1.Fluoxetine alleviates depressive-like behaviors in chronic unpredictable mild stress rats by regulating PI3K/AKT/mTOR signaling pathway

Objective To investigate the effect of fluoxetine(FLX)in allivating depressive-like behaviors in chronic unpredictable mild stress(CUMS)rats through regulation of phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin(PI3K/AKT/mTOR)signaling pathway.Methods Twenty-four rats were randomly divided into four groups according to random number table method:Con group(healthy rats),CUMS group(CUMS-induced depressive rat model),FLX group(CUMS-induced depressive rat model intraperitoneally injected with 10 mg/kg FLX),and LY294002 group(CUMS-induced depressive rat model intraperitoneally injected with 0.002 μg/kg LY294002),with six rats in each group.The rats were exposed to a chronic unpredictable stress(CUS)environ-ment to establish depressive model.Behavioral indicators of rats in each group were observed and recorded,including sucrose preference test,immobility duration in the tail suspension test,and the immobility time in the forced swimming test.The levels of adrenocorticotropic hormone(ACTH),cortisol(CORT),interleukin(IL)-1β,IL-6,and tumor necrosis factor-α(TNF-α)in the serum of rats in each group were detected.The content of brain-derived neurotrophic factor(BDNF)in the hippocampal tissue was detected by immunohistochemistry.The expression levels of related proteins in the PI3K/AKT/mTOR signaling pathway were detected by Western blotting.Results Compared with the Con group,the CUMS group showed a decreased sucrose preference rate,BDNF level,p-PI3K/PI3K,p-AKT/AKT,and p-mTOR/mTOR,as well as prolonged immobility duration in the tail suspension test,prolonged immobility time in the forced swimming test,and increased levels of ACTH,CORT,IL-1β,IL-6,and TNF-α(P<0.05).Compared with the CUMS group,the FLX group exhibited an increased sucrose preference rate,BDNF level,p-PI3K/PI3K,p-AKT/AKT,and p-mTOR/mTOR,as well as shortened immobility duration in the tail suspension test,shortened immobility time in the forced swimming test,and decreased levels of ACTH,CORT,IL-1 β,IL-6,and TNF-α,with statistically significant differences(P<0.05).Compared with the FLX group,the LY294002 group showed a decreased sucrose preference rate,BDNF level,p-PI3K/PI3K,p-AKT/AKT,and p-mTOR/mTOR,as well as prolonged immobility duration in the tail suspension test,prolonged immobility time in the forced swimming test,and increased levels of ACTH,CORT,IL-1β,IL-6,and TNF-α,with statistically significant differences(P<0.05).Conclusion FLX can regulate the secretion of ACTH,CORT,and BDNF,inhibit neuroinflammation,and improve depres-sive-like behaviors in CUMS rats,possibly by activating the PI3K/AKT/mTOR signaling pathway.
2.Deep learning model based on fundus images for detection of coronary artery disease with mild cognitive impairment

Objective To develop a deep learning model based on fundus retinal images to improve the detection rate of mild cognitive impairment(MCI)in patients with coronary heart disease,achieve early intervention and improve prognosis.Methods The study was a single-center cross-sectional study that retrospectively included patients diagnosed with coronary heart disease(CHD)by coronary angiography(≥50% stenosis of at least one coronary vessel)from Beijing Anzhen Hospital between November 2021 and December 2022.The whole data set was randomly divided into the training set and the testing set according to the ratio of 8∶2 for model development.After that,the patient data of the same center from January 2023 to April 2023 were included in the time verification method to verify the model.The diagnostic criteria for MCI were MMSE<27 or MoCA<26.Four kinds of convolutional neural network(CNN)architectures were used to train fundus images,and a comprehensive vision model of MCI detection was established through model integration.The area under the curve(AUC),sensitivity and specificity of the receiver operating curve(ROC)were used to evaluate the performance of the AI model.Results We collected 5 880 eligible fundus images from 3 368 CHD patients.Based on the results of the MMSE scale,the algorithm was labeled,including 2 898 males and 527 MCI patients.The AUC of the deep learning model in the test group is 0.733(95%CI 0.688-0.778),and the sensitivity of the algorithm in the test group is 0.577(95%CI 0.528-0.625)by using the operating point with the maximum sum of sensitivity and specificity.With a specificity of 0.758(95%CI 0.714-0.802),corresponding to a validated AUC of 0.710(95%CI 0.601-0.818).Based on the results of the MoCA scale,the algorithm labels 2 437 males and 1 626 MCI patients.The AUC of the deep learning model in the test group was 0.702(95%CI 0.671-0.733).The operating point with the maximum sum of sensitivity and specificity was selected,and the sensitivity of the algorithm was 0.749(95%CI 0.719-0.778)and the specificity was 0.561(95%CI 0.527-0.595),corresponding to the AUC value of the verification group was 0.674(95%CI 0.622-0.726).Conclusions The deep learning algorithm model based on fundus images has good diagnostic performance,and may be used as a new non-invasive,convenient and rapid screening method for MCI in CHD population.
3.Analysis of Active Components and Metabolites of Modified Ermiao Decoction Based on UPLC-Q-Exactive Orbitrap-MS/MS Technology

