1.Epidemiological characteristics and spatial-temporal clustering of severe fever with thrombocytopenia syndrome in Huai'an City from 2011 to 2024

Objective:
To investigate the epidemiological characteristics and spatial-temporal clustering of severe fever with thrombocytopenia syndrome (SFTS) in Huai'an City, Jiangsu Province from 2011 to 2024, so as to provide a basis for optimizing local SFTS prevention and control strategies, and identifying high-risk areas and key populations.
Methods:
Data on SFTS incidence and deaths in Huai'an City from 2011 to 2024 were collected from the Infectious Disease Reporting Information System of the Chinese Disease Prevention and Control Information System. The reported incidence, mortality, and fatality rates were calculated. Descriptive analysis was performed on temporal, population, and regional distribution. The average annual percent change (AAPC) was used to analyze the trend in the reported incidence of SFTS. Global and local spatial autocorrelation analyses were employed to examine the spatial distribution patterns and spatial association patterns of SFTS incidence while spatio-temporal scanning analyses was used to assess the spatial-temporal clustering of SFTS.
Results:
A total of 337 SFTS cases were reported in Huai'an City from 2011 to 2024, with the reported incidence rising from 0.17/100 000 to 1.88/100 000. There were 20 deaths, with an average annual mortality of 0.03/100 000, and a fatality rate of 5.93%. The incidence showed obvious seasonality, with a peak in May and June (148 cases, accounting for 43.92%). Spring and summer accounted for 107 cases (31.75%) and 159 cases (47.18%), respectively. The reported SFTS cases were mainly male, farmers, and individuals aged ≥41 years, accounting for 56.38%, 79.23%, and 96.74%, respectively. The population distribution of death cases was basically consistent with that of incident cases. Xuyi County was a high-incidence area, with a total of 332 reported cases, accounting for 98.52%. All death cases were reported in this county. Spatial autocorrelation analyses revealed a positive spatial correlation in SFTS incidence from 2019 to 2024, with Moran's I values ranging from 0.214 to 0.336 (all P<0.05). Heqiao Town, Tianquanhu Town, and Guiwu Town in Xuyi County were identified as high-high clustering areas. Spatio-temporal scanning analyses showed that cluster 1 was consistent with the high-high clustering areas, with an aggregation time from the second quarter of 2019 to the second quarter of 2022.
Conclusions
From 2011 to 2024, the reported incidence of SFTS in Huai'an City showed an upward trend, with a high incidence in spring and summer. Males, farmers, and the middle-aged and elderly population were the key populations for prevention and control. Xuyi County was the key area for prevention and control.
2.Effects of polysaccharide liposomes of dendrobium officinale targeting hair follicles in the treatment of androgenetic alopecia

Based on previous research on the promoting effect of dendrobium officinale polysaccharides (DOP) on hair growth, this study aimed to regulate the skin keratin penetration and hair follicle targeting ability of DOP through molecular weight and nano-carriers to enhance its therapeutic effect on androgenetic alopecia (AGA). Three molecular weight polysaccharides, namely high (DOP), medium (MDOP), and low (LDOP), were prepared by mannanase hydrolysis, and the corresponding liposomes (DOP-lip/MDOP-lip/LDOP-lip) were constructed. Studies have shown that DOP liposomes can effectively achieve follicular targeted delivery and promote efficient uptake by human dermal papilla cells through caveolin-mediated pathways. In the testosterone-induced AGA mouse model, LDOP-lip demonstrated excellent therapeutic effects, restoring the number and morphology of hair follicles to nearly normal levels. In summary, DOP liposomes show significant potential for promoting hair follicle repair through precise delivery and efficient cellular uptake.
3.Investigating Molecular Mechanisms of Qijia Rougan Prescription and Its Key Effect or Ingredients Against Hepatic Fibrosis Based on Macrophage M2 Polarization

