Conversion therapy is a treatment strategy that shifts from palliative systemic therapy to curative surgical treatment for primary and/or metastatic stage IV gastric cancer (GC).To address its clinical statements, the Korean Gastric Cancer Association aims to present a consensus on conversion therapy among experts attending KINGCA WEEK 2024. The KINGCA Scientific Committee and Development Working Group for Korean Practice Guidelines prepared preformulated topics and 9 clinical statements for conversion therapy.The Delphi method was applied to a panel of 17 experts for consensus and opinions. The final comments were announced after the statement presentation and discussed during the consensus meeting session of KINGCA WEEK 2024. Most experts agreed that conversion herapy provides a survival benefit for selected patients who respond to systemic therapy and undergo R0 resection (88.3%). Patients with limited metastases were considered good candidates (94.2%). The optimal timing was based on the response to systemic therapy (70.6%). The regimen was recommended to be individualized (100%) and the duration to be at least 6 months (88.3%). A minimally invasive approach (82.3%) and D2 lymph node dissection (82.4%) were considered for surgery. However, resection for metastases with a complete clinical response after systemic therapy was not advocated (41.2%). All experts agreed on the need for large-scale randomized-controlled trials for further evidence (100%).Recent advancements in treatment may facilitate radical surgery for patients with stage IV GC. Further evidence is warranted to establish the safety and efficacy of conversion therapy.

Background: Barriers to treatment with intravenous thrombolysis (IVT) for patients with acute ischemic stroke (AIS) in South Korea remain incompletely characterized. We analyze a nationwide prospective cohort to determine patient-level features associated with delayed presentation and non-treatment of potential IVT-eligible patients. Methods: We identified consecutive patients with AIS from 01/2011 to 08/2023 from a multicenter and prospective acute stroke registry in Korea. Patients were defined as IVT candidates if they presented within 4.5 hours from the last known well, had no lab evidence of coagulopathy, and had National Institute of Health Stroke Scale (NIHSS) ≥ 4. Multivariable generalized linear mixed regression models were used to investigate the associations between their characteristics and the IVT candidates or the use of IVT among the candidates. Results: Among 84,103 AIS patients, 41.0% were female, with a mean age of 69 ± 13 years and presentation NIHSS of 4 [interquartile range, 1–8]. Out of these patients, 13,757 (16.4%) were eligible for IVT, of whom 8,179 (59.5%) received IVT. Female sex (adjusted risk ratio [RR], 0.90; 95% confidence interval [CI], 0.86–0.94) and lower years of education (adjusted RR, 0.90; 95% CI, 0.84–0.97 for 0–3 years, compared to ≥ 13 years) were associated with a decreased likelihood of presenting as eligible for IVT after AIS; meanwhile, young age (adjusted RR, 1.12; 95% CI, 1.01–1.24 for ≤ 44 years, compared to 75–84 years) was associated with an increased likelihood of being an IVT candidate. Among those who were eligible for IVT, only age was significantly associated with the use of IVT (adjusted RR, 1.09; 95% CI, 1.03–1.16 for age 65–74 and adjusted RR, 0.83; 95% CI, 0.76–0.90 for ≥ 85 years, respectively). Conclusion Most patients with AIS present outside IVT eligibility in South Korea, and only 60% of eligible patients were ultimately treated. We identified increased age, female sex and lower education as key features on which to focus interventions for improving IVT utilization.

The utilization of health insurance claims data has expanded significantly, enabling researchers to conduct epidemiological studies on a large scale. This review examines key statistical methods for addressing baseline differences and conducting cohort analyses using Korean National Health Insurance claims data. Propensity score matching and inverse probability of treatment weighting are widely used to mitigate selection bias and enhance causal inference in observational studies. These methods help improve study validity by balancing covariates between treatment and control groups. Additionally, survival analysis techniques, such as the Cox proportional hazards model, are essential for assessing time-toevent outcomes and estimating hazard ratios while accounting for censoring. However, the application of these statistical methods is accompanied by challenges, including unmeasured confounding, instability in weight estimation, and violations of model assumptions.To address these limitations, emerging approaches, such as Doubly robust estimation, machine learning-based causal inference, and the marginal structural model, have gained prominence. These techniques offer greater flexibility and robustness in real-world data analysis. Future research should focus on refining methodologies for integrating highdimensional health datasets and leveraging artificial intelligence to enhance predictive modeling and causal inference. Furthermore, the expansion of international collaborations and the adoption of standardized data models will facilitate large-scale multi-center studies.Ethical considerations, including data privacy and algorithmic transparency, should also be prioritized to ensure responsible data use. Maximizing the utility of health insurance claims data requires interdisciplinary collaboration, methodological advancements, and the implementation of rigorous statistical techniques to support evidence-based healthcare policy and improve public health outcomes.

