1.Establishment of graded management standards for off-label use of Shenqi fuzheng injection

OBJECTIVE To establish the graded management standards for off-label use of Shenqi fuzheng injection. METHODS Systematic searches were conducted in databases including CNKI, PubMed and the Cochrane Library to retrieve guidelines/consensuses, systematic reviews/meta-analyses, and randomized controlled trials (RCTs) of Shenqi fuzheng injection. The quality of evidence was evaluated using AGREE Ⅱ, AMSTAR Ⅱ, and the Risk of Bias 1.0 tool recommended by Cochrane Collaboration, and the graded management standard for off-label use of Shenqi fuzheng injection was developed by using the Thomson grading system. RESULTS A total of 534 articles were involved, including 11 guidelines, 22 systematic reviews/meta-analysis, and 501 RCTs. They covered 79 off-label indications for Shenqi fuzheng injection: cancer-related fatigue, colorectal cancer and breast cancer, all with high-quality evidence were classified under grade A management (grade Ⅰ commendation), allowing all physicians across the hospital to prescribe relevant treatments; five diseases, such as ovarian cancer, liver cancer, leukemia, heart failure and cerebral infarction, were classified under grade B management (grade Ⅱa commendation), with prescription restricted to physicians with intermediate or higher professional titles in specific departments; eleven diseases, including sepsis, cervical cancer, esophageal cancer, etc., were classified under grade C management (grade Ⅱb commendation), requiring strict evaluation by senior physicians before prescription; the use of Shenqi fuzheng injection for other conditions was explicitly prohibited due to a lack of sufficient evidence. CONCLUSIONS Off-label use of Shenqi fuzheng injection is prevalent. The graded management standard established by evidence-based medical approach provides a scientific basis for standardizing the clinical application of traditional Chinese medicine injections and offers an operable paradigm for implementing differentiated drug use supervision in medical institutions.
2.Seroprevalence and influencing factors of low-level neutralizing antibodies against SARS-CoV-2 in community residents

ObjectiveTo understand the seropositivity of neutralizing antibodies (NAb) and low-level NAb against SARS-CoV-2 infection in the community residents, and to explore the impact of COVID-19 vaccination and SARS-CoV-2 infection on the levels of NAb in human serum. MethodsOn the ground of surveillance cohort for acute infectious diseases in community populations in Shanghai, a proportional stratified sampling method was used to enroll the subjects at a 20% proportion for each age group (0‒14, 15‒24, 25‒59, and ≥60 years old). Blood samples collection and serum SARS-CoV-2 NAb concentration testing were conducted from March to April 2023. Low-level NAb were defined as below the 25th percentile of NAb. ResultsA total of 2 230 participants were included, the positive rate of NAb was 97.58%, and the proportion of low-level NAb was 25.02% (558/2 230). Multivariate logistic regression analysis indicated that age, infection history and vaccination status were correlated with low-level NAb (all P<0.05). Individuals aged 60 years and above had the highest risk of low-level NAb. There was a statistically significant interaction between booster vaccination and one single infection (aOR=0.38, 95%CI: 0.19‒0.77). Compared to individuals without vaccination, among individuals infected with SARS-CoV-2 once, both primary immunization (aOR=0.23, 95%CI: 0.16‒0.35) and booster immunization (aOR=0.12, 95%CI: 0.08‒0.17) significantly reduced the risk of low-level NAb; among individuals without infections, only booster immunization (aOR=0.28, 95%CI: 0.14‒0.52) showed a negative correlation with the risk of low-level NAb. ConclusionsThe population aged 60 and above had the highest risk of low-level NAb. Regardless of infection history, a booster immunization could reduce the risk of low-level NAb. It is recommended that eligible individuals , especially the elderly, should get vaccinated in a timely manner to exert the protective role of NAb.
3.Analyzing the impact of individual and enterprise characteristics on occupational health literacy of key populations

