Clinical pharmacist participated in the whole process of combined drug treatment and medication monitoring of a patient with refractory edema secondary to end-stage diabetic kidney disease.By using dapagliflozin combined with sacubitril valsartan and conventional diuretics,the patient's body weight decreased by 16 kg,blood pressure returned to normal,systemic edema subsided and the symptoms of heart failure obviously improved.In this case,clinical pharmacist provided individualized pharmaceutical care and assisted clinicians to make a scientific and reasonable drug therapy,so as to provide a new idea and medication reference for clinical patients to delay the time of renal replacement therapy.

Currently,the research or publications related to the clinical comprehensive evaluation of Chinese patent medicine are increasing,which attracts the broad attention of all circles. According to the completed clinical evaluation report on Chinese patent medicine,there are still practical problems and technical difficulties such as unclear responsibility of the evaluation organization,unclear evaluation subject,miscellaneous evaluation objects,and incomplete and nonstandard evaluation process. In terms of evaluation standards and specifications,there are different types of specifications or guidelines with different emphases issued by different academic groups or relevant institutions. The professional guideline is required to guide the standardized and efficient clinical comprehensive evaluation of Chinese patent medicine and further improve the authority and quality of evaluation. In combination with the characteristics of Chinese patent medicine and the latest research achievement at home and abroad,the detailed specifications were formulated from six aspects including design,theme selection,content and index,outcome,application and appraisal,and quality control. The guideline was developed based on the guideline development requirements of China Assoication of Chinese medicine. After several rounds of expert consensus and public consultation,the current version of the guideline has been developed.

4.Therapeutic effects of the extract of Sancao Formula, a Chinese herbal compound, on imiquimod-induced psoriasis via cysteine-rich protein 61.

OBJECTIVE:
Psoriasis is a common chronic inflammatory skin disease that is prone to recurrence, and the proinflammatory factor, cysteine-rich protein 61 (Cyr61), is important in its pathophysiology. Long-term clinical practice has shown that Sancao Formula (SC), a Chinese herbal compound, is effective in the treatment of psoriasis, but the precise mechanism remains unknown. In this study, we investigate the mechanism by which SC extract alleviates imiquimod (IMQ)-induced psoriasis.
METHODS:
The expression of Cyr61 in psoriatic lesions and normal healthy skin was detected using immunohistochemical analysis to investigate the biological role of Cyr61 in models of psoriatic inflammation. A psoriatic mouse model was established by topical application of IMQ, and the effect of topical application of SC extract was evaluated using the psoriasis area and severity index (PASI) score, hematoxylin-eosin staining, and histopathological features of the skin. Next, a HaCaT cell inflammation model was established using interferon-γ (IFN-γ), and the effect of SC extract on the mRNA and protein levels of Cyr61 and intercellular cell adhesion molecule-1 (ICAM-1) was confirmed using Western blot and quantitative real-time polymerase chain reaction analyses.
RESULTS:
Immunohistochemical staining showed that the expression of Cyr61 in psoriatic lesions was higher than that in normal skin samples (78.26% vs 41.18%, P < 0.05), and the number of Cyr61-positive cells in psoriatic lesions was also significantly higher than in normal skin (18.66 ± 2.51 vs 4.33 ± 1.52, P < 0.05). Treatment in mice with IMQ-induced psoriasis showed that SC extract could significantly improve the inflammatory phenotype, PASI score (10.875 ± 0.744 vs 3.875 ± 0.582, P < 0.05), and pathological features compared with those in IMQ model group; SC treatment was also associated with decreased levels of Cyr61 and ICAM-1. In the IFN-γ-induced inflammatory cell model, the mRNA and protein levels of Cyr61 and ICAM-1 were upregulated, while the SC extract downregulated the levels of Cyr61 and ICAM-1.
CONCLUSION
The results provide a theoretical basis for the involvement of Cyr61 in the pathogenesis of psoriasis, and suggest that SC should be used to target Cyr61 for the prevention of psoriasis recurrence.

5. The research progress of P2Y receptors and its antagonists in cardiovascular diseases

ADP receptors on platelets are mainly divided into two categories: P2X and P2Y. The activation of P2Y receptor is closely related to the occurrence and development of cardiovascular diseases. It plays an important role in regulating platelet activation and aggregation, vascular inflammation, thrombosis and so on. It is the target of ADP receptor blockers. This kind of drugs is also a hot spot in the research and development of antiplatelet drugs. This article reviews the role of P2Y receptor in the occurrence and development of cardiovascular diseases, in order to provide new ideas and reference for the application of P2Y receptor antagonists in the treatment of cardiovascular diseases.
6.Clinical Application of Shengxiantang: A Review

