Objective:To investigate the differences in clinical outcomes of septic children with varying serum zinc levels, and to analyze the relationship between reduced serum zinc levels and organ dysfunction as well as 28-day mortality in septic children. Methods:This study conducted a retrospective analysis of clinical data from pediatric patients diagnosed with sepsis or septic shock in the Department of critical care medicine of the children's Hospital of Soochow University between January 2017 and December 2022.Clinical characteristics, organ dysfunction, and prognosis were compared between two groups:children with low serum zinc levels and those with normal zinc levels. Results:The serum zinc level of septic children within 24 hours of admission was 9.60(5.52,13.80)μmol/L,with 50.54%(94/186)of the children exhibiting low serum zinc levels(<10.07 μmol/L).Compared to the normal serum zinc group, the low serum zinc group had a significantly lower Pediatric Critical Illness Score(PCIS)[(78.71±9.35)vs.(85.12±8.51),P=0.005]and higher 28-day mortality(46.80%vs.14.13%,P<0.001).The low serum zinc group also had a higher proportion of invasive mechanical ventilation(64.89%vs.47.82%,P=0.019),renal replacement therapy(15.59%vs.3.26%,P=0.003),and use of vasoactive drugs(56.38%vs.30.43%,P<0.001).The rate of underlying conditions in the low serum zinc group was significantly higher than that in the normal serum zinc group(57.44%vs.36.95%,P=0.005).Additionally,the low serum zinc group had a higher incidence of disseminated intravascular coagulation(DIC),respiratory failure, acute kidney injury, shock, and multiple organ dysfunction syndrome(MODS)compared to the normal serum zinc group(P<0.05).Serum zinc levels had predictive value for 28-day mortality in septic children(AUC=0.813;95%CI:0.725~0.902;P<0.001).A serum zinc level of less than 6.950 μmol/L predicted the death of septic children with a sensitivity of 0.618 and a specificity of 0.902.Conclusion:Sepsis in children is commonly associated with low serum zinc levels, especially in those with underlying conditions such as hematologic and oncologic disorders.Sepsis patients hypozincemia with a higher incidence of DIC,respiratory failure, acute kidney injury, shock, and MODS.A serum zinc level below 6.95 μmol/L serves as a significant predictor of 28-day mortality in children with severe sepsis.
2.Construction and validation of a prognostic model for NK/T-cell lymphoma based on random survival forest algorithm
Journal of Army Medical University 2025;47(3):275-284
Objective To investigate the prognostic factors affecting survival in patients with natural killer T-cell lymphoma(NKTL),and then develop a prognostic model for predicting their overall survival(OS)based on random survival forest(RSF)algorithm. Methods Demographic and clinical pathological data of NKTL patients were collected from the SEER database during 2000 and 2020.The patients were divided into a training cohort(n=471)and a validation cohort(n=203)in a 7∶3 ratio.Cox regression analysis was performed to identify prognostic factors affecting OS,and a nomogram model was constructed based on the obtained factors.Meanwhile,RSF algorithm was used to determine prognostic factors affecting OS to build the RSF model. The models were evaluated using receiver operating characteristic(ROC)curve, calibration curve, decision curve, net reclassification improvement(NRI),and integrated discrimination improvement(IDI),and the predictive performances of the 2 models were compared.Risk scores for each patient were calculated using the 2 models. Then the patients were divided into high-and low-risk groups based on the median risk score, and survival curve was plotted for comparison. Results Ann Arbor stage, age, radiotherapy, combined treatment, and type of disease were identified as significant prognostic variables associated with OS.In the validation cohort, the area under the ROC curve(AUC)for the nomogram model at 1,3,and 5 years was 0.745,0.771,and 0.748,respectively,while the AUC for the RSF model was 0.764,0.792,and 0.761 at the same time points.ROC curve analysis indicated that both models demonstrated good accuracy and discrimination in predicting OS.Calibration curve analysis showed a strong consistency between the predicted and actual OS for both models. Both models effectively stratified the patients into poor and favorable prognosis groups, with the OS of patients in the poor prognosis group being significantly shorter than that of the favorable prognosis group(P<0.000 1).Decision curve analysis revealed that the net benefit of the RSF model was superior to that of the nomogram model.Compared to the nomogram model, the NRI for the RSF model was 0.184(95%CI:0.098~0.267,P<0.01),and the IDI was 0.300(95%CI:0.241~0.359,P<0.01).Overall,the RSF model demonstrated superior predictive capability than the nomogram model. Conclusion Ann Arbor stage, age, radiotherapy, combined treatment, and type of disease are prognostic factors affecting the prognosis of NKTL patients. Our RSF model demonstrates strong predictive capability for the prognosis of NKTL patients and can effectively assess patient outcomes.