Objective To analyze the factors influencing the occupational health literacy (OHL) level among workers in key industries from the perspectives of both individual workers and enterprises. Methods A total of 32 336 front-line workers from 12 key industries in the secondary industry in Guangdong Province were selected as the research subjects by a stratified cluster random sampling method. Their OHL level was investigated using Occupational Health Literacy Questionnaire of National Key Populations, and the influencing factors were analyzed. Results The OHL level of the research subjects was 48.5%. The OHL level of the research subjects in four dimensions from high to low was basic knowledge of occupational health protection, occupational health practice and behavior, legal knowledge of occupational health, and basic skills of occupational health protection (80.7%, 61.2%, 48.3% and 29.5%, respectively). The multivariable logistic regression analysis showed that the OHL level of female workers was lower than that of males (P<0.05). Lower OHL was also associated with older age, lower education level, lower personal monthly income of workers (all P<0.01). The workers with length of service < 3 years and ≥ 20 years had lower OHL level than those with length of service 3-<10 years and 10-<20 years, respectively (all P<0.05). Workers in larger enterprises had higher OHL levels (all P<0.01). The OHL level of workers in the sixth category of industries with occupational injuries had higher occupational injury risks than those in the third and fourth categories (all P<0.05). The OHL levels of workers in state-owned enterprises, private enterprises, foreign-funded enterprises, and other enterprises were higher than that of workers in public institutions (all P<0.05). Conclusion The influencing factors of workers′ OHL in key industries of the secondary industry include individual factors (gender, age, education level, personal monthly income, length of service) and enterprise factors (enterprise size, enterprise nature and industry injury risk category). Female, older workers, those with lower education or income, and those with short length of service represent priority groups for OHL interventions, while small and micro enterprises are priority units for future workplace health promotion intervention.
4.Epidemiological and clinical characteristics of 152 HIV/AIDS patients complicated with anorectal diseases

OBJECTIVE To explore the epidemiological characteristics of human immunodeficiency virus(HIV)/ac-quired immune deficiency syndrome(AIDS)complicated with anorectal diseases and analyze the clinical character-istics of the HIV-positive patients with perianal abscess.METHODS Totally 152 HIV/AIDS patients who were complicated with anorectal diseases and were treated in The Eighth Hospital of Wuhan from Jan.2020 to Dec.2023 were recruited as the research subjects,36 of whom had perianal abscess.The manifestations of anorec-tal diseases,treatment status and incidence rates of the diseases among the patients with different clinical charac-teristics in the different years were statistically analyzed.The clinical characteristics of the HIV-positive patients with perianal abscess were observed.RESULTS Of the 152 HIV/AIDS patients complicated with anorectal disea-ses,51.97%had mixed hemorrhoid,and 23.68%had perianal abscess.The number of patients with HIV/AIDS complicated with anorectal diseases gradually increased from 2020 to 2023;while the incidence rate of male,aged between 30 and 40 years old,married individuals,those with primary school education or below and famers were relatively high.Among the HIV/AIDS patients complicated with anorectal diseases,139 underwent surgical pro-cedures,and 23 did not.Among the 36 HIV-positive patients with perianal abscess,97.22%were male,50.00%were aged between 20 and 30 years old,and 44.44%were married.CONCLUSIONS The number of HIV/AIDS patients complicated with anorectal diseases is gradually increased from 2020 to 2023;the incidence rates are rela-tively high among the patients who are male,aged between 30 and 40 years old,married,with primary school edu-cation or below and famers.Perianal abscess and mixed hemorrhoid are the major types of anorectal diseases.Sur-gical procedures are recommended for HIV/AIDS patients complicated with anorectal diseases,while the HIV-positive patients with perianal abscess are dominated by the population of male,aged between 20 and 30 years old and married.It is necessary to attach great importance to the detection of pathological characteristics of the HIV-positive patients with perianal abscess so as to take appropriate treatment measures to improve the therapeutic effect.

