Objective:To investigate the dynamic characteristics of human papillomavirus (HPV) genomic integration during cervical lesion progression and the clinical value of HPV integration detection in stratify HPV-positive women, and to explore its molecular mechanisms in cervical carcinogenesis. Methods:A prospective cohort study was designed to enroll high-risk HPV (HR-HPV) positive women who underwent cervical cancer screening in Drum Tower Hospital Affiliated to Nanjing University Medical School and Nanjing Maternity and Child Health Care Hospital from July 2022 to July 2024. Cervical exfoliated cells samples were collected, and HPV whole genome targeted capture and high-throughput sequencing technology were used. The HPV integration patterns, host gene functional region distribution and pathway enrichment characteristics of 157 samples with different cervical lesions grades were analyzed, including 31 cases of normal cervix, 40 cases of cervical intraepithelial neoplasia (CIN) Ⅰ, 32 cases of CIN Ⅱ, 42 cases of CIN Ⅲ, and 12 cases of cervical cancer.Results:HR-HPV integration was detected in 80.2% (126/157) of the 157 HR-HPV positive samples. The incidence of HR-HPV integration in cervical cancer patients was 12/12, which was higher than that in normal women (77%, 24/31). The incidence of HPV16 integration was significantly higher in high-grade lesions, and the incidence of HPV16 integration was 43% (18/42) in CIN Ⅲ patients and 8/12 in cervical cancer patients ( P<0.001). A total of 14 438 integration events were detected in 126 samples with HPV integration. The integration sites were mainly distributed in the host intergenic region (51.0%, 7 359/14 438) and intronic region (38.1%, 5 494/14 438), and the integration frequency of viral L1 gene was the highest (28.4%, 4 498/16 781). Functional enrichment analysis showed that HPV integration-related host genes were significantly enriched in transport of small molecules, cyclic guanosine monophosphate (cGMP)-protein kinase G (PKG) signaling pathway, and purine ribonucleotide biosynthetic process, which synergistically drove carcinogenesis through multiple mechanisms. Conclusions:HPV integration events are significantly associated with the progression of cervical lesions. HPV integrated detection based on cervical exfoliated cells is expected to optimize the current screening strategy, reduce excessive intervention of HPV positive women and facilitate their accurate triage management.

Background Previous research on chlorinated paraffins (CPs) in fine particulate matter (PM2.5) has predominantly focused on short- and medium-chain chlorinated paraffins (SCCPs and MCCPs), and few studies could simultaneously determine short-, medium-, and long-chain chlorinated paraffins (LCCPs). Simultaneous extraction and determination of SCCPs, MCCPs, and LCCPs in PM2.5 could provide technical support for their environmental monitoring and human health risk assessment. Objective To establish a method based on QUEChERS pretreatment method in conjunction with ultra-performance liquid chromatography-quadrupole/orbitrap high resolution mass spectrometry for simultaneously determining the levels of SCCPs, MCCPs, and LCCPs in PM2.5. Methods The extraction solvents, extraction salts, and extraction steps of a QuEChERS method were optimized. The extraction efficiencies of the target substances were compared under 4 extraction solvents [acetonitrile, dichloromethane, and n-hexane solvents in sequence; acetonitrile: dichloromethane: n-hexane = 1: 1: 2 (v/v/v) mixed solvent; 1% acetic acid-acetonitrile: dichloromethane: n-hexane = 1: 1: 1 (v/v/v) mixed solvent; acetonitrile: dichloromethane: n-hexane = 1: 1: 1 (v/v/v) mixed solvent], 2 dehydrated salts (anhydrous MgSO4+NaCl and anhydrous Na2SO4+NaCl), 2 purification salts (C18 and PSA), and 4 vortex time (5, 7.5, 10, and 12.5 min) conditions. Then internal standard was utilized to estimate linear range and detection limit of the refined QuEChERS approach. Results The linearities of SCCPs, MCCPs, and LCCPs were good in the range of 10~

