Acquired dermal melanocytosis, characterized by the presence of melanocytes in the dermis, varies in the timing and location of occurrence. While congenital cases, such as Mongolian spots, nevus of Ota, nevus of Ito, and blue nevus, are common, acquired cases also occur. Acquired bilateral nevus of Ota-like macules is relatively common in Asians;however, extrafacial cases, particularly those involving both the face and extremities, are extremely rare. We report the case of a 66-year-old man with symmetric pigmented patches on the scalp, face, shoulders, and dorsa of the hands for 9 months, following the onset of spots on the forehead and nose that progressively spread. Notably, nasal ala pigmentation was confirmed. Histopathological examination revealed scattered melanin-containing dendritic cells in the upper dermis. Based on the clinicopathological findings, he was diagnosed with acquired dermal melanocytosis of the face and extremities, representing a rare case.

Pinitol (3-O-Methyl-D-chiro-inositol) has been reported to possess insulin-like effects and is known as one of the anti-diabetic agents to improve muscle, liver, and endothelial cells. However, the beneficial effects of pinitol on the skin are not well known.Here, we investigated whether pinitol had effects on human dermal fibroblasts (HDFs), and human dermal equivalents (HDEs) irradiated with ultraviolet A (UVA), which causes various damages including photodamage in the skin. We observed that pinitol enhanced wound healing in UVA-damaged HDFs. We also found that pinitol significantly antagonized the UVA-induced up-regulation of matrix metalloproteinase 1 (MMP1), and the UVA-induced down-regulation of collagen type I and tissue inhibitor of metalloproteinases 1 (TIMP1) in HDEs. Electron microscopy analysis also revealed that pinitol remarkably increased the number of collagen fibrils with regular banding patterns in the dermis of UVA-irradiated human skin equivalents. Pinitol significantly reversed the UVAinduced phosphorylation levels of ERK and JNK but not p38, suggesting that this regulation may be the mechanism underlying the pinitol-mediated effects on UVA-irradiated HDEs. We also observed that pinitol specifically increased Smad3 phosphorylation, which is representative of the TGF-β signaling pathway for collagen synthesis. These data suggest that pinitol exerts several beneficial effects on UVA-induced damaged skin and can be used as a therapeutic agent to improve skin-related diseases.

Background: A healthcare system’s collapse due to a pandemic, such as the coronavirus disease 2019 (COVID-19), can expose healthcare workers (HCWs) to various mental health problems. This study aimed to investigate the impact of the COVID-19 pandemic on the depression and anxiety of HCWs. Methods: A nationwide questionnaire-based survey was conducted on HCWs who worked in healthcare facilities and public health centers in Korea in December 2020. Patient Health Questionnaire-9 (PHQ-9) and Generalized Anxiety Disorder-7 (GAD-7) were used to measure depression and anxiety. To investigate factors associated with depression and anxiety, stepwise multiple logistic regression analysis was performed. Results: A total of 1,425 participating HCWs were included. The mean depression score (PHQ-9) of HCWs before and after COVID-19 increased from 2.37 to 5.39, and the mean anxiety score (GAD-7) increased from 1.41 to 3.41. The proportion of HCWs with moderate to severe depression (PHQ-9 ≥ 10) increased from 3.8% before COVID-19 to 19.5% after COVID-19, whereas that of HCWs with moderate to severe anxiety (GAD-7 ≥ 10) increased from 2.0% to 10.1%. In our study, insomnia, chronic fatigue symptoms and physical symptoms after COVID-19, anxiety score (GAD-7) after COVID-19, living alone, and exhaustion were positively correlated with depression. Furthermore, post-traumatic stress symptoms, stress score (Global Assessment of Recent Stress), depression score (PHQ-9) after COVID-19, and exhaustion were positively correlated with anxiety. Conclusion In Korea, during the COVID-19 pandemic, HCWs commonly suffered from mental health problems, including depression and anxiety. Regularly checking the physical and mental health problems of HCWs during the COVID-19 pandemic is crucial, and social support and strategy are needed to reduce the heavy workload and psychological distress of HCWs.

Objective: A wireless electroencephalography (EEG) headset was applied to patients with an unclear etiology of an altered mental status suspected of non-convulsive seizures in the emergency department. Methods: The study included patients who presented to the emergency department with an altered mental status from January 1, 2020, to April 30, 2023. The patients with abnormal findings in brain imaging or those with typical seizure movement were excluded. Real-time portable continuous EEG monitoring was conducted on patients with persistent altered consciousness despite receiving conservative treatments and showing improvement in vital signs and blood tests within 3 hours. The baseline characteristics, degree of consciousness alteration and neurological symptoms, initial clinical diagnosis, EEG findings, and medications used in cases where epileptiform discharges were examined. The clinical outcomes were analyzed retrospectively. Results: Among 26 patients, six patients showed epileptiform discharges. The final diagnoses included uremic encephalopathy (n=2), septic encephalopathy due to pneumonia (n=1), urinary tract infection (n=1), diabetic ketoacidosis (n=1), and mental change only (n=1). The associated symptoms were myoclonus in three patients and abnormal eye movements in two patients. Conclusion In cases where the causes of altered mental status remain unexplained despite appropriate evaluations and treatments, particularly when accompanied by myoclonus or abnormal eye movements, it is crucial to consider the possibility of non-convulsive seizures and promptly perform an EEG to differentiate and diagnose the underlying condition, even if initial tests and treatments have been performed.
