Objective To explore the economic toxicity perceptions and experiences of caregivers of colorectal cancer patients from the perspective of social ecological systems theory.Methods Using purposive sampling,18 caregivers of colorectal cancer patients hospitalized in the gastrointestinal sur-gery and oncology departments of a tertiary grade A hospital in Yangzhou were selected for semi-struc-tured interviews.The Colaizzi 7-step analysis method was employed to organize and analyze the data.Results Three main themes and nine sub-themes were extracted regarding the economic toxicity expe-riences and needs of caregivers of colorectal cancer patients.Microsystem included multiple negative experiences,impacted individual health,and difficulties in surrogate decision-making;mesosystem in-cluded heavy family financial burden,altered family lifestyle,and tense family atmosphere;macrosys-tem included needs for the scope and intensity of medical insurance reimbursement,needs for medical resources and services,and a desire for social support.Conclusion The ecological system of caregiv-ers of colorectal cancer patients is not optimistic,and is generally affected by economic toxicity.Inter-vention strategies can be sought from multiple aspects,including alleviating the negative experiences of caregivers,strengthening social support,and paying attention to the needs of caregivers,aiming to re-duce the level of economic toxicity among caregivers of colorectal cancer patients.

OBJECTIVES: To explore the value of serum cystatin C (CysC) levels in evaluating renal prognosis in IgA nephropathy (IgAN) patients. METHODS: We retrospectively collected the clinical data of IgAN patients diagnosed by renal biopsy at Guangdong Provincial People's Hospital from January, 2014 to December, 2018. Based on baseline serum CysC levels, the patients were divided into high serum CysC (>1.03 mg/L) group and normal serum CysC (≤1.03 mg/L) group. The composite endpoint for poor renal prognosis was defined as ≥50% decline in estimated glomerular filtration rate (eGFR) and/or progression to end-stage renal disease (ESRD). Lasso regression, multivariate Cox regression and Kaplan-Meier survival analysis were used to identify the risk factors and compare renal survival rates between the two groups. Smooth curves fitting and threshold effect analysis were used to explore the relationship between serum CysC levels and the outcomes. A nomogram model was constructed and its predictive performance was evaluated using concordance index, calibration curve, receiver operating characteristic (ROC) curve and the area under curve (AUC). RESULTS: A total of 356 IgAN patients were enrolled, who were followed up for 4.65±0.93 years. The composite endpoint occurred in 74 patients. High serum CysC was identified as an independent risk factor for poor renal prognosis in IgAN (HR=2.142, 95% CI 1.222 to 3.755), and the patients with high serum CysC levels had a lower renal survival rate (Log-rank χ²=47.970, P<0.001). In patients with serum CysC below 2.12 mg/L, a higher CysC level was associated with an increased risk of poor renal prognosis (β=3.487, 95% CI: 2.561-4.413, P<0.001), while above this level, the increase of the risk was not significant (β=0.676, 95% CI: -0.642-1.995, P=0.315). The nomogram model based on serum CysC and 3 other independent risk factors demonstrated good internal validity with a concordance index of 0.873 (95% CI: 0.839-0.907) and an AUC of 0.909 (95% CI: 0.873-0.945). CONCLUSIONS Serum CysC levels are associated with renal prognosis in IgAN patients, and high serum CysC an independent risk factor for poor renal prognosis.

A 69-year-old male patient with hypertension was treated with long-term oral administration of benidipine (8 mg, once daily) and losartan (100 mg, once daily). Due to the novel coronavirus infection, he took simnotrelvir/ritonavir (1.5 g of simnotrelvir, 0.1 g of ritonavir) by himself. After 10 hours, he experienced syncope and unconsciousness. His blood pressure was 70/40 mmHg, heart rate was 40 times/min, and consciousness recovered about 15 minutes later. Laboratory tests showed B-type natriuretic peptide 233 ng/L and blood creatinine 188 μmol/L. Electrocardiogram showed atrial fibrillation. Color Doppler ultrasound of the heart showed enlargement of both atria. Head CT showed suspicious low-density shadows in the brainstem and left frontal lobe. Immediate symptomatic treatments including fluid replacement, blood pressure increase with norepinephrine and dopamine, and anticoagulation with enoxaparin sodium were given. Antihypertensive drugs were stopped. After 1 day of symptomatic treatments, the patient′s blood pressure was 130-150/60-80 mmHg, norepinephrine and dopamine were discontinued. Three days after antihypertensive medication withdrawal, the patient′s blood pressure was 168/80 mmHg, and antihypertensive treatments were resumed (levoamlodipine 2.5 mg twice daily orally, sacubitril and valsartan 100 mg once daily orally). Eleven days later, the patient′s blood pressure was 114-135/74-78 mmHg, and blood creatinine was 96 μmol/L. It was considered that the hypotension and syncope were caused by the combination of simnotrelvir/ritonavir and benidipine, and the acute kidney injury was a prerenal injury caused by hypotension.