BACKGROUND:Slow bone repair and poor bone formation quality are still problems during masquelet technique in the treatment of large segment bone defects.H-type vessels can induce osteogenesis, enhance the local angiogenesis and osteogenesis coupling, and promote bone repair. However, there are few reports on the role of H-type blood vessels in the bone induced membrane.OBJECTIVE:To construct a large segment bone defect model of SD rat tibia, observe the expression characteristics of H-type blood vessels in the bone induced membrane, then to identify the expression peak point of H-type blood vessels in the bone induced membrane and determine the optimal period of bone grafting.METHODS:Sixty SD rats were randomly divided into a control group(n=30)and a model group(n=30)by random number table method. The two groups were further divided into three subgroups at 4,6,and 8 weeks after bone cement implantation, with 10 rats in each group.A 4 mm bone defect model of the right tibia was constructed in both the control and the model groups.Polymethyl methacrylate bone cement was implanted in the model group to induce bone biomembrane formation, while bone cement was not implanted in the control group. At 4,6,and 8 weeks after bone cement implantation,6 rats were randomly selected at each time point. The bone induction membrane tissue was cut from the model group, and the non-bone soft tissue of the corresponding part was cut from the control group. The dynamic expressions of H-type blood vessels in the bone induced membrane were identified by immunofluorescence. The morphological changes of the bone induced membrane were observed by hematoxylin-eosin staining. The formation of blood vessels in the bone induced membrane was observed by angiography. The expression levels of osteoblast-specific transcription factor in the bone induced membrane were detected by immunohistochemistry.Four rats remained at each time point. In the model group, the bone induced membrane was cut open and the bone cement was removed and autologous coccyx was implanted. In the control group, autologous coccyx was implanted in the bone defect area.Micro-CT evaluation of the tibial defect was performed 8 weeks after bone grafting.RESULTS AND CONCLUSION:(1)Immunofluorescence staining showed that the expression of H-type vessels in the model group was most obvious 6 weeks after bone cement implantation, and the expression of H-type vessels in the model group at each time point after bone cement implantation was higher than that in the control group(P<0.05).(2)Hematoxylin-eosin staining and angiography showed that the number and volume of new blood vessels at each time point after bone cement implantation in the model group were greater than those in the control group(P<0.05).The order of the number and volume of new blood vessels in the model group was:8 weeks after bone cement implantation>6 weeks after bone cement implantation>4 weeks after bone cement implantation.(3)Immunohistochemical staining showed that the positive expression of osteoblast-specific transcription factors at each time point after bone cement implantation in the model group was higher than that in the control group(P<0.05),and the positive expression of osteoblast-specific transcription factors in the model group was most obvious 6 weeks after bone cement implantation.(4)Micro-CT detection showed that the bone repair effect of the three subgroups in the model group was significantly better than that of the corresponding subgroups in the control group, and the bone repair effect of the subgroup in the model group 6 weeks after bone cement implantation was better than that of the subgroups 4 and 8 weeks after bone cement implantation. The results indicate that H-type blood vessels are dynamically expressed in the bone induced membrane and reached a peak 6 weeks after bone cement implantation.Good bone repair effects can be obtained by the bone induced membrane bone grafting 6 weeks after bone cement implantation.