Shengxiantang, one of the classical traditional Chinese medicine (TCM) prescriptions, was developed by ZHANG Xi-chun, a well-known doctor in the Republic of China, to treat the syndrome of pectoral Qi sinking. Due to the unique herbal composition, this prescription has yielded remarkable outcomes, and it is still widely used for dealing with diseases of various systems. This paper reviewed the clinical research concerning Shengxiantang in order to provide reference for its clinical application. Following the retrieval of related articles published in the past ten years from China National Knowledge Infrastructure (CNKI), Wanfang Data Knowledge Service Platform, Chongqing Weipu Database for Chinese Technical Periodicals (VIP), and Chinese Biomedical Literature Service System (SinoMed) with the Chinese key words of “Shengxiantang (升陷汤)”, “sinking of pectoral Qi (大气下陷)”, “replenishing Qi and raising the sinking (益气升陷)”, and“pectoral Qi theory (大气理论)”, this paper collated the clinical application experience of Shengxiantang. To be specific, the corresponding etiology and pathogenesis, interpretations of Shengxiantang, as well as its indications, application experience and precautions were summarized to provide a basis for its further research and application. Based on the collected more than 50 representative articles, it was found that Shengxiantang has been extensively utilized for treating complex and critical diseases of various systems, especially the circulatory and respiratory diseases and organ prolapse. The research on the clinical application of Shengxiantang has deepened, exhibiting a promising development trend, which will provide new ideas for its clinical application.
7.Key Genes of Peripheral Blood Mononuclear Cells in Hepatocellular Carcinoma and Effective Chinese Herbs: A Bioinformatics Analysis and Systematic Review

Objective:To explore the key gens of peripheral blood mononuclear cells in hepatocellular carcinoma(HCC-PBMC) and potentially effective Chinese herbs based on bioinformatics, and to verify the clinical efficacy of these Chinese herbs via a systematic review. Method:The chips GSE58208 and GSE36076 of HCC-PBMC were downloaded from the Gene Expression Omnibus (GEO), followed by the identification of differentially expressed genes (DEGs) using RStudio. After protein-protein interaction (PPI) analysis by STRING, the DAVID was employed for gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis. The DEGs of HCC-PBMC were visualized by Cytoscape. The key genes of HCC-PBMC were calculated by CytoHubba plug-in and mapped with those in Coremine Medical for screening out the potential Chinese herbs for the treatment of HCC, which were then included for subsequent systematic review. Result:A total of 203 DEGs were obtained (194 up-regulated and nine down-regulated). As revealed by DAVID analysis, the DEGs were mainly enriched in such biological processes and signaling pathways as transcriptional regulation of DNA template, hydrolysis of ribonucleic acid phosphodiester bond, positive regulation of intranuclear mitosis and division, skeletal muscle fiber development, activation of mitogen-activated protein kinase(MAPK)activity, Fanconi anemia pathway, and metabolic pathway. 8.Exploration of Stratified Evidence Scoring Method of Acupuncture Clinical Practice Guidelines.

Due to its own internal laws of development, Chinese medicine (CM) seems more inclined to empirical medicine in a relatively long historical period. It is considered to be lacking objective and unified clinical practice guidelines (CPGs), and the difficulties in diagnosis and therapeutic effect evaluation comes with it, have restricted its further inheritance, development and international communication. Over the years, our research group has been committed to improving the standardization theory and methodology of CM, also perfecting relative techniques for further application, which are all based on the stratified evidence scoring method. We have already applied this method to 45 issued guidelines, including 5 national guidelines, 3 industrial guidelines, and 37 formulation/revision social organization guidelines. The stratified evidence scoring method has been recognized and used widely. It helps scholars and applicators to study, formulate, publish and popularize the acupuncture therapy clinical practice guidelines better, thus further promotes the development of acupuncture therapy.

BACKGROUND:
The ongoing new coronavirus pneumonia (Corona Virus Disease 2019, COVID-19) outbreak is spreading in China, but it has not yet reached its peak. Five million people emigrated from Wuhan before lockdown, potentially representing a source of virus infection. Determining case distribution and its correlation with population emigration from Wuhan in the early stage of the epidemic is of great importance for early warning and for the prevention of future outbreaks.
METHODS:
The official case report on the COVID-19 epidemic was collected as of January 30, 2020. Time and location information on COVID-19 cases was extracted and analyzed using ArcGIS and WinBUGS software. Data on population migration from Wuhan city and Hubei province were extracted from Baidu Qianxi, and their correlation with the number of cases was analyzed.
RESULTS:
The COVID-19 confirmed and death cases in Hubei province accounted for 59.91% (5806/9692) and 95.77% (204/213) of the total cases in China, respectively. Hot spot provinces included Sichuan and Yunnan, which are adjacent to Hubei. The time risk of Hubei province on the following day was 1.960 times that on the previous day. The number of cases in some cities was relatively low, but the time risk appeared to be continuously rising. The correlation coefficient between the provincial number of cases and emigration from Wuhan was up to 0.943. The lockdown of 17 cities in Hubei province and the implementation of nationwide control measures efficiently prevented an exponential growth in the number of cases.
CONCLUSIONS
The population that emigrated from Wuhan was the main infection source in other cities and provinces. Some cities with a low number of cases showed a rapid increase in case load. Owing to the upcoming Spring Festival return wave, understanding the risk trends in different regions is crucial to ensure preparedness at both the individual and organization levels and to prevent new outbreaks.