OBJECTIVE To conduct qualitative analysis of chemical constituents in the crude extract of Modified Ermiao Formu-la,along with its blood-absorbed components and metabolites in rats post-administration employing ultra-performance liquid chroma-tography coupled with quadrupole-exactive orbitrap tandem mass spectrometry(UPLC-Q-Exactive Orbitrap-MS/MS).METHODS Separation was performed on a Waters HSS T3 column(100 mm×2.1 mm,1.8 μm)using gradient elution with 0.1%formic acid in water(mobile phase A)and 0.1%formic acid in acetonitrile(mobile phase B),under controlled conditions:column temperature maintained at 40℃,flow rate set to 0.3 mL·min-1,and injection volume fixed at 2 μL.Mass spectrometric data acquisition utilized an electrospray ionization(ESI)source with scanning in both positive and negative ion modes.RESULTS By analyzing precise mo-lecular weights,retention times,and MS/MS fragmentation patterns,and cross-referencing these against established databases and published literature,173 chemical constituents were definitively identified in the Modified Ermiao Formula crude extract.These prima-rily comprised flavonoids,organic acids,and alkaloids.Additionally,32 prototype components and 13 metabolites were characterized in the serum of dosed rats.Organic acids constituted the predominant class of serum prototype components,undergoing in vivo metabo-lism principally through demethylation and carboxyl glucuronidation.CONCLUSION This study provides the initial delineation of blood-absorbed prototype components derived from Modified Ermiao Formula,with concomitant identification of their metabolites,thereby establishing a foundational framework for elucidating the pharmacologically active constituents of this formula.
4.Coronary Computed Tomographic Angiography-Derived Radiomics Combing CT-Fractional Flow Reserve for Detecting Hemodynamically Significant Coronary Artery Disease.

Objective To develop a diagnostic model combining the CT angiography(CCTA)-derived myocardial radiomics signatures with the CT-derived fractional flow reserve(CT-FFR)based on coronary CCTA and investigate the diagnostic accuracy of the hybrid model for hemodynamically significant coronary artery disease(CAD).Methods The patients presenting stable angina pectoris,diagnosed with CAD,and clinically referred for CCTA examination and invasive coronary angiography were prospectively recruited.Radiomics features of the left ventricular myocardium were extracted from the three main perfusion territories demarcated according to the coronary blood supply.The extracted features were first selected by the minimum redundancy maximum relevance feature ranking method.A least absolute shrinkage and selection operator Logistic regression algorithm with leave-one-out cross-validation was then employed to construct a radiomics model.The CT-FFR value was generated for each blood vessel.The area under the receiver operating characteristics curve(AUC_ROC),sensitivity,and specificity were adopted to evaluate the performance of each model against the reference standard invasive coronary angiography/FFR.Results A total of 70 patients[42 men and 28 women;(61±10) years old] were included in this study and complemented CCTA examination,with 175 vessels and the corresponding myocardial territories undergoing invasive coronary angiography/FFR.A total of 1 656 specific radiomics parameters were extracted,from which 14 features were selected to establish the radiomics model.The AUC_ROC,sensitivity,and specificity were 0.797(95%CI=0.732-0.861),77.1%,and 73.7%for the radiomics model,0.892(95%CI=0.841-0.943),81.4%,and 88.8%for the CT-FFR model,and 0.928(95%CI=0.890-0.965),83.3%,and 88.4%for the hybrid model,respectively.The hybrid model outperformed the radiomics model and CT-FFR alone(P=0.040).Conclusions The radiomics signatures of the vessel-related myocardium from CCTA could provide incremental value to the diagnostic performance of CT-FFR and improve vessel-specific ischemia detection.The hybrid model combining CT-FFR with radiomics signatures is potentially feasible for improving the diagnostic accuracy for hemodynamically significant CAD.