Inclusion criteria:①chronic spinal cord injury patients over 18 years old(with no progression of neurological symptoms within 3 months);② presence of NDO and AD;③ inadequate response or intolerance to oral antimuscarinic agent(M-receptor antagonists or β 3-receptor agonists)④ perform clean intermittent catheterization to empty the bladder. Exclusion criteria:① primary disease in the acute or progressive phase;② previous surgeries that would affect lower urinary tract function, such as transurethral sphincterotomy, bladder neck resection, prostatectomy,or bladder surgery;③ allergy to BTX-A or its adjuvants,or those with allergic predisposition ④ patients who were pregnant, breastfeeding,or planning for pregnancy in the near future;⑤ patients did not accept or were unable to perform intermittent catheterization. Before treatment, all patients were required to maintain 3-5 day urine diary, along with urodynamic studies(UDS),incontinence specific quality of life instrument(I-QOL)and AD symptom severity assessment, and blood pressure monitored. Key UDS parameters recorded included maximum bladder capacity, maximum detrusor pressure during filling phase, changes in maximum systolic blood pressure(SBP)relative to baseline(ΔSBP)during UDS examination, and the frequency of 24-hour blood pressure exceeding baseline by 20 mmHg. After general anesthesia or epidural anesthesia,BTX-A(200 U)was injected into the bladder at 30 points(including the triangle)under the cystoscope using a special injection needle,6.7 U per injection, and then the catheter was kept for 3-5 days after treatment. Three months later, relevant indicators were collected and compared with pre-treatment data. Results:A total of 43 patients were included in this study, including 34 males and 9 females. The age was(39.23±13.17)years old and the disease course was(2.69±3.27)years old. There were 33 cervical and 10 thoracic cases. The American Spinal Injury Association Injury Scale score distribution was as follows:26(60%)A,4(9%)B,9(21%)C,and 4(9%)D. The presence of AD was confirmed in all patients during urodynamic examination(UDS),that was the systolic blood pressure(SBP)suddenly increased and exceeded 20 mmHg(1 mmHg = 0.133 kPa). Before treatment,The AD symptoms severity score(consist of headache, sweating, goose bumps, anxiety and palpitation)were(14.53±2.51),Bladder-related AD frequency was 10.67 episodes/day. Baseline SBP was(103.51±9.64)mmHg,the maximum SBP was(150.40±22.75)mmHg,and the change in SBP(ΔSBP)from maximum to baseline SBP during UDS examination was(43.83±21.01)mmHg. The UDS indicated that the maximum detrusor pressure during storage phase was(54.95±24.68)cmH 2O,and the bladder capacity was(131.70±75.29)ml. Bladder diary showed the volume of catheterization each time from was(181.16±49.86)ml,and The I-QOL score was(44.07±8.60). Three months after treatment, the AD symptoms severity score(consist of headache, sweating, goose bumps, anxiety and palpitation)were(11.37±2.39). The frequency of bladder-related AD episodes was(7.51±2.37)episodes/day,showing statistically significant differences compared to pre-treatment( P<0.05).The SBP before UDS examination was(102.12±10.28)mmHg,with no statistically significant difference from baseline( P = 0.518). The maximum SBP in perfusion phase and the ΔSBP were(132.84±16.30)mmHg and(28.72 ± 14.02)mmHg,respectively,both demonstrating statistically significant differences( P < 0.05). The UDS examination revealed that the maximum detrusor pressure during the storage phase was(29.77±13.72)cmH 2O,showed a significant decrease, and the bladder capacity was(272.63±79.75)ml,which were both statistically different before and after surgery. Bladder diary showed the volume of catheterization each time was(326.74±63.71)ml;I-QOL score was(71.86±11.45),both were significant different after treatment( P < 0.01). Conclusion:BTX-A intravesical injection in the treatment of NDO can also alleviate the severity and frequency of bladder related AD.
