OBJECTIVE To investigate the effects and mechanism of the Yishen paidu formula on renal fibrosis in rats with chronic renal failure (CRF) through the reactive oxygen species (ROS)/thioredoxin-interacting protein (TXNIP)/NOD-like receptor thermal protein domain associated protein 3 (NLRP3) pathway. METHODS Rats were randomly divided into control group, model group, Yishen paidu formula low-dose (Yishen paidu formula-L) group, Yishen paidu formula high-dose (Yishen paidu formula- H) group, Yishen paidu formula-H+pcDNA-NC group, and Yishen paidu formula-H+ pcDNA-TXNIP group, with 10 rats in each group. Except for control group, all other rats were fed a diet containing 0.5% adenine to establish a CRF model; the rats were then administered corresponding drugs or normal saline intragastrically or via tail vein, once daily, for 8 consecutive weeks. After the last administration, the levels of serum creatinine (Scr), blood urea nitrogen (BUN), ROS, superoxide dismutase (SOD), malondialdehyde (MDA), tumor necrosis factor-α (TNF-α), interleukin (IL)-6, and IL-1β were measured in each group. Pathological changes in renal tissue were observed, and the protein expression levels of Collagen Ⅲ, α-smooth muscle actin (α-SMA), transforming growth factor-β1 (TGF-β1), TXNIP and NLRP3 in renal tissue were detected. RESULTS Compared with model group, the renal histopathological damage and fibrosis of rats in Yishen paidu formula-L group and Yishen paidu formula-H group were significantly alleviated. The levels of Scr, BUN, ROS, MDA, TNF- α, IL-6 and IL-1β, and the protein expressions of Collagen Ⅲ, α-SMA, TGF-β1, TXNIP and NLRP3 were significantly decreased, while SOD levels were significantly increased (P<0.05). Moreover, the changes were more pronounced in the Yishen paidu formula-H group (P<0.05). Compared with Yishen paidu formula-H+pcDNA-NC group, above indexes of rats in Yishen paidu formula-H+pcDNA-TXNIP group were reversed significantly (P<0.05). CONCLUSIONS Yishen paidu formula can inhibit renal fibrosis in CRF rats by suppressing the ROS/TXNIP/NLRP3 pathway.

The aim of this study is to establish an gene editing method of Mesomycoplasma hyo-pneumoniae(Mhp)based on the homologous recombination principle. The restriction enzyme di-gestion and ligation method combined with gene synthesis were used to construct a shuttle plasmid to achieve replication in both Mhp and Escherichia coli(E.coli).The pGEM?-T vector was used as the skeleton. The oriC sequence of Mhp which can achieve the replication of the plasmid in Mhp was inserted into the vector.Sequences of the Spiroplasma promoter and puromycin resistance gene were then inserted into the above constructed plasmid to screen recombinant clones. The up-stream and downstream homologous arms of p97 were constructed to initiate homologous recombination. The recA gene of E.coli is inserted to improve the efficiency of homologous recom-bination.The obtained shuttle plasmid was then delivered into Mhp by electro-transformation or chemical transformation.A shuttle plasmid,pGEM?-Mhp-oriC-p 97,which can replicate in both Mhp and E.coli was constructed. With the transformation of this plasmid, the carried puromycin gene and recA gene can be expressed, the p97 gene can be edited. Finally, the genetically unstable p97 gene mutant was initially obtained. In this study,a tool for Mhp gene editing based on the principle of homologous recombination was established, which laid a foundation for the develop-ment of tools for studying the pathogenesis of Mhp.

According to statistics from the Extracorporeal Life Support Organization(ELSO),as of October 2023,a total of 198 623 patients worldwide had received extracorporeal membrane oxygenation(EC-MO)support.In recent years, the number of ECMO treatment cases in China has increased rapidly, reaching 10 656 cases in 2022.It was previously believed that fat emulsions could easily lead to ECMO system failures due to their hydrophobic nature, thereby limiting their use during ECMO.However, many clinically used drugs are lipid-soluble,and restricting their use not only increases treatment difficulty for these patients but may e-ven affect their prognosis. With advancements in material technology and membrane lung manufacturing processes, intravenous lipid emulsion(ILE)now exerts significant effects on ECMO systems. This article re-views recent domestic and international research progress regarding the effects of ILE on ECMO systems,ai-ming to provide more evidence-based medical support for ILE application in ECMO patients.

