1.Dosimetric and execution efficiency comparison of flattening filter-free VMAT plans for post-mastectomy radiotherapy in breast cancer based on halcyon and VitalBeam

Objective To compare the dosimetric differences, plan complexity, verification pass rate, and execution efficiency of volumetric modulated arc therapy (VMAT) plans designed for post-mastectomy radiotherapy of breast cancer using Halcyon 2.0 and VitalBeam accelerators in flattening filter-free mode. Methods A retrospective analysis was conducted on imaging data from 87 female breast cancer patients aged 29 to 75 (49.9 ± 9.9) years who underwent modified radical mastectomy followed by radiotherapy. VMAT plans were designed using both VitalBeam and Halcyon accelerators in flattening filter-free mode. The dosimetric parameters, plan complexity, verification pass rate, and execution efficiency were compared between the plans designed for both accelerators, with a significance level of P<0.05. Results In terms of target volume parameters, Halcyon 2.0 plans demonstrated superior homogeneity index and conformity index. For organs at risk, the Halcyon 2.0 plans showed lower doses for the lungs (D20% and Dmean) and heart (Dmean) compared to the VitalBeam plans (all P<0.05). Regarding plan complexity, Halcyon 2.0 plans exhibited better median average aperture area variation (0.35 ± 0.02), smaller aperture score, and larger median plan aperture area than VitalBeam plans (all P<0.05). However, VitalBeam plans had a superior median modulation complexity score (0.28 ± 0.01) compared to Halcyon 2.0 plans (P<0.05). Patient plans were assessed using the ArcCHECK phantom, revealing higher pass rates and significantly shorter execution time for Halcyon plans compared to VitalBeam plans (P<0.05). Conclusion Both Halcyon 2.0 and VitalBeam accelerators can provide high-quality VMAT plans for postoperative radiotherapy of breast cancer, with excellent verification pass rates ensuring treatment accuracy and efficacy. However, Halcyon 2.0 offers lower plan complexity, simplifies the treatment planning process, and reduces execution time, thereby achieving higher treatment efficiency and demonstrating greater advantages in clinical applications.
2.Study on the role of peripheral defocus lenses and reduced contrast sensitivi-ty lenses in myopia control efficacy

Objective To investigate the roles of peripheral defocus lenses(DIMS)and reduced contrast sensitivity(CS)lenses[e.g.,those utilizing optical diffusion technology(DOT)]in myopia control efficacy and their impact on visu-al quality in myopic patients.Methods A prospective controlled study was conducted to select 160 myopic children(160 eyes)aged 7-12 years old who were fitted with lenses at Harbin Purui Eye Hospital from April 2023 to April 2024.They were divided into a DIMS group(n=80)and a DOT group(n=80)according to the type of lens fitted.Spherical equiva-lent(SE),axial length(AL),choroidal thickness(CT)in various regions,and contrast sensitivity(CS)under photopic and scotopic conditions at different spatial frequencies were recorded and compared between the two groups before lens wear,and at 6 and 12 months after lens wear.Results At 6 and 12 months after lens wear,the DOT group showed lower SE,lower CT in all regions,and lower CS under both photopic and scotopic conditions at three different spatial frequen-cies,alongside higher AL,compared to the DIMS group(all P<0.05).Compared with baseline(before lens wear),the DOT group showed decreased SE and increased AL at both 6 and 12 months(all P<0.05).In the DIMS group,compared with baseline,the change in SE at 6 months was not statistically significant(P>0.05),while at 12 months,SE was de-creased and AL was increased(both P<0.05).Compared to the 6-month time point,both groups showed decreased SE and increased AL at 12 months(all P<0.05).Compared with baseline,CT in all regions increased in both groups at 6 and 12 months(all P<0.05).Compared to the 6-month time point,CT in all regions increased in both groups at 12 months(all P<0.05).Compared with baseline,CS within the same spatial frequencies under both photopic and scotopic conditions in the DOT group,and CS under scotopic conditions within the same spatial frequencies in the DIMS group,were decreased at both 6 and 12 months(all P<0.05).Compared to the 6-month time point,CS within the same spatial frequencies under both photopic and scotopic conditions in the DOT group,and CS under scotopic conditions within the same spatial frequen-cies in the DIMS group,were decreased at 12 months(all P<0.05).Conclusion Wearing DIMS lenses provides superi-or control over myopia progression(SE change,AL growth,CT increase)compared to DOT lenses,with less impairment to visual quality.This finding can serve as a reference for clinicians selecting myopia control interventions.
3.Literature review of one case of follicular lymphoma complicated with iliac bone Cryptococcus neoformans infection

