1.Differences in deltamethrin resistance and kdr gene mutation in Culex tritaeniorhynchus population in and outside the Yellow Sea wetland

Objective To gain insights into the biological characteristics of different populations of Culex tritaeniorhynchus within and around the Yellow Sea wetland from the perspective of the occurrence of resistance, we investigated the levels of resistance to deltamethrin and kdr gene mutation in the wetland and its peripheral areas. Methods Specimens were collected from Cx. tritaeniorhynchus populations at two monitoring sites in the Rare Bird National Nature Reserve and Tiaozi Ni Wetland Scenic Area, and also from two populations in Yancheng City and the Liuhe District of Nanjing, and the resistance of these mosquitoes to deltamethrin was determined using the CDC biotest bottle method. For each concentration of deltamethrin assessed, a random subset of exposed specimens was selected for amplification of the kdr gene fragment, followed by Sanger sequencing to identify and analyze resistance-associated mutations. Results The LC50 levels of deltamethrin among mosquitoes from the four populations in Luhe, Yancheng, the Rare Bird National Nature Reserve and the Tiaozi Ni Wetland Scenic Area were 2.048 5, 7.798 2, 3.473 3, and 17.695 5 mg/mL, respectively, with corresponding concentrations of deltamethrin ranging from 0.005 to 5.000,0.050 to 50.000,0.050 to 25.000 and 0.050 to 50.000 mg/mL, respectively. Furthermore, the ranges of the KT50 values were 11.76-107.43, 67.05-216.30,29.77-107.43 and 28.40-329.51 min; the 1-h knockdown rates were 34.58%-99.15%, 9.52%-43.80%, 55.09%-73.01%, and 10.09%-68.07%; and the 24-h mortality rates were 12.15%-67.52%,9.52%-79.56%,13.17%-82.21%, and 11.01%-78.99%, respectively. With respect to kdr gene mutation, we assayed a total of 63,70,59, and 57 mosquitoes for the four populations, for which we detected L1014F mutation frequencies of 14.29%, 35.00%, 20.34%, and 31.58%, respectively, with a majority of these mutations being heterozygous for resistance. In addition, five adult mosquitoes were identified has having synonymous mutations at site 1011[i. e. , AAT(asparagine)mutation to AAC(asparagine)]. Conclusions Our findings revealed the clear resistance of Cx. tritaeniorhynchus to deltamethrin in the Yancheng region of the Yellow Sea wetland, and the resistance phenotype and kdr frequency of Cx. tritaeniorhynchus in the wetland environment were comparable to those of Cx. tritaeniorhynchus in the wetland environment, thereby indicating that the resistance of different populations of Cx. tritaeniorhynchus was homogeneous under the pressure of different insecticide selection within and around the wetland. However, the underlying mechanisms need to be further studied.
2.Effect of dual task training on mobility and balance in stroke patients: a meta-analysis

ObjectiveTo systematically review the effect of dual-task training (DTT) on mobility and balance in stroke patients. MethodsCNKI, Wanfang Data, VIP, PubMed, Web of Science, Embase and Cochrane Library were searched for randomized controlled trials (RCT) on DTT in stroke rehabilitation from inception to June, 2025. The quality of the included studies was assessed using the Cochrane Risk of Bias Tool and the PEDro scale. Meta-analysis was performed using RevMan 5.4. ResultsA total of eleven RCT involving 517 stroke patients were included, and the scores of PEDro scale were four to eight. DTT was more effective on the score of Fugl-Meyer Assessment-Lower Extremity (MD = 3.70, 95%CI 1.88 to 5.53, P < 0.001) and Berg Balance Scale (BBS) (MD = 3.04, 95%CI 1.58 to 4.49, P < 0.001), fall incidence (OR = 0.32, 95%CI 0.11 to 0.87, P = 0.025), gait speed under a single-task condition (MD = 0.08, 95%CI 0.03 to 0.12, P = 0.002), gait speed under a dual-task condition (MD = 0.13, 95%CI 0.10 to 0.16, P < 0.001), Timed Up and Go Test time (MD = -2.78, 95%CI -3.85 to -1.72, P < 0.001), and score of modified Barthel Index (MD = 4.77, 95%CI 1.88 to 7.66, P = 0.001). Subgroup analysis indicated that motor-motor DTT was more effective than cognitive-motor DTT on BBS scores and single-task gait speed. ConclusionsDTT can improve mobility and balance in stroke patients, especially with motor-motor tasks.
3.Study on the Potential Suitable Planting Areas of Scutellariae Radix in Shandong Province Based on MaxEnt Model

