1.Expression characteristics of FOSB in kidney tissue from IgA nephropathy and other common kidney diseases

Objective:To evaluate the expression characteristics of the FBJ murine osteosarcoma viral oncogene homolog B(FOSB)gene in immunoglobulin A nephropathy(IgAN)and common chronic kidney diseases(CKDs),and to determine its value as a potential key candidate gene or biomarker.Methods:The RNA sequencing datasets for IgAN,diabetic kidney disease(DKD),membranous nephropathy(MN),and minimal change disease(MCD)glomerular samples were downloaded from the Gene Expression Omnibus(GEO)database.The feature genes for CKD were identified using machine learning methods including least absolute shrinkage and selection operator(LASSO)regression,random forest(RF),and support vector machine(SVM).Gene Ontology(GO)functional enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG)signaling pathway enrichment analyses were performed on the identified IgAN feature genes.The"pROC"package was used to plot the receiver operating characteristic(ROC)curves to evaluate the diagnostic efficacy of IgAN feature genes.The gene with the highest diagnostic value was selected for Gene Set Enrichment Analysis(GSEA)and correlation analysis with core immune cells in IgAN.Clinical correlation analysis between the FOSB expression level in kidney tissue and renal function was performed using the Nephroseq v5 platform.Kidney tissue samples were collected from 5 cases of IgAN patients,DKD patients,MCD patients,and MN patients,respectively,along with 5 samples of adjacent normal kidney tissues(control group).Immunohistochemistry staining method was used to detect the expression levels of FOSB protein in tissue samples in various groups.Results:A total of 110 differentially expressed genes(DEGs)were identified in IgAN glomeruli,among which FOSB,NR4A2,and DUSP1 were identified as the feature genes.Compared with control group,the expression level of FOSB mRNA in IgAN group was significantly decreased(P<0.05).The GO fuctional enrichment analysis results revealed that these IgAN feature genes were primarily enriched in biological processes related to dopamine biosynthesis,midbrain dopaminergic neuron differentiation,peptidyl-serine/threonine dephosphorylation,and response to corticosterone.The KEGG signaling pathway enrichment analysis results showed that the DEGs were significantly enriched in cocaine addiction,amphetamine addiction,interleukin 17(IL-17)signaling pathway,aldosterone synthesis and secretion,and serotonergic synapse.The ROC curve analysis results demonstrated that FOSB showed high diagnostic accuracy for IgAN.GSEA analysis revealed that arginine and proline metabolism,butyrate metabolism,erythroblastic leukemia viral oncogene homolog(ERBB)signaling pathway,mitogen-activated protein kinase(MAPK)signaling pathway,and fructose and mannose metabolism pathways were enriched in FOSB high-expression group,while allograft rejection,extracellular matrix receptor interaction,and type 1 diabetes pathways were significantly enriched in FOSB low-expression group.The immune cell infiltration analysis results identified natural killer cells,neutrophils,and M1 macrophages as core immune cells in IgAN,and the expression of FOSB gene was positively correlated with neutrophil infiltration(r=0.42,P<0.05).The immunohistochemistry analysis results demonstrated that compared with control group,the expression level of FOSB protein in glomeruli of the patients in IgAN,DKD,MN,and MCD groups were significantly decreased(P<0.05).Conclusion:The expressions of FOSB gene in the glomeruli tissue of IgAN,DKD,MN,and MCD patients ware decreased,suggesting FOSB may represent a potential biomarker for IgAN.
2.CiteSpace-based analysis of research trends and hotspots in the field of microbiomes and microbes of dental caries

Objective:
To analyze the trends and hotspots in research related to microbiomes and microbes of dental caries; in addition, the study seeks to provide a reference for caries research.
Methods:
We searched the Web of Science Core Collection (WOSCC) to extract relevant literature in the field of microbiomes and microbes of dental caries published from 2014 to 2023. We used bibliometric visualization evaluation methods such as CiteSpace to conduct visualized analysis of factors that include the number of publications, journals, countries, authors, institutions, co-cited references, and keywords.
Results:
A total of 3 192 references were extracted, including 2 664 articles and 528 reviews. The number of annual publications is increasing. The United States and China lead the number of publications, with the United States demonstrating a greater capacity for international collaboration. The top 10 journals in percentage of literature are mainly in the field of dentistry followed by the field of microbiology. The author cooperation networks with the highest number of publications include the network led by Zhou Xuedong of Sichuan University, and the network led by Xu Hockin H.K and Weir Michael D of the University of Maryland, Baltimore. The research on microbiomes and microbes of dental caries focuses on the cariogenic toxicity and interaction of microorganisms, oral microbiomes, and the relationship between dental caries and systemic diseases. The articles with high citation frequency mainly involve topics such as dental caries, oral biofilm, oral microbiota, and Streptococcus mutans. Keyword research showed that “dental caries,” “Streptococcus mutans,” “bacteria,” “dental plaque,” and “antibacterial activity” have been the primary focus of research in the last decade. The number of keywords, such as “health” and “oral health,” is on the rise. The latest emergence of “gut microbiome/microbiota” suggests that the oral gut microbiome axis is at the forefront of research in this field, and researchers’ focus is gradually shifting toward the connection between dental caries and systemic diseases.
Conclusion
Over the last decade, the number of publications in the field microbiomes and microbes of dental caries has increased annually. This research trend will be the multi-omics of the overall oral microbiome, and new research methods and techniques will contribute to the field of cariology.
3.Research on clinical application of urine sediment score in the diagnosis of acute kidney injury

