1.Clinical application and evaluation of rapid detection of carbapenem resistance genes in blood cultures by Xpert Carba-R

This work aims to evaluate a rapid detection method of carbapenem resistance genes in blood cultures based on Xpert Carba-R and preliminarily evaluate its clinical application.Methods:Sixteen strains of Enterobacterales carrying different carbapenem resistance genes were selected to prepare simulated positive blood culture samples and Xpert Carba-R was used to directly detect carbapenem resistance genes in the simulated positive blood culture. From January 2022 to June, a prospective study was conducted on a total of 117 Enterobacteriaceae-positive blood culture samples in the First Affiliated Hospital of Nanjing Medical University. Xpert Carba-R, detecting five kinds of carbapenem resistance genes in these samples, was evaluated in sensitivity and specificity compared to polymerase chain reaction sequencing. Meanwhile clinical data of positive patients was collected for prognostic analysis. Results:Of the 16 simulated specimens, 14 strains had carbapenem resistance genes detected by Xpert Carba-R, including 8 bla KPC, 5 bla NDM and 1 bla IMP, showing 100% agreement with the known results. As of the 117 clinical specimens, 28 cases were determined to be Enterobacterales harboring carbapenem resistance genes, including 24 bla KPC, 2 bla NDM and 2 bla KPC+ bla NDM. In comparison to the PCR sequencing, the sensitivity and specificity of Xpert Carba-R were both 100% for blood culture samples, and furthermore, the detection time was significantly reduced. Of the 25 positive patients, 9 cases were treated with monotherapy and 5 cases were effective, other 16 cases received combined treatment and 12 cases were effective. A total of 17 cases were effective, 8 cases were ineffective and 3 of them died, the mortality rate was 12% (3/25). Conclusion:Xpert Carba-R can rapidly and accurately detect carbapenem resistance genes in blood culture, which can provide evidence for rational drug therapy in early clinical stage.
2.Effect of esketamine on cerebral ischemia-reperfusion injury and association with mitochondrial stress in mice

Objective:To evaluate the effect of esketamine on cerebral ischemia-reperfusion (I/R) injury and the association with mitochondrial stress in mice.Methods:The experiment was performed in two parts. Part Ⅰ Eighteen SPF male C57BL/6 mice, aged 8-12 weeks, with body mass index of 28-30 g, were divided into 3 groups ( n=6 each) by a random number table method: sham operation group (S group), cerebral I/R group (IR group), and esketamine plus cerebral I/R group (E+ IR group). Cerebral I/R was produced by occlusion of middle cerebral artery for 1 h followed by 24-h reperfusion in anesthetized mice.Esketamine 10 mg/kg was intraperitoneally injected at 20 min before developing the model in E group. Neurological function was evaluated using the Zea Longa score and balance beam test (Feeney score). The cerebral infarct size was determined by TTC staining. Part Ⅱ Primary cortical neurons were isolated and cultured and then divided into 3 groups ( n=42 each) using a random number table method: control group (group C), oxygen-glucose deprivation-reoxygenation (OGD/R) group, and esketamine plus OGD/R group (group E+ OGD/R). Cells were subjected to O 2-glucose deprivation for 1 h followed by restoration of O 2-glucose supply for 24 h. The cells were treated with 25 μmol/L esketamine for 40 min before preparing the model in E+ OGD/R group. The neuronal viability was measured by the CCK-8 assay. The ultrastructure of neurons was observed with a transmission electron microscope. The levels of reactive oxygen species (ROS), glutathione peroxidase (GSH-px) and malondialdehyde (MDA) were determined, and the mitochondrial membrane potential was determined by JC-1 kit. The neuronal apoptosis was detected by TUNEL staining, and the apoptosis rate of neurons was calculated. The expression of Bax, cytochrome C (CytC), cleaved-caspase-9, caspase-3 and cleaved-caspase-3 was detected by Western blot. Results:Part Ⅰ Compared with S group, the Zea Longa score, Feeney score and cerebral infarct size were significantly increased in IR group ( P<0.01). Compared with IR group, the Zea Longa score, Feeney score and cerebral infarct size were significantly decreased in E+ IR group ( P<0.01). Part Ⅱ Compared with C group, the cell viability and activity of GSH-px were significantly decreased, the apoptosis rate of neurons, levels of ROS and MDA, mitochondrial membrane potential, and cleaved-caspase-3/caspase-3 ratio were increased, and the expression of Bax, Cyt C and cleaved-caspase-9 was up-regulated in OGD/R group ( P<0.01). Compared with OGD/R group, the cell viability and activity of GSH-px were significantly increased, the apoptosis rate of neurons, levels of ROS and MDA, mitochondrial membrane potential, and cleaved-caspase-3/caspase-3 ratio were decreased, and the expression of Bax, Cyt C and cleaved-caspase-9 was down-regulated in E+ OGD/R group ( P<0.01). Conclusions:Esketamine can alleviate cerebral I/R injury in mice, and the mechanism may be related to inhibition of mitochondrial stress in neurons, improvement in mitochondrial function, and inhibition of mitochondria-dependent apoptosis in neurons.
3.Clinical characteristics of patients with puerperal schizophrenia

