ObjectiveTo explore the possible mechanism of Danggui Sini Granules (当归四逆颗粒) in treatment of coronary heart disease with cold congealing and blood stasis syndrome. MethodsFifty SD rats were randomly divided into a blank group, a model group, a Danshen Pill (丹参滴丸) group, and a low- and high-dosage Danggui Sini Granules group, with 10 rats in each group. Except for the blank group, the rat model of coronary heart disease with cold congealing and blood stasis syndrome was established by repeated cold stimulation at low temperature combined with intraperitoneal injection of posterior pituitary hormone in all other groups. In the 6th week of modelling, 0.073 g·kg-1·d-1 of compound Danshen Pill was given to the Danshen Pill group, 20.2 and 40.4 g·kg-1·d-1 of Danggui Sini Granules were given to the low- and high-dose Danggui Sini Granules groups, respectively, and 0.2 ml/10 g of sterile water was given to the blank group and the model group, all for 2 consecutive weeks. The general conditions of the rats were recorded, and the body mass was compared weekly. At the end of the intervention, electrocardiogram, blood rheological indexes, including whole blood low-cut viscosity, whole blood high-cut viscosity, erythrocyte aggregation index, and cardiac index were detected to evaluate the effect of the medication, and HE staining was used to observe the myocardial histopathological changes, TUNEL staining to detect the apoptotic situation of cardiomyocytes, and ELISA to detect the serum thyroid-stimulating hormone (TSH), free triiodothyronine (FT3), free tetraiodothyronine (FT4) levels and thromboxane B2 (TXB2), 6-keto-prostaglandin F1α (6-Keto-PGF1α), endothelin 1 (ET-1), and nitric oxide (NO) levels. ResultsCompared with the blank group at the same time point, the body mass of rats in the remaining groups decreased at all time points (P<0.05 or P<0.01); compared with the model group at the same time point, the body mass of rats in high-dose Danggui Sini Granules group and Danshen Pill group increased at the 6th and 7th weeks (P<0.05 or P<0.01). In the blank group, the S-T segment of the electrocardiogram was close to the isoelectric line, the myocardial structure was regular, the fibres were closely arranged, and the nuclei of the cells were intact and neatly aligned; in the model group, the S-T segment of the electrocardiogram significantly elevated, and the arrangement of the myocardial fibres was obviously disordered, with myocardial cells appearing to be swollen, necrotic, and infiltrated by inflammatory cells, and the apoptosis-positive cells of the cardiac muscle cells obviously increased; in each of the medication groups, the electrocardiogram had a lowered S-T segment, and myocardial fibres were aligned, myocyte structure and morphology were improved, inflammatory cells reduced, and the number of apoptosis-positive cells significantly reduced. Compared with the blank group, the cardiac index, whole blood high cut viscosity, whole blood low cut viscosity, erythrocyte aggregation index increased in the model group, and the serum levels of TSH, FT3, FT4, NO, 6-Keto-PGF1α significantly reduced, and the levels of ET-1 and TXB2 significantly increased (P<0.05 or P<0.01). Compared with the model group, the levels of cardiac index, whole blood high cut viscosity, whole blood low cut viscosity, erythrocyte aggregation index, ET-1 and TXB2 significantly decreased and the levels of NO and 6-Keto-PGF1α significantly increased in each medication group, and the serum levels of TSH, FT3, and FT4 elevated in the high-dose Danggui Sini Granules group and Danshen Pill group (P<0.05 or P<0.01). The serum TSH level of rats in the low-dose Danggui Sini Granules group was lower than that in Danshen Pill group and the high-dose Danggui Sini Granules group (P<0.01). ConclusionDanggui Sini Granules have the effect of alleviating myocardial injury in coronary heart disease with cold congealing and blood stasis syndrome, and its mechanism may be related to reducing myocardial cell apoptosis, improving energy metabolism, and regulating the level of vasoactive factors.