Objective:To evaluate the clinical application of urine sediment score (USS) in early diagnosis, etiological differentiation, staging and prognosis of acute kidney injury (AKI), and to investigate the diagnostic efficacy of independent USS and its combination with blood urea nitrogen(Bun) serum creatinine(sCr) and uric acid(UA) in AKI.Methods:From August 23 to September 28, 2023, 9 020 morning urine samples of hospitalized patients in the First Hospital of Jilin University were detected by Sysmex UF5000.A total of 3 226 ssamples with small and round cell (SRC) > 1/μl and/or CAST>1/μl were screened for microscopic examination, and 404 cases with positive renal tubular epithelial cells and/or cast were enrolled in this study. There were 218 males and 186 females, aged 59.5 (49.0, 71.0) years. The 404 cases were divided into the USS AKI group (345 cases) and the USS non-AKI group (59 cases) according to the USS results based on the microscopic findings. According to Kidney Disease: Improving Global Outcomes (KDIGO) criteria, they were divided into KDIGO criteria AKI group (63 cases) and KDIGO criteria non-AKI group (341 cases), and the AKI group was divided into renal AKI group (33 cases) and non-renal AKI group (30 cases). According to the clinical diagnosis recorded in the medical records, they were divided into clinically diagnosed AKI group (29 cases) and clinically diagnosed non-AKI group (375 cases).The χ 2 test or Fisher exact test was used to compare USS in different AKI causes and stages. Logistic regression was used to calculate the odds ratio of renal AKI and stage 3 AKI. The area under the receiver operating characteristic curve was used to evaluate the sensitivity and specificity of USS, sCr, UA and Bun alone and in combination in the diagnosis of AKI, and the best cut-off value, sensitivity and specificity in the diagnosis of AKI were calculated. P < 0.05 was considered statistically significant. Results:The USS was used to identify the etiology of KDIGO standard AKI group,and there were significant differences in USS between renal AKI group and non-renal AKI group (χ 2=11.070, P<0.001). Compared to USS=1, the odds ratio of renal AKI was 8.125 when USS≥2 (95% CI 2.208—29.901). There was a statistically significant difference in the comparison of USS between groups in each stage of the AKI staging study based on USS (χ 2=15.724, P<0.05). Compared to USS=1, the odds ratio of stage 3 AKI was 9.714 when USS≥2 (95% CI 1.145-82.390). The AUC of independent USS in the diagnosis of AKI was 0.687 (95% CI 0.618-0.757, P<0.001), the specificity was 65.7% and the sensitivity was 61.9%. The AUC of USS combined with Bun, sCr, UA in the diagnosis of AKI was 0.794 (95% CI 0.608-0.980, P<0.05), the specificity was 82.4%, and the sensitivity was 88.9%. Conclusions:There wasan increased likelihood of renal AKI or stage 3 AKI while USS≥2,and whose combination with Bun, sCr and UA will improve the diagnostic efficiency of AKI.
4.Analysis on Medication Rule of Ruan Yan in the Treatment of Children with Allergic Rhinitis Based on Data Mining

Objective To analyze and discuss the medication rule of professor Ruan Yan in the treatment of children with allergic rhinitis by using data mining method,and to provide reference for the clinical research and patented drugs development for the treatment of children with allergic rhinitis.Methods The outpatient medical records of professor Ruan Yan for the treatment of children with allergic rhinitis were collected.Microsoft Excel 2010 software was used for frequency statistics.SPSS Clementine 12.0 software was used for association rule analysis,cluster analysis and factor analysis to obtain the data.The frequency of use of various drugs and the association rules between drugs were obtained.Then the medication rules in professor Ruan Yan's prescription were analyzed.Results A total of 308 Chinese medicine compounds were included,involving 80 kinds of Chinese medicines,among which relieving drugs and qi-invigorating herbs were high-frequently used.The distribution of traditional Chinese medicine syndrome types was mainly characterized by lung-qi deficiency-cold syndrome and lung-spleen qi deficiency syndrome.The medicinal properties were mainly spicy,warm and sweet,and most of them belonged to the lung,spleen and stomach meridians.Five core prescriptions were extracted by factor analysis.Four drug combinations were obtained by systematic cluster analysis.Conclusion Ventilating lung and opening the orifices,expelling wind and removing cold,strengthening the spleen and replenishing qi are basic therapeutic principles for professor Ruan Yan in the treatment of children with allergic rhinitis.The treatment mainly focused on dispelling evil,ventilating lung and opening the orifices,expelling wind and removing cold during the acute stage of allergic rhinitis.In the remission period,according to the principle of"treating disease must be based on its origin",the treatment should enhance children's physical fitness,tonify lung and strengthen spleen,thereby reducing recurrence.
5.Evaluation on Mechanical Damage of vWF Induced by Shear Stress in BPX-80 Centrifugal Blood Pump