Since the concept of artificial intelligence(AI)was proposed in 1956,medicine has been one of its core application fields.At present,AI technology has run through the whole diagnosis and treatment process,and has been extended to innovative scenarios such as drug research and development,surgical robots,and clinical trial optimization.Scenario application is the backbone of the technical system.Multimodal data fusion integrates heterogeneous data such as images,medical records,and genes,and federated learning realizes cross-institutional privacy protection and sharing.Deep learning achieved more than 90%sensitivity in imaging diagnosis for lung nodule detection,while generative AI accelerates drug molecule design.The core applications cover four major areas field:AI is more accurate than professional doctors in breast cancer and diabetic retinopathy screening;robotics shortens hospital stays and improves spinal screw placement accuracy;AI shortens the discovery cycle of drug targets;machine learning improves the efficiency of subject screening and enables real-time data monitoring.The application of AI in the medical field is first constrained by data quality and algorithm bias,and the"black box"characteristics of AI models and the ambiguity of responsibility attribution are the core obstacles to clinical implementation.This paper analyzes key technological breakthroughs and typical cases,discusses the application scenarios and challenges of AI in medicine,and aims to provide a reference for the future development of medical intelligence.
6.FTO regulates resistance of triple-negative breast cancer to adriamycin through Wnt/β-catenin signaling pathway

Aim To explore the effect of FTO on adria-mycin resistance in triple-negative breast cancer through the Wnt/β-catenin signaling pathway and to reveal the underlying mechanism.Methods The MDA-MB-231/ADR drug-resistant cell line was constructed using a method of gradually increasing adriamycin concentra-tion with intermittent induction.The half-inhibitory concentration(IC50)of adriamycin for MDA-MB-231 and MDA-MB-231/ADR cells and the expression of FTO were compared.After knocking down FTO in MDA-MB-231/ADR cells,CCK-8,qRT-PCR,colony formation assay,transwell,flow cytometry,and Western blot were used to assess the changes in the IC50 of adri-amycin,cell proliferation,migration,invasion,apopto-sis,and the expression of related proteins.Results FTO was highly expressed in MDA-MB-231/ADR cells.After FTO knockdown,the IC50 value of adriamy-cin in MDA-MB-231/ADR cells decreased,and the a-bilities of proliferation,migration and invasion were weakened.In the FTO knockdown group,the expres-sion levels of Bax,cleaved-caspase3,GSK-3 β proteins and the apoptosis rate significantly increased,while the expression levels of Bcl-2,Wnt5a,β-catenin,c-myc,cyclin D1,and P-gp proteins decreased.Conclusion FTO may inhibit the apoptosis of MDA-MB-231/ADR cells through the Wnt/β-catenin signaling pathway,al-ter P-gp expression,and thereby enhance the resistance of MDA-MB-231/ADR cells to adriamycin.
7.Bioinformatics analysis and experimental verification of disulfidptosis-related genes in vascular dementia

Aim To examine the pathogenesis of disul-fide death gene in vascular dementia(VD)by bioin-formatics analysis of disulfide death differentially ex-pressed genes(DEGs)combined with experimental verification.Methods The death DEGs of disulfide were screened and their correlation was analyzed.The VD patients data in the data set were analyzed by clus-tering and typing and gene set variation.The clustering risk of DEGs was tested with a nomogram model,and the optimal learning model was predicted.After the es-tablishment of VD rat model,water maze test,HE stai-ning and RT-qPCR detection were performed to verify the results of health information.Results Four DEGs including SLC7A11 were obtained,which had antago-nistic or synergistic interaction with each other.The genetic data could be divided into two subtypes with significant differences.After typing,VD disulfide DEGs were mainly concentrated in GnRH signaling pathways.The accuracy of the nomogram prediction model was high.Generalized linear was the best ma-chine learning model.Compared with the sham opera-tion group,the escape latency of rats in the model group was prolonged,the number of crossing platforms decreased,the relative mRNA expression levels of Slc3a2 and Slc7a11 decreased,and LRPPRC in-creased.Conclusions SLC7A11 and other disulfide death DEGs and its related GnRH signaling pathway may be an important part of the pathogenesis of VD di-sulfide death.SLC3A2,LRPPRC and SLC7A11 can be used as characteristic genes in the regulation of VD by disulfide death,which may affect VD progression through the regulation of disulfide death.