Objective To discuss the occurrence and characteristics of hemolytic anemia induced by piperacillin-tazobactam to provide a reference for clinically safe drug use.Methods PubMed,Web of Science,CNKI,WanFang Data,and VIP were electronically searched to collect case reports of hemolytic anemia induced by piperacillin-tazobactam from inception to December 2023,and statistical analysis was conducted on patients'basic information,drug use,hemolytic anemia,complications and outcomes.Results A total of 27 patients were included in 25 literature,including 14 males(51.85％)and 13 females(48.15％),aged from 4 days to 77 years old,and the time of first occurrence of hemolytic anemia ranged from 24 h to 18 days.The minimum hemoglobin level of hemolytic anemia was determined in 25 cases,ranging from 27 to 113 g·L-1,of which 14 cases were severe or extremely severe anemia(＜60 g·L-1).After drug withdrawal,antibiotic change,and symptomatic supportive treatment,the hemolytic anemia of 25 patients(92.59％)was improved or recovered.Among them,the anemia of 1 patient was improved after stopping piperacillin-tazobactam,but the patient died of multiple organ failure 15 days after drug withdrawal.The other two patients of anemia did not improve,and eventually discharged themselves or died clinically.Conclusion Hemolytic anemia is a rare adverse reaction of piperacillin-tazobactam,and the clinical symptoms have no obvious specificity.Hemoglobin levels should be monitored in clinical use,and once the abnormality is found,if it is clear that piperacillin-tazobactam is the suspicious drug,it should be promptly discontinued,and measures such as red blood cell transfusion should be taken if necessary.

OBJECTIVE To explore the regulatory effect and mechanism of salipurposide(Sali), salidroside(Sal) and isoflavone(Isota) from Rhodiola crenulata on senescence of human embryo lung fibroblast diploid cells(2BS). METHODS Observing and selecting young 28 generations 2BS(28PD) cells and senescence 50 generations 2BS(50PD) cells under the microscope. MTT method was used to determine the effects of 1.25, 2.5, 5.0, 7.5, 10.0 μmol·L-1 Sal, 2.5, 5.0, 7.5, 10.0, 12.5 μmol·L-1 Sali and Isota on the activity of 50PD cells. The 5.0 μmol·L-1 Sal, 10.0 μmol·L-1 Sali and Isota were used for the following experiments according to MTT results. 28PD and 50PD cells were divided into 28PD group, 50PD group, 50PD+Sali group, 50PD+Sal group, 50PD+Isota group, intervention for 24 h. The senescence associated-beta-galactosidae(SA-β-gal); malondialdehyde(MDA), reactive oxygen species(ROS), superoxide dismutase(SOD), glutathione peroxidase(GSH-PX) levels of cells were measured with the test kits; the mRNA expression of phosphatidylinositol-3-kinase(PI3K), protein kinase B(AKT), histone deacetylase 2(HDAC2) and the protein expression of PI3K, p-AKT, HDAC2 were detected by qRT-PCR and Western blotting respectively. RESULTS Compared with the 28PD group, 50PD group cell SA-β-gal expression was increased, the content of MDA and ROS were increased, the activity of SOD and GSH-PX were decreased, the mRNA expression of PI3K, AKT, HDAC2 and related protein expression were decreased(P<0.01). 50PD cells after intervention with Sali, Sal, and Isota, SA-β-gal expression were decreased(P<0.01), the content of MDA and ROS were decreased significantly(P<0.01), the activity of SOD and GSH-PX were increased(P<0.01), the mRNA expression of PI3K, AKT, HDAC2 and related protein expression were significantly increased(P<0.05 or P<0.01). CONCLUSION Sali, Sal and Isota from Rhodiola crenulata can reduce the expression of SA-β-gal of 50PD cells, its effect may related to the inhibition of oxidative stress and regulation of expression of PI3K/AKT-HDAC2 axis.