Objective To predict the current and future ecological suitability distribution areas of Scutellariae Radix in Shandong Province at present and in the future;To screen the main environmental factors affecting its suitable distribution;To provide a reference for the protection of wild resources and the site selection artificial planting of Scutellariae Radix.Methods Taking Scutellariae Radix as the research object,based on the MaxEnt model,the potential suitable habitats were predicted using 84 distribution points of and 48 environmental factors of Scutellariae Radix,and the main environmental factors affecting the distribution of Scutellariae Radix were explored.Its potential suitable habitat areas were predicted under four greenhouse gas emission concentration scenarios(SSP126,SSP245,SSP370,and SSP585)for the years 2021-2040,2041-2060,2061-2080 and 2081-2100.Results The MaxEnt model was optimized through feature combination and regularization constant parameters,and its predictive ability was good(AUC value of 0.871).The primary driving environmental factors affecting its distribution were the maximum temperature in the warmest month(bio_05),soil gravel content(cf05)and altitude(elev).Under the current climate conditions,Scutellariae Radix is mainly distributed in the hilly areas of central and southern Shandong and some areas of in the hilly areas of Jiaodong.Under the future climate conditions of SSP126,SSP245,SSP370 and SSP585,the areas of suitable habitat of Scutellariae Radix in Shandong will show a decreasing trend.Except for the SSP126,the medium and high adaptation areas of Scutellariae Radix will disappear in the other three modes.Conclusion The results of this study can provide an important reference for the protection,rational utilization and planting layout of Scutellariae Radix in Shandong Province.
4.Application of virtual case teaching integrating SP and OSCE in experimental exercise therapy courses

Objective:To evaluate the effectiveness of virtual case teaching integrating standardized patients (SP) and objective structured clinical examination (OSCE) in enhancing students' clinical thinking and practical skills in experimental exercise therapy course.Methods:In April 2023, 52 students from the 2021 cohort and 54 students from the 2022 cohort of the Rehabilitation Therapy undergraduate program at Xinjiang Medical University were selected and divided into the experimental group and control group according to their classes. In the shoulder joint mobilization experimental class of the exercise therapy course, virtual SP teaching was performed in the experimental group of both cohorts. For the control group, student-led SP teaching was performed in the 2021 cohort, while the traditional "lecture + demonstration" method was used in the 2022 cohort. Upon completion of teaching, theoretical exams, case analysis, practical exams, and a teaching satisfaction survey were conducted on the students. Data were analyzed using the t-test and chi-square test in SPSS 25.0. Results:In both cohorts, the experimental group had similar theoretical exam scores ( P=0.207 and P=0.097, respectively) to the control group, but had higher practical exam scores ( P=0.013 and P=0.002, respectively) than the control group. In the case analysis exam, the experimental group scored higher than the control group in the 2022 cohort ( t=2.84, P=0.006) but similarly to the control group in the 2021 cohort ( t=1.53, P=0.132). All students believed that virtual SP teaching was most engaging. In terms of understanding complex concepts, increasing active participation, and fostering clinical thinking, the 2022 cohort was more satisfied with virtual SP teaching, whereas the 2021 cohort experienced no significant difference between virtual SP teaching and student-led SP teaching. In addition, the 2021 cohort found that student-led SP teaching was more helpful for improving doctor-patient communication skills. Conclusions:The application of virtual SP teaching in the experimental exercise therapy course effectively enhances students' practical skills and case analysis capabilities, contributing greatly to their overall clinical management thinking.
5.The diagnostic value of high-resolution magnetic resonance black blood technology imaging and CT angiography in detecting carotid plaque in predicting stroke

