(r=-0.811~-0.728,all P<0.05),and positively correlated with oxygenation index(OI)(r=0.689,0.640,all P<0.05).APACHE II score and SOFA score were risk factors for sepsis complicated with ALI.Serum MEGF6(OR=0.769,95%CI:0.612~0.965)and SAMSN1(OR=0.778,95%CI:0.617~0.980)were protective factors(Wald χ2=5.140,4.525,all P<0.05).The area under the curve(AUC)of serum MEGF6,SAMSN1 alone and combined in the evaluation of sepsis complicated with ALI were 0.840,0.811 and 0.903,respectively.The combined detection was greater than the single detection,and the differences were statistically significant(Z=4.602,4.318,all P<0.05).Conclusion The serum levels of MEGF6 and SAMSN1 in patients with sepsis are decreased,which are the influ-encing factors of sepsis complicated with ALI.The combined detection of the two has a high evaluation value for sepsis complicated with ALI.
6.Craniocerebral penetrating injury caused by nail shooting:one case report

We reported the clinical,imaging,and treatment process of a patient with severe cranial penetrating injury caused by a nail from a nail gun that accidentally shot into the eye and penetrated into the skull while working.The patient was admitted to the Qingpu Branch of Zhongshan Hospital,Fudan University after being injured by the nail gun.The patient was admitted to the hospital 1 hour after injury with coma.After admission,the foreign body was completely removed and part of the hematoma was cleared through emergency surgery.After surgery,there were high-risk complications such as cerebral edema peak,intracranial hypertension,intracranial infection,cerebrospinal fluid leakage,and intracranial pseudoaneurysm.The patient eventually regained consciousness and was transferred to rehabilitation treatment.
7.Mediating effect of health literacy between discharge readiness and self-management behavior in patients with inflammatory bowel disease

Objective:To explore the current status of self-management behavior in patients with inflammatory bowel disease and to construct a structural equation model to explore the mediating role of health literacy between discharge readiness and self-management behavior.Methods:From March to September 2023, 310 hospitalized patients with inflammatory bowel disease in the Tenth People ′s Hospital Affiliated to Tongji University were selected as the research subjects by the convenience sampling method. A cross-sectional survey was conducted using the general information questionnaire, the Health Literacy Management Scale, the Discharge Readiness Scale, and the Self-management Behavior Scale. Results:Eventually, 303 valid questionnaires were retrieved. Among the 303 patients, there were 190 males and 113 females, with an average age of (35.63 ± 16.41) years old. The total score of health literacy of patients with inflammatory bowel disease was (94.35 ± 10.78) points, indicating a health literacy compliance rate of 74.3%(225/303). The total score of discharge readiness was (79.52 ± 9.16) points, and the total score of self-management behavior was (135.22 ± 17.36) points. In patients with inflammatory bowel disease, the discharge readiness was positively correlated with the total score of health literacy ( r=0.748, P<0.01), the discharge readiness was positively correlated with the total score of self-management behavior ( r=0.821, P< 0.01), and the health literacy was positively correlated with the total score of self-management behavior ( r=0.767, P<0.01). Health literacy may serve as a mediating variable, partially explaining the relationship between discharge readiness and self-management behavior in patients with inflammatory bowel disease. The mediating effect accounted for 47.5% of the total effect. Conclusions:Discharge readiness not only positively impacts self-management behavior among patients with inflammatory bowel disease but also indirectly influences this behavior through health literacy. It is essential for clinical healthcare professionals to prioritize the assessment of discharge readiness and health literacy among these patients. By improving discharge readiness and enhancing health literacy, clinical healthcare professionals can effectively support patients with inflammatory bowel disease in their self-management efforts.
8.Biomechanical Response of the Utricle to Linear Acceleration Based on a Bionic Physical Model

Objective To construct a transparent and visualized bionic model of human utricle and explore the biomechanical response of the utricle to linear acceleration.Methods Using three-dimensioanl(3D)printing technology and PVA-gelatin composite hydrogel fabrication method,a visual physical model of the utricle with a ratio of 10∶1 to the human body was successfully prepared.The biomechanical response of the utricle macula was investigated by varying acceleration and direction stimulation experiments.Results Under 1-5 Hz sinusoidal reciprocating linear excitation,the response amplitude of the bionic macula increased from 4.11 μm to 48.82 μm.The response amplitude of the bionic macula increased linearly with the acceleration.In addition,the macula showed deformation differences in response to acceleration in a specific direction.Conclusions The bionic utricle model prepared in this study can accurately simulate the working mechanism of human utricle,which is expected to provide a new way for the pathological study of vestibular dysfunction and expand a new direction for the application of bionic technology in the field of biomedical engineering.
9.Bibliometric analysis of hot topics and trends in thyroid cancer nursing research at home and abroad from 2004 to 2023