Objective:To explore the value of Gestational Diabetes Mellitus Risk Score in monitoring hypoglycemia among neonates delivered by mothers with gestational diabetes mellitus (GDM) .Methods:From February 2018 to June 2020, 240 neonates delivered by mothers with GDM admitted to the Shanghai First Maternity and Infant Hospital affiliated to Tongji University were selected as the study subject. The patients were assessed and divided into groups by the Gestational Diabetes Mellitus Risk Score and the Supplementary Gestational Diabetes Mellitus Risk Score from 12 to 20 weeks. The low-risk group ( n=63) was defined as the score of Gestational Diabetes Mellitus Risk Score ≤ 20 and the score of Supplementary Gestational Diabetes Mellitus Risk Score<33 from 12 to 20 weeks. The group with the score of Gestational Diabetes Mellitus Risk Score>20 and the score of Supplementary Gestational Diabetes Mellitus Risk Score <33 from 12 to 20 weeks was classified as the medium-risk group ( n=49) . High-risk group ( n=128) was defined as the score of Gestational Diabetes Mellitus Risk Score >20 and the score of Supplementary Gestational Diabetes Mellitus Risk Score >33 from 12 to 20 weeks. Three groups of patients were tested for fasting blood glucose (FPG) . Within one hour after birth, neonates were tested for blood glucose using the glucose oxidase blood glucose meter and given early sucking. Within 72 hours after birth, continuous dynamic blood glucose monitoring methods were used to measure blood glucose in neonates. This study compared the blood glucose within one hour and at 6, 12, 24, 48, and 72 hours after birth and the incidence of hypoglycemia at 72 hours after birth among three groups. Results:Among the 240 neonates in the risk group, 197 experienced hypoglycemia, with an incidence rate of 82.08%. There were statistically significant differences in the incidence of hypoglycemia before treatment, one occurrence of hypoglycemia within 72 hours, and recurrent hypoglycemia among the three groups of neonates ( P<0.01) . There were statistically significant differences in blood glucose among the three groups of newborns within one hour, 6, 12, and 24 hours after birth ( P<0.05) . There was no statistically significant difference in blood glucose among the three groups at 48 hours and 72 hours after birth ( P>0.05) . Conclusions:Neonates with gestational diabetes will suffer from repeated hypoglycemia. The higher the risk level, the higher the incidence of hypoglycemia. It is important to assess the risk of neonates with GDM, monitor their blood glucose in real time and treat them accordingly.

Objective To identify the relations of secondary cerebral edema volume with aquaporin 4 (AQP4) and reactive oxygen species (ROS) levels in patients with spontaneous deep supratentorial intracerebral hemorrhage. Methods Forty-seven patients with spontaneous deep supratentorial intracerebral hemorrhage, admitted to our hospital from December 2016 to January 2018, were chosen in our study; on the 1st, 3rd, 14th and 28th d of onset, the hematoma volume and secondary cerebral edema volume were measured by CT scan. Serum AQP4 and ROS levels were measured by ELESA. The relations of perihematomal edema volume with AQP4 and ROS levels in patients with spontaneous deep supratentorial intracerebral hemorrhage were analyzed. Results The hematoma volumes on 14th and 28th d of onset were significantly decreased as compared with those on 1st and 3rd d of onset (P<0.05); the serum AQP4 and ROS levels gradually increased on 1st, 3rd, and 14th d of onset, with significant differences (P<0.05). The cerebral edema volume, and serum AQP4 and ROS levels on 28th d of onset were significantly decreased as compared with those on 3rd and 14th d of onset (P<0.05). Serum AQP4 and ROS levels were positively correlated with cerebral edema volume (r=0.331, P=0.000;r=0.541, P=0.000); serum ROS level was positively correlated with AQP4 level (r=0.298, P=0.000). Conclusion The changes of brain edema volume, and serum AQP4 and ROS levels in patients with spontaneous supratentorial intracerebral hemorrhage are consistent and positively correlated, which suggests that the antioxidant may reduce the AQP4 protein expression, reduce the degree of brain edema, and alleviate the deterioration of patients with spontaneous intracerebral hemorrhage.
