1.Correlation between fundus blood flow parameters and carotid artery ultrasound blood flow parameters in patients with hypertensive retinopathy

AIM: To investigate the correlation between fundus blood flow parameters and carotid artery ultrasound blood flow parameters in patients with hypertensive retinopathy(HRP).METHODS: A total of 50 patients(22 left eyes and 28 right eyes)with HRP admitted to our hospital from June 2021 to June 2023 were retrospectively included as the experimental group, and 50 healthy physical examination subjects(22 left eyes and 28 right eyes)during the same period were included as the healthy group. Pearson correlation was used to analyze the correlation between fundus blood flow parameters and carotid artery ultrasound blood flow parameters.RESULTS: The AUC values of fundus blood flow parameters and carotid artery ultrasound blood flow parameters and their combined diagnosis of HRP were 0.853, 0.844 and 0.935, respectively. Pearson correlation analysis showed that carotid systolic peak blood flow velocity was negatively correlated with foveal avascular zone(FAZ)area, FAZ circumference and non-circularity index, and positively correlated with macular vascular density(all P<0.05). The end-diastolic blood flow velocity was positively correlated with FAZ area and macular vascular density(all P<0.05). The internal carotid artery resistance index was positively correlated with FAZ area(P<0.05).CONCLUSION: The combination of fundus blood flow parameters and carotid artery ultrasound blood flow parameters in the diagnosis of HRP has good application value in the diagnosis of HRP.
2.Iodine nutritional status of children aged 8-10 in Wuhan from 2019 to 2023

Objective:
To explore the iodine nutrition status of children in Wuhan from 2019 to 2023, and to evaluate the effect of iodine deficiency disorders control in focus groups in Wuhan, so as to provide a basis for consolidating elimination of iodine deficiency disorders.
Methods:
A total of 13 000 non boarding primary school students aged 8-10 were selected from 13 districts of Wuhan by stratified random sampling method.Household salt samples were collected to measure salt iodine content, random urine samples were analyzed for urinary iodine concentration. And B ultrasound was used to measure thyroid volume in students. The median of salt iodine, coverage rate of iodized salt, consumption rate of qualified iodized salt, median of urinary iodine, and the goiter rate were calculated. And Mann-Whitney U- test, Kruskal-Wallis H- test and Chi-square test were applied to compare between groups. Chi-square trend test was used to analyze the changing trends of coverage rate of iodized salt, consumption rate of qualified iodized salt and goiter rate among children in Wuhan.
Results:
The median of iodine content of children s household salt was 23.8 (21.7, 26.1) mg/kg, and the coverage rate of iodized salt was 98.7%, and the consumption rate of qualified iodized salt was 94.5 %. The consumption rates of qualified iodized salt showed an overall upward trend from 2019 to 2023 ( χ 2 trend =5.57, P <0.05). The median of urinary iodine of children was 220.1 (136.7, 326.0) μg/L,and boys had higher median of urinary iodine than girls [228.3(143.2, 336.0),210.2(129.1, 315.7) μg/L] ( Z =6.60, P <0.01). The median of urinary iodine of children in suburbs was higher than those in urban areas [236.3 (150.7, 342.2) , 207.1 (124.5, 309.8) μg/L]( Z =11.00, P <0.01). A total of 4 600 children were examined for thyroid volume, and the range of goiter rates were 1.1% to 3.4%, with an average goiter rate of 2.5%, which showed an overall downward trend from 2019 to 2023 ( χ 2 trend =5.11, P <0.05).
Conclusions
The iodine nutrition is sufficient and iodine nutrition status is good among children in Wuhan. It should continue to carry out monitoring and evaluation of children s iodine nutrition, guide the public to supplement iodine scientifically,so as to maintain the appropriate level of iodine for children.
3.A survey of adult thyroid volume and related influencing factors in Wuhan