Objective:To investigate the diagnostic value of high-resolution magnetic resonance black blood technology imaging (MRBTI) and CT angiography (CTA) in detecting carotid plaque in predicting stroke.Methods:A prospective study was conducted on 135 patients with carotid artery disease diagnosed and treated in Handan Central Hospital from January 2020 to June 2022, all patients underwent cervical MRBTI and CTA examinations. Digital subtraction angiography (DSA) was used as the gold standard to compare the diagnostic value of the two detection methods for carotid plaque properties. Patients were followed up for 2 years and divided into stroke and non-stroke according to the result of follower-up. Imaging indexes of the two groups were compared, and the predictive value of MRBTI combined with CTA for stroke was evaluated by receiver operation characteristic (ROC) curve.Results:The relevant data of 134 patients were included in statistical analysis. For severe carotid stenosis and ulcerative plaques, the accuracy of CTA examination was 85.11% and 84.91% separately, that of MRBTI examination was 91.49% and 92.45%, the combined examination was 95.74% and 96.23%, and the above three examination methods was not statistically significant ( P>0.05). The detection accuracy rates of CTA for mild, moderate carotid artery stenosis and smooth, irregular plaque, were 73.08%, 86.89%, 73.91% and 86.21%, those of MRBTI detection were 92.31%, 91.80%, 86.96% and 93.10%, and those of combined detection were 100.00%, 98.36%, 100.00% and 98.28%, the combined detection was higher than those of CTA and MRBTI alone ( P<0.05); there was no statistically significant difference in the level of tube wall area between the stroke patients and the non-stroke patients ( P>0.05), but the total vascular area and lumen area in stroke patients were significantly higher than those in non-stroke patients: (103.48 ± 22.48) mm 2 vs. (92.51 ± 16.26) mm 2, (46.18 ± 11.03) mm 2 vs. (41.32 ± 10.52) mm 2, and normal wall index was significantly lower: (0.54 ± 0.12) mm 2 vs. (0.61 ± 0.09) mm 2 ( P<0.05); ROC curve analysis showed that MRBTI combined with CTA had the best predictive efficacy for stroke, with area under the curve (AUC) value as high as 0.82, and its sensitivity (79.00%) was significantly higher than that predicted by MRBTI indexes (total vascular area, lumen area and normal wall index) (51.90%, 39.50%, 56.80%) or CTA (30.90%) alone, with statistically significant difference ( P<0.05). Conclusions:The combination of MRBTI and CTA can improve the diagnostic efficiency of carotid artery disease and the prediction efficiency of stroke, and can be used as an auxiliary examination means of DSA to provide a more reliable clinical basis for the assessment of plaque characteristics in patients with carotid artery disease and the prediction of stroke.
6.The influence of diabetes mellitus and high-sensitivity C-reactive protein on the risk of diges-tive system malignancy: a prospective cohort study

