Purpose: We present a case of Uveitis-Glaucoma-Hyphema (UGH) syndrome secondary to intraocular lens (IOL) subluxation and associated Soemmering ring.Case summary: A 56-year-old female presented with recurrent episodes of right eye uveitis, vitritis, and vitreous hemorrhage, along with progressive vision loss that had not responded to treatment at a private clinic. On examination, her best-corrected visual acuity (BCVA) was 0.4, and intraocular pressure (IOP) was elevated at 34 mmHg. Slit-lamp and fundoscopic evaluation revealed a subluxated IOL and an adjacent Soemmering ring in contact with the iris, accompanied by anterior chamber inflammatory cells, hyphema, and vitreous hemorrhage. A diagnosis of UGH syndrome secondary to IOL subluxation was made. The patient underwent pars plana vitrectomy with the removal of the subluxated IOL and Soemmering ring, followed by scleral fixation of a new IOL using the Yamane technique. Postoperative recovery was favorable, with resolution of anterior chamber inflammation. At follow-up, BCVA had improved to 1.0, and IOP had normalized to 14 mmHg without the need for antiglaucoma medications. Conclusions UGH syndrome resulting from IOL subluxation and a Soemmering ring can be managed effectively via timely surgical intervention, restoring visual acuity and normalizing IOP.

Purpose: We evaluated the treatment outcomes of intravitreal 0.625% povidone-iodine (PI) injection in patients with exogenous and endogenous endophthalmitis. Methods: We retrospectively analyzed the medical records of 21 patients (23 eyes) who received intravitreal 0.625% PI injections for endophthalmitis between March 2021 and December 2023. Results: Of the 21 patients, 19 (19 eyes) had exogenous endophthalmitis, with the most common etiology being post-cataract surgery endophthalmitis in 14 eyes (58.3%). The mean number of intravitreal PI injections administered was 1.6 ± 0.7. The mean time to hypopyon resolution was 2.5 ± 2.1 days. Pars plana vitrectomy (PPV) was required in 16 eyes (84.2%), with an average of 1.1 ± 0.5 surgical interventions. The best-corrected visual acuity (BCVA, logMAR) improved significantly from 2.06 ± 0.52 before treatment to 1.00 ± 0.96 after treatment (p < 0.001). Four eyes of two patients with endogenous endophthalmitis received a mean of 2.5 ± 1.0 intravitreal PI injections (range: 2-4). Clinical improvement was observed in all cases. Conclusions Intravitreal 0.625% PI injection appears to be a potentially effective alternative to conventional antibiotic therapy for the treatment of endophthalmitis.

Objective: Thoracic pedicle screw fixation is technically demanding because of the natural variability in pedicle anatomy, a challenge that is further exacerbated in patients with spinal tumors who often have compromised pedicles. The L-point entry, juxtapedicular, and endplate-parallel trajectory (L-JET) technique was developed to provide a uniform entry point, a predictable screw trajectory, and the capacity to accommodate larger-diameter screws at levels T3–10. This study aimed to evaluate the safety and feasibility of the L-JET technique in comparison with the conventional transpedicular method. Methods: A comparative analysis of consecutively collected data was performed to evaluate outcomes associated with the L-JET technique in patients undergoing surgery for thoracic extradural spinal tumors. The L-point was defined as the intersection of a vertical line along the lateral margin of the facet joint and a horizontal line along the upper edge of the transverse process. The screw trajectory was planned with 30° of medial convergence in the axial plane, creating a juxtapedicular path, and a straightforward trajectory in the sagittal plane. Primary outcome measures included screw diameter, screw length, cortical breach rate, and the need for revision surgery, as assessed using computed tomography scans and medical records. Results: A total of 108 screws were placed in 22 patients using the L-JET technique, while 98 screws were placed in 18 patients using the conventional technique. The L-JET group used significantly larger-diameter screws (6.06±0.65 mm) than the conventional group (5.74±0.80 mm, p=0.02), with no significant difference observed in screw length between groups. Medial cortical breach occurred in one screw in each group, and no supra- or infrapedicular breaches were identified. No screw-related neurovascular complications or revision surgeries occurred in either group. Conclusion The L-JET technique enables consistent screw placement with larger-diameter screws, even in compromised pedicles associated with thoracic spinal tumors, without increasing the risk of complications or the need for revision surgery.

