1.Cross-sectional survey of healthcare-associated infection in 5 736 medical institutions across China in 2024

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
2.Constructing and validation of a predictive model and application program for stone recurrence after endoscopic retrograde cholangiopancreatography based on machine learning algorithms in patients with common bile duct stones

Objective:To construct and validate a predictive model and application program for stone recurrence after endoscopic retrograde cholangiopancreatography (ERCP) based on machine learning algorithms in patients with common bile duct stones (CBDS).Methods:A multicenter retrospective cohort study was conducted, 862 CBDS patients underwent ERCP from June 2020 to September 2023 in Changshu First People′s Hospital (data set 1, 759 cases, including a training set of 588 cases and a validation set of 171 cases) and Changshu Hospital of Traditional Chinese Medicine (data set 2, 103 cases, used as a test set). The demographics, medical history, ERCP procedural records and laboratory indices were collected. All patients were followed up for 1 year, and the stone recurrence was recorded. In training set, the feature selection was conducted by the least absolute shrinkage and selection operator (LASSO) algorithm, and a conventional Logistic regression model was constructed based on selected features. The 3 machine learning algorithms (gradient boosting machine model, extreme gradient boosting model and random forest model) and a conventional Logistic regression model (LASSO model) were trained to fit predictive models. The model performance was assessed by area under curve (AUC) of receiver operating characteristic curve. The model interpretability was analyzed by feature importance evaluation, Shapley additive explanations (SHAP) and force plots. The best-performing model was deployed as an online application by Streamlit framework (V1.36.0).Results:Among the 862 patients, 158 patients (18.33%) developed stone recurrence after ERCP. There were no statistical difference in demographics, medical history, ERCP procedural records and laboratory indices between training set and a validation set ( P>0.05). LASSO regression analysis result showed that 6 key variables (in descending order of significance: endoscopic sphincterotomy, common bile duct angulation, stone diameter, stone count, common bile duct diameter, and periampullary diverticulum) influencing stone recurrence. ROC curve analysis result showed that the random forest model exhibited the highest predictive performance (it had the largest AUC of 0.900). SHAP analysis result showed that common bile duct angulation, common bile duct diameter, stone diameter, endoscopic sphincterotomy and stone count were the top 5 contributing factors in the random forest model. Using Python, the random forest model was implemented into a Streamlit-based application with a user-friendly visual interface, providing predictive outcomes, confidence levels, SHAP force diagram and health recommendations. In the test set, the application program achieved an accuracy of 84.5% (87/103), sensitivity of 82.6% (19/23), and specificity of 85.0% (68/80). SHAP plots and force diagram intuitively illustrated the impact of key features on stone recurrence prediction, offering a clear visualization of each variable′s role within the model. Conclusions:The predictive model and application program based on the random forest machine learning algorithms demonstrate excellent predictive performance and practical usability in predicting stone recurrence after ERCP in patients with CBDS.
3.A survey on lameness in dairy cows in 87 large-scale farms

This study investigated the incidence,prevention and control measures,diagnostic and treatment methods,and treatment costs of hoof diseases in dairy cows across 87 large-scale farms,aiming to provide a scientific basis and new insights for the treatment and prevention of hoof disea-ses in dairy herds.The results showed that the incidence rate of lameness in most farms was con-trolled between 3%and 7%,with a cure rate above 85%.The importance ranking of prevention and control measures was as follows:hoof trimming>leveling the ground>disinfection>regular foot baths>manure removal>locomotion(lameness)scoring.The importance ranking of treat-ment measures was:hoof trimming>disinfection and astringents>anti-inflammatory treatment>antibiotics>block application>traditional Chinese medicine(fire therapy).Economic analysis showed that the treatment cost of lameness was closely related to the incidence rate,with higher treatment expenses observed in farms with higher incidence rates.Farms with higher individual milk yields typically had more efficient disease management strategies,leading to lower treatment costs.Large-scale farms should continuously optimize management practices to reduce the occur-rence of lameness and improve overall farm efficiency by using effective treatment strategies.
4.Intelligent healthcare in the continuity management of patients with liver transplantation:a scoping review