Objective:To learn about the thyroid volume and goiter status of adults in Wuhan, analyze related influencing factors, and provide a basis for revising the diagnostic criteria for adult goiter.Methods:From December 2019 to November 2021, a multi-stage cluster random sampling method was used in 13 districts of Wuhan. Two communities or townships were selected from each district, 100 permanent residents over 16 years old were selected from each community or township according to the inclusion and exclusion criteria for general information collection, thyroid ultrasound examination, urinary iodine detection and physical examination were performed, and the influencing factors of thyroid volume were analyzed.Results:A total of 2 578 adults aged 16 and older in Wuhan were selected, including 1 168 males and 1 410 females, with an average age of (41.79 ± 13.01) years old. The median thyroid volume (interquartile range) in adults was 6.36 (4.88, 8.11) ml, the rate of goiter was 0.70% (18/2 578), and the detection rate of thyroid nodules was 35.49% (915/2 578). The median urinary iodine (interquartile range) was 185.32 (107.93, 277.08) μg/L. According to Spearman correlation analysis, the thyroid volume of adults in Wuhan was positively correlated with their height, weight, body mass index (BMI), and body surface area (BSA, r = 0.12, 0.22, 0.21, 0.20, P < 0.001). Through multiple linear regression analysis, the regression equation was obtained: thyroid volume = 7.72 + 1.19 × thyroid nodules - 1.04 × gender + 0.15 × age group. Patients with thyroid nodules had a thyroid volume greater than those without, males greater than females, and the elderly group greater than the younger group. Conclusion:The goiter rate in Wuhan is relatively low, and factors such as gender, age, presence of thyroid nodules, height, weight, BMI, BSA and other body measurement are the influencing factors of thyroid volume.
4.Exploration on the Chinese Medical Master Wang Qingguo's Medication Law in Treating Insomnia Based on Data Mining

Objective To explore the medication law of Professor Wang Qingguo for the treatment of insomnia based on data mining technology.Methods Paper-based prescriptions for Professor Wang Qingguo's treatment of insomnia from December 2016 to August 2023 were collected and a database was constructed.The screened prescriptions were subjected to data mining such as frequency statistics,association rule analysis,and clustering analysis.Results Totally 399 effective outpatient prescriptions were screened-out,involving 276 kinds of Chinese materia medica,with a total frequency of 6738 times.There were 38 Chinese materia medica with a frequency of use≥50.The properties were mainly warm,cold and mild,the tastes were mainly sweet,bitter and pungent,and mainly belong to the lung,spleen and heart meridians.Association rule analysis showed that the most commonly used drug combinations were"Fructus Tritici Levis-Scutellariae Radix-Bupleuri Radix","Pericarpium Citri Reticulatae-Scutellariae Radix-Pinelliae Rhizoma",etc.Association rule network analysis obtained a core prescription containing 12 kinds of Chinese materia medica for insomnia.Four new drug combinations were obtained by clustering analysis.Conclusion Professor Wang's treatment of insomnia has the characteristics of following the original meaning of Huang Di Nei Jing and Nan Jing,making good use of the classical prescription,applying combined prescription,flexible use of prescription,combining ancient and modern prescriptions,using specific medicine.It deeply reflects the academic view of"Tongping Zhihe".
5.Discussion on syndrome differentiation and treatment of acupuncture-moxibustion encephalopathy based on Shi's Xingnao Kaiqiao theory

By exploring the core ideas of the "Xingnao Kaiqiao" theory, analyzing the relationship between the "Xingnao Kaiqiao" theory and the TCM encephalopathy, and exploring the necessity of acupuncture for the treatment of TCM encephalopathy, in order to clarify how to diagnose and treat brain diseases in acupuncture and moxibustion. The "Xingnao Kaiqiao" is the core of the theory of the "Tiaoshen", the acupuncture of "Xingnao Kaiqiao" was based on the principle of "Xingnao","Xingshen" and "Tiaoshen", the idea of the "Tiaoshen" was inseparable from the diagnosis and treatment of the TCM encephalopathy. Based on the special features of acupuncture therapy and complexity and particularity of TCM encephalopathy, acupuncture and moxibustion treatment of encephalopathy was supposed to have their own unique system of syndrome differentiation and treatment. During the practice of acupuncture, the diagnosis and treatment of TCM encephalopathy should be based on "brain", "Xingnao Tiaoshen" as a general rule, adding and subtracting from the main points of the "Xingnao Kaiqiao", and the different brain diseases are matched with appropriate treatments.
6.A case of febrile neutropenia caused by meropenem in an infant after congenital heart surgery and data mining analysis