BackgroundWomen may develop severe symptoms of stress disorder following childbirth, which may be exposed to a risk of developing mental health problems, and even lead to the recurrence of the illness in female patients with schizophrenia, while comparatively limited research has been undertaken concerning the clinical characteristics and treatment of puerperal schizophrenia in China. ObjectiveTo explore the clinical characteristics of puerperal schizophrenia, so as to provide references for the clinical treatment. MethodsA total of 24 patients with puerperal schizophrenia who were hospitalized in the female ward of adult psychiatry department of the Affiliated Brain Hospital of Guangzhou Medical University from 2012 to 2020 and met the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV) diagnostic criteria for schizophrenia were included as puerperal group. Another 48 non-puerperal women with schizophrenia were concurrently enrolled as control group. Then the basic data, scores on Positive and Negative Symptom Scale (PANSS) and the discharge medication were recorded. ResultsThe percentages of newly onset and positive family history of psychosis in puerperal group were larger than those in control group, with statistical significance (χ2=9.321, 5.240, P<0.05 or 0.01). Puerperal group scored higher on PANSS excitement factor (t=-2.220, P<0.05) and lower on negative factor (t=3.377, P<0.01) compared with control group. In terms of discharge medication, puerperal group reported a higher dosage of antipsychotic drugs (t=-2.095, P<0.05), and a larger proportion of combined use of benzodiazepines or antidepressants (χ²=21.316, 5.114, P<0.05 or 0.01) compared with control group, with statistical significance. ConclusionPatients with puerperal schizophrenia display increased ratings of excitement symptoms and decreased ratings of negative symptoms, which necessitates the use of high doses of antipsychotic drugs, and combined use of benzodiazepines and antidepressants.
4.Application of serum tumor specific protein 70 for prognostic stratification in acute myeloid leukemia

Objective:To assess the value of serum tumor specific protein 70 (SP70) for prognostic stratification in acute myeloid leukemia (AML).Methods:A cohort study design was adopted. 129 newly diagnosed AML patients from September 2022 to January 2024 at the Hematology Department of the First Affiliated Hospital of Nanjing Medical University were included, as well as a control group consisted of 120 healthy individuals and 7 cases with benign hematologic diseases during the same period (total 127 cases). Clinical data were collected from Electronic Medical Records. According to the 2023 edition of the Chinese Leukemia Diagnosis and Treatment Guidelines, AML patients with good or moderate prognosis were categorized as low-to-intermediate risk, while those with poor prognosis were high-risk group. Univariate and multivariate logistic regression analyses were used to identify variables significantly associated with AML prognostic risk. ROC analysis was used to evaluate diagnostic performance. A nomogram for predicting patient prognostic risk was constructed by R 4.0.2 software, and the internal validation was performed using bootstrapping.Results:Among 129 AML patients, there were 71 males (55.0%) and 58 females (45.0%), with 42 (32.6%) classified as high-risk and 87 (67.4%) as low-intermediate risk. The high-risk group had a significantly higher median age [62 (48, 67) years] compared to the low-intermediate risk group [50 (35, 63) years, Z=-2.381, P=0.017], and a significantly higher proportion of males (30 patients, 71.4%) compared to the low-intermediate risk group (41 patients, 47.1%, χ 2=6.760, P=0.009). Multivariate logistic regression analysis indicated that serum SP70 ( OR=2.54, 95% CI: 1.68-3.84, P<0.001), hemoglobin (HB) ( OR=0.96, 95% CI: 0.93-0.99, P<0.05), and bone marrow blast (BM blast) ( OR=1.07, 95% CI: 1.02-1.13, P<0.05) were independent risk factors for high-risk prognosis in AML patients. ROC analysis showed that the area under the curve (AUC) for SP70 predicting high-risk patients was 0.908 (cut-off value of 5.74 ng/ml, 95% CI: 0.845-0.952, sensitivity 90.5%, specificity 82.8%). The combined model of serum SP70, HB, and BM blasts had an AUC of 0.931 (95% CI: 0.890-0.973); C-index=0.925 (95% CI: 0.876-0.963),with no statistically significant difference compared to serum SP70 alone ( Z=1.693, P>0.05). Conclusion:Serum SP70 may be a promising non-invasive molecular biomarker for prognostic stratification in AML.