Objective:To explore the application value of three-dimensional visualization reconstruction technology in robot-assisted laparoscopic partial nephrectomy(RAPN)for the treatment of highly complex(R.E.N.A.L. score≥10)renal hilar tumors. Methods:The clinical data of 87 patients with highly complex renal hilar tumors with R.E.N.A.L. scores ≥10 who were treated in First Affiliated Hospital of Nanchang University from January 2021 to December 2024 were retrospectively analyzed,of which 36 underwent 3D visualization reconstruction and 51 underwent conventional CT. The 3D visualization reconstruction method was to import the patient’s enhanced CT images in DICOM format into the 3D reconstruction image data processing software to produce a 3D visualization model. There were 22 males and 14 females in the 3D visualization group, with an average age of(54.2 ± 9.5)years,a body mass index of(24.8 ± 4.5)kg/m 2,and a tumor size of(4.3 ± 1.0)cm. Tumors were located on the left side in 16 cases and on the right side in 20 cases. Tumor stages were classified as T 1a in 11 cases,T 1b in 21 cases, and T 2a in 4 cases. The R.E.N.A.L. scores were distributed as follows:10 points in 21 cases,11 points in 12 cases, and 12 points in 3 cases. The estimated glomerular filtration rate(eGFR)before operation was(78.2±9.6)ml/(min·1.73 m 2). There were 35 males and 16 females in the conventional CT group, with an average age of(51.3±8.9)years,a body mass index of(25.4 ± 3.9)kg/m 2,and a tumor size of(4.1 ± 1.2)cm. Tumors were located on the left side in 25 cases and on the right side in 26 cases. Tumor stages were classified as T 1a in 12 cases,T 1b in 33 cases, and T 2a in 6 cases. The R.E.N.A.L. scores were distributed as follows:10 points in 31 cases,11 points in 18 cases, and 12 points in 2 cases . The preoperative eGFR was(80.6 ± 8.8)ml/(min·1.73 m 2). There was no statistical difference in general data and preoperative renal function between the two groups( P > 0.05). Both groups underwent RAPN. The two groups were analyzed and compared in terms of operation time, warm ischemia time, intraoperative blood loss, postoperative hospital stay, postoperative complications, and changes in renal function 3 months after surgery. Results:There were no cases of conversion to radical treatment or open surgery in both the 3D visualization group and the conventional CT group. The 3D visualization group had shorter operation time[(94.6 ± 18.5)min vs.(110.2 ± 17.2)min, P < 0.001],shorter renal artery occlusion time[(23.3 ± 4.0)min vs.(27.2 ± 3.3)min, P < 0.001],less intraoperative blood loss[120(100,250)ml vs. 150(120,300)ml, P = 0.018],and a lower proportion of intraoperative collecting system incision(19/36 vs. 38/51, P = 0.042)than the conventional CT group. There was no significant statistical difference in the time of postoperative drainage tube removal and postoperative hospital stay between the two groups( P > 0.05). One case in the 3D visualization group had postoperative fever, and two cases in the conventional CT group had postoperative obvious macroscopic hematuria. Postoperative pathological diagnosis of the patients was clear cell carcinoma in 78 cases, papillary cell carcinoma in 6 cases, chromophobe cell carcinoma in 2 cases, and oncocytoma in 1 case. No positive resection margin was found in both groups. Three months after surgery, there was no significant statistical difference in eGFR between the two groups[(70.6 ± 8.5)ml/(min·1.73 m 2)vs.(71.4 ± 9.2)ml/(min·1.73 m 2), P = 0.681]. During the median follow-up of 17.8 months,no tumor recurrence or metastasis was observed in either group. Conclusions:RAPN has good safety and feasibility in the treatment of highly complex(R.E.N.A.L. score ≥10)renal hilar tumors. Preoperative three-dimensional visualization reconstruction technology helps to reduce RAPN operation time, renal artery occlusion time and intraoperative blood loss, and has good clinical application value.