OBJECTIVE To provide a reference for the safe clinical use of meropenem. METHODS The relationship of adverse reactions was analyzed based on a case of febrile neutropenia after using meropenem in an infant after congenital heart surgery by clinical pharmacists. By retrieving Chinese and English databases, case reports of adverse reactions induced by meropenem in blood system were summarized and analyzed. Relevant risk signals of adverse events were excavated from the FDA Adverse Event Reporting System, and the characteristics of meropenem-induced hematologic adverse reactions were summarized. RESULTS This case was assessed as “very likely” according to the Naranjo’s assessment scale by clinical pharmacists. A total of 16 patients were included in the literature analysis, of which one was febrile neutropenia, 13 patients had grade 4 serious adverse reactions that were life-threatening or required urgent treatment. All adverse reactions improved after drug withdrawal. Data mining revealed that the median occurrence time of hematologic adverse events induced by meropenem was 6 d; neonatal thrombocytopenia, intravascular hemolysis and neutrophil count decrease were the top 3 signals of risk intensity for adverse events. CONCLUSIONS Early use of meropenem may induce serious life-threatening hematological adverse reactions, especially in infants and elderly, those who use higher doses of meropenem, those who have previous antibiotic allergies, those with complex underlying diseases, those who use multiple antibiotics in combination, and those who experience high fever and bleeding after medication.Close monitoring should be provided in clinical practice.
7.Analysis of the current situation and related factors of the knowledge, behavior and educational attitude of sexual abuse prevention among primary school students in Luzhou City

Objective:
To assess the current situation and related factors of the knowledge, attitude and behavior toward sexual abuse prevention among primary school students in Luzhou City, Sichuan Province, so as to provide a reference basis for the development of sexual abuse prevention education for primary school students.
Methods:
A total of 4 563 primary school students in Luzhou City were sampled from December 2023 to January 2024 by stratified cluster sampling, and self administered questionnaires were conducted. The binary Logistic regression analysis was used to analyze related factors of the knowledge, attitude and behavior toward sexual abuse prevention.
Results:
The qualified rates of knowledge, behavior and attitude among primary school students towards sexual abuse prevention in Luzhou were 76.2%, 21.3%, and 90.2 %, respectively. The binary Logistic regression showed that males ( OR=0.66, 95%CI =0.56-0.78), attending rural schools ( OR=0.77, 95%CI =0.64-0.92), left behind status ( OR=0.66, 95%CI =0.53-0.81), and household registration in Lu County ( OR=0.57, 95%CI =0.46-0.70) and Gulin County ( OR=0.70, 95%CI =0.57-0.86) had lower qualified rates of the knowledge of sexual abuse prevention among primary school students ( P < 0.05 ); while qualified knowledge of sexual abuse prevention was positively associated with grade ( OR =3.05-18.81); attending rural schools ( OR=0.82, 95%CI =0.70-0.95) and not having received education on prevention of sexual abuse ( OR=0.55, 95%CI = 0.47-0.64) had lower qualified rates of the behavior of sexual abuse prevention among primary school students ( P <0.05); household registration in Lu County ( OR=0.73, 95%CI =0.58-0.92) and not having received education on prevention of sexual abuse ( OR=0.33, 95%CI =0.26-0.40) had lower qualified rates of the educational attitudes of sexual abuse prevention among primary school students ( P <0.05).
Conclusions
The qualified rates of knowledge and behavior of sexual abuse prevention among primary school students in Luzhou City needs to be improved. Health education regarding sexual abuse prevention might help enhance primary school students awareness and skills of sexual abuse prevention.
8.Analysis of a case of imported malignant malaria death