6.CT-guided hook-wire localization of ≤10 mm pulmonary ground-glass nodules via different path ways before video-assisted thoracoscopic surgery:a comparative study

Objective To compare the clinical safety and efficacy of CT-guided hook-wire localization of≤10mm pulmonary ground-glass nodule(GGN)via different path ways before video-assisted thoracoscopic surgery(VATS).Methods The clinical data of a total of 128 patients with 10 mm pulmonary GGN,who received CT-guided hook wire localization before VATS at The Third Hospital of Mianyang of China between July 2018 and March 2023,were retrospectively analyzed.According to the puncturing localization path way mode,the patients were divided into vertical puncturing group(n=88)and non-vertical puncturing group(n=40).The number of puncturing times,the time spent for puncturing localization,the success rate of puncture,the operation time of VATS,and puncture-related complications of the two groups were recorded.Results No statistically significant differences in the gender,age,smoking history,GGN location,puncture position,nodule size,density characteristics of GGN,emphysema,and nodules-pleura distance existed between the two groups(all P>0.05).Compared with non-vertical puncturing group,in vertical puncturing group the number of puncturing times was smaller,the time spent for localization was shorter,the incidence of pneumothorax was lower,and the operation time of VATS was shorter,the differences in all the above indexes between the two groups were statistically significant(all P<0.05);and the subgroup analysis of patients whose GGN was overlapped with rib shadow obtained the same results.Binary logistic regression analysis revealed that non-vertical puncturing and the number of puncturing times were the independent risk factors for the occurrence of pneumothorax.Conclusion CT-guided hook-wire localization of≤10mm pulmonary GGN before VATS is clinically safe and effective.Under the condition when the lesion can be localized within the range of 2.0cm and the shadow overlapping of GGN with the rib and blood vessel can be effectively avoided,vertical puncturing path way mode should be preferred,which can effectively reduce the incidence of pneumothorax and shorten the operation time of VATS.
7.Epidemiological and clinical characteristics of 101 patients with brucellosis

Objective:To analyze the epidemiological and clinical characteristics of patients with brucellosis.Methods:Medical records of confirmed brucellosis patients ( n = 101) were collected from January 2015 to December 2022 at the First Affiliated Hospital of Nanjing Medical University. Patients were divided into acute phase group (≤3 months, n = 89) and non acute phase group (> 3 months, n = 12) according to the course of the disease. The epidemiological characteristics, clinical symptoms, laboratory indicators, treatment plan and curative effect of the patients were analyzed retrospectively. Results:Data of a total of 101 cases of brucellosis were collected, including 72 males and 29 females. The disease occurred throughout the year, with summer (June to August) being the peak period (43.56%, 44/101); 72.28% (73/101) cases had a clear history of contact with animal. Ninety-two point zero eight percent (93/101) of the patients visited the Department of Infectious Diseases for the first time. Clinical manifestations included fever, accounting for 82.18% (83/101), chills accounting for 36.63% (37/101), backache accounting for 33.66% (34/101), night sweats accounting for 22.77% (23/101), and arthralgia accounting for 20.79% (21/101). The symptoms of backache (75.00%, 9/12) and arthralgia (41.67%, 5/12) were more common in patients in the non acute phase group than those of the acute phase group [28.09% (25/89), 17.98% (16/89), P < 0.05]. The most common laboratory test abnormal items were elevated C-reactive protein (CRP, 68.32%, 69/101), erythrocyte sedimentation rate (ESR, 61.39%, 62/101), aspartate aminotransferase (AST, 58.42%, 59/101), and alanine aminotransferase (ALT, 48.51%, 49/101). Elevated ESR (66.29%, 59/89) was more common in patients in the acute phase group than that of the non acute phase group [25.00 (3/12), χ 2 = 7.48, P = 0.006]. All patients were treated with a combination therapy, with a recovery rate of 100.00% (101/101). Conclusions:Brucellosis patients are more common in males, with a higher incidence in summer and often accompanied by a history of contact with animal. The clinical manifestations are diverse and non-specific.
8.Effects of mind mapping based on evidence-based theme in health education of patients undergoing thoracentesis and drainage