ObjectiveBased on the regulation of macrophage M2 polarization, this study aims to explore the molecular mechanism and action targets of the Qijia Rougan prescription and its key effector ingredients in anti-fibrosis, thereby providing a basis and reference for the development of new drugs for hepatic fibrosis. MethodsA rat model of hepatic fibrosis was established by subcutaneous injection of 40%CCl4, followed by oral administration of Qijia Rougan granules. The volume of collagen fibers was detected using Masson staining, the fibrosis markers Collagen Ⅰ and α-SMA were detected using immunohistochemistry, the proportion of M2 macrophages was detected by flow cytometry. The expression levels of M2 macrophage phenotype markers CD163 and CD206 were detected using immunofluorescence double staining. Western blot was used to detect the levels of the transforming growth factor-β (TGF-β), platelet derived growth factor subunit B (PDGFB), interleukin-10 (IL-10), phosphorylated Janus kinase 1 (p-JAK1), and phosphorylated signal transducer and activator of transcription 6 (p-STAT6). Real-time fluorescent quantitative PCR was used to detect the relative expression levels of JAK1, STAT6, Arginase 1(Arg1), and Fizz1. Based on the theory of serum pharmacology, liquid chromatography-mass spectrometry and WENN analysis were used to obtain the active ingredients of Qijia Rougan prescription. Molecular docking and molecular dynamics simulation were performed to analyze the effector ingredients and their targets. The identified effector ingredients were interfered with IL-4-induced M2 polarization of RAW264.7 macrophage in vitro to validate the targets. ResultsQijia Rougan prescription significantly reduced the content of fibrosis markers α-SMA and Collagen Ⅰ, as well as collagen fiber content (P<0.05). It decreased the proportion of M2 macrophages and the levels of related cytokines IL-10, TGF-β and PDGFB, and up-regulated the levels of p-JAK1 and p-STAT6 (P<0.05). A total of 1 214 compounds were identified from Qijia Rougan prescription, medicated serum and blank serum, and 29 ingredients were finalized by Venn analysis, including 15 blood-entry prototypes and 14 drug metabolites. Molecular docking showed that enoxolone and berberine bound more strongly to JAK1, with binding free energies of -9.6 kcal·mol-1(1 cal≈4.184 J) and -9.1 kcal·mol-1, respectively. Molecular dynamics simulations showed that JAK1-enoxolone and JAK1-berberine exhibited stable simulation trajectories within 100 ns, with essentially identical conformations and high protein overlap before and after simulation. Their binding free energies were -25.18 5.0.81 kcal·mol-1 and -27.39 7.0.85 kcal·mol-1, respectively. The number of hydrogen bonds formed between JAK1 and enoxolone ranges from 0 to 5, and most of the time can be maintained at 2-3. In vitro intervention with enoxolone or berberine significantly reduced p-JAK1 and p-STAT6 levels (P<0.05). ConclusionQijia Rougan prescription inhibits M2 macrophage polarization in hepatic fibrosis. Enoxolone and berberine are the key effector ingredients of Qijia Rougan prescription to inhibit macrophage M2 polarization through targeting JAK1 and modulating the JAK1/STAT6 signaling pathway, thereby ameliorating hepatic fibrosis. This study provides a basis for prescription optimization, clinical application and new drug development, as well as a reference for monolithic anti-hepatic fibrosis research.
4.Epidemiological characteristics and prediction of incidence trend of hepatitis E in Huai'an City in 2015-2024

Objective To analyze the epidemiological characteristics of hepatitis E (HE) in Huai'an City from 2015 to 2024, and construct and validate the prediction effect of the Seasonal Autoregressive Integrated Moving Average (SARIMA) model, and to provide a scientific basis for HE prevention and control. Methods Data of reported HE cases in Huai'an City from January 2015 to December 2024 were extracted from the National Disease Control and Prevention Information System. Descriptive epidemiological methods were used to analyze the temporal, regional and population distribution characteristics of HE incidence. After data preprocessing, stationarity test and randomness test, the SARIMA model was constructed. The optimal model was selected by minimizing the Akaike Information Criterion (AIC). The model performance was evaluated by adjusted R-squared (Adj R2) and root mean square error (RMSE). The monthly incidence rate of HE in Huai'an City in 2025 was predicted and compared with the actual incidence data to validate the model. Results A total of 1 713 HE cases were reported in Huai'an City from 2015 to 2024, showing a fluctuating upward trend. Huaiyin District (339 cases) and Lianshui County (334 cases) were high-incidence areas. The cases were mainly concentrated in people aged 45 and above (83.3%), males (gender ratio 2.37:1) and farmers (70.8%). The optimal model was SARIMA (1,1,1)×(2,0,1)12, with residual as white noise sequence. There was no statistical difference between the actual incidence and predicted value from January to August 2025 (P>0.05). Conclusion HE incidence in Huai'an has obvious aggregation and temporal fluctuation characteristics. The SARIMA model has a good prediction effect, which can provide reference for formulating targeted prevention and control strategies.
5.Surveillance of soil-transmitted nematodiasis in Hubei Province from 2016 to 2023