OBJECTIVE To conduct a literature review of one case of follicular lymphoma complicated with iliac bone Cryptococcus neoformans infection so as to raise the understanding of such pathogen in the field of HIV-negative Hodgkin's/non-Hodgkin's lymphoma.METHODS The clinical data were collected from one case of follicular lymphoma patient complicated with C.neoformans infection who was treated in Xinrui Hospital of Xin-wu District,Wuxi City on Feb.24,2023 and retrospectively analyzed.All of relevant literatures regarding to the subject were retrieved in Pubmed,CNKI,Wanfang and VIP databases,and the clinical data of the patients with HIV-negative Hodgkin's/non-Hodgkin's lymphoma with C.neoformans infection were screened out and summa-rized.RESULTS The case was a 28-year-old male and had the underlying disease of follicular lymphoma.The C.neoformans was detected by iliac bone histopathology and metagenome next generation sequencing(mNGS).The condition of the patient improved after the treatment with amphotericin B cholesterol sulphate com-pound and fluconazole.A total of 28 patients,with this case included,were involved in the literature review,23 of whom were male,and 5 were female,and the age ranged between 16 and 79 years old.With respect to major in-fection sites,there were 15(53.57%)cases of cerebral infection,9(32.14%)cases of blood infections,7(25.00%)cases of pulmonary infection,4(14.285%)cases of skin infections,2(7.14%)cases of muscle tis-sue infections,2(7.14%)cases of pleural effusion infections,2(7.14%)cases of bone infections and 1(3.57%)case of bone marrow infection.Totally 11 patients had disseminated Cryptococcus infection,accounting for 39.28%.CONCLUSIONS The C.neoformans infection is seldom detected in the patients with HIV-negative Hodgkin's/non-Hodgkin's lymphoma.The brain is the major infection site with the high probability of dissemina-ted infection.It is necessary for the hospital to deepen the understanding of the pathogen in the field of HIV-negative Hodgkin's/non-Hodgkin's lymphoma.
4.Trend of Incidence Rates of Gallbladder Cancer in Qi-dong City of Jiangsu Province from 1972 to 2021

[Purpose]To analyze the trend of incidence rates of gallbladder cancer in Qidong City of Jiangsu Province from 1972 to 2021.[Methods]The incidence data of gallbladder cancer from 1972 to 2021 were collected from the Qidong Tumour Registry database,the crude incidence rate(CR),age-standardized rate by Chinese standard population(ASRC),age-standardized rate by world standard population(ASRW)of gallbladder cancer were calculated.Trend analysis was per-formed using Joinpoint 4.9.1.0 software to calculate the average annual percentage change(AAPC)of gallbladder cancer incidence rates,and time trend analysis was performed on the overall inci-dence rate by sex and age.[Results]A total of 1 369 cases of gallbladder cancer occurred in Qi-dong City from 1972 to 2021,accounting for 0.93%of all malignant tumors.The overall CR of gallbladder cancer was 2.44/105 in 50 years,ASRC was 0.88/105 and ASRW was 1.45/105.The truncated rate of 35~64 years old was 1.98/105,the cumulative incidence of gallbladder cancer at 0~74 years old was 0.16%,and the risk of cumulative incidence of gallbladder cancer was 0.16%.CR was slightly higher in women than that in men,but after standardization it was slightly higher in men than that in women.The sex ratio of CR,ASRC and ASRW was 0.89,1.07 and 1.06,respectively.With the increase of age,the incidence of gallbladder cancer was also increased.The age of onset was slightly increased in last 50 years.In last 50 years,there was an upward trend in the overall incidence of gallbladder cancer,the incidence both for men and women,and the inci-dence of each age group.[Conclusion]The incidence of gallbladder cancer in Qidong City has increased considerably during the past 50 years,and continuing attention should be paid to the prevention and treatment of gallbladder cancer especially for the key populations.
5.Analysis of Survival Rate of Breast Cancer from 1972 to 2019 and Prediction for Next 10 Years in Qidong City of Jiangsu Province