Objective To evaluate the damage of von Willebrand factor (vWF) induced by shear stress in BPX-80 centrifugal blood pump, and determine whether it can be used as a reference pump for vWF damage research. Methods An in vitro hemolysis test platform was established according to the ASTM standards. The BPX-80 centrifugal blood pump was tested for 8 hours by using fresh porcine blood. The hemolysis level and vWF damage of hourly blood samples were then evaluated and compared with the static control group. ResultsThe hemolysis index of BPX-80 was stable and low during the test; vWF polymer with high molecular weight had a small amount of degradation, and showed no significant difference compared with the static control group; there was no significant change in the concentration of vWF antigen, which was basically consistent with the trend of the static control group. Conclusions BPX-80 centrifugal blood pump has good blood compatibility and can be used as the reference pump for vWF damage and hemolysis evaluation, thereby providing guidance for the design and optimization of new blood pumps.
6.Clinical efficacy of Nd∶YAP laser in the treatment of acute wisdom tooth pericoronitis

Objective To study the clinical efficacy of Nd∶YAP laser in the treatment of acute localized pericardial periodontitis. Methods A total of 120 patients with acute localized pericardial periodontitis were randomly divided into 3 groups, including Nd∶YAP laser group, minocycline group and control group (iodine glycerol group). All patients were underwent pretreatment of intracrevicular washing with 3.0%hydrogen peroxide and normal saline alternately. After the pretreatment, the patients in the Nd∶YAP laser group were given 3 min local Nb:YAP laser irradiation (1 time/d for 3 times), in the minocycline group were injected with minocycline hydrochloride ointment (1 time), and in the control group were treated with 2%iodine glycerol (1 time/d for 4 times) in gingival sulcus. One day after the treatment, the gingival index (GI), pain visual score (VAS), and opening degree of all patients were recorded, and the therapeutic effect was observed 5 days after the treatment. Results Compared with the minocycline group and the control group (iodine glycerol group), the GI value and VAS value of the Nd∶YAP laser group decreased and the openmouthed size increased (all P<0.05). At 5 days after the treatment, the patients in the Nd∶YAP laser group and the minocycline group had significant improvement in local gingival sulcusinflammation, and the total effective rate was 90%and 85%, which was significantly better than the control group (all P<0.05) and no significant difference in efficacy between the two groups (P>0.05). Conclusions The Nd∶YAP laser treatment inacute wisdom tooth pericoronitis can significantly reduce pain and improve openmouthed size, and has a good clinical efficacy.

Objective To investigate the effects of 12-lipoxygenase (12-LO) and angiotensin Ⅱ (Ang Ⅱ) on the CIP/KIP family of cyclin-dependent kinase inhibitors (CKIs) p21,p27 and p57 related to cell hypertrophy.Methods Mesangial cells were treated with high glucose for 24 hours and 48 hours respectively.12(S)-hydroxyeicosatetraenoic acid [12(S)-HETE] and Ang Ⅱ were infused to rats by osmotic mini-pump for 1 week and 2 weeks respectively.Rats fed high fat diet were received low dose streptozotocin (STZ) to make type 2 diabetes (DN).The rats were divided into normal control group,DN group,DN+Ang Ⅱ type 1 receptor blocker (ARB) group or 12-LO inhibitor (CDC) group.DN+ARB rats were treated by losartan for 6 weeks,and DN+CDC rats were treated for 8 weeks.Urine albumin and protein expressions of p21,p27 and p57 were detected by ELISA and Western blotting respectively.Glomeruli injury and expressions of p21 and p27 were detected by PAS staining and immunohistochemistry respectively.Results High glucose increased p21 and p27 protein expression in mesangial cells significantly compared with the relative control (all P < 0.05),but had no effect on p57.Ang Ⅱ increased p27 protein expression in gloneruli significantly (P < 0.05),but had no effect on p21 and p57 protein expression.12(S)-HETE increased both p21 and p27 protein expression in glomeruli significantly (all P < 0.05),but had no effect on p57 protein expression.Blood glucose,kidney/body weight,urinary protein,and glomerular p21 and p27 protein expressions were increased in DN group (all P < 0.05) compared with those in control group,with little change of p57 protein expression (P < 0.05).Moreover,glomerular hypertrophy and extra cellular matrix accumulation were observed in DN group.However,urine protein,kidney/body weight,renal injury,but not blood glucose,were decreased in DN+ARB group and DN+CDC group compared with DN group respectively (P< 0.05).Further DN+CDC rats had decreased both p21 and p27 protein expressions in glomeruli,but DN+ ARB rats only had decreased p27 protein expression (all P < 0.05).Conclusions 12-LO may induce both p21 and p27 protein expression in DN glomeruli,but Ang Ⅱ may induce only p27 expression.
9.Infection and drug resistance of staphylococcus aureus in patients with hospital-acquired pneumonia