Objective To investigate the prevalence of soil-transmitted nematode human infections in Hubei Province from 2016 to 2023, so as to provide insights into formulation of soil-transmitted nematodiasis control strategies and evaluation of soil-transmitted nematodiasis control effectiveness. Methods There were 10 surveillance points established in Hubei Province for soil-transmitted nematodiasis each year from 2016 to 2023. The surveillance points were divided into eastern, western, southern, northern and central regions according to geographical locations, and one administrative village was sampled from a township in each region as a surveillance point. Then, at least 200 permanent residents at ages of over 3 years were randomly sampled at each point, with at least 1 000 individuals sampled. Soil-transmitted nematode infection was detected using a modified Kato-Katz thick smear method (two slides for one stool sample), and Enterobius vermicularis infection was additionally detected with a cellophane tape test among children at ages of 3 to 9 years. The prevalence of soil-transmitted nematode human infections was descriptively analyzed. Results A total of 80 547 individuals underwent surveillance for soil-transmitted nematodiasis in Hubei Province from 2016 to 2023, and a total of 262 individuals were detected with soil-transmitted nematode infections, including two cases with mixed infections, yielding an overall prevalence rate of 0.33% (262/80 547). The prevalence rates of hookworm, Ascaris lumbricoides, Trichuris trichiura, and E. vermicularis infections were 0.02% (20/80 547), 0.07% (55/80 547), 0.02% (15/80 547) and 0.22% (174/80 547), respectively, appearing a parasite species-specific prevalence rate (χ2 = 250.235, P < 0.05), and the prevalence rates of soil-transmitted nematode infections were 0.50%(51/10271),0.37%(34/9120),0.39%(39/10099),0.40%(41/10168), 0.36% (37/10 234), 0.45% (46/10 211), 0.03% (3/10 238) and 0.11% (11/10 206) in Hubei Province from 2016 to 2023, respectively, showing a year-specific prevalence rate (χ2 = 60.589, P < 0.05). Soil-transmitted nematode infections were detected across 36 surveillance points, and there was a significant difference in the prevalence of soil-transmitted nematode infections among surveillance points (0.02% to 3.30%, P < 0.001). In addition, the differences in prevalence rates of human soil-transmitted nematode infection among different age groups (χ2 = 531.091, P < 0.05), occupations (P < 0.001), and education levels (P < 0.001) were all statistically significant. Among these, the children at ages of 3 to 9 years old (1.40%, 174/12 461), nursery-school children (1.69%, 90/5 312) and preschool-aged children (1.56%, 93/5 969) were relatively high. Conclusions The overall prevalence of soil-transmitted nematodiasis was low in Hubei Province from 2016 to 2023; however, the prevalence varied across regions and among populations, with E. vermicularis as the predominant species of soil-transmitted nematode. Soil-transmitted nematodiasis control should be given a high priority in key areas in Hubei Province, and preschool children, primary school students, and individuals with a low educational level are recommended as key target populations for health education and interventions.
6.Logistic Regression Analysis of Helicobacter Pylori Infection Status and Influencing Factors among Occupational Population in Chengdu City

Objective To analyze the status and influencing factors of Helicobacter pylori(Hp)infection in the occupational population of Chengdu city.Methods A retrospective analysis was conducted on the general data of 8860 occupational population who underwent health examinations in our hospital from June 2021 to June 2023.All study subjects underwent 14 C urea breath test during health check ups.Analyze the Hp infection status of 8860 occupational population,and use logistic regression analysis(single factor and multiple factor)to explore the influencing factors of Hp infection in occupational population in Chengdu.Results Among the 8860 occupational population undergoing physical examinations in Chengdu,2890 cases were positive for Hp infection,with a total Hp infection rate of 32.62%.Univariate analysis found that occupational groups with age≥40 years,male,BMI<18.5 kg/m2(underweight)/24.0 kg/m2~27.9 kg/m2(overweight)/≥28 kg/m2(obese),workers,history of alcohol consumption,smoking,unclean diet,family history of Hp infection,and history of spicy and jelly diet had a higher Hp infection rate(P<0.05).Multivariate analysis showed that age≥40 years old,low BMI,overweight,obesity,alcohol consumption history,unclean diet history,family history of Hp infection,and spicy diet history were all risk factors affecting Hp infection in the occupational population of Chengdu(P<0.05).Conclusion The positive rate of Hp infection in the occupational population of Chengdu is relatively high,associated with age,alcohol consumption history,BMI,history of unclean diet,family history of Hp infection,and history of spicy eating.Therefore,attention should be paid to the Hp infection status of the occupational population.It is not only necessary to strengthen Hp infection publicity and education,but also to intervene in its risk factors early,in order to reduce and prevent Hp infection in the occupational population.
7.Summary of the best evidence for the physical restraint management of elderly dementia patients

