1.Establishment of a rat model of comorbid chronic inflammatory pain and depression using different doses of complete Freund's adjuvant

Objective To compare the success rate and stability of rat models of comorbid chronic pain and depression induced by different doses of complete Freund's adjuvant(CFA).Methods Sixty SD rats were divided randomly into a control group,low-dose CFA group(CFA-L),and high-dose CFA group(CFA-H)(n=20 rats per group).Rats in the CFA-L and CFA-H groups were injected with 50 and 100 μL CFA,respectively,and rats in the control group were injected with 0.9%sodium chloride solution.The general state,body weight,mechanical withdrawal threshold(MWT),and thermal withdrawal latency(TWL)were observed at 0,7,14,21,and 28 days after modeling.Depressive behavior was evaluated using the open field test(OFT),forced swim test(FST),and tail suspension test(TST).Glutamate(Glu)and γ-aminobutyric acid(GABA)levels in the anterior cingulate cortex were detected by enzyme-linked immunosorbent assay.Brain-derived neurotrophic factor(BDNF)expression in the anterior cingulate cortex was detected by immunohistochemistry,and pathological changes in the anterior cingulate cortex were observed by HE staining.Results(1)Regarding the general condition of the rats,the left ankle joint and toes were obviously red and swollen in the CFA-L and CFA-H groups on the 7th day after modeling,and the swelling was more severe in the CFA-H group.The redness and swelling of the left hind foot and ankle joint and toes gradually recovered in the CFA-L group on days 14,21,and 28 after modeling,but were still obvious in the CFA-H group,and the water and food intake decreased.(2)The body mass was significantly lower in rats in the CFA-H group compared with those in the blank and CFA-L groups on days 14,21,and 28 after modeling(P<0.05,P<0.05).(3)Regarding pain-related behavior,the MWT and TWL were significantly decreased in the CFA-L and CFA-H groups on the 7th and 14th days after modeling,compared with the control group(P<0.05,P<0.05).On day 21 after modeling,MWT was significantly lower in the CFA-H group than in the blank and CFA-L groups(P<0.05,P<0.05),and TWL was significantly lower in the CFA-L and CFA-H groups than in the blank group(P<0.05,P<0.05).On day 28 after modeling,MWT and TWL were significantly lower in the CFA-H group than in the blank and CFA-L groups(P<0.05,P<0.05).(4)In terms of depression-related behaviors,the total OFT movement distance was significantly lower in the CFA-H group than in the blank and CFA-L groups on day 7 after modeling(P<0.05,P<0.05).The total OFT distance and central dwell time were significantly lower in the CFA-H group than in the blank and CFA-L groups on days 14,21,and 28 after modeling(P<0.05,P<0.05),and the result in the FST and TST were significantly higher than in the blank and CFA-L groups(P<0.05,P<0.05).(5)Glu,GABA,and BDNF expression levels were significantly higher in the CFA-H group than in the blank and CFA-L groups(P<0.05,P<0.05),while GABA,Glu/GABA,and BDNF levels were significantly lower in the CFA-H group than in the blank and CFA-L groups(P<0.05,P<0.05,P<0.05).(6)The CFA-L group showed less damage in the anterior cingulate cortex,more pyramidal cells,more arranged cells,clear nucleoli,and a small number of cells with karyokynesis and deep staining.Compared with the CFA-L group,rats in the CFA-H group showed a disordered cell arrangement in the injured area of the anterior cingulate cortex,a large number of pyknotic and hyperchromatic neurons,significantly fewer or absent pyramidal cells,and vacuoles,red blood cells,and neurofibrillary tangles in the interstitial space.Conclusions Injection of CFA 100 μL can be used to establish a rat model of chronic inflammatory pain and depression,showing hyperalgesia,depression-like behavioral changes,changes in levels of Glu,GABA,and BDNF in the anterior cingulate cortex,and pathological changes in the anterior cingulate cortex,consistent with the pathophysiological characteristics of chronic pain and depression.
2.Improved DeepSurv model for survival analysis in lung cancer and exploration of influencing factors

