1.Application of floating acupuncture in the treatment of military training injuries

Objective To explore the clinical effect of floating acupuncture on military training injuries,so as to provide reference for the prevention and treatment of military training injuries.Methods A total of 300 patients with ankle,knee,hip,lower back,neck and shoulder diseases caused by military training were divided into two groups.One group was treated with floating acupuncture,and the other group(control group)was treated with traditional physical therapy,medicine and acupuncture.Visual analogue scale(VAS)and Functional Movement Screening(FMS)were used to assess pain and functional movement before and after treatment,respectively.Results After treatment,VAS scores in the floating acupuncture subgroups were significantly lower than those in the control group,and FMS scores in the floating acupuncture subgroups were significantly higher than those in the control group(all P<0.05).Conclusion Floating acupuncture has significant effect on ankle,knee,hip,lower back,neck and shoulder diseases caused by military training,and it is worthy of popularization.
2.Effect of varicocele on semen quality of officers and soldiers stationed along the coast

Objective:To investigate the grade of varicocele and its impact on semen quality of officers and soldiers stationed in coastal areas.Methods:The research subjects were 146 patients with left-sided varicocele and 32 healthy men selected from the male officers and soldiers who had visited the Beidaihe Sanatorium of PLA Joint Logistics Support Force. They were given physical and ultrasonic examination of spermatic vein and were divided into different groups by ultrasonic grading according to the inner diameter of their spermatic vein. Their semen quality was routinely analyzed.Results:There was no significant difference in semen volume or sperm deformity rate among the groups( P > 0.05). As varicocele aggravated,the sperm survival rate,sperm density,and the proportion of Grade A + B sperm all decreased significantly in the groups of clinical type II and III compared with those in the groups of normal type,subclinical type,and clinical type I( P < 0.05). Conclusion:The incidence of varicocele in officers and soldiers of coastal garrisons is significantly higher than that in the general population. The varicocele grade is closely related to the semen quality. Varicocele of clinical type I and below has less impact on the semen quality.
3.Comparative analysis of semen quality before and after operation in the military personnel stationed at coast areas with clinical varicocele

Objective:To observe the changes in semen quality before and after operation in the officers and soldiers stationed at coastal areas with clinical varicocele,and to evaluate the outcome of operation.Methods:According to the results of the color Doppler ultrasound,a total of 113 patients with clinical left varicocele who underwent high ligation of the spermatic vein in the Department of Urology of the Beidaihe Rehabilitation and Recuperation Center of PLA Joint Logistics Support Force were divided into three groups:clinical grade Ⅰ( n=31),clinical grade Ⅱ( n=36),and clinical grade Ⅲ( n=46). The semen qualities of all patients were analyzed before and after operation. Results:There was no significant change in semen quality before and after operation in the clinical grade Ⅰ group( P>0.05);the semen quality in the clinical grade Ⅱ group was significantly improved after operation( P<0.05);the semen quality in the clinical grade Ⅲ group after operation was generally improved( P<0.05). 4.Ultrasonographic characteristics and correlation analysis of asymptomatic hyperuricemia and gout knee joint lesions in naval officers and sailors