Objective:To investigate the influence of diabetes mellitus (DM) and high-sen-sitivity C-reactive protein (Hs-CRP) on the risk of digestive system malignancy.Methods:The pro-spective cohort study was conducted. The clinical data of 93 928 participants who participated health examination in 9 hospitals at Tangshan, including Kailuan General Hospital Affiliated to North China University of Science and Technology et al, in 2006 were selected. According to the presence or absence of DM and the level of Hs-CRP, all participants were divided into 4 groups, including the DM(-)CRP(-) group defined as absence of DM and Hs-CRP ≤3 mg/L, the DM(-)CRP(+) group defined as absence of DM and Hs-CRP>3 mg/L, the DM(+)CRP(-) group defined as presence of DM and Hs-CRP ≤3 mg/L, and the DM(+)CRP(+) group defined as presence of DM and Hs-CRP >3 mg/L. The data of participants were collected by a fixed team of physicians. The first physical examination in 2006 was taken as the starting point for follow-up. The end event of follow-up was defined as the occurrence of digestive system malignancy or death, and the follow-up was up to December 31, 2021. Observation indicators: (1) comparison of clinical data among the 4 groups of participants; (2) the incidence and cumulative incidence rate of digestive system malignancy in participants; (3) influence of DM and Hs-CRP level on the risk of digestive system malignancy; (4) the combined influence of DM and Hs-CRP level on the risk of digestive system malignancy; (5) sensitivity analysis. Comparison of measurement data with normal distribution among multiple groups was conducted using the one-way analysis of variance. For pairwise comparison, least significant difference test was used for homogeneity of variance, and Dunnett′s T3 test was used for heterogeneity of variance. Comparison of measurement data with skewed distribution among multiple groups was conducted using the Kruskal-Wallis rank sum test, and Dunn-Bonferroni test was used for pairwise comparison. Comparison of count data among multiple groups was conducted using the chi-square test, and Bonferroni test was used among multiple comparisons. The Kaplan-Meier method was used to plot cumulative incidence curve, and Log-rank test was used for cumulative incidence rate analysis. The Cox proportional risk model was used for multivariate analysis. All models were adjusted for relevant confounders. Results:(1) Comparison of clinical data among the 4 groups of participants. Of the 93 928 participants, there were 70 743 cases in the DM(-)CRP(-) group, 14 644 cases in the DM(-)CRP(+) group, 6 425 cases in the DM(+)CRP(-) group, and 2 116 cases in the DM(+)CRP(+) group. There were significant differences in gender, age, fasting blood glucose, Hs-CRP, triglyceride, alanine aminotransferase, body mass index, marrital status, smoking, drinking, high school degree or above, physical exercise, high salt diet, high fat diet, positive hepatitis B virus surface antigen, fatty liver, liver cirrhosis, gallstone, taking hypoglycemic drugs, taking lipid-lowering drugs among the 4 groups of participants ( P<0.05). (2) The incidence and cumulative incidence rate of digestive system malignancy in participants. At the end-up of follow-up, 2 008 cases developed digestive system malignancy in the 93 928 participants, including 717 cases of colorectal cancer, 456 cases of liver cancer, 396 cases of gastric cancer, 195 cases of esophageal cancer, 144 cases of pancreatic cancer, 65 cases of gallbladder cancer or extrahepatic cholangiocarcinoma, 35 cases of small bowel cancer. The cumulative incidence rates of digestive system malignancy were 2.19%, 2.42%, 2.86%, 3.59% in participants of the DM(-)CRP(-) group, DM(-)CRP(+) group, DM(+)CRP(-) group, DM(+)CRP(+) group, respectively, showing a significant difference among the 4 groups ( χ2=31.72, P<0.05). (3) Influence of DM and Hs-CRP level on the risk of digestive system malignancy. After adjusting for the confounding factors of the participants, results of multivariate analysis showed that DM and Hs-CRP >3 mg/L were independent influencing factors for the incidence of digestive system malignancy ( hazard ratio=1.32, 1.19, 95% confidence interval as 1.13-1.56, 1.06-1.33, P<0.05). Futher analysis showed that there was a significant difference in interaction between DM and Hs-CRP >3 mg/L ( P<0.05). (4) The combined influence of DM and Hs-CRP level on the risk of digestive system malign-ancy. After adjusting for confounding factors, results of multivariate analysis showed that using the DM(-)CRP(-) group as the control group, the risk of incidence of digestive system malignancy increased in the DM(-)CRP(+) group, DM(+)CRP(-) group, and DM(+)CRP(+) group, respectively ( hazard ratio=1.14, 1.23, 1.79, 95% confidence interval as 1.01-1.29, 1.02-1.48, 1.38-2.31, P<0.05). In the site-specific analysis of digestive system malignancy, using the DM(-)CRP(-) group as the control group, the risk of incidence of liver cancer increased in the DM(-)CRP(+) group ( hazard ratio=1.37, 95% confidence interval as 1.07-1.75, P<0.05), the risk of incidence of liver cancer and pancrea-tic cancer increased in the DM(+)CRP(-) group ( hazard ratio=1.60, 1.74, 95% confidence interval as 1.16-2.21, 1.00-3.02, P<0.05), the risk of incidence of small bowel cancer, pancreatic cancer and colorectal cancer increased in the DM(+)CRP(+) group ( hazard ratio=5.05, 2.31, 2.23, 95% confidence interval as 1.57-16.21, 1.00-5.31, 1.54-3.24, P<0.05). (5) Sensitivity analysis. After adjusting for confounding factors of excluding 3 types of participants (103 cases of digestive system malignancy within 1 year of follow-up, 2 370 cases of taking glucose-lowering drugs, and 915 cases of taking lipid-lowering drugs), results of multivariate analysis showed that using the DM(-)CRP(-) group as the control group, the risk of incidence of digestive system malignancy increased in the DM(+)CRP(-) group, and DM(+)CRP(+) group, respectively ( hazard ratioexcluding cases of digestive system malignancy within 1 year of follow-up=1.26, 1.66, 95% confidence interval as 1.04-1.52, 1.26-2.18, P<0.05; hazard ratioexcluding cases taking glucose-lowering drugs=1.23, 1.75, 95% confidence interval as 1.02-1.49, 1.31-2.33, P<0.05; hazard ratioexcluding cases taking lipid-lowering drugs=1.24, 1.80, 95% confidence interval as 1.03-1.49, 1.39-2.34, P<0.05). Conclusions:DM and Hs-CRP >3 mg/L are independent influencing factors for the incidence of digestive system malignancy. There is an interation and synergistic effect between DM and Hs-CRP to promote the incidence of digestive system malignancy.

Hepatocellular carcinoma (HCC) remains a leading cause of cancer-related death worldwide, emphasizing the utmost importance of early diagnosis, therapeutic monitoring, and pro-gnostic assessment. Circulating tumor DNA (ctDNA), as an innovative liquid biopsy biomarker, has demonstrated unique advantages in HCC management. The authors systematically summarized recent advances in the application of ctDNA for early diagnosis, therapeutic monitoring, and prognostic evaluation of HCC, focusing on its advantages and clinical application value in precision medicine.
8.Effects of methimazole on endocrine function in patients with hyperthyroidism at different NRG4 levels