[Purpose]To analyze the survival rate of breast cancer in Qidong City of Jiangsu Province from 1972 to 2019 and to predict the trend in the next 10 years.[Methods]The data of breast cancer collected from Qidong Cancer Registry from 1972 to 2019 were extracted.Observed survival rate(OSR),relative survival rate(RSR),age-adjusted relative survival(ARS)and average annual percentage change(AAPC)of breast cancer were calculated.ARIMA model was used to predict the trend of breast cancer survival rate.[Results]The 5-year RSR increased from 57.30%during 1972-1977 to 89.01%during 2014-2019,and the uptrend of RSR in the 8 periods was statistically significant(P<0.001).The 5-year ARS increased from 48.12%during 1972-1977 to 85.64%during 2014-2019 with an AAPC of 1.85%(t=10.113,P<0.001).The 5-year RSR during 1972-2019 for males was 85.22%,and for females was 74.51%.For females,the 5-year RSR in-creased from 56.44%during 1972-1977 to 88.93%during 2014-2019 with an AAPC of 1.29%(t=13.087,P<0.001),and the 5-year ARS increased from 46.14%during 1972-1977 to 85.23%during 2014-2019 with an AAPC of 1.90%(t=10.369,P<0.001).The 5-year RSR in the age groups of 25~34,35~44,45~54,55~64,65~74,and over 75 years old were 66.91%,74.69%,76.97%,75.52%,73.44%and 66.40%,respectively;the corresponding AAPCs of 5-RSR in above age groups were 1.02%(t=3.816,P=0.009),1.03%(t=4.936,P=0.003),1.23%(t=5.826,P=0.001),1.86%(t=5.997,P=0.001),2.13%(t=10.245,P<0.001),and 1.44%(t=6.405,P=0.001),respectively.ARIMA modeling of survival trend prediction showed that 5-RSR and 5-ARS for breast cancer will be ascended to 98.76%and 98.33%by 2028,respectively.[Conclusion]The overall survival rate of registered breast cancer cases in Qidong City has been greatly improved and will be further improved in the future,more attention should be paid to the prevention and treatment of breast cancer.

7.The influence of phase coding on streaking artifact of Star-volumetric interpolated body examination sequence

Objective To discuss the mechanism of the streaking artifact of Star-volumetric interpolated body examination(Star-VIBE)sequence and the influence of phase coding on it.Methods Twenty-six volunteers underwent many times Star-VIBE sequences scanning with different phase coding(320,640,960,1 280,1 600).The severity of streaking artifact of images in different phase cod-ing groups was evaluated qualitatively.The signal-to-noise ratio(SNR)of muscle,thyroid and contrast-enhanced artery,contrast-to-noise ratio(CNR)between soft tissues,and CNR between muscle and contrast-enhanced artery of the images in different phase coding groups were evaluated quantitatively.Results In qualitative evaluation,with the number of phase coding increased,the streaking artifact decreased(H=83.022,P<0.005).In quantitative evaluation,SNRmuscle,SNRthyroid,SNRcontrast-enhanced artery and CNRcontrast-enhanced gradually increased with the increase of phase coding number(SNRmuscle:F=6.913,P<0.005;SNRthyroid:F=3.930,P=0.005;SNRcontrast-enhanced artery:F=6.980,P<0.005;CNRcontrast-enhanced:F=6.482,P<0.005),while CNRsoft tissue showed no statistical difference(F=1.114,P=0.339).Conclusion There is streaking artifact on the image of the Star-VIBE sequence,which can be reduced by increas-ing the number of phase coding appropriately.The most suitable phase coding is 960.

9.A case of using acceptance and commitment therapy matrix for pre-exam anxiety

Objective:To explore the effect of acceptance and commitment therapy matrix(ACT Matrix)for somatic symptoms associated with pre-exam anxiety in a high school student.Methods:An 8-session intervention was conducted,conceptualized by the ACT Matrix.The comprehensive assessment of Acceptance and Commitment Therapy processes(CompACT)and the evaluation of inverted U-shaped pressure curve were used for pre-and post-assessment.Results:The client's CompACT score increased from 56 to 71,and his stress level score decreased from 9 to 6.The client's somatic symptoms were significantly alleviated,the sleep improved,the relationship with his par-ents enhanced.Furthermore,The client exceeded expectation in the final semester exam.Conclusion:ACT Matrix effectively alleviated pre-exam anxiety-related somatic symptoms in a high school student,which is a useful tool to guide and facilitate the counseling.