7.Nasolabial groove through the skin flap repair nasal vestibular benign and malignant lesions Application of postoperative tissue defects.

Objective:To investigate the application value of nasolabial flaps in addressing tissue defects after resection of benign and malignant nasal vestibular lesions. Methods:The clinical data of patients with benign and malignant nasal vestibular lesions were analyzed retrospectively. There were 4 cases of squamous cell carcinoma, 2 cases of black hairy nevus and 1 case of chronic proliferative inflammatory lesions, all of which were repaired by adjacent nasolabial flap. Results:After 6 months of follow-up, none of the patients developed nasal vestibular contracture or nostril stenosis, and postoperative nasal ventilation function was good. Conclusion:The preoperative design of individual nasolabial flaps is very important for maintaining maxillofacial aesthetics, protectingthe nasolabial framework, and preserving postoperative nasal ventilation function.

8.Hematological Characteristics of Neonates with Abnormal Hemoglobin and Their Parents in Guangzhou Area

Objective:To analyze the incidence of abnormal hemoglobin(Hb)in neonates in Guangzhou area,as well as the results of quantitative analysis of Hb in neonatal umbilical cord blood and genetic diagnosis of thalassemia in neonates with abnormal Hb;And to explore the hematological phenotypes and clinical characteristics of neonates with abnormal Hb and their parents,providing a reference for eugenics and childcare.Methods:650 neonates born at Guangdong Provincial People's Hospital who underwent Hb electrophoresis were included in this study.The results of routine blood test of umbilical cord blood,Hb electrophoresis and α-,β-thalassemia gene detection of the neonates were collected.The genotype distribution of thalassemia in the neonates was analyzed.Additionally,the abnormal Hb content of α and β variants was studied.Furthermore,the differences in hematological parameters between abnormal Hb neonates and normal neonates and α-thalassemia neonates,as well as between the parents of abnormal Hb neonates and normal adults were compared.Results:Among the 650 neonates,332(51.08%)were diagnosed with thalassemia,including 235 cases of α-thalassemia(36.15%),79 cases of β-thalassemia(12.15%),and 18 cases of compound α β-thalassemia(2.77%).Among all the α-thalassemia genotypes,the most prevalent one was--SEA/α α(48.94%),followed by-α3.7/α α(20.00%),-α42/α α(11.06%),and α α CS/α α(8.94%).The four most common genotypes of β-thalassemia were β CD41-42(32.91%),βIVS-Ⅱ-654(26.58%),β-28(21.52%),and β E(10.13%),respectively.275 cases of abnormal bands were found in Hb electrophoresis of umbilical cord blood,with a detection rate of 42.31%.The abnormal Hb content ofα-variant in the neonates was significantly higher than that of β-variant(P<0.001).The levels of Hb,MCV,MCH,Hb A,and Hb F in neonates with abnormal Hb were lower than those in normal neonates,while the RDW-CV was higher than that in normal neonates,with statistical significantce(P<0.05).The levels of RBC and Hb A in neonates with abnormal Hb were lower than those in neonates with α-thalassemia,while the level of MCH was higher than that in neonats withα-thalassemia,with statistical significance(P<0.05).The levels of Hb,MCV,MCH,and Hb A in parents of neonates with abnormal Hb were lower than those in normal adults,while the RDW-CV was higher than that in normal adults,and the differences were statistically significant(P<0.05).Conclusion:The abnormal Hb content of α-variant in the neonates is significantly higher than that of β-variant in the neonates in Guangzhou,which can help to presume whether it isα chain or β chain based on the abnormal Hb content,providing a reference for globin gene sequencing.Meanwhile,analysis of various hematological screening-related indicators in neonates in the early stage is beneficial for early warning of the occurrence of abnormal Hb combined with thalassemia,reducing missed diagnoses to a certain extent.
9.Current status of randomized controlled trials of traditional Chinese medicine in treating coronary heart disease with angina pectoris after percutaneous coronary intervention.