Objective To explore the effects of methimazole on endocrine function in patients with hyperthyroidism at different neuregulin 4(NRG4)levels.Methods A total of 60 patients with hyperthyroidism admitted to Huzhou First People's Hospital from January 2022 to January 2023 were selected.According to the level of NRG4,they were divided into case Ⅰ group(NRG4<80ng/ml,28 cases),case Ⅱ group(NRG4 80-120ng/ml,18 cases)and case Ⅲ group(NRG4>120ng/ml,14 cases).The clinical efficacy,thyroid hormone levels,glucose and lipid metabolism indexes and adverse reactions of three groups were compared.Results The difference of total effective rate among three groups was statistically significant(χ2=10.836,P<0.001).There were statistically significant differences in thyroid-stimulating hormone(TSH),free triiodothyronine(FT3),free thyroxine(FT4)between groups,time points and interactions among three groups(P<0.05).TSH levels increased and FT3 and FT4 levels decreased in all three groups after treatment(P<0.05).One month after treatment,the levels of fasting blood glucose,2h postprandial blood glucose,glycated hemoglobin,total cholesterol and low density lipoprotein cholesterol in three groups were significantly lower than before treatment(P<0.05),and the differences in above indicators among the three groups were statistically significant(P<0.05).There was no significant difference in total incidence of adverse reactions among three groups(χ2=5.349,P=0.069).After treatment,NRG4 levels in three groups decreased significantly over time(P<0.05).There were significant differences in NRG4 levels among the three groups at different time points(P<0.05),and case Ⅰ group
9.Bedside transesophageal echocardiography-guided atrial septostomy under extracorporeal membrane oxygenation support

Objective To investigate the feasibility and safety of bedside transesophageal echocardiography(TEE)-guided atrial septostomy in patients undergoing extracorporeal membrane oxygenation(ECMO)support.Methods Eight patients(2 females and 6 males)who developed pulmonary edema during VA-ECMO treatment between January 2020 and October 2024 were enrolled.All patients underwent bedside TEE-guided atrial septal puncture,followed by gradual balloon dilation using an Inoue balloon(diameter:18-24 mm).The endpoint for dilation was defined as mean left atrial pressure<15 mmHg.Post-procedure evaluation included assessment of the septal defect size,mean left atrial pressure,and improvement in bilateral pulmonary edema.Results The mean age of all 8 patients was(43.50±16.84)years old.Atrial septostomy was successfully performed in all patients.The mean balloon dilation diameter was(22.75±2.68)mm,resulting in a mean septal defect size of(6.31±0.37)mm.The mean left atrial pressure significantly decreased from(26.63±1.77)mmHg pre-procedure to(12.75±1.28)mmHg post-procedure(P<0.001).All patients demonstrated marked improvement in bilateral pulmonary edema,and six patients also showed a significant reduction in the severity of mitral regurgitation following the procedure.No procedure-related complications were observed.Conclusions Bedside TEE-guided atrial septostomy is safe and effective for reducing left heart loading during ECMO support and improving pulmonary edema.
10.Study on the consistency of ossification ratio between left and right sides in ultrasound assessment of bone age

Objective:To study whether the bone age of the left and right limbs in children was consistent when the ossification ratio was measured by ultrasound,and to compare this consistency between ultrasound bone age of left and right limbs,and the bone age that was determined by X-ray. Method:A total of 526 children aged 3 to 18 years,who were treated at the Affiliated Hospital of Guizhou Medical University from October 2022 to November 2023,were retrospectively included. The ossification ratios of the left and right ulna,radius,and femur of these children were measured by using ultrasound to calculate the bone age of ultrasound. Simultaneously,all children underwent X-ray examination on left-hand for bone age within two weeks before and after the ultrasonic examination for bone age. The differences in ossification ratios of the ulna,radius and femur of dual sides in assessing bone age by using ultrasound were compared,and the differences of assessing bone age among left ultrasound,right ultrasound and X-ray were further compared. Results:In the ultrasound test,Ossification rate of the left ulna was 48.0%,Less than 49.5% on the right side,The difference was statistically significant (W=25425.5,P<0.001),the left femoral ossification rate was 77.0%,Higher than 76.0% on the right side,The difference was statistically significant (W=57829,P=0.041). The overall ossification rate of the left ulna,radius,and femur was 184.0%,Below 185.0% on the right side,The difference was statistically significant (W=46314.5,P<0.001). The ultrasound bone age of the left limb was 10.1 years,Below 10.3 years on the right,The difference was statistically significant (W=67958.5,P<0.001). The median X-ray bone age of the included children was 10.2 years between ultrasound bone age and X-ray bone age (P>0.05). Conclusion:There is difference in the bone age between left and right side of limbs in assessing bone age by using ultrasound to detect ossification ratio. Therefore,the left and right sides should be assessed by using ultrasound to assess bone age.