Background/Aims: Rectal hyposensitivity (RH), as defined by the London Classification, has been linked to sensory dysfunction caused by diabetes mellitus and Parkinson’s disease (PD); however, its clinical interpretation has not been sufficiently validated. In this study, we aim to explore the correlations between rectal sensory thresholds and the clinical characteristics of patients with functional defecatory disorders. Methods: We reviewed data from patients who underwent high-resolution anorectal manometry and acquired their clinical characteristics using a standardized questionnaire. The associations between RH based on either 1 (borderline RH) or 2 (RH) abnormal rectal sensory thresholds and patients’ clinical and demographic characteristics were analyzed using linear and logistic regression models in the overall sex-stratified populations. Results: We enrolled 2540 patients, of whom 1046 (41.2%) were men. Overall, 150 (5.9%) patients were diagnosed with RH, whereas 422 (16.6%) had borderline RH. Multivariate linear regression analysis revealed that the Cleveland Clinic Constipation Score (CCCS) increased linearly with the increase in the number of abnormal rectal sensory thresholds (effect per threshold: 0.900 [standard deviation: 0.188]). Upon stratification by sex, borderline RH was positively associated with diabetes mellitus, PD, and CCCS (adjusted odds ratio [aOR] = 2.11, 95% confidence interval [1.08, 4.15]; aOR = 1.49 [1.03, 2.14]; aOR = 1.03 [1.01, 1.05], respectively) in women. However, RH was positively associated with only the CCCS. Conclusions Defining RH based on 1 or more abnormal sensory thresholds showed better clinical correlation with patient characteristics. However, further prospective studies are needed to validate these findings before proposing revisions to the current London classification criteria.

Objective: The Daegu region experienced the first wave of the pandemic at the beginning of the coronavirus disease 2019 (COVID-19) outbreak in Korea. Other non-COVID-19-related treatments during a community outbreak, such as cardiovascular diseases, were expected to impact emergency departments. In acute myocardial infarctions, time is an important factor affecting the patient outcome. This study examined how community COVID-19 outbreak affected STsegment elevated myocardial infarction (STEMI) care in emergency departments. Methods: A retrospective analysis was performed on patients visiting five emergency departments in the Daegu area who were diagnosed with STEMI from February 18 to April 17 each year from 2018 to 2020. The demographic characteristics, prehospital variables, in-hospital time variables, and treatment results were collected. The cases were divided into the pre-COVID period and the COVID period for comparison. Results: The study included 254 patients (194 pre-COVID, 60 during COVID). The symptom-to-door time did not differ. Although the door-to-first doctor time was shortened (4 min vs. 2 min, P=0.01), the rate of coronary angiogram along with the door-to-angiogram time and the door-to-balloon time did not change. The length of stay in the emergency department was delayed during COVID-19 (median, 136 min vs. 404 min; P<0.01). The in-hospital length of stay and mortality were similar in both groups. Conclusion The time to treat STEMI was not delayed significantly during the first wave of the COVID-19 outbreak in the Daegu area compared with the pre-pandemic period. Mortality did not change. The length of stay was elongated significantly in the emergency department but not in the hospital.