This study systematically reviewed the randomized controlled trial(RCT) of traditional Chinese medicine(TCM) treatment of coronary heart disease patients with angina pectoris after percutaneous coronary intervention(PCI). The basic elements of these RCTs, including sample size and estimation method, randomizing scheme, allocation concealment, blind method implementation, data integrity, statistical method, TCM syndrome, intervention measures, treatment course, follow-up time, and outcome indicators, were analyzed to provide reference for the design of future RCT and the clinical application of TCM in treating angina pectoris after PCI. CNKI, Wanfang, VIP, SinoMed, PubMed, EMbase, Cochrane Library, Web of Science, ClinicalTrials.gov, and Chinese Clinical Trial Registry were searched for the RCT about TCM treatment of coronary heart disease patients with angina pectoris after PCI according to pre-defined criteria, with the time interval from inception to January 31, 2024. A total of 188 RCTs were included, of which 184 were clinical research articles and 4 were clinical trial registration schemes. These RCTs involved a total of 15 521 patients, with an average sample size of 83 patients and a maximum sample size of 248 patients. Among them, 126 RCTs reported TCM syndromes, the top three of which were Qi deficiency and blood stasis(38.89%), phlegm combined with stasis(17.46%), and Qi stagnation and blood stasis(9.52%). The control group received guideline-directed medical therapy(GDMT) or GDMT combined with placebo, and the treatment group received GDMT combined with TCM. The treatment mainly lasted for 4-8 weeks, most of the RCTs did not set the follow-up period or the follow-up period was unknown. A total of 160 outcome indicators were used, with the total frequency of 1 348. According to functional attributes, the outcome indicators can be categorized into 6 groups: symptoms/signs(403, 29.90%), TCM syndromes/symptoms(182, 13.50%), physical and chemical examination(468, 34.72%), quality of life(89, 6.60%), long-term prognosis(5, 0.37%), and safety evaluation(201, 14.91%). The clinical trial design of TCM intervention in angina pectoris after PCI of coronary heart disease is becoming more and more rigorous, while it remains to be improved. It is expected that more clinical trial schemes with rigorous design and taking into account the TCM advantages can be adopted in the future to provide a basis for the TCM treatment of angina pectoris after PCI of coronary heart disease.

10.Environmental hygiene of metro line X in Nanjing in 2023

Background The subway environment, characterized by dense crowds, relatively enclosed architectural structures, and insufficient ventilation, poses various health hazards. Population health risk attributable to subway hygiene have become an important public health issue. Objective To evaluate the current environmental hygiene of metro line X in Nanjing, and to provide a scientific basis for the prevention and control of harmful factors and the establishment of relevant standards. Methods Station halls, platforms, and carriages of Nanjing metro line X were monitored in March 2023. Six stations were selected by stratified sampling, and monitored once during the morning peak hours (9:00—11:00). Carriage monitoring was conducted after a full day of operation (21:00—22:00). The monitoring indicators included physical factors such as temperature, relative humidity, wind speed, illuminance, and noise (A-weighted); air quality factors such as carbon dioxide (CO2), carbon monoxide (CO), respirable particulate matter (PM10), fine particulate matter (PM2.5), ammonia (NH3), formaldehyde, benzene, toluene, xylene, total volatile organic compounds (TVOC), ozone (O3), total bacterial count, and total fungi count; hygiene status of the inner surface of the central air conditioning ventilation system, such as dust accumulation, total bacterial count, and total fungi count; supply air, such as PM10, PM2.5, total bacterial count, and total fungi count; and surface microorganisms on objects, such as total bacterial count, total fungi count, coliform bacteria, and Staphylococcus aureus. Results were evaluated and discussed according to Hygienic indicators and limits for public places (GB 37488-2019) and Hygienic specification of central air conditioning ventilation systems in public buildings (WS 394-2012). Results The environmental temperature medians (P25, P25) in the platforms and carriages of Nanjing metro line X were 20.7 (19.95, 21.65) ℃ and 21.8 (19.80, 35.80) °C, respectively, which exceeded the standard limits. The relative humidity and noise level in the carriages were 38.80% (24.6%, 46.3%) and 79.50 (76.25, 82.00) dB, respectively, failing to meet the national health standards. The total fungi count in supply air in 22.2% of the monitoring points exceeded the standard value. Coliform bacteria and Staphylococcus aureus were detected on the surfaces of some high-touch objects, with the bathroom faucet being the most severely contaminated. The medians (P25, P25) of temperature [21.8 (19.8, 35.8) ℃], wind speed [0.48 (0.39, 1.02) m·s−1], noise [79.5 (76.25, 82.00) dB, and CO2 content [0.079% (0.070%, 0.091%)] in the carriages were higher than those in the station halls and platforms (P<0.05). There were significant differences in microclimate, air particulate matter, and air microbial levels between urban and suburban stations (P<0.05); the concentrations of PM10 [0.076 (0.046, 0.079) mg·m−3)] and PM2.5 [0.063 (0.044, 0.068) mg·m−3)] in suburban stations were 2 times higher than those of urban stations.