Objective To evaluate the performance of an improved DeepSurv model for survival analysis in lung cancer patients,and investigate key factors affecting the prognosis of lung cancer.Methods The lung cancer data from the SEER database(2018-2021)was used in the study,and the DeepSurv model was optimized by incorporating a self-attention mechanism,a residual network,a LIME module and an entropy regularization term to enhance prediction accuracy and interpretability.Model performance was assessed using C-index and Brier score,and the improved model was utilized to evaluate the effects of various features on the prognosis of lung cancer.Results The improved DeepSurv model achieved a C-index of 0.852 and a Brier score of 0.139.Feature importance analysis identified age as the primary determinant of the survival of lung cancer patients.Conclusion The improved DeepSurv model outperforms both the Cox proportional hazards model and the original DeepSurv model in terms of accuracy,robustness and interpretability,offering a novel methodology for personalized medicine and survival analysis.
3.Diabetic retinopathy research based on deep converged network

A converged network based on deep learning is proposed to realize the efficient and accurate diagnosis of diabetic retinopathy.Both data augmentation technology and generative adversarial network are used to augment the fundus images in EyePACS dataset for effectively addressing the problem of uneven classification of fundus images.The proposed model uses Inception-Resnet-V2 as the main network,and incorporates deep residual shrinkage network and pyramid split attention module for effectively filtering out the irrelevant information in the feature learning process and focusing on the lesion information,thereby improving the network's ability to capture important features.Experimental results show that the optimized model achieves accuracy,recall,specificity,sensitivity,and F1 score of 0.951,0.950,0.990,0.950,and 0.950,respectively,without the need to specify lesion characteristics in advance,demonstrating its superiority in evaluation indicators.
4.TTP combined with inflammatory indicators identifies CoNS bloodstream infection and contamination in tumor patients

Objective To explore the value of the time to positivity(TTP)combined with inflammatory indicators for identifying coagulase-negative Staphylococcus(CoNS)bloodstream infection and contamination in tumor patients.Methods A retrospective analysis was performed on totally 368 tumor patients with blood culture-positive admitted to the hospital from September 2022 to April 2023,and 47 patients with blood cul-ture-positive CoNS and complete clinical data were selected as research objects.The patients were divided into CoNS infection group(24 cases)and CoNS contamination group(23 cases)according to the diagnostic criteria of bloodstream infection and in combination with the clinical manifestations and laboratory examination re-sults.In addition,5 tumor patients with blood culture Staphylococcus aureus positive admitted to the hospital during the same period were selected as the control group.The TTP,percentage of immature granulocytes(IG%),absolute value of immature granulocytes(IG#),white blood cell count(WBC),neutrophil percent-age(NE%),neutrophil-to-lymphocyte ratio(NLR),C-reactive protein(CRP)and procalcitonin(PCT)levels in all groups were compared.Results Among the 47 patient isolates of CoNS from blood cultures,Staphylo-coccus hominis was the most common,accounting for 42.55%.There were significant differences in TTP,NE%,NLR and CRP levels between CoNS infection group,CoNS contamination group and control group(P<0.05).The TTP in CoNS infection group was shorter than that in CoNS contamination group,and the levels of NE%,NLR,CRP and PCT were higher than those in CoNS contamination group,with statistical sig-nificance(P<0.05).TTP combined with NLR,CRP and PCT had the largest area under the curve,which was 0.971(P<0.000 1),with a sensitivity of 91.67%,specificity of 95.65%,likelihood ratio of 21.08,positive predictive value of 91.67%,negative predictive value of 95.65%,and an accuracy of 93.62%.Conclusion TTP combined with NLR,CRP and PCT have important clinical value in the differential diagnosis of CoNS blood-stream infection and contamination in tumor patients.
5.Study of the protocol of handgrip exercise stress echocardiography