Objective:To study the best extraction technology of Qishan sugar-free granule. Methods:The effects of boiling dura-tion, boiling times and water addition ratio on the extraction process were investigated by using L9 (34 ) orthogonal design, and the con-tents of total flavonoids, baicalin and berberine were taken as the indices. Results:The optimum conditions were as follows:10-fold a-mount of water for 2 cycles, and every extracting time was 1 hour. Conclusion: The best extraction technology of Qishan sugar-free granule is obtained, and the process is stable and feasible, and the contents of active components are high.

Objective:To explore the role of clinical pharmacist in clinical medication by the pharmaceutical care for a patient with acute myocardial infarction complicated with gastrointestinal bleeding .Methods: Clinical pharmacist made an individualized pharma-ceutical care and provided medication guidance for the patient according to the characteristics of acute myocardial infarction and gastro -intestinal bleeding combined with the patient ’ s self factors .Results:The pharmaceutical care improved the compliance of the patient and the efficacy and safety of the drug treatment .Conclusion:The safety and rationality of drug use can be improved by the practice of clinical pharmacist in clinical medication .

Objective Pulmonary hypertension (PH) was an independent predictor of mortality and new onset cardiovascular events in maintenance hemodialysis (MHD) patients. Recent studies revealed PH complicated by pericardial effusion (PE) increased mortality in non-CKD (chronic kidney disease) population. No such evidence existed in MHD patients. Methods We enrolled 108 MHD patients with diagnosed PH by echocardiography (61 with PE) between 2009 and 2011. All patients had been followed-up for 2 years. The endpoints were all-cause mor-tality, cardiovascular (CV) mortality and new onset CV events. Results The mean age was (60.2 ± 14.1) years, 55.6% were males and SPAP was (53.0 ± 15.4) mmHg. 12.0% were severe PH and 1.9% were moderate-to-severe PE. The overall mortality, cardiovascular mortality and new on-set cardiovascular events in PE and non-PE group were 34.4% vs. 21.3%、 23.0% vs. 12.8% and 54.1% vs. 42.6%, respectively (P > 0.05). Kaplan-Meier curve showed there was no difference on all-cause mortality , CV mortality nor new-onset CV events between PE and non-PE group (Log Rank P > 0.05). Conclusions Compared with patients without PE, the presence of PE in patients with PH did not increase the mortality and worsen the cardiovascular outcome in MHD patients.

Objective To investigate the related risk factors of contrast-induced acute kidney injury (CI-AKI)after cerebrovascular intervention. Methods The clinical data of 5423 patients performed cerebrovascular angiography and intervention at the Departments of Neurology and Neurosurgery,Guangdong People′s Hospital from January 2005 to December 2013 were analyzed retrospectively. The patients who underwent cerebrovascular angiography and intervention were evaluated and screened. A clinical history database was established. All the selected patients used iodixanol,an isotonic contrast agent. The occurrence of CI-AKI was used as an endpoint. The patients were divided into either a CI-AKI group or a non CI-AKI group. A multivariate Logistic regression model was used to analyze the risk factors associated with the occurrence of CI-AKI. Results A total of 4164 patients were finally enrolled,including 137 had CI-AKI. The incidence of CI-AKI was 3. 3%. The results of multivariate Logistic regression showed that age >60 years (OR,1. 965,95%CI 1. 244-3. 136),baseline estimated glomerular filtration rate (eGFR)<60mL/(min·1. 73 m2)(OR,4. 163,95%CI 2. 422-5. 873),diabetes (OR,3. 140,95%CI 1. 983-3. 902),and anemia (OR,1. 524,95%CI 1. 226 -3. 253)were the influencing factors for occurring CI-AKI after cerebrovascular angiography and intervention. Conclusion Chronic kidney disease (eGFR<60 mL/[min·1. 73 m2 ]),diabetes,anemia,and old age (age >60 years)are the independent risk factors for occurring CI-AKI after cerebrovascular angiography and intervention.