Objective:To explore the effect of mind mapping based on evidence-based theme in health education of patients undergoing thoracentesis and drainage.Methods:From September 2020 to December 2021, 78 patients with pleural effusion and thoracentesis drainage diagnosed by infection in Beijing Chaoyang Hospital affiliated to Capital Medical University were selected by convenience sampling as the research object. The patients from September 2020 to March 2021 were set as the control group, and the patients from May to December 2021 were set as the observation group, with 39 cases each. The control group was given traditional health education, while the observation group received evidence-based health education with mind mapping. The health education knowledge examination score, self-care ability, adverse event rate and nursing satisfaction were compared between the two groups.Results:The scores of health education knowledge examination in the observation group were higher than those in the control group before , 24 hours after catheterization and before decannulation, with statistically significant differences ( P<0.01) . The total score and each dimension score of self-care ability in the observation group were higher than those in the control group, with statistically significant differences ( P<0.01) . The incidence of adverse events in the observation group was lower than that in the control group, and the score of Nursing Satisfaction Questionnaire was higher than that in the control group, with statistical differences ( P<0.05) . Conclusions:The mind mapping on the basis of evidence-based themes can effectively improve patients' health education knowledge and self-care ability, reduce the incidence of adverse events, and increase nursing satisfaction.
9.Effect of Danggui Shaoyaosan on Improvement of Cognitive Ability of SAMP8 Mice and Its Mechanism via Regulating Ubiquitin Proteasome Pathway

ObjectiveTo investigate the mechanism of Danggui Shaoyaosan (DSS) in the improvement of the cognitive ability of SAMP8 mice with Alzheimer's disease (AD) via regulating the ubiquitin-proteasome pathway (UPP). MethodFifteen SAMR1 mice were used as a normal group, and 60 SAMP8 mice were randomly divided into a model group and DSS high, medium, and low-dose groups (57.6, 28.8, and 14.4 g·kg-1·d-1), with 15 mice in each group. Intragastric administration was conducted for eight continuous weeks. Place navigation and spatial capacity were evaluated by Morris water maze. Pathological structure changes in neurons in the hippocampal CA1 area was detected by hematoxylin-eosin (HE) staining. The protein expression levels of hippocampal β-amyloid protein(Aβ) and phosphorylation(p)-Tau were determined by immunohistochemical staining (IHC) and enzyme-linked immunosorbent assay (ELISA), and the mRNA and protein expression levels of hippocampal ubiquitin (Ub), ubiquitin ligase E3 (E3), 26S proteasome, ubiquitin carboxyl terminal hydrolase-1 (UCHL1), and UCHL3 were determined by real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively. ResultAs compared with the normal group, the escape latency was prolonged in the model group (P<0.05) with the reduced number of crossing platform quadrants and time ratio in the platform quadrant (P<0.05). The model group decreased neurons and condensed cell bodies in the CA1 area, and increased β-amyloid precursor protein (β-APP) and p-Tau positive cells (P<0.05). In the model group, the protein expression levels of Aβ and p-Tau were increased (P<0.05), the mRNA and protein expression levels of Ub were increased (P<0.05), and the mRNA and protein expression levels of E3, 26S proteasome, UCHL1, and UCHL3 were decreased (P<0.05). As compared with the model group, the escape latency was shortened in the DSS high and medium-dose groups (P<0.05) with an increased number of crossing platform quadrants and residence time ratio (P<0.05). The pathological changes in CA1 of each DSS group were significantly improved, and the number of β-APP positive staining cells decreased (P<0.05). The number of p-Tau positive staining cells decreased in the DDS medium and low-dose groups (P<0.05). The protein expression levels of Aβ and p-Tau in each DDS group decreased (P<0.05), and the mRNA expression level of Ub in each group decreased (P <0.05). The mRNA expression levels of 26S, E3, and UCHL3 in the DDS high and medium-dose groups increased (P<0.05), and the mRNA expression level of UCHL1 in the DDS medium-dose group increased (P<0.05). The protein expression level of Ub in each DDS group decreased, and the protein expression levels of 26S, E3, UCHL1+3 in the DDS high and medium-dose groups increased (P<0.05). ConclusionDSS can improve the cognitive ability of SAMP8 mice, and its mechanism may be related to the reduction of the abnormal deposition of Aβ and p-Tau via decreasing the expression of Ub and increasing that of E3, 26S, UCHL1, and UCHL3 in the UPP.
10.Protective Effect of Danggui Shaoyaosan-contained Serum on Aβ1-40-injured PC12 Cells via Regulating UPP and Its Mechanism