Objective:To explore the protocol for handgrip exercise stress echocardiography by comparing different maximal voluntary contraction(MVC)handgrip groups with bicycle exercise stress.Methods:Forty-one healthy volunteers were enrolled prospectively from June to October 2024 in Tangdu Hospital and utilized a color Doppler echocardiography system,supine cycle ergometer,and handgrip dynamometer to collect echocardiographic data at baseline,during handgrip exercises at 20%(3 min),30%(3 min),and 40%(2 min)of MVC,and spine bicycle exercise stress at peak. Parameters measured included left ventricular ejection fraction(EF),stroke volume(SV),cardiac output(CO),mitral inflow E-wave and A-wave velocities,lateral and septal mitral annular e' velocities,E/A and E/e' ratios,global longitudinal strain(GLS),left atrial reservoir strain(LAS R),conduit strain(LAS CD),and contractile strain(LAS CT). The non-invasive myocardial work indices were also assessed,including global work index(GWI),global constructive work(GCW),global wasted work(GWW),and global work efficiency(GWE). Statistical analyses were performed using repeated measures analysis of variance,with corrected paired t-test for comparisons between two exercise stress states. Results:Compared with the baseline state,heart rate,blood pressure,CO、GWI、GCW、GWW and LAS CT gradually increased,while EF、E/A、GLS、GWE、LAS R and LAS CD gradually decreased under 20%,30% and 40% of MVC states. The changes were most obvious at 40% of MVC state,which was selected for the handgrip exercise stress echocardiography protocol.Compared with the peak of bicycle exercise,at 40% of MVC,heart rate was significantly lower[(81.2 ± 9.7)bpm vs.(164.6 ± 11.3)bpm, P<0.05)],systolic blood pressure was slightly lower[(152.9 ± 13.2)mmHg vs.(165.1 ± 20.4)mmHg, P<0.05],diastolic blood pressure was higher[(96.0 ± 9.5)mmHg vs.(89.5 ± 10.9)mmHg, P<0.05],GLS was lower[(19.1 ± 1.5)% vs.(23.5 ± 1.7)%, P<0.05],GWI was similar[(2 254.2 ± 417.3)mmHg% vs.(2 227.5 ± 389.0)mmHg%, P>0.05],but GWE was higher[(95.3 ± 2.0)% vs.(93.7 ± 2.0)%, P<0.05],and LAS R was lower[(39.4 ± 4.2)% vs.(43.9 ± 4.1)%, P<0.05]. Conclusions:The 40% of MVC lasting 2 min can cause the most significant handgrip-related changes in cardiac function and can be used as the standard protocol for handgrip exercise stress echocardiography. Handgrip stress exercise can cause significant changes in cardiac systole,diastole,and work performance,showing different characteristics compared with bicycle exercise stress.
6.Criteria and prognostic models for patients with hepatocellular carcinoma undergoing liver transplantation

Hepatocellular carcinoma (HCC) is a leading cause of cancer-associated death globally. Liver transplantation (LT) has emerged as a key treatment for patients with HCC, and the Milan criteria have been adopted as the cornerstone of the selection policy. To allow more patients to benefit from LT, a number of expanded criteria have been proposed, many of which use radiologic morphological characteristics with larger and more tumors as surrogates to predict outcomes. Other groups developed indices incorporating biological variables and dynamic markers of response to locoregional treatment. These expanded selection criteria achieved satisfactory results with limited liver supplies. In addition, a number of prognostic models have been developed using clinicopathological characteristics, imaging radiomics features, genetic data, and advanced techniques such as artificial intelligence. These models could improve prognostic estimation, establish surveillance strategies, and bolster long-term outcomes in patients with HCC. In this study, we reviewed the latest findings and achievements regarding the selection criteria and post-transplant prognostic models for LT in patients with HCC.
7.Application of microflow imaging (MFI) technology based on Image Pro Plus software in BI-RADS 4 breast nodules and correlation between MFI parameters and Ki-67 expression

Objective:To evaluate the diagnostic value of micro flow imaging (MFI) supported by Image Pro Plus (IPP) software for BI-RADS classification of breast nodules and the correlation between its parameters and Ki-67 expression level.Methods:A total of 205 cases of breast solid nodules with ultrasound BI-RADS classification of 4 categories were enrolled in this study. BI-RADS classification, CDFI score, MFI score, CDFI-IPP score and MFI-IPP score by IPP software were performed for each nodule. Ki-67 immunohistochemical staining of invasive ductal carcinoma was performed to analyze the correlation between MFI-IPP-VI and Ki-67 expression levels.Results:The combined BI-RADS classification for diagnosis of malignant nodules had the largest area under the ROC curve, which was significantly higher than that of BI-RADS, MFI-IPP-VI, MFI score, CDFI-IPP-VI, and CDFI score (all P<0.05). The combined BI-RADS classification had the best diagnostic efficacy for the diagnosis of malignant nodules with a sensitivity of 96.4%, specificity of 65.6% and accuracy of 82.4%. In addition, the agreement between experienced and inexperienced sonographers applying MFI-IPP-VI was higher than MFI (Kappa=0.780 vs. Kappa=0.743). The mean value of Ki-67 in invasive ductal carcinoma was 21.93%±16.37%, and Spearman's correlation analysis showed a positive correlation between MFI-IPP-VI and Ki-67 expression level ( r=0.815, P<0.05). Conclusions:MFI-IPP can assist sonographers to classify breast nodules by BI-RADS, providing an effective basis for puncture biopsy. MFI-IPP-VI offers the possibility of noninvasively predicting the expression level of Ki-67 in invasive ductal carcinoma.