Objective To investigate the infection distribution of the hospital-acquired pneumonia (HAP) in patients with Staphylococcus aureus (S.aureus) and its drug resistance to those commonly-used antimicrobial agents, so as to provide evidence for rational use of antibacterial drugs clinically .Methods Retrospective analysis of the infection status and drug resistance of Staphylococ-cus aureus in 336 cases of HAP treated in the hospital from January 2011 to December 2015 was made in the study .Results (1) Nine-ty-three strains of Staphylococcus aureus were isolated from the 336 cases of HAP (27.7%), including 78 strains of methicillin-resist-ant Staphylococcus aureus (MRSA), accounting for 83.9%.(2) The infection rates of Staphylococcus aureus for the HAP patients in the five years were respectively 29.6%, 26.7%, 26.9%, 25.3%and 30.1%.There were no statistical differences in infection rates , when comparisons were made between the years (P>0.05).(3) Bacterial resistance analysis indicated that the patients with HAP-MRSA infection were completely sensitive to vancomycin and linezolid ( by 100%) , fairly sensitive to compound sulfamethoxazole and rifampicin ( by about 70%) and other antibiotics , such as beta lactams , macrolides and fluoroquinolones , was poorly sensitive ( by as low as less than 30%).Conclusion In HAP patients, Staphylococcus aureus was the main Gram positive pathogen , with MRSA infec-tion being the main case .Vancomycin and linezolid should be the first choice of drugs in clinical application of antibiotics for the treat -ment of HAP-MRSA infection.
10.Clinical effects of gamma knife combined with pemetrexed plus carboplatin in concurrent or sequential chemoradiotherapy of locally advanced non-small cell lung cancer

Objective To observe the clinical effects and adverse reactions of gamma knife combined with pemetrexed plus carboplatin in concurrent or sequential chemoradiotherapy of locally advanced non-small cell lung cancer( LANSCLC) .Methods A to-tal of 125 cases of LANSCLC were randomly divided into 3 groups, in accordance with different therapeutic methods.The concurrent group ( or group A, 30 cases) received body gamma knife combined with pemetrexed plus carboplatin.The sequential group ( or group B, 31 cases) was given 2 weeks of chemotherapy with pemetrexed and carboplatin, then sequential body gamma knife radiotherapy and finally underwent two cycles of chemotherapy with the same regimen.The radiotherapy group ( or group C, 64 cases) was given just body gamma knife radiotherapy.Clinical therapeutic effects and adverse reactions were evaluated.Results The short-term effects for group A, group B and group C were respectively 83.33%, 77.42%and 73.44%, and no statistical significance could be seen when comparisons were made between them(P<0.05).The 1-2-year survival rate and the median survival time for group A were respectively 73.33%, 43.33%and 17.5 months, those for group B were respectively 64.52%, 35.48%and 15.4 months, and those for group C were respectively 46.88%, 23.44%and 10.6 months.Analyses indicated that group A was superior to group B, and group B was su-perior to group C, with the former having a better survival rate, and no statistical significance could be noticed when comparisons were made between them(P>0.05).However, there was statistical significance when comparisons were made between group A and group C (P<0.05).With regard to toxic reactions, all the adverse reactions were from gradeⅠ-Ⅲ, with bone marrow suppression and gastro-intestinal reactions as the main manifestations.GradeⅣadverse reaction never occurred.There was no statistical significance in the in-cidence rates of radiation-induced esophagiti and pneumonitis, when comparisons were made between the 3 groups(P>0.05).With re-gard to the incidence rates of bone marrow suppression and adverse reactions of the digest system, there was no statistical significance, when comparisons were made between group A and group B(P>0.05).However, statistical significance could be noted, when compari-sons were made between group A/B and group C(P>0.05).Conclusion Body gamma knife combined with pemetrexed and carboplatin in concurrent chemoradiotherapy was more superior to body gamma knife alone, furthermore it displayed better safety and higher tolerance.