ObjectiveTo investigate the protective effect of Danggui Shaoyaosan (DSS)-contained serum on β-amyloid (Aβ)1-40-injured rat adrenal pheochromocytoma PC12 cells and its mechanism in regulating ubiquitin-proteasome pathway (UPP). MethodAβ1-40 was used to intervene PC12 cells to prepare the cell models of Alzheimer's disease (AD), and the experiment was divided into the blank, model, and DSS-contained serum high, medium, and low-dose groups (10%, 5%, and 2.5%). Cell viability and apoptosis were detected using cell counting kit-8 (CCK-8) method and flow cytometry, respectively. The content of Aβ and p-Tau protein was determined by enzyme-linked immunosorbent assay (ELISA). The ubiquitin (Ub), ubiquitin ligase E3 (E3), 26S proteasome, ubiquitin carboxyl terminal hydrolase1 (UCHL1), and UCHL3 protein expressions of UPP were displayed using immunofluorescence cytochemistry (ICC), and the mRNA and protein expression levels of Ub, E3-parkin, 26S, UCHL1, and UCHL3 were determined by real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively. ResultThe data of the CCK8 experiment verified that 5 μmol·L-1 and 48 hours were the optimal conditions for modeling Aβ1-40-injured PC12 cells. As compared with the blank group, the cell viability rate in the model group decreased (P<0.05) with an increased apoptosis rate (P<0.05), the content of Aβ and p-Tau contents was elevated (P<0.05), the mRNA and protein expression levels of Ub increased, and the mRNA and protein expression levels of 26S, E3, and UCHL1+3 decreased (P<0.05). As compared with the model group, the cell viability rate in the DSS-contained medium-dose group increased (P<0.05), whereas the apoptosis rate in each DSS-contained group decreased (P<0.05). The content of Aβ in each DDS-contained group decreased (P<0.05), and the content of p-Tau in the DDS-contained high and medium-dose groups decreased (P<0.05). The mRNA expression level of Ub decreased, and that of 26S increased in each DDS-contained group (P<0.05). The mRNA expression level of UCHL1 in the DDS-contained medium-dose group increased (P<0.05), and the mRNA expression levels of E3 and UCHL 3 in the DDS-contained high and medium-dose groups increased (P<0.05). The protein expression level of Ub in each DDS-contained group decreased, and the protein expression levels of 26S, E3, and UCHL1+3 in the DDS high and medium-dose groups increased. The DSS-contained serum medium-dose group exerted the optimal effect. ConclusionDSS-contained serum can increase cell viability rate, reduce cell apoptosis rate, eliminate Aβ and p-Tau protein deposits, and exert protective effects on Aβ1-40-injured PC12 cells. Its mechanism may involve UPP via decreasing the expression of Ub and increasing that of 26S, E3, UCHL1, and UCHL3.