Purpose: The aim of this study is to summarize cancer risk among patients with clinical indications of immunosuppressive and antineoplastic drugs in Korea, which are pharmaceuticals defined as group 1 by International Agency for Research on Cancer. Materials and Methods: We conducted a nationwide population-based retrospective cohort study using the Korean National Health Insurance Service claims data from 2002 to 2018. Patients with clinical indications for group 1 pharmaceuticals from 2002 to 2017 were selected as baseline population, and followed up until 2018. Cox proportional hazards regression model was used to analyze the risk of cancer and dose-response relationship between group 1 pharmaceuticals and cancer. Results: Azathioprine use increased the risk of skin and hematologic cancer (hazard ratio [HR], 4.63; 95% confidence interval [CI], 2.91 to 7.39 and HR, 3.15; 95% CI, 2.41 to 4.13). Cyclosporine use increased the risk of skin and hematologic cancer (HR, 2.30; 95% CI, 1.79 to 2.95 and HR, 2.96; 95% CI, 2.59 to 3.40). Cyclophosphamide use increased the risk of bladder and hematologic cancer (HR, 2.69; 95% CI, 1.92 to 3.78 and HR, 3.83; 95% CI, 3.20 to 4.59). Chlorambucil use increased the risk of hematologic cancer (HR, 3.51; 95% CI, 2.53 to 4.87) and melphalan use increased the risk of hematologic cancer (HR, 16.31; 95% CI, 13.41 to 19.85). Methoxsalen use increased the risk of skin cancer (HR, 2.32; 95% CI, 1.36 to 3.95). Conclusion Group 1 pharmaceuticals were associated with increased risk of cancer. The results are expected to help establish alternative clinical strategies and policies for patients with clinical indications of group 1 pharmaceuticals, by continuous risk analysis and discussions on the surveillance systems.

SUBJECTS/METHODS: A total of 419 breast cancer survivors were included in this crosssectional study. We considered 30 plasma metabolites, quantified by high-throughput nuclear magnetic resonance metabolomics. Clusters were obtained based on metabolites using 4 different unsupervised clustering methods: k-means (KM), partitioning around medoids (PAM), self-organizing maps (SOM), and hierarchical agglomerative clustering (HAC). The t-test, χ2 test, and Fisher’s exact test were used to compare sociodemographic, lifestyle, clinical, and dietary characteristics across the clusters. P-values were adjusted through a false discovery rate (FDR). RESULTS: Two clusters were identified using the 4 methods. Participants in cluster 2 had lower concentrations of apolipoprotein A1 and large high-density lipoprotein (HDL) particles and smaller HDL particle sizes, but higher concentrations of chylomicrons and extremely large very-low-density-lipoprotein (VLDL) particles and glycoprotein acetyls, a higher ratio of monounsaturated fatty acids to total fatty acids, and larger VLDL particle sizes compared with cluster 1. Body mass index was significantly higher in cluster 2 compared with cluster 1 (FDR adjusted-PKM < 0.001; PPAM = 0.001; PSOM < 0.001; and PHAC = 0.043). CONCLUSION The breast cancer survivors clustered on the basis of plasma metabolites had distinct characteristics. Further prospective studies are needed to investigate the associations between metabolites, obesity, dietary factors, and breast cancer prognosis.

Background/Aims: Atrial fibrillation (AF) significantly contributes to global morbidity and mortality. Paroxysmal atrial fibrillation (PAF) is particularly common among patients with cryptogenic strokes or transient ischemic attacks and has a silent nature. This study aims to develop reliable artificial intelligence (AI) algorithms to detect early signs of AF in patients with normal sinus rhythm (NSR) using a 12-lead electrocardiogram (ECG). Methods: Between 2013 and 2020, 552,372 ECG traces from 318,321 patients were collected and split into training (n = 331,422), validation (n = 110,475), and test sets (n = 110,475). Deep neural networks were then trained to predict AF onset within one month of NSR. Model performance was evaluated using the area under the receiver operating characteristic curve (AUROC). An explainable AI technique was employed to identify the inference evidence underlying the predictions of deep learning models. Results: The AUROC for early diagnosis of PAF was 0.905 ± 0.007. The findings reveal that the vicinity of the T wave, including the ST segment and S-peak, significantly influences the ability of the trained neural network to diagnose PAF. Additionally, comparing the summarized ECG in NSR with those in PAF revealed that nonspecific ST-T abnormalities and inverted T waves were associated with PAF. Conclusions Deep learning can predict AF onset from NSR while detecting key features that influence decisions. This suggests that identifying undetected AF may serve as a predictive tool for PAF screening, offering valuable insights into cardiac dysfunction and stroke risk.

Purpose: To report a case of macular serous retinal detachment and macular thinning associated with optic disc pit maculopathy successfully treated with vitrectomy, fovea-sparing internal limiting membrane (ILM) peeling, and inverted ILM flap technique.Case summary: A 27-year-old male patient presented with a 1-month history of decreased right eye vision. Corrected visual acuity in the right eye was 0.025 at initial visit. Fundus examination revealed an oval, deep-seated defect in the inferotemporal portion of the optic nerve and a chorioretinal coloboma located inferior to the optic nerve with a one-third disc diameter. Optical coherence tomography (OCT) showed severe macular serous detachment and central foveal thinning. Surgical treatment comprised vitrectomy, ILM peeling preserving an amount of ILM equivalent to the diameter of one disc in the central fovea and covering the optic disc pit with an inverted ILM flap from its nasal portion. The flap was fixed with a dispersible viscoelastic material and intravitreal 20% sulfur hexafluoride gas injection. The patient was maintained in a facedown position for 3 days postoperatively. After 15 months, the best-corrected visual acuity of the right eye improved to 0.63. Repeat OCT revealed resolution of retinoschisis and serous retinal detachment, with the ILM flap effectively covering the optic disc pit. Conclusions Fovea-sparing ILM peeling with an inverted ILM flap on the nasal side over the optic disc pit in patients with thin inner retinal layers and excessive serous retinal detachment effectively prevent the occurrence of macular holes and treat optic disc pit maculopathy.