Objective:To evaluate and summarize the evidence related to the physical restraint of elderly dementia patients, so as to provide a reference for clinical practice.Methods:Relevant literature on the physical restraint of elderly dementia patients in domestic and foreign databases was systematically retrieved, including clinical decisions, guidelines, expert consensus, recommended practices, evidence summaries, systematic reviews/Meta-analyses, and original studies. The retrieval time limit was from the establishment of the database to April 2024. The researchers evaluated the literature according to relevant evaluation criteria and extracted useful evidence.Results:A total of 11 articles were included, including one clinical decision, four guidelines, four systematic reviews, one evidence summary, and one expert consensus. Twenty-one best evidence items were summarized from five aspects: organizational norms, education and training, restraint assessment, alternative therapies, and clinical practice.Conclusions:The evidence on the physical restraint of elderly dementia patients can provide a theoretical basis for medical institution practitioners. When applying the best evidence, medical staff should combine their clinical experience, fully consider the individual specificity of elderly patients and the actual clinical situation, and analyze the differences between the evidence and practice, so as to implement personalized physical restraint for elderly dementia patients in clinical practice.
8.Correlation between serum uric acid/high density lipoprotein cholesterol and carotid atherosclerosis in patients with type 2 diabetes mellitus

Objective To explore the correlation between serum uric acid/high density lipoprotein cholesterol(UHR)and carotid atherosclerosis(CAS)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 322 patients with T2DM admitted to Department of Endocrinology,Tongling People's Hospital were enrolled in this study from November 2021 to March 2023.They were divided into CAS group(n=165)and T2DM group(n=157)according to whether accompanied with CAS.The medical history,physical examination,biochemical examination and UHR were compared between the two groups.Results Compared with T2DM group,the age,SBP,TG,Scr,Cys-C,Hcy,SUA and UHR in CAS group were increased(P<0.01),while HDL-C and HbA1c were decreased(P<0.05 or P<0.01).Pearson or Spearman correlation analysis showed that TG,Scr and Hcy were correlated with UHR(P<0.01).Logistic regression analysis showed that UHR,age and SBP were the independent risk factors of CAS in T2DM patients.ROC curve analysis showed that the sensitivity and specificity of UHR in predicting CAS were 82.40%and 73.90%.Conclusions UHR is the risk factor of CAS in T2DM patients and has a certain predictive value for CAS.
9.KAP Investigation and Influential Factor Analysis on the Risk of Medication Safety Among Pregnant and Lactating Women in Certain Areas of Hubei Province

Objective To investigate the knowledge,attitude,and practice(KAP)related to medication use among pregnant and lactating women in Hubei province,to comprehend the specific requirements of pregnant and lactating women for pharmaceutical assistance,to assess the critical stages in the process of medication,and to furnish factual backing for devising a secure medication education framework tailored to pregnant and lactating women.Methods Through a multi-center study design,with pregnant and lactating women as the research subjects,a questionnaire survey was carried out to gather fundamental information and assess their knowledge,attitude,and practices towards medication use.Data analysis involved utilizing one-way analysis of variance,non-parametric tests,and multiple linear regression.Results A total of 548 valid questionnaires were collected.The average scores for knowledge,practice and attitude among pregnant and lactating women were(58.26±30.68),(52.18±22.32),and(46.60±14.66)points,respectively.Overall,the scores for medication knowledge and behavior were good,while the attitude scores were passable.These results suggest that pregnant and lactating women have a certain level of understanding about safe medication practices and hold a positive attitude towards receiving medication education.However,there are still potential safety hazards in their daily medication behavior.The results of multiple linear regression indicate that age,educational level,and occupation are the main factors influencing the knowledge,attitude,and practice of medication use among pregnant and lactating women.Conclusions Pregnant and lactating women require urgent education on the safe use of medication.It is recommended that targeted drug education should be carried out based on the knowledgegaps and medication misconceptions of different objects.Additionally,it is crucial to effectively utilize new media platforms,such as WeChat official accounts and channels,and to engage in offline scientific outreach activities,such as offering free consultations and giving lectures on rational drug use.This will provide pregnant and lactating women with convenient and practical educational content on safe medication.
10.A case of Kabuki syndrome with short stature

This article reported the diagnosis and management of a case of a child with Kabuki syndrome. Kabuki syndrome is characterized by distinct facial features, skeletal anomalies, abnormal skin texture, and intellectual disabilities, and is primarily caused by heterozygous mutations in the lysine methyltransferase 2D(KMT2D) gene. The onset in this patient was insidious, presenting with short stature and intellectual impairment. Genetic testing identified a pathogenic heterozygous variant in the KMT2D gene. This article focuses on analyzing the necessity and appropriateness of growth hormone therapy in children with Kabuki syndrome, and includes a review of relevant literature to improve clinicians′ understanding of this rare condition.