Objective:To explore the ultrasonographic characteristics of asymptomatic hyperuricemia(HUA)and gout knee joint lesions in naval officers and sailors,so as to improve the HUA prevention and treatment in them.Methods:A total of 168 HUA cases were selected from the naval officers and sailors admitted to the Beidaihe Rehabilitation and Recuperation Center of Joint Logistic Support Force from January 2016 to December 2021,of whom 22 cases were diagnosed as gout(gout group),and the other 146 cases were asymptomatic hyperuricemia(HUA group). Another 58 cases with normal blood uric acid who were admitted to the center during the same period were selected as the control group. All three groups underwent high-frequency and color doppler musculoskeletal ultrasonography on knee joints,ankle joints,first metatarsophalangeal joints,and the surrounding soft tissues. The correlations between serum uric acid level and articular cartilage lesions in the three groups were analyzed and compared.Results:Compared with the HUA group,the gout group had significantly higher serum uric acid level and body mass index(BMI)( P<0.05),and a significant higher detection rate of sonographic features,such as synovial hyperplasia,synovitis,synovial sac effusion,double track sign,bone erosion,and gout stone( P<0.05);besides,the mean cartilage thickness and maximum gout stone diameter in the gout group were significantly increased( P<0.05). Logistic regression analysis showed that the double track sign and gout stone in the HUA group were the influencing factors of the incidence of HUA in naval officers and sailors( P<0.05 or P<0.01). In the gout group,six typical sonographic features of synovial hyperplasia,synovitis,synovial sac effusion,double track sign,bone erosion,and gout stone,as well as mean cartilage thickness and maximum gout stone diameter,were the risk factors of gout in naval officers and sailors( P<0.05 or P<0.01). Conclusion:Musculoskeletal ultrasonography can detect the uric acid salt crystallization and the changes of joint structure in early stage of HUA,thus providing basis for early diagnosis of HUA and gout.

8.Antifibrotic effect of pirfenidone on orbital fibroblasts in patients with thyroid-associated ophthalmopathy and its mechanisms

Objective To investigate the effects of pirfenidone on orbital fibroblasts (OFs) from patients with thyroid-associated ophthalmopathy (TAO) and its underlying mechanisms.Methods OFs from patients with TAO were isolated and cultured in DMEM.Cells were divided into four groups and treated with 0,250,500 and 1 000 μg/ml pirfenidone for 24,48 or 72 hours,respectively.Cell proliferation was detected by tetramethyl azo salt (MTT) assay,and cell viability was determined by trypan blue.Transforming growth factor (TGF) β1 mRNA level was determined by real-time fluorescence quantitative PCR (RT-qPCR).Type Ⅰ and type 11Ⅲ collagen secreted from cultured cells were measured by enzyme-linked immuno sorbent assay (ELISA).Results (1) The primary cultured OFs had typical fibroblast spindle-like morphology.(2) MTT assay showed that pirfenidone treatment significantly inhibited the proliferation of OFs in a dose-dependent manner (P<0.05) with the proliferation rates of pirfenidone treated groups of-15.31%,-24.92%,-48.53% from 250,500,1 000 μg/ml after 72 h,respectively,in which the inhibition effect of 1 000 μg/ml pirfenidone was significantly different from the other two treated groups (P<0.05).There were no significant differences in the inhibitory effect of the same concentration group among different time points at 24 h,48 h and 72 h (P>0.05).Trypan blue showed that the survival rate of OFs in different concentrations of pirfenidone from 0,250,500,1 000 μ-g/ml at 72 h were 78.37%,79.21%,78.24% and 76.28%,respectively.There were no significant differences between each drug treated and the control group (P>0.05).(3) RT-qPCR results showed that the mRNA expression levels of TGFβ1 at 250,500,1 000 μg/ml pirfenidone treated groups at 72 h were 0.760±0.010,0.440±0.006,and 0.290±0.002,respectively.Compared with the control group (0.950±0.014),the differences were statistically significant (all P<0.05).Moreover,TGFβ1 mRNA expression level in 1 000 μg/ml pirfenidone treated group was significantly lower than those in the other two treated groups (all P<0.05).The secretion of type Ⅰ collagen (0.633 ± 0.006,0.527 ± 0.003 and 0.402±0.008) and type 11Ⅲ collagen (0.511±0.003,0.439±0.007 and 0.223±0.006) in 250,500 and 1 000 μg/ml pirfenidone treated groups at 72 h were significantly lower than those in the control group (0.794±0.005,0.527±0.007,all P<0.05).Type Ⅰ and type Ⅲ collagen secretion in 1 000 μg/ml pirfenidone treated group were significantly lower than those in the other two groups (P<0.05).Conclusions Pirfenidone inhibits the cell proliferation,TGFβ1 expression and collagen secretion of OFs,which may contribute to the anti-fibrotic effect of pirfenidone.