Objective:To analyze the hotspots and trends in thyroid cancer nursing research from 2004 to 2023 at home and abroad through bibliometric methods, so as to provide references and ideas for future research in this field.Methods:A systematic search was conducted for core journal documents on thyroid cancer nursing research included in China National Knowledge Infrastructure, Wanfang Database, VIP Chinese Journal Network and Web of Science Core Collection from January 1, 2004 to December 31, 2023, using CiteSpace6. 3.R1 software performs visual analysis.Results:A total of 119 domestic documents and 417 international documents were included for analysis. From 2004 to 2023, the number of publications in the field of thyroid cancer nursing research at home and abroad showed an upward trend. Domestic research hotspots mainly include four directions: perioperative nursing, symptom nursing, care for radioactive iodine-131 treatment, and physical and mental health promotion. Foreign research hotspots mainly include three directions: nursing practice exploration and guideline formulation, care for radioactive iodine-131 treatment, and quality of life.Conclusions:The field of thyroid cancer nursing has received continuous attention from scholars at home and abroad. In future research, it is recommended that domestic scholars actively carry out cross-institutional and multi-center team cooperation in accordance with local needs, focus on the research of physical and mental care and health promotion of thyroid cancer patients at different survival stages, and the construction of clinical nursing guidelines. With the help of core research institutions and expert teams, actively promote the development of high-quality nursing research in this field.
10.Distribution of pathogens isolated from patients with eczema and clinical laboratory test indexes of patients with TCM syndrome type of eczema

OBJECTIVE To explore the differences in the clinical laboratory test indexes between the patients with two different TCM syndrome types of eczema(dampness-heat infiltration type and non-dampness-heat infiltration type)and observe the distribution and drug resistance rate of pathogens isolated from the skin lesions so as to pro-vide bases for syndrome differentiation and reasonable use of antibiotics.METHODS A total of 180 patients with eczema who were positive for bacterial culture of skin secretions and were treated in Beijing Hospital of Traditional Chinese Medicine from Jan.2021 to Dec.2023 were enrolled in the study and were divided into the dampness-heat infiltration group with 134 cases and the non-dampness-heat infiltration group with 46 cases.The data of clini-cal laboratory test indexes were collected from the two groups of patients,the secretion specimens were sampled from the skin lesion sites,the isolated pathogens were identified by VITEK 2 Compact automatic microorganism analysis system,and the drug susceptibility testing was performed.RESULTS The direct bilirubin level of the dampness-heat infiltration group was(3.99±1.62)umol/L,higher than(3.46±1.12)umol/L of the non-damp-ness-heat infiltration group(P<0.05),but both were in the normal range;there were no significant differences in other test indexes between the two groups.Totally 180 strains of pathogens were isolated from the 180 patients with eczema,156(86.67%)of which were gram-positive bacteria;Staphylococcus aureus(98 strains),Staphy-lococcus epidermidis(29 strains)and Staphylococcus haemolyticus(15 strains)were the predominant species of the gram-positive bacteria.The gram-negative bacteria accounted for 12.22%(22 strains).The drug resistance rate of the S.aureus strains to penicillin was up to 82.65%,and the isolation rate of methicillin-resistant Staphy-lococcus aureus(MRSA)was 12.24%(12/98);the drug resistance rates of the S.epidermidis strains to penicil-lin and erythromycin were 75.86%,and the drug resistance rate of the S.haemolyticus strains to erythromycin was 100.00%.CONCLUSIONS The gram-positive bacteria(dominated by the S.aureus)are dominant among the pathogens isolated from the skin lesion specimens of the eczema patients and are highly resistant to penicillin and erythromycin.The isolation rate of MRSA is relatuvely low.There is limited clinical significant difference in the direct bilirubin between the two groups.It is necessary to further explore more reliable indexes for syndrome dif-ferentiation.