To analyze the cause of death and artemisinin resistance of an imported malignant malaria case in Wuhan City in 2024, with the aim of accumulating experience in the treatment of severe malaria cases and reduce or avoid the occurrence of severe cases and deaths. Epidemiological data, case investigation forms, clinical diagnosis and treatment data, and laboratory test results of patients were collected and analyzed. The patient, who had worked as a bricklayer in the Democratic Republic of Congo for about 9 months, became ill on the eighth day after returning to China, with no history of malaria. The initial diagnosis of the case was at a private clinic, and there was a misdiagnosis. After 6 days, the condition became severe and the patient was admitted to the hospital.Approximately 8 hours after admission, the patient was diagnosed with malignant malaria and immediately received anti-malarial treatment and symptomatic supportive treatment. Treatment with artesunate injection for 12, 24, and 48 hours showed the density of malaria parasites densities of 3.2×105/µL, 3.1×105/µL, and 3.1×105/µL, respectively. There was no significant order of magnitude change in the density of malaria parasites compared to before medication, and the count of malaria parasites slightly increased, suggesting the possibility of artemisinin resistance. The patient died of multiple organ failure at 18:30 on January 11, three days after diagnosis. The initial diagnosis occurred in a private clinic where the patient was misdiagnosed. The interval between the onset of the disease and diagnosis was more than 5 d, which resulted in the loss of optimal time for the patient's treatment. At the same time, the malaria parasite-infected by the patient may have artemisinin resistance, which ultimately led to the occurrence of the patient's death.
9.The survival, complications and prognosis of the extremely preterm infants

Objective:To study the complications and prognosis of extremely premature infants(EPIs) with gestational age (GA) <28 w.Methods:From January 2016 to December 2020, EPIs with GA <28 w admitted to NICU of our hospital were retrospectively studied. Clinical data of the infants and their mothers during pregnancy were reviewed. According to the prognosis, the infants were assigned into the survival group, the death group and the withdrawal group. According to GA, the infants were assigned into <26 w goup, 26~26 +6 w group, ≥27 w group. According to birth weight (BW), the infants were assigned in to ≤750 g group, 750~999 g group and ≥1 000 g group. SPSS 26.0 was used for data analysis. Results:A total of 265 EPIs were included, 122 (46.0%) in the survival group, 47 (17.7%) in the death group and 96 (36.2%) in the withdrawal group. GA and BW of the survival group were significantly higher than the death group and the withdrawal group ( P<0.05). The incidences of tracheal intubation (92.2%) and pulmonary hemorrhage (42.2%) in the death group were the highest among the three groups. The survival group had the highest application of prenatal glucocorticoids (80.3%) and pulmonary surfactants (99.2%) ( P<0.05). In the survival group, the top 3 common complications were bronchopulmonary dysplasia (BPD) (68.0%), pulmonary infections (65.6%) and neonatal sepsis (34.4%). The survival rate increased with GA and BW. Conclusions:The survival rate of EPIs is closely correlated with GA and BW. EPIs with pulmonary hemorrhage and tracheal intubation have poor prognosis. Prenatal glucocorticoids and pulmonary surfactant may improve clinical outcome. BPD and pulmonary infections are common complications of surviving EPIs.
10.A preliminary study on reducing the formation of intra-abdominal hernia and postoperative intestinal obstruction in laparoscopic Bricker operation

【Objective】 To reduce the incidence of postoperative intestinal obstruction, we tried to improve surgical techniques by closing the cavity formed during radical cystectomy + ileal passage (Bricker) via laparoscopy to prevent the formation of abdominal hernia. 【Methods】 During Oct.2018 and Feb.2022, 41 patients were involved (conventional group). After standard laparoscopic radical cystectomy + pelvic lymphadenectomy, the ileum channel was established. The right medial retroperitoneum was sutured to cover the mesothelium and end of the ileum channel under open operation or endoscope. The space between the ureter and mesothelium of the ileum channel was sealed, and the end of the ileum channel and both ureters were externalized. During Feb.2022 and Dec.2022, 15 patients were involved (modified group). The right inner and outer lateral peritoneums below the ileal conduit were sutured to "bottom out" the gap between the ileal conduit and the right abdominal wall in addition to standard procedures. The recovery of intestinal function and incidence of bowel obstruction were compared between the two groups. 【Results】 In the conventional group, the intestinal function recovered within 2 to 6 days after surgery, with a median ventilation time of 3 days. Intestinal obstruction occurred in 3 patients, 2 of whom improved after conservative treatment while 1 underwent surgical exploration after ineffective conservative therapy. There were no significant differences in the time of discharge and ventilation between the two groups, but no intestinal obstruction occurred in the modified group. 【Conclusion】 Peritoneal externalization at the end of ileal passage can reduce the incidence of intra-abdominal hernia and postoperative intestinal obstruction, which is worthy of clinical application.