Objective To provide a scoping review of studies on the application of intelligent healthcare for continuity management in liver transplant patients,in order to provide a reference for the future development of intelligent liver transplant management.Methods A systematic search of relevant studies was conducted in PubMed,Embase,CINAHL,Web of Science,Cochrane Library,Scopus,CNKI,Wanfang Database,VIP Database,and CBM from the establishment of the databases to 30 September 2024.Following this search,the included literature was screened,summarized,and analyzed.Results A total of 19 papers were included.The intervention forms included application,intelligent medical device,and remote rehabilitation platform.The content elements included transplant health management,rehabilitation program planning,health data monitoring,telemedicine consultation,psychosocial support,and follow-up supervision.The evaluation indicators included physiological indicators,self-management ability,health-related quality of life,transplantation outcomes,patient satisfaction,and feasibility indicators.Conclusion The application of intelligent healthcare in the continuity management of liver transplantation patients is feasible and effective,and it is recommended that healthcare professionals integrate the diverse practice forms of intelligent healthcare,deepen the content elements and refine the evaluation indicator system based on the actual clinical situation,in order to improve the quality of the continuity management of liver transplantation patients.
5.Measurement method based on EBT3 film technology for quality control detection of INTRABEAM PRS500 intraoperative radiotherapy equipment

Objective To propose an EBT3 film technology-based quality control measurement method for the INTRABEAM PRS500 intraoperative radiotherapy equipment to solve the problems of the traditional methods in cumbersome operation and setup error.Methods According to HJ 1198-2021 Requirements of radiation safety and protection for radiotherapy and GBZ 121-2020 Requirements for radiological protection in radiotherapy,the environmental radiation testing of the INTRABEAM PRS500 intraoperative radiotherapy equipment was carried out point by point by means of the radiation inspection instrument.The INTRABEAM PRS500 intraoperative radiotherapy equipment was characterized by a point X-ray source(XRS),and the XRS was detected in terms of the probe linearity,radiation dose,dynamic deviation,isotropy and dose rate.The EBT3 film technology was used to verify the symmetry and isotropy of the XRS planar dose of INTRABEAM PRS500 intraoperative radiotherapy equipment.Results The X-γ dose equivalent rate of each monitoring site of the INTRABEAM PRS500 intraoperative radiotherapy device was lower than the method detection limit(MDL).The results of SQA quality control showed that the INTRABEAM PRS500 intraoperative radiotherapy equipment XRS met the quality control requirements in terms of the probe linearity,radiation dose,dynamic deviation and etc,and the isotropy differences in the+X,-X,+Y,and-Y axis directions ranged from-1.40%to 1.79%,which were all within the allowable range of measurement tolerance(5.60%to 5.65%).The results of measuring the isotropy of the INTRABEAM PRS500 intraoperative radiotherapy equipment based on the EBT3 film technology showed that the dose distribution of the XRS in the directions of the+X,-X,+Y,and-Y axes at the same plane was well isotropic,and that the doses in the directions of the X and Y axes were symmetrically distributed,and that the maximum skewness value for the isotropy of the XRS in the XY plane was-1.581%,which met the requirements of AAPM TG61 report on the reference dosimetry of low-energy and medium-energy X-rays for radiotherapy and radiobiology of≤±5.3%.Conclusion The EBT3 film technology-based measurement method gains high simplicity and feasibility for the isotropy of the INTRABEAM PRS500 intraoperative radiotherapy equipment in the directions of the+X,-X,+Y,and-Y axes at the same planet,which realizes the dynamic monitoring of the dosimetric changes and facilitates the whole-process quality control management of the intraoperative radiotherapy equipment.[Chinese Medical Equipment Journal,2025,46(6):47-53]

7.Curative effect observation on combined application of ear canal injected administration dispenser and triamcinolone acetonide econazole nitrate cream in the treatment of mycosis externa

Objectives To investigate the clinical efficacy on combined application of ear canal injected administration dispenser and triamcinolone acetonide econazole nitrate cream in the treatment of mycosis externa.Methods Clinical data of 130 patients(166 ears)diagnosed with mycosis externa from January 2023 to December 2023 were collected and randomly divided into two groups.In the observation group,65 cases(86 ears)were treated with ear canal injected administration dispenser combined with triamcinolone acetonide econazole nitrate cream,while in the control group,65 cases(80 ears)applied clean cotton swabs with triamcinolone acetonide econazole nitrate cream to the external auditory canal by themselves to treat mycosis externa.All the patients were treated for 4 weeks and followed up for half a year.The clinical efficacy,recurrence rate and adverse reactions were statistically analyzed and compared.Results The total effective rate of the observation group was 97.67%,which was significantly higher than that of the control group was 73.75%,there was a significant difference between the two groups(P<0.05).There were no obvious adverse reactions in either group.No obvious recurrence was observed in the recovered patients of both groups during the follow-up period.Conclusion The efficacy of combined of ear canal injected administration dispenser and triamcinolone acetonide econazole nitrate cream in the treatment of mycosis externa is significant.Compared with the traditional way of medicine application,the adverse reactions are less.It can improve the cure rate and reduce the recurrence rate,which is worthy in clinical application.