Objective:To investigate the characteristics of anorectal dynamics in elderly patients with functional defecation disorders(FDD), and to provide a basis for their diagnosis, treatment and prevention. Methods:In this retrospective study, 226 patients with FDD receiving 3D high-resolution anorectal manometry were divided into an elderly group(93 cases)and a non-elderly group(133 cases). Results from anorectal manometry parameters were compared and analysis of patterns of anorectal pressure changes in elderly participants based on sex, the Bristol stool classification and clinical symptoms was conducted. Results:The resting anal pressure, rectal pressure and anal relaxation rate were lower( t=-3.407, -2.051, Z=2.548, P=0.001, 0.040, 0.011)and the volume of first sensation was higher( t=1.998, P=0.047)in the elderly group than in the non-elderly group. The maximum anal squeezing pressure, residual anal pressure and maximum tolerated volume were higher( t=4.589, 4.730, 2.025, all P<0.05), whereas the anal relaxation rate and anorectal pressure gradient were lower in elderly men than in elderly women( Z=4.059, t=-3.714, P<0.001 for both). Regarding the types of FDD, both the elderly group and the non-elderly group were dominated with type Ⅱ defecation disorder, with more men than women having type Ⅱ defecation disorder in the elderly group( χ2=10.343, P=0.001). In cases of paradoxical sphincter contraction during simulated defecation, the incidence in the elderly group was 80.65%(75/93), which was higher than 68.42%(91/133)in the non-elderly group( χ2=4.194, P=0.041). The volume of first sensation, volume of first defecation sensation, and maximum tolerated volume of patients in the elderly group without the urge to defecate were(59.86±23.84)ml, (96.76±34.61)ml, and(144.32±30.57)ml, respectively, higher than those of patients with the urge to defecate(46.79±17.20)ml, (75.26±28.75)ml, and(120.00±40.28)ml( t=-2.241, -2.493, -2.891, P=0.027, 0.014, 0.005). The rectal pressure(26.52±16.08)mmHg of patients with defecation dyssynergia was lower than that of patients without defecation dyssynergia(39.91±8.82)mmHg(1 mmHg=0.133 kPa)( t=-3.128, P=0.002), while the resting anal pressure of patients with defecation dyssynergia(90.60±28.44)mmHg was higher than that of patients without defecation dyssynergia(73.65±27.10)mmHg( t=-2.201, P=0.030). The resting anal pressure and maximum anal squeezing pressure in patients with anal blockage sensation[(87.11±24.64)mmHg, (149.28±48.29)mmHg]were higher than those in patients without anal blockage sensation[(72.43±20.02)mmHg, (121.76±26.35)mmHg]( t=2.954、3.066, P=0.004、0.003). There was no significant difference in values from parameters of anorectal dynamics between patients with different Bristol stool types, with and without incomplete defecation or with different degrees of abdominal distension(all P>0.05). Conclusions:Anorectal dynamics in patients with FDD are characterized by paradoxical anal sphincter movements, but older patients with FDD are mainly characterized by inadequate rectal propulsion, pelvic floor muscle dysfunction and reduced rectal sensitivity.

Objective To evaluate the current status of radioactive occupational hazards in the construction of some medical institutions in Tianjin, discover existingproblems and deficiencies, and provide a technical basis for radiation protection management. Methods Based on the requirements of relevant regulations and standards, we comprehensivelyevaluated the results of the pre-evaluation and control effect evaluation of 81 radioactive occupational hazards in Tianjin. Results Among the 35 proposed protective measures for pre-evaluation of radiation occupational hazards constructionprojects, the lowest compliance rate is warning signs and personal protective equipment. Among the 46 construction projects of radioactive occupational hazard control evaluation, the pass rate of protective measures was between 73.9% and 100%, with the lowest pass rate for personal protective equipment. Besides, the pass rate of radiation protectionmanagement evaluation was 95.7% ~ 100%, with the lowest pass rate for the protection management system. The initial inspectionpass rateof the equipment quality control acceptancewas 98.6%, while the pass rate was 97.1% for the relevant workplace radiation protection during the initial inspection. Conclusion The pre-evaluation completion rate of radiological occupational hazard evaluation in Tianjin medical institution construction project, unilateral and area of construction room, workplace layout and zoning, ventilation, shielding measures, personal dose management and occupational health inspection all meet the requirements of regulations and standards. However, there are still some deficiencies in the personal protective equipment, warning signs and the radiation protection management system. Therefore, the regulatory department should further strengthen the evaluation of medical institutions, especially the key supervision and management of non-conformities.

Objective:To understand the current status of quality control of medical X-ray diagnostic equipment in Tianjin, discuss the problems existing in the process of quality control, and provide technical basis for the relevant regulatory agencies. Methods:Using stratified random sampling, a total of 401 pieces of medical X-ray diagnostic equipment distributed over 111 medical institutions in Tianjin was used for this study. X-ray multifunctional dosimeters and other types of equipment were used to perform quality control testing and evaluation under the relevant health industry standards. Results:The initial testing pass rate of these equipment was 90.0%, with the pass rates of X-ray imaging equipment lower than those of X-ray fluoroscopy equipment, digital subtraction angiography (DSA) and X-ray computed tomography (CT) ( χ2=9.203, 3.383, 5.754, P<0.05). Among the imaging equipment, the screen X-ray equipment had the lowest pass rate, followed by the breast X-ray equipment and medical digital radiography (DR). The main unqualified items were output repeatability, deviation of tube voltage indication and limit spatial resolution. DR accounted for the highest percentage of X-ray equipment that were below testing condition. The unqualified proportion of X-ray imaging equipment was higher than that of X-ray fluoroscopy equipment ( χ2=8.216, P <0.05). Among the imaging equipment, the screen X-ray equipment was seen to have the highest unaqualified proportion, followed by computed radiography (CR). Among the 401 devices in this study, the initial testing pass rates of private medical institutions was not statistically different from those of public medical institutions ( P>0.05). The initial pass rates of domestic equipment was lower than those of imported equipment ( χ2=4.576, P<0.05). Conclusions:The performance of medical X-ray diagnostic equipment in Tianjin needs to be further improved. Medical institutions should pay attention to the quality control testing of medical X-ray diagnostic equipment. The relevant regulatory agencies should strengthen oversight and management of equipment that contains unqualified items, no baseline values and below the testing conditions.
