Journal of Nanjing University of Traditional Chinese Medicine 2025;41(10):1320-1326
The Song,Jin,and Yuan dynasties marked a period of transformation in traditional Chinese medicine for visceral syn-drome differentiation, with the emergence of numerous new ideas and theories.Zhang Yuansu was a key figure in the transition from Han and Tang to Jin and Yuan medicine. His Medication Patterns for Treating Deficiency-Excess and Cold-Heat of Five Viscera embod-ies his thinking on visceral syndrome differentiation. This work breaks away from the pre-Song classification of visceral syndrome differ-entiation based on deficiency and excess, focusing on the specific physiological and pathological patterns of the five zang organs and proposing corresponding treatments for their specific syndromes.Medication Patterns propose the diagnosis and treatment of reproductive system diseases based on the Mingmen, assuming that the Mingmen undertakes some of the functions of the kidneys. This enriched the connotations of visceral syndrome differentiation and provided an important theoretical basis for identifying certain complex pathogenesis. The visceral syndrome differentiation concept in the Medication Patterns has promoted subsequent research on visceral syndrome differentiation, exemplified by works such as Li Dongyuan's"Treatise on the Spleen and Stomach",Zhu Danxi's"Treatise on the Xianghuo"and the"Mingmen Theory".In-depth exploration of the inherent differentiation logic and theoretical framework of the Medication Patterns is of great significance to current research on differentiation standards and norms.

Objective To evaluate the reliability of Vitek 2 AST-N335 card for determining the susceptibility of Acinetobacter bauman-nii(AB)to cefoperazone/sulbactam.Methods A total of 318 non-repeated clinical isolates of AB collected in 2023 were tested for antimicrobial susceptibility to cefoperazone/sulbactam using broth microdilution(BMD),the AST-N335 card, and the Kirby-Bauer(K-B)disk diffusion method.Using BMD as the reference method, the reliability of AST-N335 card was assessed, and the accuracy of K-B disk diffusion method as the confirmatory test was validated. Results Compared with BMD,the susceptibility testing of 318 AB strains to cefoperazone/sulbactam using the AST-N335 card showed categorical agreement(CA)of 87.8%(279/318),very major er-ror(VME)of 6.0%(19/318),major error(ME)of 0%(0/318),and minor error(mE)of 1.9%(6/318),which fall outside of the acceptable error range. In contrast, the K-B method achieved CA of 99.4%(316/318),VME of 0%,ME of 0.3%(1/318),and mE of 0.3%(1/318),all within acceptable limits.Of these, the errors with AST-N335 card occurred within the minimum inhibitory concentration(MIC)range of 8-32 μg/mL.Using BMD as the reference method, further analysis was performed on the 171 AB strains with AST-N335 card MIC values of 8-32 μg/mL for cefoperazone/sulbactam.It was revealed that at MIC of 32 μg/mL,the CA was 0%;at MIC of 16 μg/mL,CA was 5.3%(1/19)and VME rate was 84.2%(16/19),both of which substantially exceeded accepta-ble error ranges. At MIC of 8 μg/mL,the CA was 94.9%(131/138)and VME was 2.2%(3/138),both approaching the acceptable ranges. Conclusion The results obtained with Vitek 2 AST-N335 card in determining for cefoperazone/sulbactam are unreliable when the MIC values fall within the range of 8-32 μg/mL,which leads to an underestimation of the resistance rate to cefoperazone/sulbac-tam.This issue requires urgent attention in both laboratories and clinical practice. The K-B disk diffusion method could serve as a sup-plementary verification approach in routine laboratories.

Objective:To investigate the correlation between oral cleanliness and secondary Pulmonary infection in patients with severe chronic obstructive pulmonary disease (COPD) in mechanical ventilation, and to investigate the predictive effect of oral cleanliness on the risk of secondary pulmonary infection. Methods:Using the cross-sectional survey method, the purposeful sampling method was adopted to select 216 patients with severe COPD who were hospitalized in Hangzhou First People′s Hospital from June 2020 to December 2023 and received mechanical ventilation. The oral cleanliness index and general clinical data of patients at admission were collected using the hospital electronic medical record system. The independent influencing factors of secondary lung infection were analyzed by univariate analysis and multivariate Logisitic regression. The predictive value of oral cleanliness index on secondary lung infection was analyzed by patient operating characteristic (ROC) curve.Results:216 patients with severe COPD who underwent mechanical ventilation were included.Patients aged 37-84 (66.81 ± 8.98) years were included, including 125 males and 91 females.Among them, 89 cases developed secondary pulmonary infection, with an infection rate of 41.20%.Univariate analysis and multivariate Logistic regression analysis showed that, Beck Oral Rating Scale (BOAS) score ( OR = 1.371), visual simulation score of oral odor ( OR = 1.405), gum index ( OR = 3.508), plaque index ( OR = 14.357), smoking history ( OR = 6.772), duration of disease ( OR = 1.391), COPD assessment test score ( OR = 1.269) and mechanical ventilation time ( OR = 1.302) were independent factors for secondary pulmonary infection (all P<0.05). ROC curve analysis showed that oral cleanliness index combined with infection prediction was effective (the area under the ROC curve was 0.833) . Conclusions:Oral cleanliness was closely related to secondary pulmonary infection in patients with severe COPD with mechanical ventilation. BOAS score, visual simulation score of oral odor, gingival index and plaque index could predict secondary pulmonary infection independently, and combined test could predict secondary pulmonary infection.