Objective:To investigate the clinical efficacy of trimetazidine,dopamine combined with furosemide in eld-erly patients with rheumatic heart disease(RHD)and chronic heart failure(CHF)and its effect on the levels of me-teorin-like protein(Metrnl),soluble triggering receptor expressed on myeloid cells-1(sTREM-1)and syndecan-1.Methods:This randomized controlled study enrolled 106 elderly patients with RHD and CHF who were hospi-talized in the First People's Hospital of Xianyang between March 2022 and December 2023.Patients were divided in-to control group(n=53,routine treatment)and intervention group(n=53,additional trimetazidine,dopamine and furosemide combined treatment),and treated for 1 week.The clinical efficacy,left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDd),left ventricular end-systolic diameter(LVESd),6-min walking distance(6MWD),Minnesota living with heart failure questionnaire(MLHFQ)score,serum levels of B-type natriuretic peptide(BNP),Metrnl,sTREM-1 and syndecan-1,and incidence of adverse reactions during treatment were compared between the two groups.Binary Logistic regression was used to analyze the association be-tween influencing factors and therapeutic effect.Results:The total effective rate of intervention group was signifi-cantly higher than that of control group(96.23%vs.83.02%,P=0.026).Compared to patients in the control group after treatment,those in the intervention group had significantly lower LVEDd[(45.27±5.95)mm vs.(50.11±4.47)mm],LVESd[(34.85±4.19)mm vs.(40.93±4.77)mm],MLHFQ score[(34.51±4.47)points vs.(45.62±5.08)points],levels of BNP[(95.29±8.31)pg/ml vs.(145.39±25.71)pg/ml],sTREM-1](43.59±4.21)pg/ml vs.(50.61±5.48)pg/ml]and syndecan-1[(7.01±1.88)μg/L vs.(11.02±2.36)μg/L],and significantly higher LVEF[(58.14±4.02)%vs.(50.61±6.15)%],6MWD[(433.48±45.74)m vs.(386.51±31.25)m]and Metrnl level[(205.08±27.45)ng/L vs.(188.52±22.37)ng/L](P<0.001 all).Bi-nary Logistic regression analysis showed that the combination of trimetazidine,dopamine and furosemide was an in-dependent protective factor for the efficacy of elderly patients with RHD and CHF(OR=0.096,95%CI:0.011~0.837,P=0.034).There was no significant difference in the total incidence of adverse reactions between interven-tion group and control group(13.20%vs.11.32%,P=0.767).Conclusion:The combination of trimetazidine,do-pamine and furosemide could significantly improve cardiac function and exercise tolerance,reduce serum sTREM-1 and syndecan-1 levels,and increase Metrnl level with good safety in elderly patients with RHD and CHF.
9.Optimal strength of grip device exercise for arteriovenous fistula in patients with chronic kidney disease

Objective:To investigate the effects of different intensities of grip strength training on arteriovenous fistula (AVF) in patients with chronic kidney disease.Methods:Different intensities of grip strength training programs were developed through literature analysis, expert argumentation, and group discussion. From July to November 2024, 90 hospitalized patients with chronic kidney disease and AVF in the Department of Nephrology of the First Affiliated Hospital of Hunan Traditional Chinese Medical College were selected as study subjects using convenience sampling. Using the random number table method, study subjects were divided into group A, group B, and group C, with 30 cases in each group. Group A underwent low-intensity grip strength training, group B underwent moderate-intensity grip strength training, and group C underwent high-intensity grip strength training. The intervention period lasted six weeks. After intervention, the internal diameter of the cephalic vein, anastomotic blood flow, and incidence of complications were compared among three groups of patients with chronic kidney disease.Results:After intervention, the internal diameter of the cephalic vein in group C was larger than that in group A and group B ( P<0.05). The anastomotic blood flow in group B and group C was higher than that in group A ( P<0.05). There was no statistically significant difference in the incidence of complications among the three groups ( P>0.05) . Conclusions:Appropriately increasing the intensity of grip strength training has a positive effect on enlarging the internal diameter of the cephalic vein and enhancing anastomotic blood flow.