9.Sini Powder Alleviates Stress Response and Suppresses Hepatocellular Carcinoma Development by Restoring Gut Microbiota.

OBJECTIVES:
To explore the underlying pharmacological mechanisms and its potential effects of Chinese medicine herbal formula Sini Powder (SNP) on hepatocellular carcinoma (HCC).
METHODS:
The active components of SNP and their in vivo distribution were identified using ultraperformance liquid chromatography coupled with quadrupole time-of-flight mass spectrometry. Construction of component-target-disease networks, protein-protein interaction network, Gene Ontology function and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis, and molecular docking were employed to analyze the active components and anti-HCC mechanisms of SNP. Cell viability assay and wound healing assay were utilized to confirm the effect of SNP-containing serum (2.5%, 5.0%, 10%, 20%, and 40%), isoprenaline or propranolol (both 10, 100, and 1,000 µ mol/L) on proliferation and migration of HepG 2 or Huh7 cells. Meanwhile, the effect of isoprenaline or propranolol on the β 2 adrenergic receptor (ADRB2) mRNA expression on HepG2 cells were measured by real-time quantitative reverse transcription (RT-qPCR). Mice with subcutaneous tumors were either subjected to chronic restraint stress (CRS) followed by SNP administration (364 mg/mL) or directly treated with SNP (364 mg/mL). These two parallel experiments were performed to validate the effects of SNP on stress responses. Stress-related proteins and hormones were quantified using RT-qPCR, enzyme-linked immunosorbent assay, and immunohistochemistry. Metagenomic sequencing was performed to confirm the influence of SNP on the gut microbiota in the tumor-bearing CRS mice.
RESULTS:
The distribution of the 12 active components of SNP was confirmed in various tissues and feces. Network pharmacology analysis confirmed the anti-HCC effects of the 5 active components. The potential anti-HCC mechanisms of SNP may involve the epidermal growth factor receptor (EGFR), proto-oncogene tyrosine-protein kinase Src (SRC) and signal transducer and activator of transcription 3 (STAT3) pathways. SNP-containing serum inhibited the proliferation of HepG2 and Huh7 cells at concentrations of 2.5% and 5.0%, respectively, after 24 h of treatment. Furthermore, SNP suppressed tumor progression in tumor-bearing mice exposed to CRS. SNP treatment also downregulated the expressions of stress-related proteins and pro-inflammatory cytokines, primarily by modulating the gut microbiota. Specifically, the abundance of Alistipes and Prevotella, which belong to the phylum Bacteroidetes, increased in the SNP-treated group, whereas Lachnospira, in the phylum Firmicutes, decreased.
CONCLUSION
SNP can combat HCC by alleviating stress responses through the regulation of gut microbiota.

10.Salvianolic Acid B Exerts Antiphotoaging Effect on Ultraviolet B-Irradiated Human Keratinocytes by Alleviating Oxidative Stress via SIRT1 Protein.

OBJECTIVE:
To explore the anti-photoaging properties of salvianolic acid B (Sal B).
METHODS:
The optimal photoaging model of human immortalized keratinocytes (HaCaT cells) were constructed by expose to ultraviolet B (UVB) radiation. The cells were divided into control, model and different concentrations of Sal B groups. Cell viability was measured via cell counting kit-8. Subsequently, the levels of oxidative stress, including reactive oxygen species (ROS), hydroxyproline (Hyp), catalase (CAT), and glutathione peroxidase (GSH-Px) were detected using the relevant kits. Silent information regulator 1 (SIRT1) protein level was detected using Western blot. The binding pattern of Sal B and SIRT1 was determined via molecular docking.
RESULTS:
Sal B significantly increased the viability of UVB-irradiated HaCaT cells (P<0.05 or P<0.01). Sal B effectively scavenged the accumulation of ROS induced by UVB (P<0.05 or P<0.01). In addition, Sal B modulated oxidative stress by increasing the intracellular concentrations of Hyp and CAT and the activity of GSH-Px (P<0.05 or P<0.01). The Western blot results revealed a substantial increase in SIRT1 protein levels following Sal B administration (P<0.05). Moreover, Sal B exhibited good binding affinity toward SIRT1, with a docking energy of -7.5 kCal/mol.
CONCLUSION
Sal B could improve the repair of photodamaged cells by alleviating cellular oxidative stress and regulating the expression of SIRT1 protein.