Recent studies have revealed a significant association between Porphyromonas gingivalis(P.gingivalis)infection and poor prognosis in esophageal squamous cell carcinoma(ESCC).Although cer-tain circular RNAs(circRNA)have been shown to suppress ESCC tumorigenesis and progression, their regulatory mechanisms in P.gingivalis infection-associated ESCC remain elusive. In this study,RT-qPCR analysis demonstrated that P.gingivalis infection downregulated hsa_circ_0057552 expression in ESCC cells and tissues in a time-and dose-dependent manner.Actinomycin D assays further confirmed that P.gingivalis infection reduced the RNA stability of hsa_circ_0057552 in ESCC cells(P<0.05).Functional assays in vitro and a subcutaneous tumor xenograft model in vivo revealed that hsa_circ_0057552 overexpression significantly inhibited ESCC cell proliferation, migration, invasion, and tumor growth(P<0.05).Additionally,PCR array screening combined with RT-qPCR and Western blotting in-dicated that P.gingivalis infection markedly upregulated human antigen R(HuR)expression at both RNA and protein levels(P<0.05).Mechanistic investigations demonstrated that HuR knockdown signifi-cantly increased hsa_circ_0057552 expression(P<0.01),whereas hsa_circ_0057552 overexpression had no regulatory effect on HuR.Finally,si-HuR treatment reversed the inhibitory effect of P.gingivalis on hsa_circ_0057552 transcription. This study demonstrated that P.gingivalis may promote the progression of ESCC through a novel mechanism involving the regulation of HuR/hsa_circ_0057552,thereby identif-ying a novel therapeutic target and molecular marker for P.gingivalis-associated ESCC.

8.Acute cardiac tamponade caused by misperforation of the aorta during transseptal puncture without thoracotomy:a case report
Chinese Journal of Interventional Cardiology 2025;33(8):474-476
The penetrating cardiovascular injury is a complication that may lead to serious adverse consequences in electrophysiology.Studies have shown that the risk of aortic injury during transseptal puncture is about 0.05%-0.09%.Improper treatment will cause acute cardiac tamponade, delay the rescue time and even threaten the patient's life.At present, there are few reports on specific procedures for such situations, and the guidelines do not provide operational plans. Therefore, the collection and the statistical analysis is necessary. The author reported the treatment procedure of accidental penetration of the aorta and acute cardiac tamponade during transseptal puncture.

Isoflavones which mainly distributed in leguminous plants have plenty of health benefits.Isoflavone synthase(IFS)is a membrane-associated cytochrome P450 enzyme(CYP450)which carries out the unique aryl-ring migration and hydroxylation. So far,few crystal structures of plant P450s have been obtained. We determined the crystal structure of IFS from Medicago truncatula at 1.9 ? by MAD method using a selenomethionine substituted crystal and conducted molecular docking and mutagenesis study. The structure of IFS complexed with imidazole exhibits the helix Ⅰa-loop-helix Ⅰβ motif which cor-responds to helix Ⅰ of other P450s.Compared with structures of common P450s,IFS/imidazole structure contains an extra domain,i.e.,the γ-domain.The structure reveals a homodimer in which the γ-domain of one molecule interacts with the β-domain of another. The plane of heme group makes an angle of ap-proximately 40° with the helix Ⅰa-loop-helix Ⅰβ motif.Molecular docking combined with mutagenesis study suggested that Trp-128 and Asp-300 might play important roles in substrate binding and recogni-tion.Phe-301,Ser-303 and Gly-305 from the helix Ⅰa-loop-helix Ⅰβ motif may play important roles in the aryl-ring migration. These novel structural features reveal insights into the unique reaction mechanism of IFS and provide a basis for engineering IFS in leguminous crops for health purpose.