Objective: It has been 1 year since the start of the worldwide coronavirus disease-19 (COVID-19) pandemic. This study analyzed the indirect effects of COVID-19 on treating patients with non-infectious diseases by comparing the incidence of complicated appendicitis before and after the pandemic. Methods: The target group included patients aged at least 16 years diagnosed with acute appendicitis between February 23 and July 31, 2020. Patients diagnosed during the same period in 2019 were selected as the control group. The differences in the incidence of complicated appendicitis before and after COVID-19 were investigated, and the association with various variables was analyzed using the odds ratios and 95% confidence intervals (CIs). Results: The study included 120 subjects in 2019 (pre-COVID group) and 119 cases in 2020 (post-COVID group). The pre-COVID group included 25 cases (20.8%) of complicated appendicitis, while the post-COVID group included 48 cases (40.3%). The median time from symptom onset to visit (pre-hospital time) increased from 15 to 22 hours, and the median time from the visit to surgery (in-hospital time) increased from 7 to 11 hours. Multivariate regression analysis of the three variables revealed odds ratios (95% CIs) of pre-hospital time, erythrocyte sedimentation rate, and inclusion in the post-COVID group of 1.02 (1.01-1.02), 2.07 (1.11-3.86), and 2.15 (1.12-4.11), respectively. Conclusion The incidence of complicated appendicitis increased after the COVID-19 pandemic. Therefore, a healthcare system that can minimize the delay in treating non-infectious emergency patients is needed.

Background: Syphilis, an infectious and chronic disease primarily transmitted through sexual contact, is caused by the spirochete bacterium Treponema pallidum. Although the introduction of penicillin has significantly reduced its incidence, syphilis continues to have a substantial impact on public health. Objective: The purpose of this study was to investigate trends in syphilis and to analyze its epidemiologic, clinical, and serologic characteristics. Methods: We retrospectively reviewed the medical records of 330 patients who tested positive for the Venereal Disease Research Laboratory (VDRL) test, including 192 patients who received a diagnosis of syphilis and who visited the Dermatology Department of Gachon University Gil Medical Center between 2000 and 2023. Statistical analyses were performed to evaluate trends in syphilis incidence and related variable factors, such as sex, age, and serological results. Results: The average age of the 192 syphilis patients was 34.18±15.81 years, with a male-to-female ratio of 1:1.2. The most prevalent age group was 20∼29 years, comprising 71 patients (37.0%). Over time, the incidence of syphilis demonstrated a decreasing trend (p＜0.001). A total of 105 patients (54.7%) were diagnosed with symptomatic syphilis, with a male-to-female ratio of 1.5:1. The proportion of symptomatic syphilis patients tended to decrease with increasing age (p＜0.05). Maculopapular syphilis was the most frequently observed presentation in secondary syphilis (36.2%). The proportion of symptomatic syphilis patients significantly increased with higher VDRL titers (p＜0.001). Conclusion The incidence of syphilis has shown a consistent decline over the past two decades. However, the relatively higher prevalence among younger age groups indicates the need for continued public health efforts.

Regorafenib is an oral multikinase inhibitor used to treat advanced solid tumors, including hepatocellular carcinoma (HCC), gastrointestinal stromal tumors, and colorectal cancer. Adverse dermatological reactions are common after regorafenib treatment. The most frequent cutaneous adverse reaction is hand-foot skin reaction (HFSR). Compared to HFSR, reports on adverse reactions on the scrotum associated with regorafenib are limited. Only a few cases of adverse reactions on the scrotum associated with HFSR have been reported. We present the case of a 55-year-old male patient with a 1-year history of scrotal dermatitis and HFSR, who had been undergoing treatment for HCC.One year after administration of oral regorafenib (160 mg/d), multiple vesicles, pustules, desquamation, and hyperkeratosis developed on both hands and feet, accompanied by scaly erythematous plaques around the scrotum.The lesions improved after two weeks of treatment with topical and systemic steroids. Herein, we report a rare case of scrotal dermatitis associated with regorafenib-induced HFSR.

Anaplastic thyroid carcinoma (ATC) is a rare and highly aggressive malignancy typically associated with dismal outcomes. Most patients present with advanced disease, often precluding curative resection. We report a case of intrathyroidal ATC incidentally identified in a 72-year-old male who underwent diagnostic lobectomy for a low-suspicion, predominantly cystic huge thyroid nodule. Final pathology revealed a completely resected, intrathyroidal ATC without extrathyroidal extension. Postoperative imaging showed no evidence of residual or metastatic disease. Based on multidisciplinary discussion, the patient underwent completion thyroidectomy and central neck dissection without adjuvant therapy. No residual malignancy was identified, and the patient remains recurrence-free at six months. This case highlights that early-stage intrathyroidal ATC may present without overt malignant features and can be curatively treated with complete surgical excision. Careful evaluation and multidisciplinary decision-making are essential in managing rapidly enlarging thyroid nodules with atypical features.
